Sonia is in touch withJustin Hottinger (Public Health)• Mental Health and Children’s Services attending the meeting today3. Work Programme ProjectsInformation Services – Information, Advice and Advocacy Projects (IAA)• Progressing and being led by Lorna Bright• Jo Cook gave a brief update (Update paper attached) -o An extensive consultation exercise has been undertakeno The engagement exercise comprised of a number ofelements: Survey (paper and online) has been completed– 270responses received Face to face days with key groups A full day workshop with providers, County <strong>Council</strong>Officers and key stakeholders A Community Services Practitioner Workshopo Tendering exercise now taking place but model is a long term
piece of worko Key elements of the model are – Portal hub (or hub) - Implementation 2 nd quarter of2012 Location Contact Assessments Channel and formats Raising Awareness People who fund their own careo There are some early messages from the engagementexercise generally around access to IAA and a longdiscussion took place in particular on how to consider ruraland deprived areas and people who are housebound orsocially isolatedo Jo confirmed that there is currently some work being done onhelping people access digital TVo All were in agreement that access to information had to bemuch more interactive and smarto SS asked if results from the survey could be shared and JCagreed to send the responseso It was agreed that to invite Lorna Bright to attend the nextmeeting but Sonia and Jen to liaise on dateso Full analysis of consultation exercise will be available fromthe end of NovemberSonia/JenEnabling Community Groups – Ageing well – Rural Isolation Project -Sonia Shuter• For those new to the Forum, Sonia explained that this was one ofthe 3 key areas discussed at the last <strong>NNHIF</strong> meeting• This fits in with Localism/Big Society and also the project with theYoung Foundation on Ageing Well. Sonia, Rebecca and JohnEverson attended a meeting recently and have completed somework. A further meeting will be taking place shortly in Cambridge.• <strong>North</strong> <strong>Norfolk</strong> <strong>District</strong> <strong>Council</strong> (NNDC) held an evening meeting inJuly with Parish and Town <strong>Council</strong>s on improving Health andWellbeing in communities when a pleasing number attended• A questionnaire has been distributed through voluntary groups aboutissues to them and currently public transport appears to be the keyissue. The deadline for this is the beginning of December and Soniaasked if anybody would like to take some to distribute locally• Nick mentioned a discussion that recently took place with NNDCCabinet when it was is clear that the there is political will to supportlocal groups at a low level• Sonia confirmed that she had met with Lucy McLeod recently who iskeen to help identify issues and areasActivity Referral – Rebecca Champion• There has been an Exercise Referral Scheme in <strong>North</strong> <strong>Norfolk</strong> for
several years. The involvement of GP Surgeries has varied acrossthe district.• Results have been positive, but has been largely reliant on individualGPs interest, it has mainly focussed around Fakenham and <strong>North</strong>Walsham practices.• Sonia advised that a multi agency group had been looking at thecurrent scheme and it has been agreed that it will be re branded andre vitalised and will ensure equitable opportunity for access across<strong>North</strong> <strong>Norfolk</strong>. The scheme will focus on activity rather than exercisereferral and will also include recommendations to other activityopportunities such as Fit Together project. There will be a re-launchof the rebranded scheme in March 2012• Rebecca gave a brief update on Broadland <strong>District</strong> <strong>Council</strong>’s,Broadly Active Scheme.• It was suggested that this website address is advertisedwww.readytochange.org.uk – anyone with a referral can log intohere to find a self help area and access some informationSonia4. Updates from last meetingStaff/Organisational/service changes• Jayne Rose (replacing Liz Haistead – Stockwell )• Up to compliment with Case Managers• Contract Variation agreed for half time case manager, so there willbe one in each of the community teams• Each Case Manager will be taking the 25 patients from the Frail andOlder People group and work with them to manage their long termconditions to essentially keep them out of hospital and away fromGP Surgeries. Linking this to the wellbeing programme would be ofbenefit• Community Nursing Trust is geared to working closer to home,rather than patients travelling to acute unit or community hospital,this is linked with preferred placement areas. There is a WellbeingBusiness Unit and Jayne will feedback from this groupJayneChristine BarnetHealth Visiting and School Nursing -• Training 42 students this year and again next year• Working on Sure Start tenders for more centres• Children/Baby Clinics do take place in Village Halls fordevelopmental checks• Possibility that Health Trainers may be interested in attending thismeetingIan Bell• Wellbeing Service started in April 20<strong>11</strong> essentially the Trust winning
the bid for the IAP service to include LINK workers plus IAP workers,partners with MAP, MIND and RELATE therefore forming theWellbeing Service• <strong>North</strong> <strong>Norfolk</strong> area is particularly high in self referring• Launch takes place on 28 November and will help to widen theability to self refer in communities. Promotion leaflets will be placedin libraries, village halls etc• Referrals to the Wellbeing Service from GPs Service are triaged andgiven to the right organisation. Work is based in GP Surgeries,mainly in market towns, where clinics are held for IAP workers• Self referral can be by telephone, on line (Wellbeing link above) but28 November the new website will supersede the Trust Website• Euan said that we have a comprehensive mental health service andit is useful to interact with other agencies to understand how we canchange and make a differenceEuan Williamson –• NHS <strong>Norfolk</strong> and Waveney – single Executive Board• In the new year a restructure will take place to align the organisationwith Commissioning Care Groups will take place• There is currently a tender out for Substance Misuse Treatmentcentre, to include alcohol and drugs, with a new provider taking overin October 2012• Older People – will be looking at frequent attendees at A & E interms of alcohol abuse and looking at putting in a service for these• Sonia mentioned that the Benjamin Foundation are working inpartnership with the Police on Crime Innovation and putting in a bidfor a grant to include working with young people and alcoholHanne Lene Schierff –• Social care element – personalisation projects• Contracts with big and small providers helping people to makechanges and making awareness on personalised budgets• Quarterly forums will be taking place5. Health Watch• Taking over from LINKS• A paper giving a brief summary was circulated and discussed• Looking at preferred modelsAny other business• Rob Lewis was keen for members of the Forum to be aware of therole Environmental Health plays in terms of Housing conditions andtriggers. Safeguarding Adults will become mandatory as from nextyear and EH will be organisations eyes and ears Funding
opportunity – Funding available to help reduce incidents of winterfuel deaths. Closing date deadline is 3 weeks from today and £20mavailable with only local authorities allowed to bid. This will gotowards providing information and help on how to keep warm andwell, energy efficiency, voluntary sector, hot meals, shopping• All keen for Sonia to put in a bid• Rebecca went through the Winter Plan• Jen was asked to add Mental Capacity Act on next Agenda – Ianand Euan to invite a colleague to the next meeting who is wellbriefed on the ActSoniaJen/Ian/Euan6.
