Hello and Happy “Year of the <strong>Nurse</strong>”! As KNA<br />

President and on behalf of the entire KNA Board<br />

of Directors, I salute all of you who have devoted<br />

your lives to our amazing profession. From Paducah<br />

to Pikeville and all parts in between, nurses are<br />

working to improve the lives of their patients and<br />

communities. One of my personal favorite things<br />

about nursing is the variety of settings and roles<br />

on which a nurse can focus. <strong>Nurse</strong>s can work<br />

in the acute environment of the hospital setting,<br />

in the behavioral health arena, in a primary care<br />

practice, in patients’ homes as a home health nurse,<br />

assist patients in navigating their way through our<br />

challenging healthcare system, and more. I’ve<br />

personally had the privilege to serve as a staff nurse<br />

in various parts of the hospital, a flight nurse and<br />

later a chief flight nurse, a nurse executive, a patient<br />

safety and quality consultant and now mainly a<br />

volunteer. Our work can be exhausting, heartbreaking<br />

and extremely difficult, but it can also be<br />

energizing, encouraging and very rewarding.<br />

Last year, we celebrated what would have been<br />

Florence Nightingale’s 200th birthday on May 12<br />

with a year-long celebration – the Year of the <strong>Nurse</strong><br />

and Midwife which has been extended into <strong>2021</strong>.<br />

The American <strong>Nurse</strong>s Association is leading the<br />

charge nationally with various publications, toolkits<br />

and other promotional tools. See these key talking<br />

points for your use:<br />

• The aim of the “Year of the <strong>Nurse</strong>” is to raise<br />

the visibility of the nursing profession in policy<br />

dialogue and spur greater investment in the<br />

development and increased capacity of the<br />

nursing workforce;<br />

• We rank as the most trusted professionals for<br />

19 years consistently;<br />

• Registered nurses are by far the largest<br />

healthcare workforce in the US;<br />

• We are also the largest healthcare profession<br />

worldwide, but a huge shortfall by 2030 is<br />

predicted, unless radical action is taken soon;<br />

• With the aging US population, more than one<br />

million additional nurses will be needed by 2022;<br />

• We advocate and<br />

innovate in our<br />

communities,<br />

clinics, hospitals<br />

and throughout the<br />

healthcare system;<br />

• We influence policy and<br />

practice standards that impact access to quality<br />

and affordable healthcare, and advocate for<br />

healthcare for all;<br />

• We must work together with educators,<br />

employers and policy makers to expand nursing<br />

roles and explore new, nurse-led models of<br />

care and<br />

• Through collective efforts to elevate<br />

contributions of nurses and create an<br />

imperative to invest in nursing and nurses, we<br />

will improve the health of our state, nation and<br />

world.<br />

To recognize the “Year of the <strong>Nurse</strong>” here at<br />

home, the <strong>Kentucky</strong> <strong>Nurse</strong>s Association is leading<br />

