YOUR MEDICAL PLAN OPTIONS
Your medical plan options are offered through BCBSIL or Kaiser (California, Colorado, D.C., Maryland and Northern Virginia residents
only). When you enroll in one of the following plans, you will be required to select which carrier you want to administer your plan.
Medical coverage offers valuable benefits to help you stay healthy and pay for care if you or your covered family members
become sick or injured.
$400 and $900 Deductible Plans with Copays
For the $400 Deductible Plan with Copays and $900 Deductible Plan with Copays:
• You pay a portion of the covered expenses before the plan pays for certain benefits; this amount is called the deductible.
• You’ll pay a copay for certain services.
• After the deductible is paid, you’ll pay a percentage of your covered medical expenses; this amount is called your coinsurance.
• If your share of medical expenses reaches an amount called the out-of-pocket maximum, you won’t have to pay anything for the
rest of the calendar year.

$1,850, $2,850 and $6,550 Deductible Plans with HSA
The $1,850 Deductible Plan, $2,850 Deductible Plan and $6,550 Deductible Plan are designed to encourage you to know your
treatment options and the cost implications of your choices.
• You have a higher deductible, but the tax-free Health Savings Account (HSA) and your lower monthly contributions can help
you cover out-of-pocket costs.
• When you enroll in the $1,850 Deductible Plan, $2,850 Deductible Plan or $6,550 Deductible Plan, you will also automatically be
enrolled in Critical Illness insurance at no cost to you.
