Referring Professionals
OUR REFERRING PROFESSIONALS ARE PART OF THE TEAM
When professionals refer a patient to Lakeview Health, they are a valuable
part of the treatment team. We believe that a seamless support structure
is critical to the healing process. Our partnership includes:
• Weekly patient progress phone updates with a Lakeview Health therapist
• Onsite visits and/or phone meetings with the patient and his or her
therapist for discharge and aftercare planning
• Transitioning of patients to the referring clinician after treatment
• Inclusion of the referring clinician’s services in the Lakeview Health
aftercare database
HOW TO REFER A PATIENT
Connecting with your Client Services Representative
We are available 24 hours a day to help you. To initiate a referral, we
invite you to get acquainted with your Lakeview Health Client Services
Representative. They are seasoned experts who are your liaison to our
facility and local resources. They are available to answer questions about
our clinical program, connect with our staff, facilitate the admissions
process for your patient, or connect you with other important resources.
PATIENT ADMISSIONS AND INSURANCE
We are available 24 hours a day to help anyone seeking care
and we accept most commercial insurance plans.

Lakeview Health accepts most commercial insurance plans.
Medicaid and Medicare are not accepted.
Your Client Services Representative is available to speak with your patients
to help them make an informed decision about their recovery options.
