BapayeRecording a good quality surgical video is essential for documentation ofsurgeons’ surgical skill. Creating a quality video is an important tool foreducating not just postgraduate students and colleagues but also patients.Creation of a surgical video involves recording the surgical procedure, editingit to highlight important aspects and then storing it in suitable format for laterviewing.Recording of posterior segment surgery remains a challenge due to lowresolution of single chip CCD (Charge Coupled Device) cameras that arecommonly used in anterior segment surgery recording. The high resolution3-chip CCD cameras that are used with C-mount on beam splitter are costlyand system remains cumbersome. There have been reports where handhelddigital video camera (camcorder) has been manually placed on one of theeyepieces of assistant scope to record the surgery. 1We describe a system using FlexioMotion TM adapter by Carl Zeiss Inc and SonyHandycam TM to record the posterior segment surgery with good resolutionand ease of archiving the videos.Recording the videoIn the OR we use OPMI MDO (Carl Zeiss Inc.) microscope with S5 stand forposterior segment surgery. The microscope has X-Y and zoom functions. Noncontact wide angle viewing system Binocular Indirect Ophthalmomicroscope(BIOM IV) (Occulus, GmBH, Germany) is used for posterior segmentvisualization. For macula work we use Flat lens of Landers’ lens system withself stabilizing ring. Flexio Motion TM adapter developed by Carl Zeiss Inc isconnected to one port of dual port beam splitter. This adapter can be connectedto a wide range of handheld digital camcorders commercially available. Weuse Sony HDR XR-500E Handycam TM . The handycam records videos in ‘FullHD’ AVCHD format having resolution of 1080i. The camera has hard discwith 120GB capacity. It records videos for approximately 1200 minutes beforehard disc is entirely occupied. The camcorder has automatic functions forfocussing, adjusting white balance and illumination. These functions can bechanged manually if required though we prefer to use autofocus function aswe can avoid manual adjustment during critical stages of surgery. Camerazoom can be adjusted separately using camera controls. Once desired level ofmagnification is achieved, it further varies proportionately with microscopezoom function. The camera comes with video output cable which is connectedto external monitor for real-time viewing.1044
