East Side <strong>Case</strong> Management Consortium <strong>Case</strong> <strong>Manager</strong>Lenox Hill Neighborhood House is the oldest and largest social services and educationalorganization on the Upper East Side of Manhattan. Operating out of multiple East Sidesites, we are a vibrant community organization and serve 20,000 people in need each yearwho live, work or go to school on Manhattan’s East Side and on Roosevelt Island. Ourconstituents include older adults, homeless and formerly homeless adults, children andfamilies, recent immigrants, disabled persons and thousands more who make up New YorkCity’s wonderful mosaic.We have a 117-year history rich in community activism and innovation, addressing systemicproblems such as the lack of affordable housing, the needs of an aging population and theinadequacies of the public education system. Lenox Hill Neighborhood House leads theEast Side <strong>Case</strong> Management Consortium, a partnership with Search & Care and the CarterBurden Center on Aging, agencies that have been a long-standing service provider to theolder adults in the community and whose staff are experts in the field of aging.<strong>Job</strong> Responsibilities: Working as part of a team, you will assess the needs of older persons,develop a care plan to help meet these needs, and implement the care plan, which includesproviding information on appropriate services and programs, making referrals for services asneeded, assisting with the collection of documents and complete necessary paper work forbenefits, providing linkages with other service providers and assisting with bill paying andhealth insurance reimbursements. You will also:Initiate and maintain contact with representatives of the community and governmentagencies on behalf of the clientParticipate in case conferences as an advocate for the client as needed.Maintain statistical, narrative, and financial records including written records ofcontact with clients according to Consortium and NYC Department for the AgingstandardsEnsure each client has up to date emergency contact information and that there is anemergency protocol developed for each clientBecome knowledgeable about community resources; make all staff aware of any newresourcesParticipate in weekly staff meetings, in-service trainings and conferences. Attend therequired number of funder sponsored trainings as outlined in NYC Department forthe Aging standards


Provide office and/or intake back up as neededIn cases of emergency, when the client’s home is uninhabitable, begin the process ofsorting and disposing of items with client’s permission, arranging for and executing aprofessional cleaning and extermination, and securing foundation funding to pay forsuch services for clients without the means to do soWhen necessary, refer to and ensure APS follows up on cases where clients’competency is questionable. Support and work with the client through the processof guardianship hearingsSecure foundation funding allowing clients to remain in their own homes as long aspossible which includes clients whose rent and monthly expenses exceed theirincome.Provide emergency shopping assistance in the event a client runs out of food orother necessities and is not scheduled for homecareOther tasks and assignments as indicated depending on the needs of the individualclients, the program and the ConsortiumQUALIFICATIONS:BA or BSW with relevant experience acceptedBilingual Spanish/English preferredExperience with the elderly a plus but some experience in the field of human services isrequiredPlease send cover letter and resume to:Chris Chin, LMSWLenox Hill Neighborhood House331 East 70 th StreetNew York, NY 10021Email: cchin@lenoxhill.org