the charge. Our boards, committees, cabinets,<br />

affiliate organizations and staff recently wrapped up<br />

<strong>Nurse</strong>s Month with fantastic messages and activities<br />

to celebrate, improve awareness, educate nurses and<br />

educate the public about all nurses can do. Look at<br />

this snapshot of activities:<br />

• The opportunity to recognize or remember<br />

a nurse and benefit the <strong>Kentucky</strong> <strong>Nurse</strong>s<br />

Foundation through its’ Honor a <strong>Nurse</strong>,<br />

Remember a <strong>Nurse</strong> campaign;<br />

• “Thank You <strong>Nurse</strong>s – A Cinema of Gratitude”<br />

video series sharing heartfelt thanks from key<br />

leaders in our state;<br />

• A Proclamation from Governor Andy Beshear<br />

recognizing the month of May as “<strong>Kentucky</strong><br />

<strong>Nurse</strong> Suicide Prevention Month” (see the video<br />

of this proclamation being signed on the KNA<br />

KNA Bluegrass Chapter<br />

The Bluegrass Chapter continues to meet via<br />

zoom and will offer CE programs monthly, July<br />

– January <strong>2021</strong> that will focus on issues such as<br />

community development and diversity and workforce<br />

development and diversity. The chapter recently<br />

hosted, “Preparing Graduate <strong>Nurse</strong>s for Patient<br />

Rescue,” facilitated by Drs. Keri New and Heather<br />

Chapter members provided breakfast to the<br />

Anderson County Health Department during Public<br />

Health Week. Members continue to volunteer at<br />

local health departments and articles were placed in<br />

local newspapers in recognition of their service.<br />

Students participating in vaccination clinic at the<br />

Anderson County Health Department were given<br />

“goodie bags” which included information about<br />

KNA, how to become a student subscriber, and<br />

chapter scholarships. Members continue to keep<br />

nursing faculty members/program directors and<br />

Student Subscribers informed of chapter activities.<br />

The chapter recently hosted “<strong>Kentucky</strong>, COVID-19,<br />

and School <strong>Nurse</strong>s <strong>2021</strong>” and “The Kidz Club: A<br />

Medical Day Treatment Program for Medically Fragile<br />

Children.” Officers include:<br />

- Lisa Lockhart, MSN, MHA, RN, NE-BC, co-chair<br />

- Denise Alvey, MSN, RN-BC, secretary<br />

Michael Tain, KNA Nightingale Chapter member,<br />

volunteered his time administering the COVID<br />

vaccine at the Garrard County Health Department.<br />

Lisa Lockhart, MSN, MHA, RN, NE-BC and<br />

Regina Rice, BSN RN FCN, took a quick break<br />

for a photo op after administering vaccines<br />

at the COVID-19 clinic at their workplace,<br />

Ephraim McDowell Hospital in Danville. Lisa is<br />

sporting a special bracelet that promotes organ<br />

donation and a poster behind the two ladies also<br />

publicizes this important action.<br />

KNA Northeastern Chapter<br />

Chapter members will soon host a t-shirt<br />

fundraiser to donate an AED for the local Christian<br />

Academy and have set a goal to raise funds to<br />

purchase thermometers for the Academy. The<br />

chapter sent “Thank You <strong>Nurse</strong>s” postcards to all<br />

members to promote <strong>Nurse</strong>s Month and recent<br />

chapter meetings.<br />

Members have volunteered their time at vaccine<br />

clinics at the St. Claire Regional Medical Center in<br />

Morehead and seek to recruit and retain members<br />

by increasing the chapter’s presence in local<br />

communities. Other recruitment and retention<br />

efforts include “bring-a-friend” raffles to encourage<br />

members to bring non-KNA members to meetings.<br />

The Chapter recently hosted a CE program,<br />

“Implementation of a Dedicated Education Unit<br />

(DEU): An Evidenced-Based Quality Improvement<br />

Project.” Elizabeth Beuke, DNP(s), RN, CMSRN,<br />

presented the program. Plans are underway for<br />

Dr. Dora Savani speak at an August meeting, more<br />

details TBD. Officers include:<br />

KNA Pennyrile Chapter<br />

Chapter members continue to volunteer with the<br />

health departments to provide COVID-19 vaccines<br />

and testing – two members recently volunteered<br />

their time or reached out to the White Plains and<br />

Christian County Health Departments. To help all<br />

nurses navigate mental health issues and stress,<br />

the Pennyrile Chapter developed this helpful list of<br />

resources.<br />

Pennyrile members recently formed scholarship and<br />

fundraising subcommittees to establish a sustainable<br />

nursing student scholarship like the one offered by<br />

the <strong>Kentucky</strong> <strong>Nurse</strong>s Foundation. Immediate plans<br />

