Where this is a third country, boxes 16 and 17 must contain details of therelevant permit. Where specimens <strong>or</strong>iginating in a Member State of the Union are exp<strong>or</strong>ted from another,only the name of the Member State of <strong>or</strong>igin must be mentioned in box 15.18 to 20. The country of last re-exp<strong>or</strong>t is, in the case of a re-exp<strong>or</strong>t certificate, the re-exp<strong>or</strong>ting third country fromwhich the specimens were imp<strong>or</strong>ted bef<strong>or</strong>e being re-exp<strong>or</strong>ted from the Union. In the case of an imp<strong>or</strong>tpermit, it is the re-exp<strong>or</strong>ting third country from which the specimens are to be imp<strong>or</strong>ted. Boxes 19 and 20must contain details of the relevant re-exp<strong>or</strong>t certificate.21. The scientific name must be in acc<strong>or</strong>dance with the standard references f<strong>or</strong> nomenclature referred to inAnnex VIII to Regulation (EC) No 865/2006.23. Provide as many details as possible and justify any omissions to the inf<strong>or</strong>mation required above.
