We usually don’t give our spines much thought,<br />

that is, unless this critical part of our body<br />

starts grousing back at us. Only when the back pain<br />

begins do we then look for places to comfortably<br />

THE PLAGUE OF<br />

BACK PAIN:<br />

PREVENT IT BEFORE IT STRIKES<br />

According to reports from the World<br />

<strong>Health</strong> Organization, more people in<br />

the United States are disabled from<br />

work as a result of musculoskeletal<br />

disorders — especially back pain —<br />

than from any other group of diseases.<br />

When back pain strikes, sufferers may reach<br />

for over-the-counter drugs, grab heating pads,<br />

take to bed or head to the doctor. Once there,<br />

some request surgery, thinking it will provide immediate<br />

relief.<br />

Not so fast, said Dr. M. Gordon Whitbeck Jr.,<br />

a fellowship trained orthopaedic spine surgeon<br />

with Rochester (N.Y.) Regional <strong>Health</strong>’s Whitbeck<br />

Spinal Associates.<br />

“The chance for a good to excellent result (after<br />

surgery) for low back pain is 65 percent to 75<br />

percent,” he explained, adding that you probably<br />

wouldn’t buy a new car if there were a 30 percent<br />

chance that it wouldn’t run.<br />

The best thing to do? Avoid it in the first place<br />

with preventive measures. One place to start is<br />

quitting smoking.<br />

“Studies have found that smoking is deleterious<br />

to the health of intervertebral disks,” Whitbeck<br />

said. “Nicotine and other substances lead to<br />

greater disk degeneration.”<br />

Being sedentary also promotes low back pain,<br />

explained Elizabeth Wetmore, a coordinator of<br />

the Spine Rehabilitation Program at UR Medicine’s<br />

Sports and Spine Rehabilitation.<br />

“Staying active is so important in a society<br />

where we sit too much and sit with poor posture,”<br />

Wetmore said.<br />

Part of the problem is sitting and slouching.<br />

“Most people know they slouch,” she said.<br />

“We spend weeks or months slouched over a<br />

computer or a smartphone.”<br />

“It’s a huge problem,” Wetmore said. “People<br />

come in experiencing chronic neck pain. They<br />

don’t realize bending over a smartphone for long<br />

periods of time every day can be the equivalent<br />

of some 50 pounds of stress (on the neck).”<br />

An alternative to “cellphone hunch posture,”<br />

she said, is to “hold the phone up level to your<br />

face while your thumbs are<br />

clicking over the<br />

keys.”<br />

Maintaining a good BMI ratio (body mass<br />

index, the measure of body fat based<br />

on height and weight) is crucial to a<br />

healthy back. Without some exercise, e,<br />

weight inevitably will increase.<br />

Morbid obesity’s effect on<br />

the<br />

entire body — hips and knees<br />

— is obvious, but a BMI above<br />

30 affects the spine and can<br />

increase low back pain.<br />

Even brief walks are<br />

helpful, Wetmore said,<br />

“but depending on what<br />

people can do and their<br />

flexibility, doing planks<br />

and bridges are important<br />

also.”<br />

Most people have<br />

heard the warning about<br />

bending at the knees<br />

not the waist to pick<br />

up something, but few<br />

make the connection<br />

between a pattern of<br />

prolonged sitting and low<br />

back pain.<br />

Whitbeck said the solution<br />

can be as simple as<br />

“getting up to stand or walk<br />

around a bit every hour.”<br />

When it comes to exercise,<br />

what’s good for your<br />

overall health is good for your<br />

back, Whitbeck said: low-impact<br />

aerobic exercise — walking, swimming<br />

or cycling; resistant t or bearing activity like lifting weights; and<br />

weight-<br />

strengthening the abdominal muscles<br />

that support the back.<br />

“Pilates, crunches, planks are good. Yoga<br />

is also good for developing flexibility,” Whitbeck<br />

said.<br />

But when people do experience back pain, they<br />

sometimes opt for braces to ease the pain, or manipulation<br />

or electrical stimulation in a doctor’s office.<br />

Wetmore is not a proponent pon<br />

of braces. “If people use braces, they’re<br />

not requiring their muscles to work. The only exception is for those people identified as ‘industrial<br />

athletes’—people whose work requires lifting or restocking heavy items all day or who have a high risk<br />

history of injuries.”<br />

Manipulation, similar to treatments used in chiropractic offices, and electrical stimulation are also<br />