for the scholarship committee, chaired by Dr. Linda<br />

Thomas, include providing two $500 scholarships<br />

awarded annually to Madisonville and Hopkinsville<br />

nursing students as well as those throughout the<br />

Several of nursing faculty members attended the<br />

<strong>Kentucky</strong> League for Nursing (KLN) virtual conference<br />

recently and represented Madisonville Community<br />

College by being elected to committees and a board<br />

position. Chapter members congratulate Ava Cook<br />

for being elected to the KLNM Board of Directors<br />

The chapter Fundraising Committee will raffle a<br />

“Summer Fun Basket” with proceeds going toward a<br />

The River City Chapter members to work<br />

with community members to present continuing<br />

education opportunities with proceeds benefitting<br />

charities. In January, the chapter hosted a CE<br />

program, “Blessings in a Backpack – Who Will<br />

Feed the Children this Weekend?” and garnered<br />

$600 to benefit the non-profit. The chapter’s<br />

next presentation, facilitated by Matthew<br />

LaRocco, manager of Norton Hospital’s Harm<br />

Reduction Program, “One of These People is<br />

Not Like the Other,” noon-1pm, Monday, <strong>June</strong><br />

21 with proceeds going to the <strong>Kentucky</strong> <strong>Nurse</strong>s<br />

Foundation (KNF).<br />

River City Chapter members are helping with<br />

COVID-19 testing and vaccination clinics through<br />

the KNA Office, the local health department and<br />

other community organizations. Chapter members<br />

participated in a community health fair providing<br />

blood pressure screenings and education.<br />

To celebrate <strong>Nurse</strong>s Month, River City Chapter<br />

members randomly selected four members<br />

each Friday to receive a prize. At each meeting,<br />

members who bring a non-KNA member nurse<br />

friend are entered in a drawing to win a prize or<br />

gift card. The chapter presents its first scholarship<br />

opportunity by providing a $500 scholarship for<br />

a River City Chapter member pursuing a graduate<br />

degree and a $250 scholarship for a Student<br />

Subscriber pursuing an undergraduate degree. The<br />

chapter will also offer KNA Summit scholarships<br />

for first time conference/summit attendees and has<br />

developed a student liaison team to communicate<br />

with schools of nursing to increase Student<br />

Subscriber participation.<br />

To celebrate National Certification Day,<br />

members were asked to complete a survey to<br />

make the chapter aware of their certification. A<br />

total of 44 members participated sharing their<br />

certification and were entered in a drawing<br />

where 10 members received prizes to celebrate<br />

their accomplishment and dedication to nursing<br />

excellence.<br />

KNA School <strong>Nurse</strong> Task Force<br />

Members of the KNA School <strong>Nurse</strong> Task Force are<br />

promoting the importance of school nurses in every<br />

school every day with Dr. Jason Glass, commissioner<br />

for the Department of Education, and with the<br />

Lieutenant Governor Jacqueline Coleman, as well as<br />

the superintendents of schools. <strong>Nurse</strong>s across the<br />

state regularly attend Dr. Glass’ virtual town hall<br />

meetings.<br />

Members have applied for a grant to fund<br />

studying the outcomes for children in schools that<br />

have a dedicated school nurse in their schools<br />

for their sustainability. Members are also seeking<br />

Coronavirus Aid, Relief, and Economic Security<br />

(CARES) and Elementary and Secondary School<br />

Emergency Relief (ESSER) funds to place nurses in<br />

schools for the next budgeted year.<br />

KNA Western Chapter<br />

Murray State University and Western <strong>Kentucky</strong><br />

Community & Technical College (WKCTC) KNA<br />

members recently participated in several COVID-19<br />

vaccine clinics in the chapter’s service area which<br />

includes Ballard, Calloway, Carlisle, Crittenden,<br />

Fulton, Graves, Hickman Livingston, Lyon, Marshall<br />

and McCracken counties.<br />

Students in the area who joined KNA as Student<br />

Subscribers in January and February were entered<br />

into a drawing to win $50 gift cards. <strong>Nurse</strong> faculty<br />

members that joined KNA in May also were entered<br />

into drawing for $50 gift cards. WKCTC first<br />

semester nursing students will join KNA during their<br />

orientation on Monday, August 16.<br />

mental health and suicide prevention;<br />

• Additional educational opportunities included a<br />

webinar hosted by the KNA Northern Chapter<br />

on Dedicated Education Units and a webinar<br />

hosted jointly by KNA and the KBN that focused<br />

on nursing laws, and how to protect your license<br />

and understand scope of practice and<br />

The <strong>Kentucky</strong> <strong>Nurse</strong>s Association needs nurse and<br />

