Contractor and all subcontractors (collectively “Contractors”) shall, unless otherwise approvedin writing by Owner, obtain and maintain throughout the duration of the Project (or as otherwisespecified) insurance of the types and in the amounts described below (the “RequiredInsurance”):1.1 Commercial General Liability Insurance.1.1.1 Combined single limit of:$1,000,000 Each Occurrence$2,000,000 General Aggregate$2,000,000 Products/Completed Operations Aggregate$1,000,000 Personal and Advertising Injury$100,000 Fire Legal Liability$5,000 Medical Payments1.1.2 Coverage Required: Per project aggregate, premises-operations, explosion,collapse, underground, products/completed operations, independent contractors,blanket contractual liability, personal injury liability, general liability primary(non-contributory), additional insured (CG 20 10 11/85 or its equivalent)including products/completed operations coverage and waiver of subrogation,Designated Construction Project(s) General Aggregate Limit endorsement (CG25 03 03 97 or its equivalent), and Designated Location(s) General AggregateLimit endorsement (CG 25 04 03 97 or its equivalent). Insurance is to be on anoccurrence form insuring bodily injury and property damage against the hazardsof: construction operations, subcontractors and independent contractors, andshall include an aggregate limit per job site endorsement. Products andcompleted operation insurance shall remain in force five (5) years followingcompletion of project.Owner and all parent corporations including Sisters of Mercy Health System shallbe included as an insured under the commercial general liability insurance, usingAdditional Insured Endorsement (CG 20 10 11/85 or its equivalent).3262341


1.2 Business Auto Liability Insurance.1.2.1 Combined Single Limit of:$1,000,000 each Occurrence Bodily Injury and Property Damage1.2.2 Coverage Required: Primary for all owned automobiles; including liability forall owned hired/non-owned automobiles leased, rented and/or borrowedcars/trucks; medical payments, uninsured and underinsured motorists coverage.Owner and all parent corporations including Sisters of Mercy Health System shallbe included as additional insured. An MCS-90 endorsement (or its equivalent)shall be attached when operations require this coverage.1.3 Workers’ Compensation Insurance.1.3.1 Workers’ Compensation Limits: Statutory1.3.2 Employers’ Liability Limits:Bodily Injury by Accident $1,000,000 Each AccidentBodily Injury by Disease $1,000,000 Policy LimitBodily Injury by Disease $1,000,000 Each Employee1.3.3 The alternative employer endorsement shall be attached showing Owner in theschedule as the alternate employer.1.3.4 Workers compensation limits shall not be less than the law requires.1.4 Excess Umbrella Insurance.$10,000,000 Each Occurrence$10,000,000 Aggregate$10,000,000 Products/Completed Operations Aggregate$10,000 RetentionCoverage is following form of underlying general liability, automobile liability andemployers’ liability, coverage including per project aggregate, primary, additionalinsured/non-contributory, and waiver of subrogation.1.5 Builder’s Risk. Limits vary depending upon the value of the Project. Builder’s Riskcoverage may be provided by the Contractor or by the Sisters of Mercy Health Systemproperty policy. Please note that the property policy carries a $50,000 per occurrencedeductible, which will be the responsibility of Contractor, unless agreed to otherwise withthe Contractor.