options at the Spine Rehabilitation Program where she works.<br />

“Nerves can’t carry two impulses, so the electrical stimulation will override the pain,” Wetmore<br />

explains.<br />

Local health care practitioners are readying for more back pain complaints as people do more activities<br />

with the onset of summer.<br />

In the end, when everything in your painful back says “Don’t move a muscle,” remember that it’s<br />

important to set yourself in motion. It’s just like the philosopher William James suggests, “Nothing<br />

changes until something moves,” and the “something” has to be you.<br />

BUT IT MAY NOT BE THE HEALTHIEST STATE OF MIND.<br />

When Ashley Hartkorn was trying<br />

to lose weight a few years<br />

ago, she followed a plan that<br />

advocated the “cheat day”<br />

philosophy. Hartkorn would<br />

adhere to her diet six days a week, but on the<br />

seventh day, she would indulge in all the foods<br />

she had been craving the days before.<br />

“What I learned is that I would take them to<br />

the extreme,” Hartkorn, of Des Moines, Iowa,<br />

said. “Every single Sunday, my cheat day, I<br />

would eat a whole medium pizza, a whole box<br />

of breadsticks and two cans of soda. And that<br />

was just one of my meals on Sunday.”<br />

Despite her once-a-week binges, Hartkorn did<br />

lose weight, but the changes didn’t stick.<br />

“My habits weren’t healthy,” she said. “I’ve<br />

learned since then that if I don’t deprive myself<br />

during the week, I don’t feel the need to binge;<br />

I can feel good about the choices I’m making<br />

instead of feeling like the food is in control.”<br />

Hartkorn isn’t the only dieter to embrace the<br />

“cheat day” philosophy — a newly published<br />

book by “Biggest Loser” trainer Dolvett Quince<br />

bases its premise entirely on the method.<br />

The book, “The 3-1-2-1 Diet — Eat and<br />

Cheat Your Way to Weight Loss,” advocates<br />

that dieters eat “cleanly,” adhering to a<br />

1,500-calorie-a-day diet, for three days,<br />

then follow those days with one “cheat,” or<br />

indulgence, day. After that, he or she follows<br />

with two more “clean” days before ending the<br />

week with one more “cheat” day.<br />

The plan works, Quince maintains, in part<br />

because the body’s metabolism revs up when it<br />

has extra food to digest.<br />

“Indulging a little during the week isn’t<br />

going to ruin your diet; it will actually make<br />

your metabolism work better,” Quince writes.<br />

“Alternating between ‘clean’ and ‘cheat’ days<br />

will confuse your body into optimal fat-burning<br />

performance.”<br />

It sounds too good to be true — so, is it?<br />

At least one Iowa nutritionist isn’t a fan of the<br />

method.<br />

“I don’t like the terms ‘cheat days’ and ‘clean<br />

days’ — I advocate more of a well-balanced<br />

“I also don’t like the fact that consumers are<br />

bombarded with so much new information; it’s<br />

hard for them to figure out what’s beneficial<br />

and what’s not. People who want to lose weight<br />

are left wondering, ‘What am I supposed to<br />

believe, and what am I supposed to do?’ In my<br />

opinion, it helps to stick to a few key things to<br />

make the process of weight loss less stressful.”<br />

Paulson suggests rather than “diet” for a set<br />

number of days and throw caution to the wind<br />

the rest of the time, people who are trying to<br />

lose weight are better off focusing on whole<br />

foods with a nutritional balance as well as<br />

taking a hard look at their relationships with<br />

food.<br />

“When we work with people, we look at<br />

many things — total calories coming in, physical<br />

activity, how the person handles stress, what<br />

his or her relationship is with food in general,”<br />

Paulson said. “So many things influence how we<br />

deal with food, and that needs to be looked at<br />

as a whole package.”<br />

Paulson said dieters need to understand that<br />

when it comes to weight loss, there’s no such<br />

thing as a quick fix.<br />

“It’s all about getting a balance of nutrition<br />

while enjoying indulgences in moderation,”<br />

she said. “When you do that, there’s no need<br />

for ‘cheating’ because you’re not depriving<br />

yourself. Focus on eating things that are good<br />

for you, and eating in a way that you can<br />

maintain over time, not eating in a way that<br />

you’re going to lose weight very quickly, then<br />

turn around and put it back on.”<br />

An “80-20” or “90-10” way of eating is a<br />

sensible way to lose weight over time, Paulson<br />

said.<br />

“Make 80 percent or 90 percent of your<br />

choices healthier choices,” she said. “Then the<br />

other 10 to 20 percent can be indulgences —<br />

