Dental
The Victory Benefit and Discount Card enables the card holder to save their hard earned dollars on
all their dental care needs. No discount will be given if a person is referred to a specialist.
This program is not only designed to save you money on routine procedures, such as cleaning,
fillings, x-rays, but to also include major procedures such as root canals, dentures, crowns, bridges,
oral surgery etc, when performed by a general dentist.
Depending on the plan you choose this card will save you anywhere from 10% to 60% on your
dental needs.
Choose a dentist from the “Provider list” enclosed for your dental procedures.
Just show your card when you get to the dentist’s office and start saving immediately.
*Important: The fee you pay has already been discounted – pay directly to the dental office.
*Important: If 48 hrs notice has not been given to cancel or change an appointment a $50.00
charge will be applied.

Invisible Aligners
The following is a list of the most common dental procedures:
Adjunctive Services
Adjunctive services are additional therapeutic services rendered including prevention of
pain and/or treatment for relief.
Amalgam
An alloy made up of many metals, in which mercury is one of them. It is also known as a
silver filling. It is primarily used on the back teeth (molars).
Bridgework
A fixed prosthetic device consisting of artificial teeth (pontics). It can replace one or more
missing teeth, and can not be removed from the mouth. The pontics are supported by
attaching them to abutment teeth (the teeth beside the pontic).
Composite
A man-made resin that is used for coloured restorations, primarily on the front teeth.
Crowns
When a filling is not possible a crown is fabricated. A crown is an artificial restoration that
covers the entire surface of the tooth above the gum line.
Denture Repair
After a certain period of time, a denture goes through normal wear and tear and may need
to be repaired. This simple repair may be repaired by the dentist in the office, but if it is a
more difficult repair that is required, it must be sent to a laboratory.
Denture
A removable prosthesis used to replace all or some missing teeth within the same arch.
Partial Denture
A prosthetic device containing artificial teeth. It is supported by its attachment to natural
teeth with special retainers fabricated by a laboratory. This type of denture can be worn on
upper and lower arches, and can be removed for cleaning etc.
Full Denture
An impression is takes by the dentist, of the dental arch that is missing teeth. The denture
is fabricated by the laboratory. It can be removed for cleaning or at bedtime etc. This type
of denture can be made for both upper and lower arches.
Diagnostic Services
Diagnostic services are necessary procedures that are performed to recognize and identify
any condition that may be abnormal, and their causes. These diagnostic procedure may
include one or more of the following: examinations, x-rays, clinical and laboratory tests.
Endodontic Services
Services performed to diagnose and treat diseases that occur within the pulp and tissues
surrounding the tooth root. Common procedures include root canal and periapical surgery.
Examinations
Initial Examination
This is performed on a new patient. A complete medical and personal history is recorded,
and then a full diagnostic examination of the teeth, gums and surrounding tissue is
achieved.
Recall Examination
A re-examination of the gums, teeth and tissue, that occurs on a regular interval basis. Its
purpose is to maintain a level of dental health.
Orthodontics
The specialty concerned with the detection, prevention and correction of improper
positioning of the teeth in relationship to the jaws.
Periodontal Services
The diagnosis and treatment of the tissues and bone, which surround and support the teeth.
Pit and Fissure Sealants
A clear plastic material placed in deep grooves and crevices found on the biting surfaces
of the back teeth. This protects the grooves from trapping plaque or other bacteria that
may cause decay.
Preventative Services
Services that are performed to help avoid, seize, or decrease any undesirable conditions,
which may cause diseases in the oral cavity. These preventative services are home care
instructions, scaling, sealants, polishing, and fluoride treatments.
Rebase
Rebasing is to replace the entire denture’s base material, on an existing prosthesis.
Reline
Relining is to resurface the tissue side or side that fits next to the gum of a partial or full
denture, so it fits more accurately.
Restoration
A restoration is the use of a dental material to replace the diseased or lost portion of a
tooth, to replace its function.
Root Planning
Root planning is the removal of plaque and other toxins from the root of a tooth, resulting
in a smooth surface by using specific instruments.
Scaling
Scaling is the removal of plaque, tartar, and stains from the tooth above and below the
gum line by using specific instruments.
Space Maintainers
An appliance that is used after the loss of a primary tooth to maintain the space in the arch
until the permanent tooth erupts.
Surgical Services
A surgical service is the diagnosis and surgical management of oral diseases, injuries, or
certain defects of the jaws. The surgical services include extraction (removal) of teeth.

