This solution is onlyavailable through Siemens.SIEMENS16-ChannelDiagnostic CoilThe NORAS BI 320-PA-SI was developed for the Siemens TIM system and serveswith its 4Ch array coil for diagnostics as well as biopsy removal. The design of thewhole setup is very open to ensure optimal access to the breast for interventions.Optionally, you can use the 16Ch coil for optimized imaging (spectroscopy possible)with the same Patient Rest.The biopsy set consists of a complete positioning unit and two disposable gridsfor medial, lateral and cranio-caudal access. BIOPSYManually operated calculation of coordinates for the MR-supportedbreast biopsy with the NORAS Biopsy Unit on Siemens MR-systems.1. Perform a measurement with transversal slices and look for the slice onwhich one you can see the marker.2. Start the distance-tool (menu, right center → „Extras“ → „distance“(ruler)).3. Draw a line, straight through the pointer.18


BIOPSY4. Mark that line with a left-click and select the marked line with a right-click.In the context-menu, select „copy“. By doing so, the line is copied and canbe inserted into other slices.5. Scroll through the other slices until you found the lesion that you want toperform a biopsy on. Right-click on this slice und select „paste“.6. Select the „distance-tool“ again and measure the distance betweenthe lesion and the reference line that you inserted in step5. In this example the biopsy-unit has to be moved 22mm fromthe marker zero position to anterior (because the patient is lyingon prone position).19


BIOPSY7. In order to determine the shift from head to feet, view the slice positionof the marker and lesion at the transversal slices. The marker from step1. is located on slice-position („SP“=Slice Position) H 7.0, therefore 7mmtowards the head from the isocenter.8. The lesion you have chosen in step 5. is located in our example on sliceposition H 25.0. Hence, the difference is 25-7=18mm. In order to reach thelesion, you have to move the biopsy unit by 18mm towards the head.9. Move the Post&Pillar unit with the marker according to the calculatedcoordinates (in our example 22mm downwards (anterior) und 18mm towardsthe head).20


BIOPSY10. Perform a control scan in order to check that marker is located infront of the lesion.11. Use the distance-tool again in order to measure the puncture depthfrom the surface of the skin to the middle of the lesion (in this case68mm). Deduct, depending on the biopsy system, a certain offset(normally about 5-10mm), in order to make the cannula lying infront of the lesion. Now puncture with this offset-corrected depth(starting at skin surface) in the breast using the trocar. Afterwardspull the needle out of the trocar and insert a plastic-bar to the cannulainstead. The plastic-bar causes less artifacts than the needleand prevents blood flow into the cannula.12. Perform a control scan in order to check that the end of the needle is positionedin front of the lesion.Now perform the actual biopsy with properlytrained personal.21


BIOPSYAngulated, manual calculation of coordinates for the MR supportedbreast biopsy with NORAS biopsy unit on Siemens MRsystems.Perform the steps 1 to 10 from the user manual for the straight, manual calculation of coordinates.If you notice during the straight biopsy that, for example, one of the bars of the grid is in the wayof the biopsy, or if you prefer to continue with an angulated biopsy for any other reason, you canalso work with an angulation of 15 or 30 degrees toward the top or the bottom.14. Now, an angle of 15 degrees (in this example) has to be drawn to the originalmarker line. To do so, one needs to use the tool, which you find on theright hand side under “Extras” → „angle“.15. Draw the first line, in this example from right to left, through the markertoward the lesion.16. Draw the second line with an angle of 15 degrees from left to right. Thiscan also be done slightly underneath the first line.22


BIOPSY17. Now, go ahead and move this second line (which forms an angle of 15 degreeswith the first line) so that it has its left endpoint in the center of thelesion. Then, left-click the second line. Finally, hold down the left mousekey and move the line.18. Choose the “distance” tool und draw the new line with alength of 22mm from the tip of the marker to the back,where the pivot point of the mechanic is located.19. The second line has to have appropriate length toward the right so it lastsat least underneath the pivot point. You can also extend that line afterwardsby pulling its right endpoint.23


BIOPSY20. With an additional line, measure the distance of the pivot point on themarker (22mm behind the tip) to the line with 15 degrees angle (here 25.4mm). This is the amount you need to move the biopsy unit posterior (towardthe ceiling).21. BIOPSYGrid-BiopsyIn the following the Grid localization-process withmedial/lateral access while using sagittal imaging slices isbriefly described.The grid pattern itself is used to locate the position ofthe lesion.The grid plate is mounted on the biopsy unit for the fixation of the breast. The pattern of this gridis visible on the sagittal MR-image at the edge of the breast. Therefore the grid position lyingdirectly in line with the lesion can be identified manually. A needle block is inserted into that gridposition.A marking is created around, respectively on the lesion (using the imaging tools on the scanneruser interface, e.g. a circle or a cross). This marking is copied and pasted into the parallel sagittalslice, which shows the grid pattern. Now it can be easily seen, where the needle block and theneedle need to be positioned. Additionally a MR-visible marker block for better orientation on thegrid can be used (in this imaging example, the marker is sitting in the middle of the grid).With the knowledge of the number and thickness (plus a potential gap between slices) of thesagittal imaging slices from the surface of the grid to the lesion, the penetration depth can beeasily calculated.25

Duringdevelopment, NORAS emphasized on the need to allow the useof existing coils as well as most of the already existing head holdercomponents.The basic concept is to create a sterile seal or chamber in betweenthe patient’s head and the top coil with two sterile drapes.A new reference star and head holder adapter has been developedin collaboration with BrainLab AG. The reference star cannow be tightly attached to the head holder without perforatingthe sterile drapes.The new system is going to be offered to the current clients asan upgrade kit.Complete setup with new sterile concept. NORAS products in development16-Channel CPC versionWe are currently developing a bigger version of the CPC coilwith 8+8 channels instead of the usual 4+4. NORAS Neurosurgery Solution 5, FlexibilityThis new version of our Neurosurgery Solution, which is indevelopment right now, will be released in Q4 2012. It willprovide much more flexibility in the way the patient canbe positioned.The whole headholder is height-adjustable, which willenable optimal positioning in 70cm bore systems (forinstance Philips Ingenia or Siemens MAGNETOM Aera &Skyra)Additionally there is the possibility to move the setup inz-direction, along the bore-direction, which is very convenientwhen placing the patient on the transfer-board inthe first place. The setup can be adjusted to the patientposition and not the other way round.We will also implement a mechanically improved solutionfor the anchorage of the 3-point fixation on the c-arc,which will block any movement at the wing point.VisionAll this new and improved features will of course be ontop of the already available new sterile concept (all coilsand cables can remain non-sterile all the time) and thevarious accessories.