but keep in mind that an indulgence can be<br />

relatively healthy, like a piece of dark chocolate.<br />

It doesn’t have to be chocolate cake every day.”<br />

She said in terms of the big weight-loss<br />

picture, “cheating” amounts to “trying to cheat<br />

the system, and there’s no real way to do that.”<br />

“It all comes down to knowing yourself,<br />

making good choices, and practicing portion<br />

control,” Paulson said. “If there were a magic<br />

way to lose weight that didn’t require any<br />

lifestyle changes, everyone would be doing it!”<br />

to notice small changes in their vision, which<br />

can impact their daily lives and job performance.<br />

Whether having difficulty reading a book or<br />

working on a computer screen, such changes can<br />

be frustrating, but they can often be addressed<br />

by an eye care professional.<br />

A comprehensive dilated eye exam is the best<br />

way to detect diseases and conditions that can<br />

cause vision loss and blindness. That’s because<br />

many have no symptoms in their early stages.<br />

According to the National Eye Institute,<br />

all adults aged 60 and older should have a<br />

comprehensive dilated eye exam, with the<br />

exception of African Americans, who are advised<br />

to get the exams starting at age 40 due to a<br />

higher risk of developing glaucoma at an earlier<br />

age. Even if<br />

you haven’t experienced any issues with your<br />

sight, a dilated exam can detect serious eye<br />

diseases and conditions such as age-related<br />

macular degeneration, cataract, diabetic eye<br />

disease, and glaucoma.<br />

During a dilated eye exam, your eye care<br />

professional places drops in your eyes to dilate,<br />

or widen, the pupil. This lets more light enter<br />

the eye, similar to the way an open door lets<br />

more light into a dark room and allows your eye<br />

care professional to get a good look at the back<br />

of your eyes to examine them for any signs of<br />

damage or disease.<br />

Regular eye exams go a long way in helping<br />

you see well for a lifetime. But there is more you<br />

can do. Experts at the National Eye Institute<br />

recommend following these additional steps to<br />

protect your vision.<br />

1. Live a healthy lifestyle. Living an overall<br />

healthy life is good for your eyes. This includes:<br />

Maintaining a healthy weight. Being<br />

overweight or obese increases your risk of<br />

developing diabetes and other systemic<br />

conditions, which can lead to vision loss<br />

from diabetic eye disease or glaucoma.<br />

Eating healthy foods. You’ve heard carrots<br />

are good for your eyes, but eating a diet<br />

rich in fruits and vegetables, particularly<br />

dark leafy greens such as spinach, kale, or<br />

collard greens is important for keeping your<br />

eyes healthy, too. Research has also shown<br />

there are eye health benefits from eating<br />

fish high in omega-3 fatty acids, such as<br />

salmon, tuna, and halibut.<br />

Not smoking. Smoking is as bad for<br />

your eyes as it is for the rest of your<br />

body. Research has linked smoking to an<br />

increased risk of developing age-related<br />

macular degen eration, cataract and optic<br />

nerve damage, all of which<br />

can lead to blindness.<br />

2. Know your family history. Talk to your<br />

family members about their eye health<br />

history. It’s important to know if anyone<br />

has been diagnosed with an eye disease<br />

or condition since many are hereditary.<br />

This will help to determine if you are at<br />

higher risk for developing an eye disease or<br />

condition.<br />

3. Use protective eyewear. Protect your<br />

eyes when playing sports, working with<br />

hazardous materials, or doing chores like<br />

mowing the lawn. Protective eyewear<br />

includes safety glasses and goggles, safety<br />

shields and eye guards specially designed<br />

to provide the correct protection for a<br />

certain activity. Most protective eyewear<br />

lenses are made of polycarbonate, which is<br />

10 times stronger than other plastics. Many<br />

eye care providers sell protective eyewear,<br />

as do some sporting goods stores.<br />

4. Wear sunglasses. Sunglasses are a great<br />

fashion accessory, but their most important<br />

job is to protect your eyes from the<br />

sun’s ultraviolet rays. When purchasing<br />

sunglasses, look for ones that block out<br />

99 to 100 percent of both UV-A and UV-B<br />

radiation. Exposure to excess sunlight<br />

can increase your risk of cataract and<br />

age-related macular degeneration, tissue<br />

growth on the white part of eye that can<br />

cause discomfort<br />

and blurred vision.<br />

Common Eye Diseases and<br />

