NEW STUDENT ENROLLMENT
Thank you for your interest in enrolling your student in the San Marcos Unified School
District (SMUSD). SMUSD uses ParentVUE, an online platform that allows parents to
process all of their student information electronically.
This new enrollment process will take approximately 20-30 minutes to complete and you
will be required to upload a copy of your Government Issued Photo ID, your student's
age verification and residency verification documents.
Please ensure you allow yourself enough time to complete the form and have the
necessary documents ready to upload, if you don’t have all the documents listed, you
may return to your application at any time. Please note that your application will not be
submitted until all documents are loaded.
___ Student’s last Report Card or Withdrawal Grades
___ Birth Certificate
___ Immunization Records
___ IEP, 504 & Psych Report for Special Education (if applicable)
One Proof of Residency from EACH category:
___ CATEGORY ONE:
□ MORTGAGE STATEMENT or PAYMENT RECEIPT (with address of
□ RENTAL AGREEMENT or PAYMENT RECEIPT (with address of residency)
□ PROPERTY TAX STATEMENT or RECEIPT (with address of residency)
□ GRANT DEED (with address of residency)
□ ESCROW PAPERS (with address of residency)
___ CATEGORY TWO:
□ CURRENT UTILITY BILL
(SDG&E, WATER, TRASH OR CABLE [Must be current within 60 days)
□ CORRESPONDENCE FROM A GOVERNMENT AGENCY
□ VOTER REGISTRATION
□ CURRENT PAY STUB W/ADDRESS
□ AFFIDAVIT OF RESIDENCY (needed if shared housing-Parent/Guardian not
listed on proof of residency)
