ModerationYou will automatically be allocated a visiting moderator for June 2012 unless you request to attend an area moderationmeeting.Centres who took part in area moderation in 2011, will be assigned to a meeting, where possible, unless they wish to opt outfor 2012. If you wish to apply for area moderation or opt out, please complete form CW/AREA/APP2012, available at:www.aqa.org.uk/areamod2012. Completed forms must be submitted to AQA by 14 November 2012.Administration checklistCandidate Record FormsOne for each unit for each candidate, which must be signed by the candidate and teacher, and available for the moderationvisit/area moderation meeting.Centre Declaration SheetsEach centre must complete a Centre Declaration Sheet for each endorsement to indicate that internal standardisationhas taken place, even if only one teacher did all the marking. Moderators will collect Centre Declaration Sheets at the visit/meeting.Centre Mark FormsSent to centres in April (one for each unit). The Centre Mark Forms must be completed and posted to be received by AQAand your moderator by 31 May at the latest. The top (white) copy is to be sent to AQA and the remaining two (yellow/pink)copies to your moderator. Your moderator will return one copy to you with the sample indicated.Lost courseworkIf work is lost or damaged due to no fault of the candidate then the candidate should be given extra time to re-do the lostwork. If it is not possible for the work to be reproduced then the centre should complete JCQ Form 15 and send completedform to AQA.Secure storage of candidates workThe centre must retain the work of all candidates, with Candidate Record Forms attached and under secure conditions,from the time it is internally assessed until at least Results Day, to allow for the possibility of an enquiry about results. If anenquiry about results is made, the work must remain under secure conditions until the outcome of re-moderation. Pleasenote that enquiries about results will not be accepted if the work has been returned to candidates at any time.Area Moderation meetingsYour moderator will send an Area Moderation Mark Sheet (MOD/AREA/A&D) to your centre with details of the selectedsample.
