ELIGIBILITY All active full-time employees regularly scheduled to work 30 hours or more per week are eligible for the benefits described after the required waiting period. ELIGIBLE DEPENDENTS Eligible dependents include: • Your legal spouse • Your biological, adopted, foster or step-children and legal dependents up to age 26 • Disabled children over the age 26 NEW HIRES New hires are eligible on the 1 st of the month following 30 consecutive days of employment. You will be notified by the ADP benefits team when it is time to enroll. You will have a limited time to log into the RCA HR Portal and complete your elections. Your elections will remain in effect for the remainder of the plan year (through December 31, 2018). Your next opportunity to change your elections will be during the 2018 open enrollment period for an effective date of January 1, 2019, unless you experience a qualifying life event. Some examples of qualifying life events include: • Marriage • Birth, adoption or placement for adoption • Divorce, legal separation or annulment • A change in your or your spouse’s employment status that results in a gain or loss of coverage • Loss of dependent status • You, your spouse or child gains or loses Medicare or Medicaid coverage • Receiving a Qualified Medical Child Support Order Qualified life event changes must be requested within 30 days of the event. Supporting documentation consistent with the qualifying event is required. If you do not provide notification within the required timeframe, you will not be able to make benefit changes until the next annual open enrollment period.