nursing student volunteers to help with COVID-19 testing<br />

and vaccinations. Those interested in Louisville, may<br />

click Volunteer Registration. To volunteer in other parts of<br />

the state, contact your chapter leader or reach out to the<br />

KNA at admin@kentucky-nurses.org. As vaccinations are<br />

becoming more available to Kentuckians, remember it’s still<br />

very important to get tested for COVID-19. Visit ky-covidtesting<br />

for a list of testing sites throughout the state.<br />

To date, more than two million Kentuckians have received<br />

at least one vaccine dose.<br />

We thank the more than 560 KNA COVID volunteers and<br />

all nurses across the state who are lending their time and<br />

expertise to help combat the pandemic. Keep in mind that<br />

we provide and require PPE for all volunteers!<br />

the state recognizing our profession and once<br />

again, saying “Thank You” to nurses.<br />

• Describe the historical basis for viewing<br />

some groups, races and individuals as<br />

inherently superior to others and more<br />

deserving of fundamental rights based off<br />

of genetics;<br />

• Discuss the use of eugenics and genetics<br />

in the sterilization of certain populations<br />

of women;<br />

• Discuss the myth of “racial” health<br />

disparities and how they are based on<br />

falsely interpreted and employed science<br />

and<br />

• Discuss the importance of evaluating all<br />

science, including nursing science, with a<br />

clear eye and willingness to questions its<br />

use in creating inequities throughout our<br />

society, particularly within healthcare.<br />

There is a dangerous idea that has<br />

threatened the American Dream from the very<br />

beginning. It is a strong current of biological<br />

determinism which views some groups, races<br />

and individuals as inherently superior to<br />

others and more deserving of fundamental<br />

rights. Despite the founders’ assertion that<br />

“all are created equal,” this idea was used to<br />

justify disenfranchising women, blacks and<br />

Native Americans from the earliest days of the<br />

Republic.<br />

This film reveals how this dangerous idea<br />

gained new traction in the 20th century with<br />

an increasing belief in the concept of an allpowerful<br />

“gene” that predetermines who is<br />

worthy and who is not. The film reveals how<br />

this new genetic determinism provided an<br />

abhorrent rationale for state sanctioned crimes<br />

committed against America’s poorest, most<br />

vulnerable citizens and for violations of the<br />

fundamental civil rights of untold millions.<br />

are considering leadership positions within<br />

KNA?<br />

(MW) This is a great way to have a voice and<br />

support nursing throughout the Commonwealth.<br />

(KN) What is your message to nurses on the<br />

frontline of the COVID-19 pandemic?<br />

(MW) I know you are weary…. I know you are<br />

tired…. I know you may have permanent scars/<br />

marks on your face from wearing your masks<br />

13 hours at time…. I know you are frustrated<br />

thinking how can I get the general population<br />

to take COVID-19 seriously?! I have heard from<br />

several of my former students working the<br />

frontlines who have shared all these concerns.<br />

Please know, I hear you and KNA hears you.<br />

We are all putting forth effort to vaccinate and<br />

educate the public in hopes of alleviating some of<br />

your struggle. Take time for self-compassion and<br />

self-love and always know you are worthy.<br />

(KN) What would you say to others who<br />

are considering leadership positions within<br />

KNA?<br />

(KH) We need every nurse to be part of our<br />

mission. The more members we have, the more<br />

we can do. If you are busy with family and work<br />

now, join anyway. The more of us there are, the<br />

more we can share the workload.<br />

(KN) What would you like fellow nurses to<br />

know about your KNA membership? What<br />

does it mean to you?<br />

(KH) It means that we can lead the way<br />

