What is Home Health?<br />

Home Health provides clinical support to<br />

patients by providing services to help them<br />

regain their independence through medical<br />

care in their familiar home environment. Think<br />

of home health care as an extension of the<br />

care you receive from your doctor’s office or<br />

hospital, provided at home.<br />

From medication management to treatments<br />

to therapy that helps you gain your strength<br />

back, we have the skilled clinical services your<br />

doctor says you need to get better – all in your<br />

What is Hospice?<br />

Hospice provides supportive care to<br />

terminally ill patients who usually have a life<br />

expectancy of six months or less. Hospice<br />

promotes palliative care, which means<br />

that efforts will be made to keep patients<br />

as comfortable as possible with primary<br />

emphasis on preventing, reducing and<br />

controlling pain. Hospice can also prevent<br />

unnecessary hospitalizations.<br />

While family members or other<br />

caregivers attend to the patient on a daily<br />

basis, members of the Hospice team make<br />

regular home visits to offer medical,<br />

emotional and spiritual support.<br />