Conditions<br />

Just as the rest of your body ages, so do the<br />

eyes. The following are common diseases and<br />

conditions associated with aging that can be<br />

detected with proper monitoring from an eye<br />

care professional:<br />

Cataract. People who experience this<br />

clouding of the lens in the eye often report<br />

increased glare and fading of colors.<br />

Diabetic eye disease. A complication of<br />

diabetes and a leading cause of blindness, its<br />

most common form is diabetic retinopathy,<br />

which happens when the disease damages<br />

small blood vessels inside the retina.<br />

Dry eye. Dry eye can feel like stinging or<br />

burning, and can lead to blurred vision or<br />

even vision loss if left untreated.<br />

Glaucoma. Glaucoma is a group of diseases<br />

that can damage the eye’s optic nerve and<br />

result in vision loss and blindness. Openangle<br />

glaucoma is the most common form of<br />

the disease.<br />

Age-related Macular Degeneration. Agerelated<br />

macular degeneration (AMD) is a<br />

disease that blurs the sharp, central vision<br />

you need for “straight-ahead” activities such<br />

as reading, sewing, and driving. AMD affects<br />

the macula, the part of the eye that allows<br />

you to see fine detail.<br />

of a child’s home. In fact, in 23 percent of the<br />

cases in which a child under age 5 mistakenly<br />

ingested an oral prescription drug, the medication<br />

belonged to someone who did not live with the<br />

child, such as an elderly relative or grandparent.<br />

Medications can be poisonous, too<br />

Parents know to keep household cleaners and<br />

other chemicals out of the reach of children, but<br />

should also be cautious about prescriptions, overthe-counter<br />

medications and vitamins.<br />

There is no better time than now to learn more<br />

about preventing accidental and unintentional<br />

poisonings. Here are some important tips you can<br />

share with family and friends.<br />

Be cautious of colors: Medications are colorful<br />

and attractive to children and can be mistaken<br />

for candy. For example, Tums look like SweeTarts,<br />

and Advil and Ecotrin resemble Skittles or M&M’s.<br />

Parents should not encourage children to take<br />

their medication by comparing it to candy, as this<br />

may lead to improper use.<br />

Lock it up: Don’t leave your next dose out on<br />

the counter where a child can reach it. Tightly<br />

secure caps and lock up all medications and<br />

vitamins in a cool, dry place. Keep medications<br />

in their original labeled containers so if there is<br />

an emergency, you can tell medical personnel<br />

exactly what the child ingested.<br />

Do not share: Be sure to remind children that<br />

they should never share their medications. When<br />

playing “doctor,” friends and younger siblings of<br />

those taking a medication are often the recipients,<br />

leading to an accidental poisoning. This is an<br />

important message for teens, too, as this behavior<br />

often leads to prescription-drug abuse.<br />

How to respond to accidental<br />

poisonings<br />

If you suspect a child has ingested a potentially<br />

poisonous substance, here is what you should do:<br />

Know your numbers: If the child has collapsed<br />

or is not breathing, dial 911 immediately. If the<br />

child is awake and alert, call the poison hotline<br />

at 800.222.1222 and follow the operator’s<br />

instructions. If possible, have available the victim’s<br />

age and weight, the container or bottle of the<br />

poison, the time of the poison exposure and the<br />

address where the poisoning occurred.<br />

Know the signs: Reactions to ingested<br />

medications or household products may vary.<br />

Look for signs such as vomiting, drowsiness and<br />

any residue or odor on the child’s mouth and<br />

teeth. But know that some products cause no<br />

immediate symptoms, so if you suspect that<br />

your child has ingested a potentially hazardous<br />

substance, call the poison hotline immediately.<br />

Keep calm: It’s important to remain calm so<br />

you can effectively communicate with emergency<br />

personnel. If the child ingested medication, do<br />

not give anything to the child by mouth until<br />

advised by the poison control center. If chemicals<br />

or household products have been swallowed, call<br />

the poison control center immediately or follow<br />

the first aid instructions on the label.<br />

Medications can keep you healthy but can<br />

be extremely dangerous if taken by the wrong<br />

person or in the wrong amount. Add in a child’s<br />

insatiable curiosity, and you have the ingredients<br />

for a very serious and dangerous situation.<br />

