President’s Pen
Hello and Happy “Year of the Nurse”! As KNA
President and on behalf of the entire KNA Board
of Directors, I salute all of you who have devoted
your lives to our amazing profession. From Paducah
to Pikeville and all parts in between, nurses are
working to improve the lives of their patients and
communities. One of my personal favorite things
about nursing is the variety of settings and roles
on which a nurse can focus. Nurses can work
in the acute environment of the hospital setting,
in the behavioral health arena, in a primary care
practice, in patients’ homes as a home health nurse,
assist patients in navigating their way through our
challenging healthcare system, and more. I’ve
personally had the privilege to serve as a staff nurse
in various parts of the hospital, a flight nurse and
later a chief flight nurse, a nurse executive, a patient
safety and quality consultant and now mainly a
volunteer. Our work can be exhausting, heartbreaking
and extremely difficult, but it can also be
energizing, encouraging and very rewarding.
Last year, we celebrated what would have been
Florence Nightingale’s 200th birthday on May 12
with a year-long celebration – the Year of the Nurse
and Midwife which has been extended into 2021.
The American Nurses Association is leading the
charge nationally with various publications, toolkits
and other promotional tools. See these key talking
points for your use:
• The aim of the “Year of the Nurse” is to raise
the visibility of the nursing profession in policy
dialogue and spur greater investment in the
development and increased capacity of the
nursing workforce;
• We rank as the most trusted professionals for
19 years consistently;
• Registered nurses are by far the largest
healthcare workforce in the US;
• We are also the largest healthcare profession
worldwide, but a huge shortfall by 2030 is
predicted, unless radical action is taken soon;
• With the aging US population, more than one
million additional nurses will be needed by 2022;
• We advocate and
innovate in our
communities,
clinics, hospitals
and throughout the
Donna Meador
healthcare system;
• We influence policy and
practice standards that impact access to quality
and affordable healthcare, and advocate for
healthcare for all;
• We must work together with educators,
employers and policy makers to expand nursing
roles and explore new, nurse-led models of
care and
• Through collective efforts to elevate
contributions of nurses and create an
imperative to invest in nursing and nurses, we
will improve the health of our state, nation and
world.
To recognize the “Year of the Nurse” here at
home, the Kentucky Nurses Association is leading
the charge. Our boards, committees, cabinets,
affiliate organizations and staff recently wrapped up
Nurses Month with fantastic messages and activities
to celebrate, improve awareness, educate nurses and
educate the public about all nurses can do. Look at
this snapshot of activities:
• The opportunity to recognize or remember
a nurse and benefit the Kentucky Nurses
Foundation through its’ Honor a Nurse,
Remember a Nurse campaign;
• “Thank You Nurses – A Cinema of Gratitude”
video series sharing heartfelt thanks from key
leaders in our state;
• A Proclamation from Governor Andy Beshear
recognizing the month of May as “Kentucky
Nurse Suicide Prevention Month” (see the video
of this proclamation being signed on the KNA
website!);
KNA Needs Volunteers to Help with COVID Testing and Vaccinations
Make a difference, lend your time and expertise to help us serve all Kentucky communities
President’s Pen continued from page 1
• Through the efforts of the KNAC (Kentucky
Nurses Action Coalition) and the KNA
Professional Nursing Practice and Advocacy
Cabinet, weekly educational sessions took place
throughout the month focusing on nurses’
mental health and suicide prevention;
• Additional educational opportunities included a
webinar hosted by the KNA Northern Chapter
on Dedicated Education Units and a webinar
hosted jointly by KNA and the KBN that focused
on nursing laws, and how to protect your license
and understand scope of practice and
The Kentucky Nurses Association needs nurse and
nursing student volunteers to help with COVID-19 testing
and vaccinations. Those interested in Louisville, may
click Volunteer Registration. To volunteer in other parts of
the state, contact your chapter leader or reach out to the
KNA at admin@kentucky-nurses.org. As vaccinations are
becoming more available to Kentuckians, remember it’s still
very important to get tested for COVID-19. Visit ky-covidtesting
for a list of testing sites throughout the state.
