Welcome to Preferred Health Plan, Inc.!
Preferred Health Plan, Inc. is a preferred provider organization (PPO), developed to provide you and your
family with quality, cost-effective health care services through its network of health care providers. With
Preferred Health Plan’s network, you may see the physician of your choice. When you utilize the services
of a network provider, you maximize your benefits.
Using the Network
When making an appointment with a Preferred Health Plan network provider, always identify yourself as
a member of the network, and verify your doctor’s continuing participation in it. At the time of your
appointment, the physician’s office will verify your coverage by checking your identification card and/or
by calling Preferred Health Plan. The physician’s office will then collect any applicable office visit
copayment and will file your claim for you.
Using this Directory
This directory includes a list of all network providers. The first list is sorted alphabetically by
physician’s last name and includes ALL practice locations. The list reads left to right across the page.
The second list is sorted alphabetically by Practice Specialty, then alphabetically by physician last name
within each specialty.
This directory is subject to change, so always confirm with the doctor’s office that your doctor is currently
a network provider. When your doctor refers you to a specialist or hospital, make sure that the referral is
to a Preferred Health Plan, Inc. network provider in order to minimize your out-of-pocket expense.

Maximizing Your Benefits
To maximize your benefits, choose a Preferred Health Plan network provider whenever possible. This
can result in direct savings based on discounted rates negotiated with the participating providers. In
addition, we suggest that you become familiar with your health plan and its pre-certification requirements,
if any. Many plans include significant penalties for non-compliance with pre-certification procedures.
