Your business and church have the potential of<br />

becoming the epicenter for social change.<br />

Within your community there are needs. People<br />

need education. They need jobs. They need<br />

quality healthcare. They need development and<br />

opportunity. People need you, your church and<br />

they need Jesus Christ.<br />

The Bible calls all Christians to compassion,<br />

justice and sacrifice. Professionals within every<br />

church desire to stand up for justice, to help<br />

those in need and to restore their community to<br />

one that is truly thriving.<br />

But, they don’t know how to do it on their own.<br />

Seed to Oaks is a non profit that brings real and<br />

lasting community change through a collaboration<br />

between local churches, businesses and civic<br />

agencies, filling the space between the three.<br />

Our name, Seed to Oaks, comes from Isaiah<br />

61, the great justice chapter in the Bible.<br />

Scripture uses a metaphor to describe local<br />

churches and Christians who love mercy and<br />

do justice; they become “Oaks of<br />

Righteousness,” a planting of the LORD for<br />

the display of his glory (Isaiah 61:3; NIV).<br />

We invite you to plant a movement of social<br />

good through the local church — from seed<br />

to oaks.<br />

We help socially-driven pastors and professionals<br />

connect and mobilize their resources and<br />

influence through guided social initiatives. These<br />

initiatives are proven, strategic and<br />

straightforward to implement — designed as a<br />

sustainable means for community change that is<br />

real and measurable.<br />

Executive Summary<br />

We believe real and lasting change is within reach. In FY <strong>2018</strong>, we increased the number of<br />

churches we have mobilized from 6 to 25. We helped churches place 37 people into full-time<br />

jobs – accruing over $750,000 in earned income, in collaboration with 12 businesses. We<br />

helped over 750 families receive free health services, including 128 tooth extractions, at a time<br />

when Kentucky is tightening requirements for health coverage among the poor. We launched<br />

two educational summer academies with a proprietary gospel-centered curriculum that helped<br />

100% of children who attended every day to become academically ready for the school year.<br />

And, we facilitated 8 neighborood assessments helping to position churches for greater gospel<br />

reach in their own neighborhoods.<br />

This year is even more promising. We are expanding<br />

our health services, bringing job taining and<br />

placements to new regions, multiplying our education<br />

efforts and launching our neighborhood assessments<br />

to chuches outside Kentucky.<br />

Seed to Oaks projected results from our <strong>2018</strong>/<strong>2019</strong><br />

strategic plan, and the $580,000 operating budget<br />

include:<br />

1) The expansion of our neighborhood assessments<br />

to 20 new church partners. Our Assessments are<br />

designed to help churches and church plants<br />

understand their own neighborhood, so they can<br />

build a strategic plan to reach them with the<br />

gospel.<br />

2) The expansion of our health efforts to 30 health<br />

clinics with the goal of reaching over 3,000+<br />

residents along with the launch of a navigation<br />

system that will connect people to a medical<br />

home for long-term care.<br />

3) The placement of 75 full-time placements (over<br />

$1.1MM in earned income projected for families<br />

not currently working)<br />

and a 70% 6-month retention rate through<br />

JobOne, our job skills and placement program,<br />

which is operated through local churches.<br />

4) The multiplication of our summer academies to<br />

six next summer. Our academies are a reading<br />

readiness and mathematics program run by certified<br />

teachers that helps ready children for the academic<br />

school year.<br />

5) The introduction of a new leader’s cohort, which<br />

concentrates on leadership development for<br />

ministry leaders and pastors, designed to help them<br />

to mobilize church members to community<br />

involvement and commitment to mercy ministry.<br />

We believe that the success of Seed to Oaks is<br />

measured by the degree local churches, not Seed to<br />

Oaks, having a powerful and visible presence in their<br />

neighborhoods. We are unique in that our aim is to<br />

mobilize local churches to the front lines of social<br />

change so that more people hear the gospel of Jesus<br />

Christ and trust him as Savior.

Phase 2 begins with an assessment<br />

community needs using our program expertise.<br />

Whole Health<br />

HEALTH SERVICES<br />

Our Health Clinic model empowers churches to<br />

host their own free medical and dental clinics as<br />

a way to collaborate with health professionals to<br />

meet the health needs of their neighbohroods.<br />

Our health strategists help churches identify and<br />

then coach Clinic Coordinators to guide them,<br />

keep them on track, trouble shoot problems, and<br />

spur them on to good work.<br />

Job One<br />

ECONOMIC OPPORTUNITY<br />

A job skills and placement program founded on<br />

the conviction that every individual ought to have<br />

the opportunity to have a job. We work with<br />

churches to identify people who need a job and<br />

improve access to employment in underserved<br />

neighborhoods. We work with businesses to<br />

increase employer profitability with Work<br />

Opportunity Tax Credits, reduce cost per hire and<br />

increase employee retention rates.