for other nurses to find their perfect nursing<br />

position – to provide the best care possible, to<br />

provide mentorship for nurses moving from one<br />

position to another and from one area of nursing<br />

to another. We can be their bridge and support<br />

structure.<br />

(KN) What is your message to nurses on the<br />

frontlines of the COVID-19 pandemic?<br />

(KH) Thank you for putting your life on the<br />

line every time you entered your place of work…<br />

not knowing what lay beyond those doors…<br />

having to protect your family from yourself. In the<br />

beginning, you did not even know the monster<br />

that lurked inside… there could be no one more<br />

dedicated to the health and welfare of a patient<br />

Nursing Care and Teaching for the Patient with a Recent STEMI<br />

Patient Description<br />

A 62-year-old white female was admitted to the hospital<br />

with a recent ST elevated myocardial infarction in addition<br />

to present diagnoses of hypotension, type 2 diabetes,<br />

hyperlipidemia, anxiety, gastroesophageal reflux disease,<br />

history of hypertension and history of ST elevated myocardial<br />

infarction. During this admission, the patient had received<br />

cardiac catheterization and stenting of the lateral anterior<br />

descending artery because of her recent myocardial infarction.<br />

Previously, the patient went through a similar cardiac<br />

catheterization process after a previous ST elevated myocardial<br />

infarction. The patient admitted to smoking and partaking in<br />

a diet high in sweets, fats, and sodium. She also stated that she does not exercise, but<br />

she does a lot of cleaning around her apartment and works at a nursing home doing<br />

laundry for a living. Currently she lives alone and does not see friends or family very<br />

often.<br />

Pathophysiology of Admitting Diagnosis<br />

The patient’s admitting diagnosis, an ST elevated myocardial infarction, is caused<br />

by an occlusion in any of the major arteries of the heart. This occlusion leads to tissue<br />

ischemia within the heart wall, causing decreased blow flow, muscle ischemia, and<br />

ability for it to contract. In this patient, the lateral anterior descending artery was<br />

occluded, causing her ST elevated myocardial infarction (Norris, 2019 p. 767). Lab<br />

values tested related to her diagnosis include brain natriuretic peptide, which was<br />

elevated at 3,200 pg/mL, and troponin, which was elevated at 4.8 ng/dl. Increased<br />

brain natriuretic peptide often indicates fluid overload and is often elevated when the<br />

heart is under acute stress (Norris, 2019 p. 798). Troponin levels in the blood stream<br />

indicate damage of the heart muscle, as this protein is not released from the heart<br />

muscle into the bloodstream unless the muscle is damaged (Norris, 2019 p. 43). Other<br />

diagnostic tests included an electrocardiogram with evident ST elevation.<br />

Other medical diagnoses identified include anxiety, gastroesophageal reflux disease,<br />

hypotension, type 2 diabetes, hyperlipidemia and hypertension. Generalized anxiety<br />

disorder is characterized by excessive worry about events and certain activities (Norris,<br />

2019, p. 533). This disorder typically causes stress and a decrease in one’s quality of life.<br />

Gastroesophageal reflux disease is defined as the symptoms caused by the abnormal<br />

reflux of gastric contents into to esophagus (Norris, 2019, p. 1080). Hypotension is<br />

classified as a blood pressure lower than 90/60. Hypertension is classified as a blood<br />

pressure above 130/80 and puts undue stress on the heart muscles (Norris, 2019, p.<br />

742). Type 2 diabetes is a condition that causes high glucose levels in the bloodstream<br />

due to insulin resistance or underproduction (Norris, 2019, p. 1219). Hyperlipidemia<br />

is the presence of high amounts of lipids within the bloodstream and can lead to<br />

occlusion of blood vessels (Norris, 2019, p. 722). In addition to the diagnoses of type 2<br />

diabetes, hypertension, and hyperlipidemia, the patient is a smoker and is overweight.<br />

