Information to persons undergoingprocedures• 15. (4) Before performing an assistedreproduction procedure that makes use ofhuman reproductive material or an in vitroembryo, a licensee shall disclose to the personundergoing the procedure the health reportinginformation in its possession respecting thedonor, but the identity of the donor — orinformation that can reasonably be expected tobe used in the identification of the donor —shall not be disclosed without the donor'swritten consent.
Destruction of information• 16. (2) A licensee or any other person that hascontrol of the health reporting informationprovided by a donor of human reproductivematerial or an in vitro embryo, by a person whohas undergone an assisted reproductionprocedure or by a person who was conceived bymeans of such a procedure shall, at the requestof the donor or that person, as the case may be,destroy that information in the circumstancesand to the extent provided by the regulations,and shall inform the donor or that person thatthe destruction has occurred.
