Abstract

DiabetesisconsideredastheseventhleadingcausesoffatalityintheUSA.Thisis

despiteadvancementsinthemedicalfield(DiseaseControlCenter,2013).Management

andeducationaboutDiabetesisakeytocurtailingthespreadofthisilness.Asbaby

boomerscontinuetoageintheUS,diagnosesofDiabetescontinuetoincrease,

causingthisdiseasetoreachunprecedentedepidemicproportions(Shakar&Matel,

2015).A nursinghomeintheNew YorkCityperformedanassessmentin2014,

revealinganincreaseintheoveralreadmissionofresidentswhoaredealingwith

diabetes,aswelasrelatedcomplications.Thesefactsledtothequestionpresentedin

thisproject:Doesaneducationalprogram aboutdiabetesinasub-acutesetinglessen

the30-dayreadmissionratesofbabyboomersinthehospital?Theaim ofthis

evidence-basedprojectwastofocusontheimpactofadiabeteseducationalprogram

onreadmissionrates.Theendgoalofthisprojectistoenhancethemanagementof

caregiversandpatientsofthiscomplicatedilness.

Managementsofthisilnessalsoincludebeterexercise,eatinghabits,self-careand

educationofthediabeticpatientstothereductionintheadverseeffectsoftheilness.

Theresearcherutilizedapreandpostsurveydesignedinbeterunderstandingthe

knowledgeofpatientsandnursesregardingdiabetesbeforetheactualeducation

program.Thefindingsoutoftheassessmenthassuggestedthatdiabeticeducationfor

thepatientsmayimproveitsmanagementwhilereducingreadmissionrates.

TheDiseaseControlandPreventionCentershavereportedthattheoveralcost

involvedindiagnosingandtreatingdiabetescontinuestoriseyearafteryear.Type2

diabeteshappensinabout99%ofalthediabeticpatients.Mostbabyboomerswhoare

sufferingfrom thisdiseaseeventualydevelopType2diabetes,whichwaspreviously

referedtoasadultonsetdiabetes.Stewart,BlakeandAnderson(201)definedbaby

boomersasthosewhowerebornbetweentheyears1946and1964.Inthisagegroup,

Type2diabeteshappenswhenthebodynolongerproducesenoughamountofinsulin

withinthebodycelsorifthecelshavealreadybecomeinsulinresistant.

NatureoftheProject

Inordertofurtherunderstandthecurentknowledgeofpatientsandcaregivers

regardingmanagementofdiabetes,anursinghomeinNewYorkCitywassuccumbed

intoaneedsassessment.Theanalysisoftheassessmenthasshowedanincreasein

thereadmissionratesofthepatientsamongtheso-caledbabyboomersgenerationas

aresultofalackinknowledgeregardingproperdiabetesmanagement.Theuseofthe

PICOTframeworkisanidealmodeltouseinhighlightingtheelementsinvolvedinthe

clinicalquestion,whiledevelopingaspecificdiabeteseducationalprogram particularly

meantforbabyboomers.Thisframeworkhelpedinstructuringtheclinicalquestion

whichresultedtomoreusefulresults.

Limitations

Thesurveymethodusedinthisstudyhaselicitedvariousresponsescomingfrom

potentialparticipants.Thetruthfulnessandaccuracyoftheresponsesarenotableto

bemeasuredfuly.Ontopofthat,alimitedsampledatausingtheN=9formulationhas

dischargedpatientswhoparticipatedintheeducationalprogram.Thisposedfurther

chalengesandlimitations.Asaresult,thestudymaynotbeviewedassomething

generalizedwithotherhealthcarepractitioners.Thelimitationswerealsooffsetbya

numberofstrengthsthatareinherenttothemodelused,takingintoaccountthe

completesystem,aswelasthepatients,organizationandproviders.Despitealof

thesestrengths,themodelusedisnotwithoutlimitations,whichshouldalsobe

consideredasalimitationofitsown.Restrictionsalsoincludethecreationofan


Conclusions

Thiscapstoneprojectpresentedthattheimplementationofadiabeteseducation

program atnursinghomeshasreducedreadmissionrates,especialyamongbaby

boomers.Soastobecomeanexpertintheenvironmentofhealthcare,cliniciansand

nursesshouldtakeownershipoftheirresponsibilitiesbyenhancingtherequiredskils,

discovering and managing the responsescoming from the patients,aswelas

completingtheneededhealthtasks.Asaresult,ilnessesarereducedtoamanageable

levelwhenhealthcareworkersworkinpromotingactivitiesthatencourageschanging,

aswelasmaintainingbehaviorswhichleadtokeepinghealthychoices.