All of these factors combine to form something called metabolic syndrome which is a<br />

collection of factors that put somebody at higher risk for heart attacks and other heart<br />

problems (Norris, 2019, p. 725).<br />

Nursing Care Required in the Acute Care Setting<br />

At first glance, the patient seems to be in general good health. Her appearance is<br />

clean, skin color is pink, and hair is evenly distributed. The patient is fully conscious<br />

and aware of her surroundings. Upon the first encounter the patient was asleep but<br />

smiled upon awakening. Her behavior is appropriate for time, place, and situation.<br />

Her mood seems to be happy as she was very talkative and polite. The patient<br />

is independent of all activities of daily living. Her skin had no obvious lesions or<br />

abnormal skin discoloration other than the cardiac catheterization incision site.<br />

Upon physical assessment it is apparent that the patient is alert and oriented<br />

to person, place, time, and situation. No abnormal head, ears, eyes, nose, throat,<br />

skin, neurological, musculoskeletal, or abdominal/gastrointestinal findings evident<br />

upon assessment. The patient has verbalized previous chest, left arm and jaw<br />

pain that has disappeared with time. Her vital signs present a blood pressure of<br />

90/53, a respiratory rate of 18, an oxygen saturation of 96, a heart rate of 97, and<br />

a temperature of 98.7. The patient’s blood pressure is low evidenced by her blood<br />

pressure reading. Her lungs are clear to auscultation bilaterally with no apparent<br />

signs of dyspnea. Auscultation of her heart revealed present S1 and S2 sounds with<br />

no S3 sounds or pedal edema. Her point of maximum impulse is located at the 5th<br />

intercoastal space, mid clavicular line.<br />

During their educational journey, every nursing student<br />

is inevitably asked the question, “Why do you want to be a<br />

nurse?” Although it maintains a sentiment of caring curiosity,<br />

this inquiry has the potential to conjure anxiety in even the<br />

most confident of students as they ponder the seemingly<br />

endless career possibilities in nursing. I regularly find myself<br />

lost in thought at the various nursing career prospects in<br />

my future, and I thoroughly survey each individual clinical<br />

experience in an incessant effort to uncover the one type of<br />

nursing perfectly suited to my skill set.<br />

This scrutinizing, fervent attitude undoubtedly presented<br />

an obstacle as I tried to focus on the present and learn the<br />

lessons existent in each unique clinical experience. The problematic nature of this<br />

mindset became strikingly evident when I experienced an unforgettable moment of<br />

inspiration while following and assisting different nurses in an emergency department<br />

setting. I observed as they treated patients from all walks of life. I was in awe of<br />

how the nurses prioritized and treated all patients in an orderly manner to provide<br />

individualized care effectively and efficiently.<br />

While these aspects provided important education, the indelible inspiration<br />

this experience lent arose from the genuine care and compassion the nurses<br />

demonstrated for their patients. Every single person was treated with the same<br />

dignity and respect regardless of the circumstances of their presentation. I saw nurses<br />

look into the eyes of every patient with a gaze of assurance, comfort, and genuine<br />

compassion. It is this unforgettable interaction that revolutionized my perception of<br />

nursing and reframed consideration of my future career considering others’ needs<br />

instead of my own desires. While certain amounts of anxiety about the future remain,<br />

I no longer wonder, “Why do I want to be a nurse?” Instead, I ponder the question,<br />

“How can I use my nursing career to give the fullest gift of myself to others?”

A large factor that played into her most recent ST elevated myocardial infarction is<br />

lack of education regarding the risks of lifestyle habits and smoking. Despite having<br />

a very similar episode in 2005, the patient admitted to smoking, not exercising, and<br />

eating a diet high in fats, sugar, and sodium. No changes in her lifestyle habits had<br />

been made between her ST elevated myocardial infarctions, which indicates that<br />

patient education is needed as a primary intervention in maintaining her health.<br />

Several nursing diagnoses fit her case very well, but the most important one’s<br />

deal with prevention of future episodes. After her most recent myocardial infarction,<br />

the patient seems stable and is not at immediate risk of life-threatening harm, so<br />

preventing another similar episode is essential as the episode itself is life threatening<br />

