Healthcare in Nigeria • All three tiers of government — Federal, State and Local —share responsibilities for providing health services in Nigeria. • All the three tiers of government also participate in the management of primary healthcare, resulting sometimes in duplication and overlap of responsibilities, conflict and waste. Disease Burden Infrastructure Human Resource for Health Nigeria is one of the developing countries faced with the “double burden” of persisting high prevalence of communicable diseases and rising prevalence of non-communicable diseases. Key health indicators such as maternal and infant mortality are worse than the Sub-Saharan African average. The highest burdens of morbidity and mortality are from Maternal and Child Health, HIV/AIDS, Malaria, Tuberculosis and Non-Communicable Diseases. Under investment in health systems over the years has resulted in poor infrastructure, inadequate modern equipment and technology and an inadequate number and mix of healthcare workers. The resultant effect is the absence of key specialist services, poor quality of care, loss of confidence in the healthcare system and increase in medical tourism. Nigeria has approximately 10,600 registered primary healthcare centers, 1,000 secondary healthcare centers and 50 tertiary centers. Nigeria has one of the largest stocks of (HRH) in Africa but the densities of nurses, midwives and doctors are still too low (1.95 per 1,000) to effectively deliver essential health services.
