<strong>Personal</strong> <strong>Leave</strong> of <strong>Absence</strong>The Company affords Managed Time and other benefits to provide employees with approvedtime out of the office to tend to personal matters. However, there may be rare instances in whichthe employee does not have a sufficient amount of available paid time off to cover a particularpersonal situation. While employees must first utilize any and all accrued paid time off or otherapplicable benefit, a <strong>Personal</strong> <strong>Leave</strong> of <strong>Absence</strong> (PLA) may be requested. A PLA is an unpaidleave of absence that may be granted at the discretion of the Company. If you are an eligibleEmployee, you may request a PLA for situations such as:Your or an immediate family member’s health matters:pregnancy or adoption;continuing education;unique travel opportunities; orpolitical opportunities.If you are not eligible for medical leave according to the Family and Medical <strong>Leave</strong> Act (FMLA)policy, you may be able to request a PLA.PLA will not be granted in increments of less than five consecutive work days.Eligibility for a PLAYou are eligible to request a <strong>Personal</strong> <strong>Leave</strong> of <strong>Absence</strong> (PLA) if you are a regular full-time orregular part-time Employee of the Company.The maximum amount of leave that can be taken is 12 weeks within a rolling 18 month period. Allcarry over Managed Time and accrued Managed Time must be taken before the start date of theunpaid leave.You are prohibited from taking unaccrued Managed Time for a PLA.When you are on a PLA from the Company, you will not accrue Managed Time Credits. When youreturn to work from PLA, you will start to accrue Managed Time Credits from the date of yourreturn. Managed Time will not be earned retroactively. If you do not return to work, any earned,unused Managed Time you have as of your anticipated return to work date will be paid out in thesame manner as it is for termination from the company.Since PLAs are granted at the discretion of the Company, the Company has the right to denyyour request.Regular, predictable attendance is an essential function of all roles. If your PLA causes you to beaway from work for more than the amount of time that the PLA is granted (maximum 12 weeks),your employment with the company will be terminated.For LFG internal use only. Requesting a PLATo request a <strong>Personal</strong> <strong>Leave</strong> of <strong>Absence</strong> (PLA), you will need to contact your HR Representative.Your request will include:The specific reason for the requested leave;the amount of time you will be on your leave; andthe beginning and ending dates of your leave.If you wish to request a PLA, you must notify your Manager and your HR Representative of yourrequest 30 days prior to the beginning date of your PLA. If the date is not known 30 days inadvance, please notify your Manager or your HR Representative as soon as any informationconcerning your beginning date becomes available.Each request for a PLA will be an individualized evaluation and the Company may considerfactors such as, but not limited to: the reason for the request; the employee’s performance; theemployee’s previous attendance history; the reasons for previous absences; and business needs.Benefits While on PLACertain changes to your benefits may occur if you have a life event change such as a familystatus change, disability, leave of absence, termination of employment, retirement or death.Taking an Approved Unpaid <strong>Personal</strong> <strong>Leave</strong> of <strong>Absence</strong>If you take an approved unpaid <strong>Personal</strong> <strong>Leave</strong> of <strong>Absence</strong>, all of your current benefit coveragesmust continue during the leave. You will be required to pay for your benefits for this period ofleave. If your leave is for less than 30 days, when you return to work, you will have doubledeductions taken from your pay until you are caught up. If your leave is expected to last longerthan 30 days, you will be billed monthly.You should call the HR Service Center at 866-922-6543 for more information.Company paid core benefits are not affected by family status changes. Returning from an Approved Unpaid <strong>Personal</strong> <strong>Leave</strong> of <strong>Absence</strong>When you return from an approved unpaid <strong>Personal</strong> <strong>Leave</strong> of <strong>Absence</strong>, your benefits willcontinue without change and payroll deductions will be reinstated. If you fail to return to work, youwill be charged retroactively for all benefits costs that were paid by the Company during theleave.Company paid core benefits are not affected by family status changes.
