Abstract

Background

Recentsurveysindifferenthospitalshaveshownthatthelackofnursestaffresultstoa

decreaseinthesatisfactionandsafetyamongpatientsduringtheirstayinthehospital.

Itisalsofoundtofurtherincreasetherisksinvolvedinpatientinjuries.Thiscapstone

paperdiscussestheseveralreasonsforthisshortageinnursingstaff,aswelasthe

differentwaysinwhichpatientsatisfactionandsafetycanbeimproved.

Objective

Toachieveabeterunderstandingofthenegativeimplicationsofinadequatestaffingor

nursesinafacility,thedevelopmentofasuccessfulplanwhichpromotessatisfaction

andsafetyamongpatients,andthecreationofasuccessfulplanwhenitcomesto

improvingstaffingamonghospitals.

Methodology

Methodsusedinthisstudyincludesevidence-basedpractice,literaturereviews,aswel

asclinicalexperience.Thelateralsoincludedinterviewswithpatients,nurses,aswel

asnursemanagersabouttheirviewsonsatisfactionandsafetyinconnectionwith

staffing.

Conclusions

Mandatedratiobetweennurseandpatientandstaffingaccordingtopatientacuity

levelshavebothenhancedpatientcare.Theimplementationofastaffingprogram that

isbasedonthelevelsofpatientacuitywouldserveasamoreviablesolutioncompared

tomandatedratiobetweennursesandpatients.Withtheshortageofnursesentering

thepicture,hospitalsneedtofindawaytomaintainstaffinginordertoimprovecare

amongpatients.


Introduction

Asofthemoment,staffingisconsideredasahotissueamongnursesindifferent

hospitals.Thesefacilitiesarebeingfrequentlyunderstaffed.Itisveryimportanttofix

thisproblem becauseofitshugeimpactonthesatisfactionlevelsamongpatients,as

welastheirsafety.Needlesstosay,ahospitalcannotoperatewhenthepatientsno

longerwanttogotherebecausetheydonotreceivethelevelofcarethattheyexpect

anddeserve.Itisalsohighlydangeroustoputthelivesofpatientsatrisk(Department

ofLaborintheUS,2014).Theaim ofthispaperistoresearchthereasonsrelatedto

understaffinginhospitals,andtheirimpactonpatientcare.Solutions wil also be

presentedinordertohelpinfixingtheproblem,withimplementationsthatwilbedone

ontheunit.Thispaperwilalsolookatthecurentviewsofemployeesregarding

patientcare,anditsrelationtostaffing.Theprimaryhospitalinvolvedinthisstudyisthe

OhioHospital.

ReasonstoUnderstaffing

Oneofthereasonsofunderstaffingamonghospitalsisnursingshortage.However,it

hasbeendiscoveredthattherearemoreregisterednursescomparedtonursingjobs

intheUSA.Howhasthisresultedtoanunderstaffing?Intheyear2007alone,about

49%ofhospitalfacilitieshavereportedthattheyarehavingdifficultieswhenitcomes

torecruitingnewregisterednursescomparedtotheirrecruitmentprocessesin2006

(Malone&Hughes,201,p.340).Atthesametime,therearehighturnoverrateson

medical-surgicalunits.Adifferentreasonisthatseveraloftheseregisterednursesare

nothiredonlyforinpatientunitsamonghospitals.Assuch,fastergrowthisalso

expectedeveninnon-hospitalfacilities,includingphysician’shomehealthcare,offices,

aswelasoutpatientunits.Thisisbecauseofthewayinwhichthehealthcareindustry

isevolving.Processesthatusedtobecompletedinhospitalsarenowdoneinphysician

officesandoutpatientunits.Atthesametime,elderlypopulationarenow gearing

towardsreceivingcareintheirhomes.Thiscanfurtherresulttoadecreaseinthe

availabilityofregisterednurseswhoaresearchingforworkinahospitalseting.


Anotherreasonishassomethingtodowithfinances.Mosthospitalorganizationsare

folowingastrictbudget.Theincreaseinfederalandbudgetdeficitsinthestatehave

furtherresultedtoanincreaseinthepressureforalorganizationsinthehealthcare

industrytoreducecost.(Thompson,201).Forthisreason,differenthospitalsalsoneed

toimplementcostcutingmeasures.Cutingasmalpercentageonpersonnelbudget

canalreadyresulttohugesavingsonthepartofthefacility.This,however,hasnegative

impactsonpatientcare.
