LABORATORY<br />

at <strong>Nouna</strong> District Hospital<br />

The new laboratory of CRSN<br />

was integrated into the<br />

District Hospital in 2005.<br />

The research laboratory<br />

thus creates a spin off into<br />

the community through<br />

training of hospital technicians<br />

and provision of<br />

laboratory services to pati<strong>en</strong>ts.<br />

Laboratory services<br />

offered to in- and outpati<strong>en</strong>ts<br />

inclu<strong>de</strong> routine diagnostic<br />

tests in hematology, biochemistry,<br />

microbiology, serology and parasitology.<br />

Pati<strong>en</strong>ts <strong>en</strong>rolled in national<br />

health programs and research<br />

projects are offered specific tests,<br />

e.g. voluntary HIV testing, CD 4<br />

count for HIV infected pati<strong>en</strong>ts on<br />

treatm<strong>en</strong>t.<br />

The CRSN laboratory has become the<br />

leading PCR laboratory for HIV <strong>de</strong>tection<br />

in new born infants from HIV<br />

infected mothers. The microbiology<br />

unit is forese<strong>en</strong> as a regional refer<strong>en</strong>ce<br />

laboratory for m<strong>en</strong>ingitis surveillance.<br />

Special equipm<strong>en</strong>t: ELISA rea<strong>de</strong>r<br />

(computer assisted), cell counter<br />

(Sysmex and Becton Coulter), FACS<br />

counter, FACS Scan, laminar flow<br />

b<strong>en</strong>ch, spectrophotometry, PCR<br />

(thermocycler, gel camera etc), <strong>de</strong>ep<br />

freezer (-80°C), and high quality<br />

microscopes .<br />

CLINICAL RESEARCH<br />

at <strong>Nouna</strong> District Hospital<br />

CRSN and <strong>Nouna</strong> district hospital are<br />

accumulating an impressive record of<br />

community based and clinical trials.<br />

Clinical research activities started in<br />

1999 with a large WHO-fun<strong>de</strong>d community-based<br />

randomized controlled trial<br />

(RCT) on the effects of zinc supplem<strong>en</strong>tation<br />

on malaria morbidity. The zinc trial<br />

was followed by a RCT on the long-term<br />

effects of insectici<strong>de</strong>-treated mosquito<br />

nets (ITN) on the malaria morbidity and<br />

mortality and on the overall mortality<br />

in childr<strong>en</strong>. Several malaria drug trials<br />

have be<strong>en</strong> carried our rec<strong>en</strong>tly (methyl<strong>en</strong>e<br />

blue and azithromycin combination<br />

therapies). With increasing experi<strong>en</strong>ce<br />

with phase II / III clinical trials, CRSN and<br />

<strong>Nouna</strong> District Hospital are <strong>de</strong>veloping<br />

into a site which meets international<br />

standards.<br />

To improve and standardize individual<br />

pati<strong>en</strong>t care, clinical managem<strong>en</strong>t procedures<br />

are being investigated, such<br />

as diagnosis and treatm<strong>en</strong>t of childr<strong>en</strong><br />

with impaired consciousness with particular<br />

emphasis on complicated malaria<br />

and bacterial m<strong>en</strong>ingitis. In the area of<br />

HIV, <strong>de</strong>terminants of mother-to-child<br />

transmission are investigated.<br />

High standards of clinical data docum<strong>en</strong>tation<br />

and managem<strong>en</strong>t and laboratory<br />

facilities are a prerequisite for clinical research<br />

and for fulfilling the requirem<strong>en</strong>ts<br />

of a clinical trial site. CRSN and <strong>Nouna</strong>,<br />

together with partners from Hei<strong>de</strong>lberg<br />

University Hospital, have substantially<br />

invested in this field in rec<strong>en</strong>t years.<br />
