Reliance Standard—Life/AD&D and Disability Life/AD&D Insurance RCA provides eligible employees with Life/AD&D and Disability benefits through Reliance Standard at no cost to you. BASIC LIFE INSURANCE* Basic Life insurance pays a cash benefit to your chosen beneficiary in the event of your death. This benefit can be used to pay for final expenses, replace lost income, cover debts, pay your mortgage, fund a child’s education, and more. The amount of your benefit is $50,000. ACCIDENTAL DEATH & DISMEMBERMENT (AD&D)* AD&D insurance pays for a covered accident that results in loss of life, speech, hearing or sight, paralysis and other losses. The plan will pay a varied amount depending on the loss; this amount can vary from 50% to 100% of the Basic Life benefit. Beneficiary Reminder When you signed up for the company benefits, you likely named a beneficiary and promptly forgot about it. But it’s important to periodically update your beneficiaries to make sure the right people receive your assets. Whenever you experience a life change such as marriage, divorce, death of a spouse or child, birth of a child or grandchild or similar events that alters your family, you should review your beneficiary designations and make any necessary changes. You should also update your beneficiary listing when a beneficiary changes his or her name, such as an adult child getting married. Contact HR if you wish to update your beneficiary. *This policy contains limitations and exclusions that may affect benefits. Please see the certificate or ask your benefits administrator for information on Elimination Periods, Waiting Periods, and Pre-Existing Conditions limitation. Disability Insurance Disability insurance provides you with a cash benefit to help pay for everyday expenses if a covered disability takes you away from work for a period of time. All eligible full-time employees are provided with Short Term (STD) and Long Term Disability (LTD) insurance, paid for by RCA. SHORT TERM DISABILITY* Begin receiving benefits—after your claim is approved—on the 8 th day from the date you are unable to work due to an injury or illness. You will receive a weekly check for up to 12 weeks—as long as you are still unable to return to work due to a covered disability. The weekly check replaces 60% of your income, up to $2,000. LONG TERM DISABILITY* Begin receiving benefits—after your claim is approved—in as soon as 90 days. You will receive a monthly check for as long as you are still unable to work due to a covered disability—until you reach the Social Security Normal Retirement Age. The monthly check replaces 60% of your income, up to $10,000.
