United Cerebral Palsy of Greater Cleveland, Inc.JOB TITLE: Adult Day Services Supervisor/Habilitation Specialist #59SUPERVISED BY:Adult Day Services ManagerPOSITIONS SUPERVISING: Adult Day Services AssistantsEXEMPT POSITION; Job Grade 35BASIC FUNCTION: Provides case management/service coordination, advocacy, support andpersonal care for clients enrolled in Adult Day Services. Develops and implements personcenteredgoals for individual plans. Observes, assesses, instructs and monitors progress ofindividual clients by spending time in the activity rooms with them. If needed, implementsbehavioral interventions. Supervises the Adult Day Services Assistants to ensure they areengaging the clients in meaningful activities and providing them with quality personal careservices. Interacts cooperatively with UCP staff, referral sources, family/guardians and otherstakeholders to promote and demonstrate a team approach to service delivery and integration.QUALIFICATIONS:1. Bachelor’s degree in vocational rehabilitation, special education, social work or relatedhuman services field required.2. Minimum of three years experience in direct service delivery to adults with developmentaland/or physical disabilities preferred.3. Experience in functional assessment, case management, and counseling strongly preferred.4. Supervisory experience preferred.5. Must be computer-literate, possess effective verbal and written communication skills, and theability and willingness to function as an active team member.6. Physical capacity to perform lifting (minimum of 50 lbs.) and assume body positions relatedto routines of light industrial and personal care tasks (e.g. pushing, pulling, bending, twisting,reaching, crouching, stooping, kneeling).7. Reliable transportation and a valid Ohio driver’s license with no more than three (3) movingviolations.RESPONSIBILITIES:1. Gather work, social, community living and behavioral data through observation, written andverbal input from the client and relevant sources to assist in the development of theIndividual Service Plan (ISP).2. Coordinate data, observations and client’s expression of needs and wants to develop anIndividual Service Plan that includes long- and short-term habilitation goals and objectives.Conduct annual ISP meetings in cooperation with the Adult Day Services Manager,according to agency procedures.3. Directly observe, assess and work with clients assigned to caseload.4. Serve as an advocate on behalf of clients assigned to caseload as well as all UCP clients. Actas lead in resolving issues and crises related to optimal program participation.5. Identify needs and initiate appropriate referrals for ancillary services as needed.


6. Provide line of communication regarding participants to VS staff, ancillary services staff,residential providers and/or families to ensure consistency of overall programming concerns.7. Document services provided in compliance with agency policies and procedures, Medicaidand CARF regulations and funding requirements--i.e. Individual Service Plans, case notes,referrals, incident reports, data sheets, etc. Review monthly data sheets for accuracy andcompleteness. Submit for billing in a timely fashion.8. Supervise ADS Assistants; assign work tasks and respond to problems, complaints andgrievances. Complete introductory and annual performance evaluations for staff. Underguidance of the ADS Program Manager, devise methods of staff development and growth.Ensure cross-training and rotational assignments to familiarize staff with the needs of allclients. Complete staff timesheets.9. Participate in the interview and selection process for new staff. Provide orientation toVocational Services program and day services setting for new staff.10. Ensure that supply and service requests are filled in a timely manner and that programmaterials and equipment are in good repair and updated as needed.11. Perform duties of ADS Assistants and assist with various activities when needed (e.g. ADL,habilitation programming, bus loading/unloading, etc.).12. Attend all related agency meetings, Vocational Services staff meetings, special team orcommittee meetings as required and as requested.13. Perform other reasonably related job duties or assignments as requested.