To date, more than two million Kentuckians have received
at least one vaccine dose.
We thank the more than 560 KNA COVID volunteers and
all nurses across the state who are lending their time and
expertise to help combat the pandemic. Keep in mind that
we provide and require PPE for all volunteers!
• Editorials appeared in newspapers throughout
the state recognizing our profession and once
again, saying “Thank You” to nurses.
So, in closing, enjoy this issue of Kentucky
Nurse. Read about student research, KNA
activities, the upcoming KNA election and
candidates and more. And make sure to thank
your colleagues for the work they do every day,
“Intersection of Social Justice and Healthcare
Movie Night 6: A Dangerous Idea: Eugenics,
Genetics and the American Dream,” 6:30 -
9:15 p.m., Thursday, June 24. At the end of this
program, participants will be able to:
• Describe the historical basis for viewing
some groups, races and individuals as
inherently superior to others and more
deserving of fundamental rights based off
of genetics;
• Discuss the use of eugenics and genetics
in the sterilization of certain populations
of women;
• Discuss the myth of “racial” health
disparities and how they are based on
falsely interpreted and employed science
and
• Discuss the importance of evaluating all
science, including nursing science, with a
clear eye and willingness to questions its
use in creating inequities throughout our
society, particularly within healthcare.
There is a dangerous idea that has
threatened the American Dream from the very
beginning. It is a strong current of biological
determinism which views some groups, races
and individuals as inherently superior to
others and more deserving of fundamental
rights. Despite the founders’ assertion that
“all are created equal,” this idea was used to
justify disenfranchising women, blacks and
Native Americans from the earliest days of the
Republic.
This film reveals how this dangerous idea
gained new traction in the 20th century with
an increasing belief in the concept of an allpowerful
“gene” that predetermines who is
worthy and who is not. The film reveals how
this new genetic determinism provided an
abhorrent rationale for state sanctioned crimes
committed against America’s poorest, most
vulnerable citizens and for violations of the
fundamental civil rights of untold millions.
Meet the Kentucky Nurses
Association Board of Directors
We hope you enjoy this continuing series,
“Meet the Kentucky Nurses Association Boards”
that regularly appears in Kentucky Nurse. KNA
leaders tirelessly volunteer their time to advocate
for fellow nurses and those in their care. Get
to know these nurses, ask them questions and
reach out. This is your professional nursing
organization:
Kathy Hager, DNP, APRN,
Kathy Hager
(KN) What made you decide to serve in a
leadership role within our organization?
(KH) I believe that our professional nursing
organization defines us and that we must actively
participate in moving the nursing profession
forward. Nurses need to be at every table that
determines the healthcare of our state and
country and at every table that defines our
nursing profession. This role extends into the
smallest community, defining population health
and the lifestyle changes that will make nurses,
Kentuckians and Americans healthy. Nurse can
do this by helping the community access healthy
food choices, promoting access to safe walk
paths, bike paths, playgrounds, neighborhoods
and most importantly, fighting for the right for
every nurse, Kentuckian and American to be
respected and enjoy mental and spiritual health.
Nurses can do this by researching the data that
leads us to these goals. Our Kentucky Nurses
Association, Kentucky Nurses Foundation and
the Kentucky Nurses Action Coalition have
these goals as their mission, the only Kentucky
organizations devoted totally to such a mission.
than the nurse who entered those doors from
the very beginning of the pandemic and then
continuing that dedication with testing and
vaccinating. Thank you, thank you, thank you!
Marsha Woodall, DNP,
Nurses Action Coalition
(KNAC)
Current Professional
Role: Nursing Program
Administrator/Division Chair
Marsha Woodall
Additional KNA Positions: president of
the KNA Pennyrile Chapter and serves on
the Finance, Nominating and Membership
Recruitment & Retention Committees.