(Dreyer, et al., 2020, p. 1). One diagnosis that fits this patient’s current state is<br />

decreased cardiac tissue perfusion related to insufficient knowledge of modifiable<br />

factors. This is evidenced by her unchanged lifestyle habits and seemingly indifferent<br />

demeanor regarding a need to change her diet and exercise habits. For this reason,<br />

the patient unfortunately suffered another myocardial infarction and resulting<br />

decreased cardiac tissue perfusion that could have been prevented. In order to<br />

reduce her risk of heart attack and promote health, one of the essential nursing<br />

interventions is education regarding an appropriate cardiac diet that includes foods<br />

she enjoys eating. An appropriate cardiac diet that is low in fats, sugar, and salt<br />

is essential in maintaining a healthy heart (Wood & Gordon, 2019, p. 15). Upon<br />

interview, the patient expressed that she enjoys southern cooking and sweets.<br />

For many, eating is strongly based on culture and evidence shows that many are<br />

reluctant to give up the foods that they grew up with (Sullivan, Hays, & Alexander,<br />

2019, p.81). For this reason, her favorite recipes should be altered with the help<br />

of the nurse so that they fit into her new cardiac diet. Information regarding how<br />

to get in touch with a dietician to help her implement a cardiac diet should also<br />

be offered as an educational and supportive resource. Research has shown that<br />

collaboration with other individuals and healthcare providers has been shown<br />

to improve results and the desire to change health practices (Sullivan, Hays, &<br />

Alexander, 2019, p.80 -81). Another area of her lifestyle that needs to be addressed<br />

is her exercise habits. The patient should be properly educated on the importance<br />

of exercise and its benefits regarding the cardiovascular system. Studies have shown<br />

that an increase in exercise results in a decrease in factors related to metabolic<br />

syndrome, and thus better heart health (Wright et al., 2018, p. E5-E7). Like changing<br />

one’s diet, changing one’s exercise habits can be very difficult. For this reason, her<br />

exercise goals need to work with her schedule and be kept within her limits. Upon<br />

interview, the patient expressed that she likes to watch TV in her free time. An<br />

appropriate goal would be to watch one of her favorite episodes on TV while riding<br />

a stationary bike for approximately 30 minutes 4 times a week.<br />

In addition to insufficient knowledge regarding diet and exercise, the client also<br />

does not seem to recognize or acknowledge the effects of her smoking. Thus,<br />

another nursing diagnosis is ineffective health maintenance related to denial of<br />

the effects of smoking. An appropriate nursing intervention should be to educate<br />

the client about smoking’s effect on the heart. Smoking in and of itself is very<br />

dangerous to overall health and has detrimental effects on the cardiovascular<br />

system (Wood & Gordon, 2019, p. 13-14). Smoking cessation is necessary in order<br />

to prevent her heart health from getting worse. Therefore, before discharge the<br />

nurse should collaborate with the patient to develop a patient centered plan for<br />

smoking cessation. This plan should include a gradual decrease in the number of<br />

cigarettes consumed per week and at a rate that the patient is comfortable with.<br />

Research has shown that people are more likely to follow a smoking cessation plan<br />

with the help of others going through smoking cessation. Thus, a support group is<br />

a good recommendation, and the information regarding how to get involved in one<br />

should be provided (Arslanian-Engoren et al., 2015, p. 481-482).<br />

Patient teaching regarding healthy eating, diet, and lifestyle practices is essential<br />

to meet both the discharge and long-term goals of this patient. Outcomes for each<br />

nursing diagnosis identified can be evaluated through vital signs and evaluating<br />

her understanding of the plans to be implemented after discharge. Her vital signs<br />

should be within normal limits as an appropriate goal prior to her discharge.<br />

The patient should be able to verbalize a plan for diet and exercise and express<br />

familiarity with the negative effects of smoking on her heart. These goals can be<br />

further evaluated through follow up appointments with her healthcare provider as<br />

well as a therapist and/or dietician.<br />

Removal of CAPA-CS Requirement for APRNs Who Meet Criteria – To<br />

ensure nurse practitioners can practice to the full extent of their education and<br />

training to increase access to underserved populations throughout <strong>Kentucky</strong>.<br />

