That is why the company has decided to offer a Wellness Benefit. This program will focus on educating you about your health risk factors and provide you with resources and support to help you set goals to live a healthier life. In addition, as a New Hire, you will first be eligible to receive the incentive next year (2019) if you and your spouse (if applicable) participate in the screening. Here’s how it works: 1. You and your spouse (if applicable) go online to complete a short questionnaire about your lifestyle choices and schedule a 10-minute appointment for a health screening. 2. You participate in the screening which will include documenting your height and weight (BMI), taking your blood pressure and collecting a blood sample through a finger poke. A nurse will then review the results with you. This blood sample is taken to determine your glucose and cholesterol levels—two important measures that, if not within a normal range, can indicate serious health problems that may arise such as diabetes or heart disease. 3. Your Complete Health Risk Assessment Report will be mailed to your home 3-4 weeks following the screening. 4. The screening will be done at no cost to you. And your information is confidential. The company will not receive individual results. That’s it! Employees and Spouses (insured under the company medical plan) will be eligible for the Wellness Incentive deposited into your HSA account just for participating. Employees that do not have medical insurance with the company and decide to participate, will receive a $100 cash or gift card incentive. Wellness Benefit—What’s Next? Once you have your results, you’ll confirm that you are a healthy person or that you have some things to work on to live a healthier life. Regardless, throughout the year, the company will provide educational campaigns and activities focused on wellness that you’ll want to participate in. In future years: The results of your HRA will be scored. You may be in great health, extremely poor health or somewhere in between. If you have significant room for improvement, in future years in order to receive the incentive, you’ll need to have worked to make small improvements. You’ll also be offered a reasonable alternative to receive the incentive if you are struggling to improve, such as health coaching sessions or required attendance in an educational campaign. You don’t have to participate, but if you do, you can lower your health risk factors and live a healthier, happier, more vital life. -4-
