November 18, 2020 www.TheTownCommon.comPage 3Bicycle, from page 1Bicycle ridinggrows in the pandemicIn the Livable Streets survey,about 40 percent of the respondentslive in the North End, 20percent in the South End, 14 percentdowntown and 13 percent inthe West End.Nearly all of the respondentssaid they cycle for recreation, butalmost 40 percent ride their bikesto shopping, about 20 percentbike to school, 13 percent to visitthe doctor and other services and12 percent commute to work ontheir bicycle.Nearly 60 percent of the respondentssaid they cycle daily or severaltimes a week. The most frequentcyclists are under 18. Almost 100percent of the under 18 respondentscycle several times a week.In general, respondents over 40years old cycle more frequentlythan younger adults.Parking for bicycles was a majorissue. The places downtownwith the greatest need for morebicycle parking are the CentralWaterfront and Market LandingPark, Market Square, the Tannery,State Street and the library.Trainor said the lack of bikeracks downtown force riders tochain their bikes to light poles.At the library, the bike rackswere moved from the front of thebuilding to the back. Peter St. Clair, DMDDo you have any problems withdental decay (cavities), gum recession,and/or dental erosion (thechemical breakdown of tooth structure)?These dental issues are complicatedmultifactorial diseases of epidemiclevels affecting both childrenand adults. A healthy mouth sometimesrequires more than brushing,flossing, and “fillings”. With currentscientific evidence and new technologies,patients and practitioners needto begin to look at these problemsnot just from a drilling and fillingapproach, but also from a medical(preventive/therapeutic) approach.There are over 19,000 differentbacteria that have been found inmouths and every person has about1,000 different types. Not all of themcause decay, but many of them havealso been found to grow on arterywalls. The medical/dental systemicconnection is real and we must paymore attention to it.Why the increase in decay? Most ofit has to do with dietary trends. Wesnack more, eat more sugar/carbs,drink more soda, have more gastricreflux, take more mouth-dryingmedications, etc. Dental caries (decay)is a pH specific disease. The rightbacteria, plus sugar, create acid whichbreaks down the enamel of the teeth.Add an already acidic environmentand it is even worse. In most casesit is a preventable disease. And don’tthink that just because you don’t eat“sugar” that you are safe. If you aregetting decay, something is causingit. The problem is that changing behaviorcan be very hard to do.It is time for the dental professionalto take a different approachwhen treating this disease. More focusneeds to be shifted to preventionof decay rather just treating it. Fillingteeth is treating the result of thedisease, but does nothing to preventit. The dentist needs to take a moreactive role in assessing individual’srisk factors. In the dental world thisis referred to as CAMBRA, whichstands for Caries Management ByRisk Assessment.Based on assessing an individual’srisk factors, such as quality of homecare, quality of salivary flow, medicationissues, and dietary issues, acaries-preventive strategy can beestablished. There are many newproducts on the horizon to helpcombat and virtually eliminate thisdisease. However, dentists must takesome responsibility and be open toa different management of this disease.They must also be able to motivatepeople to change habits, whichcan be challenging. The bottom lineis that if you want to be decay-free,you can be.For those at higher risk, there aresome great products currently availablefrom a company called Carifree.Everything from new toothpastesand gels with ions in them to rebuildtooth structure, sprays to neutralizepH, and probiotics are on their way.Right now you can use things likethe sweetener replacement Xylitol,which by itself is cavity-fighting, butalso works synergistically with fluoride.Prescription level toothpastesare also available, and there is strongresearch for the topical applicationof fluoride varnish, the same stuffthe kids get, for adults.The evidence is very clear – this is apreventable disease. Next time you goto the dentist and find out you havea new cavity, stop blaming the dentistor yourself, and ask to get a specificprotocol for prevention of this disease,based on your specific risk factors.