How to use the Vision Benefit Discount
1. Call the Optical provider(s) on the provider list located in your booklet
2. Inform the receptionist that you are a “Victory member”
3. NOTE: Discounts DO NOT apply to items that are on sale
4. Pay directly to the receptionist, submit the vision claim to your group insurance
company(if applicable)
5. You will be responsible for any amount not covered by your insurance company

Group Insurance Carrier And/Or are covered under the Victory Benefit and Discount

How to use the discounted feature
1. Call the Paramedical service provider listed in your package
2. Give the receptionist your membership number, and inform them that you are a

reimbursement (if applicable)
Important: By using this added discount feature, it will enable you and your employer to
reduce Paramedical claims.

WHAT TO EXPECT FROM YOUR PLAN
S.I.L.P. provincially-based lawyer will give you preliminary
telephone legal guidance and information with regards to
your legal matter, in order to achieve understanding of your
situation and your options going forward. If you have a
legal problem, the lawyer will give you information and guidance
as to your rights and how best to deal with your issue.
NOTE: Your Plan provides you with unlimited telephone
preliminary legal guidance calls. However, in order to maintain
levels of services for all members, you may make only one call
on each matter, as only one should be necessary.
If you decide to retain a lawyer to further assist you, you’ll receive a
direct referral to one of S.I.L.P.’s network of experienced, community
based lawyers for personal service at preferred members’ rates.
Our referral specialist will select the most appropriate lawyer for
your legal matter.
USING REFERRAL SERVICES
If you wish to retain a lawyer, simply:
Call the number on the front of your card, during business hours
Choose the option for a referral to a lawyer.
Give the S.I.L.P. intake person your name and membership number.
Once the referral has been arranged, the referral specialist will
contact you with the referral lawyers’ contact information and you
may schedule an appointment at your convenience.
USING TELEPHONE GUIDANCE SERVICES
When unexpected issues arise, preliminary telephone legal guidance
can help you understand your rights and options.
With your membership number handy, call the telephone number on
the front of you membership card during business hours.
There is a short one-time card activation process if you have not
already activated your membership.
Identify yourself as a S.I.L.P. member. Your immediate family members
living with you may also use your membership.
We do our best to have a lawyer available with expertise in your area
of need. If you have to leave a message, your call will be returned
as soon as possible and not later than with 48 hours during the
regular business week.
IMPORTANT
THINGS TO KEEP
IN MIND:
1. Member’s fixed fees and hourly rates do not include disbursements
(out-of-pocket expenses incurred by your lawyer on your behalf), or taxes
2. Lawyers customarily require retainer before providing services to clients.
This is paid to the lawyer o“n account” when service begins and the full
account is credited against the total bill after service is provided. Please
discuss with your lawyer the potential requirement for a retainer and the
amount, before requesting service.
3. Members may not use the plan against a plan sponsor (any organization
through which you received S.I.L.P. or for collective agreement matters.
4. Neither S.I.P. nor a plan sponsor is responsible for any acts or
omissions on the part of any legal service provider in our legal

Premiums and level of discounts are subject to change. The Victory Benefit
Program makes every effort to keep the fees to a minimum for our members. The
terms & conditions of this discount program may be changed at anytime without
prior notice.
This discount program may be cancelled by either party (Victory Benefit
plan/Secure It Insurance Services or the subscriber) by submitting 30 days
written notice to the other party.
The service providers are all licenced professionals, authorized to practice their
profession in the province of Ontario. The service providers are subject to
change. The Victory Benefit and Discount Card, has the right to add or delete
service providers at any time.
The Victory Benefit and Discount card and Secure It Insurance Services will not
be responsible for the availability, quality or results of any treatment, care,
supplies, or services provided by the service providers listed in the provider list.