Prevention has become the focus, and<br />

many aging Americans are turning to<br />

time-tested methods for keeping their<br />

bodies and minds healthy so they can live<br />

longer, higher-quality lives.<br />

Kristen Johnson, certified personal<br />

trainer, registered dietician and nutrition<br />

expert at ontargetliving.com points out<br />

five time-tested strategies for aging well:<br />

Daily exercise<br />

“Daily movement is the real fountain of<br />

youth. It keeps us healthy from the inside<br />

out,” says Johnson.<br />

She notes that quality over quantity is<br />

what really matters.<br />

“When it comes to improving overall<br />

fitness, high-intensity exercise for a<br />

short amount of time may be much<br />

more beneficial than low intensity for<br />

a long amount of time,” Johnson says.<br />

“Research suggests that fat-burning<br />

hormones like human growth hormones<br />

and testosterone are stimulated by<br />

high-intensity exercise, while fat-storing<br />

hormones like cortisol may be lowered.<br />

Try increasing the intensity and frequency<br />

of your exercise, while decreasing the<br />

time spent.”<br />

Superfoods<br />

The foods you eat influence how you<br />

look and feel, from glowing and confident<br />

to lethargic and sick. Selecting foods<br />

that people have eaten historically as<br />

nutritional powerhouses can help boost<br />

“Superfoods are nutrient-rich foods, such as<br />

fruits, vegetables, ancient grains, healthy fats<br />

and lean proteins,” says Johnson. “These foods<br />

naturally contain high amounts of antioxidants,<br />

vitamins and minerals, which all contribute to<br />

healthy aging.”<br />

A few to focus on:<br />

Carrots, squash and sweet potatoes are<br />

extremely beneficial for eye and skin health,<br />

thanks to high levels of beta-carotene, a<br />

type of vitamin A.<br />

Any brightly colored fruits and vegetables<br />

will have an abundant amount of<br />

antioxidants, and these help prevent<br />

oxidation and cell damage. Examples:<br />

raspberries, kale and cabbage.<br />

Carbohydrates like healthy grains, beans<br />

and potatoes help you produce serotonin,<br />

a calming and satiety hormone that helps<br />

fight stress and anxiety’s negative effects.<br />

Nutrients<br />

Supplements help fill nutritional gaps,<br />

especially as the aging body requires greater<br />

amounts of certain vitamins and minerals.<br />

Johnson points out the importance of omega-3s<br />

for aging well.<br />

“Omega-3 fats are essential for getting you<br />

healthy from the inside out, all while helping<br />

improve hormonal balance, brain health, weight<br />

loss and metabolism,” she says. “Omega-3 fats<br />

are also extremely helpful for healthy skin, hair<br />

and nails.”<br />

Her favorite? Nordic Naturals Cod Liver Oil. “This<br />

contains EPA and DHA, both of which contribute<br />

to a healthy heart and brain,” she says. “Cod liver<br />

oil also helps improve cellular function, energy and<br />

mood. Did you know cod liver oil can actually taste<br />

Sleep<br />

“Chronic lack of sleep is one of the fastest<br />

ways to age the human body,” Johnson says.<br />

“Lack of sleep can have a huge impact on the<br />

appearance of skin, causing fine lines, wrinkles<br />

and dark under-eye circles. Not getting enough<br />

sleep can also cause your body to release a stress<br />

hormone called cortisol.”<br />

She notes that adequate sleep can positively<br />

influence cognitive ability, mood, weight loss and<br />

skin rejuvenation, so it should be a top priority for<br />

an aging-well routine. While the right amount of<br />

sleep will vary between individuals, the goal for<br />

most adults is around 7 to 8 hours a night.<br />

Social activity<br />

Human interaction can decrease as people<br />

age, but it’s more important than ever to form<br />

and maintain bonds with others. Participating in<br />

social activity is a fun way to enjoy life and reap<br />

real health benefits.<br />

“The American Medical Association has noted<br />

that stress is the basic cause for more than 60<br />

percent of all human illnesses and diseases,”<br />

says Johnson. ‘”When you are socially active and<br />

surround yourself with people you enjoy, you may<br />

be less likely to feel lonely, unhappy, or unfulfilled,<br />

all of which can cause unwanted stress.”<br />

Finally, there’s no need to become<br />

overwhelmed; start an aging-well routine by<br />

taking one small step and building healthy habits<br />

over time. This is what will lead to long-term<br />

success.<br />

“Remember that it’s never too late to start<br />

living a healthy and happy life,” Johnson says.<br />

“Give yourself more reasons to smile and laugh!<br />
