It is offeredsimultaneously with all otherappropriate medical treatment.”Palliative Care can help at any stage ofa serious illness and at the same timeas curative treatment. It is differentfrom Hospice care where you need tohave a prognosis of 6 months or lessand forego curative treatment.ow does it work?HThe Palliative Care Team providesassistance with:Pain & symptom management(shortness of breath, nausea,vomiting, etc.) for patients withadvanced illnessEmotional and spiritual support forpatients and family membersAdvance Care Planning (DNR,Healthcare Proxy, Living Wills,etc.)End-of-Life decision makingALICE HYDE MEDICAL CENTER’SPALLIATIVE CARE TEAM INCLUDES:PHYSICIANSNURSESCASE MANAGERSSPIRITUAL COUNSELORSPHARMACISTSAHMC’S PATIENT REPRESENTATIVEho is appropriateWfor a referral?Hospitalized patients and Alice HydeNursing Home residents with:Life threatening or life limitingillness—typically cancer, dementia,congestive heart failure (CHF),chronic obstructive pulmonarydisease (COPD), stroke or, endstagerenal or liver disease.Deteriorating medical conditionsand in need of symptommanagement.Unacceptable level of pain,uncontrolled symptoms (nausea,vomiting), or uncontrolledpsychosocial or spiritual issues.ow do I get aHPalliative CareReferral?Anyone can request a consultation(patients, family members, or anyhealthcare professional). Once arequest is made for a consultation,your primary care provider iscontacted for his/her approval.The Palliative Care Team evaluatespatients and makes the appropriaterecommendations to the referringphysician and provides follow-up asneeded. The Team does not take overthe patient care.