Mercy Cohorts<br />

LEADERSHIP DEVELOPMENT<br />

A two-week academic academy, run by certified<br />

educatiors, that is focused on providing academic<br />

instruction, spiritual enrichment and character<br />

development to equip children to improve<br />

reading skill levels through the summer months<br />

with a goal of being better prepared by the start<br />

of the school year. 100% who attended every<br />

day, in our two pilot academies, became school<br />

ready.<br />

Our mercy cohorts concentrate on leadership<br />

development for ministry leaders. Our crosscultural<br />

training helps develop leaders who are<br />

looking to understand the complexity of inner<br />

city realities, the nature of poverty, and give<br />

them practical help from seasoned mercy<br />

leaders.

1. Conduct 20 neighborhood studies with<br />

new church partners and church plants<br />

which will further position our studies to<br />

2. Develop video consultation study pilots<br />

for churches outside of the state of<br />

Kentucky.<br />

Whole Health core ojectives are:<br />

1. Rapid expansion of our health clinic<br />

model to reach a greater number of<br />

people who are in need of healthcare in<br />

their neighborhoods.<br />

2. Integrate technology into our clinic<br />

system to increase the breadth, speed,<br />

accuracy, and mobility of clinic services<br />

by using electronic medical records and<br />

tablets.<br />

3. Explore, train, and pilot a Health<br />

care to our clinic guests in their home,<br />

business, or place of worship.<br />

1. Enlist 15 leaders into the 6-month online program<br />

so that we can train and multiply more servants<br />

and churches to reach neighborhoods.<br />

2. Develop three new aspects to our training<br />

including community engagement, recreation<br />

activities, and civic involvenent.<br />

3. Develop a Cohort operations manual that includes<br />

hand-on mercy training, character development,<br />

and deacon training.<br />

Job One core objectives are:<br />

1. Complete feasibility study, by end of first quarter,<br />

evaluating the launch of Job One in New Albany,<br />

Indiana.<br />

2. Deliver 75 job placements with a 70% six-month<br />

retention rate by identifying, training and placing<br />

people into employment.<br />

3. Train 110 people in our self-developed basic job<br />

skills boot camp hosted at our local church<br />

partners.<br />

automotive, hospitality, retail and trades.<br />

The Kia Stores of Louisville and Clarksville promoted one of our candidates to a $50,000 position and<br />

career after only 3 months of employment. Our 39 candidates have generated over $750,000 in<br />

earned income for families not working a year ago.<br />

2. Hepatitis A Awareness /<br />

Mar <strong>2018</strong>, 5 churches<br />

We led a collaborative effort between Louisville<br />

Metro Health Department and local churches to<br />

inform every household in effected<br />

neighborhoods how to prevent the disease.<br />

3. 990 Health Clinic<br />

Volunteers / 15 churches<br />

We facilitated the collaboration of almost<br />

1,000 volunteers and health professionals to<br />

provide over $210,000 in health services to<br />

780 families, including 129 tooth extractions.<br />

All for free.<br />

4. 100% School Ready /<br />

Oaks Academies Launch<br />

100% of the students who attended our<br />

Academies every day are now school ready.<br />

Each student is pre and post-assessed by<br />

certified educators using the Brigance<br />

Building upon our existing programs, we plan to eventually offer the following programs and services that have<br />

been researched and proven to successfully address many of the deeper systemic problems existing in the inner<br />

city.<br />

<strong>Plan</strong> 1: Venture Capital &<br />

<strong>Business</strong> Acquisition<br />

As part of our long-term strategy for<br />

financial sustainability, we envision<br />

purchasing several businesses;<br />

beginning with a staffing agency.<br />

Owning and operating a staffing<br />

agency will increase revenue as well as<br />

provide services to help Job One<br />

candidates get through initial hurdles<br />

in the placement process. Bottom line<br />

profits will be allocated to Seed to<br />

Oaks operating budget, which will<br />

increase the sustainability of our<br />

model and progams. Our feasibility<br />

study for the agency is scheduled to be<br />

complete 3QTR FY <strong>2018</strong>/19.<br />

<strong>Plan</strong> 2: Health Navigation<br />

System<br />

To ensure better continuity of health<br />

care, we are working to establish a<br />

health navigator program, in which<br />

patients from health clinics or<br />

neighborhoods are paired with a<br />

navigator who will help them identify<br />

and overcome their personal obstacles<br />

to healthcare (e.g., transportation,<br />

childcare, lack of insurance)<br />

The overarching goal across patients<br />

will be to promote self-efficacy<br />

(confidence) in managing one’s<br />

health and to improve quality of life.<br />

Studies have shown that navigation<br />

programs improve screening followup<br />

rates, reduce anxiety, increase<br />

adherence to treatment, reduce<br />

emergency department and hospital<br />

visits, reduce medical costs, and<br />

improve quality of life. Our<br />

feasibility study is scheduled to be<br />

complete by 2QTR FY <strong>2018</strong>/19.<br />

<strong>Plan</strong> 3: Educator<br />

Support Network<br />

Our Academies have uncovered a<br />

gap in both public and private<br />

education for support among<br />

Christian education professionals. u<br />

Our education support network will<br />

connect Christian educators to each<br />

other to help address existing gaps in<br />

both student’s social and academic<br />

performance, as well as, addressing<br />

gaps between professionals who are<br />

seeking mentoring and those who<br />

can give of their expertise and<br />

experience.

Seed to Oaks is seeking to raise $600,000 for the purpose of financing our fiscal<br />