<strong>Nurse</strong> practitioners fill a large gap in access to care. This will not only improve<br />

access, but health outcomes for Kentuckians. Result: SB78 with amendment<br />

considered; bill not passed.<br />

School <strong>Nurse</strong> in Every School, All Day, Every Day – This initiative ensures<br />

access to health providers for public school children throughout KY. Studies<br />

suggest that schools with a school nurse have higher graduation rates and better<br />

overall outcomes. Result: Not a GA bill in the <strong>2021</strong> session; KNA School <strong>Nurse</strong><br />

Task Force members are seeking Coronavirus Aid, Relief and Economic Security<br />

(CARES) and Elementary and Secondary School Emergency Relief (ESSER) funds<br />

to place nurses in schools for the next budgeted year.<br />

Increase Access to Sexual Assault <strong>Nurse</strong> Examiners (SANE) – Sexual<br />

Assault <strong>Nurse</strong> Examiners are specifically trained to care for victims of sexual<br />

assault. Care by a SANE nurses results in better patient outcomes and higher<br />

conviction rates. Sexual assault nurse examiners have become the gold standard<br />

in care for victims of sexual assault, but <strong>Kentucky</strong> has a huge deficit which results<br />

in a delay in care. We must ensure that victims have access to this specialized care<br />

to improve outcomes, decrease costs, and improve the safety of our communities.<br />

Result: Introduced, not assigned as a bill.<br />

Evacuation of Surgical Smoke – Surgical smoke contains hundreds of<br />

carcinogens that puts patients and medical providers at risk. The answer, that<br />

many states have already mandated, is very simple. We must mandate the use<br />

of smoke evacuation systems in our operating rooms. We must ensure we are<br />

safeguarding those who save lives. Result: Gov. Beshear signed <strong>Kentucky</strong> SB<br />

38 into law, requiring hospitals and outpatient centers to use a surgical smoke<br />

evacuation system during any procedure that generates smoke.<br />

Accomplishments<br />

Since its inception in 2017, our “Honor a<br />

<strong>Nurse</strong> – Remember a <strong>Nurse</strong>” campaign has<br />

brought in more than $20,073. This campaign<br />

affords us all a great way to highlight those<br />

special nurses in our lives. Our COVID-19 <strong>Nurse</strong>s<br />

Support Fund brought in more than $20,300 to<br />

help nurses and nursing students. We need to<br />

raise more funds to give more funds.<br />

We are happy to report that we just kicked<br />

off our <strong>2021</strong> <strong>Kentucky</strong> <strong>Nurse</strong>s Foundation<br />

Virtual Raffle. From chances on exclusive<br />

packages to a “Split-the-Pot” drawing, there’s<br />

something for everyone. Proceeds benefit<br />

nursing education, scholarships and research<br />

condolences and let them know that we will<br />

conduct in-person Tributes when it is safe to<br />

Conducted by Kathryn Showalter, PhD, assistant<br />

professor, College of Social Work at the University<br />

of <strong>Kentucky</strong>, the survey aims to understand the<br />

stressful experiences of working while being<br />

employed as a nurse. Two components of the<br />

survey include a questionnaire related to COVID-19<br />

and work-stressors that takes about 10 minutes and<br />

the main survey with questions about relationships<br />

and intimate partner violence for qualifying<br />

participants that takes about 25 minutes. In total,<br />

the survey should take about 35-45 minutes. If<br />

you qualify for both sections, they must be taken<br />

at once. Findings will help <strong>Kentucky</strong> healthcare<br />

employers better support nurses during this time<br />

and in the future.<br />

The study will not ask for identifying information<br />

and cannot be tracked to you. If you qualify for<br />

participation in the first portion you will receive<br />

one $5.00 Walmart electronic gift card upon<br />

completion and if you qualify for both portions, you<br />