Past KNA Positions: education role on the KNA
Professional Nurse Practice & Advocacy Cabinet
(KN) What made you decide to serve in a
leadership role within our organization?
(MW) I am a charter member of KNAC and
immediate past president of Kentucky Council
of Associate Degree Nursing (KCADN) and co-
chair of Kentucky Deans and Directors (KNDD). I
am active on several KNA committees and enjoy
serving/supporting Kentucky nurses. By serving
on the KNAC Board, I can influence and advocate
for the future of nursing by communicating with
nursing leaders around the state to share in KNA
endeavors. It is also important for me to serve on
the KNAC Board to represent Western Kentucky.
(KN) What would you say to others who
are considering leadership positions within
KNA?
(MW) This is a great way to have a voice and
support nursing throughout the Commonwealth.
(KN) What is your message to nurses on the
frontline of the COVID-19 pandemic?
(MW) I know you are weary…. I know you are
tired…. I know you may have permanent scars/
marks on your face from wearing your masks
13 hours at time…. I know you are frustrated
thinking how can I get the general population
to take COVID-19 seriously?! I have heard from
several of my former students working the
frontlines who have shared all these concerns.
Please know, I hear you and KNA hears you.
We are all putting forth effort to vaccinate and
educate the public in hopes of alleviating some of
your struggle. Take time for self-compassion and
self-love and always know you are worthy.
(KN) What would you say to others who
are considering leadership positions within
KNA?
(KH) We need every nurse to be part of our
mission. The more members we have, the more
we can do. If you are busy with family and work
now, join anyway. The more of us there are, the
more we can share the workload.
(KN) What would you like fellow nurses to
know about your KNA membership? What
does it mean to you?
(KH) It means that we can lead the way
for other nurses to find their perfect nursing
position – to provide the best care possible, to
provide mentorship for nurses moving from one
position to another and from one area of nursing
to another. We can be their bridge and support
structure.
(KN) What is your message to nurses on the
frontlines of the COVID-19 pandemic?
(KH) Thank you for putting your life on the
line every time you entered your place of work…
not knowing what lay beyond those doors…
having to protect your family from yourself. In the
beginning, you did not even know the monster
that lurked inside… there could be no one more
dedicated to the health and welfare of a patient
Student Spotlight
Nursing Care and Teaching for the Patient with a Recent STEMI
Christina DiFalco
Nursing Student, Eastern Kentucky University
Patient Description
A 62-year-old white female was admitted to the hospital
with a recent ST elevated myocardial infarction in addition
to present diagnoses of hypotension, type 2 diabetes,
hyperlipidemia, anxiety, gastroesophageal reflux disease,
history of hypertension and history of ST elevated myocardial
infarction. During this admission, the patient had received
cardiac catheterization and stenting of the lateral anterior
descending artery because of her recent myocardial infarction.
Previously, the patient went through a similar cardiac
catheterization process after a previous ST elevated myocardial
Christina DiFalco
infarction. The patient admitted to smoking and partaking in
a diet high in sweets, fats, and sodium. She also stated that she does not exercise, but
she does a lot of cleaning around her apartment and works at a nursing home doing
laundry for a living. Currently she lives alone and does not see friends or family very
often.
Pathophysiology of Admitting Diagnosis
The patient’s admitting diagnosis, an ST elevated myocardial infarction, is caused
by an occlusion in any of the major arteries of the heart. This occlusion leads to tissue
ischemia within the heart wall, causing decreased blow flow, muscle ischemia, and
ability for it to contract. In this patient, the lateral anterior descending artery was
occluded, causing her ST elevated myocardial infarction (Norris, 2019 p. 767). Lab
values tested related to her diagnosis include brain natriuretic peptide, which was
elevated at 3,200 pg/mL, and troponin, which was elevated at 4.8 ng/dl. Increased
brain natriuretic peptide often indicates fluid overload and is often elevated when the
heart is under acute stress (Norris, 2019 p. 798). Troponin levels in the blood stream
indicate damage of the heart muscle, as this protein is not released from the heart
muscle into the bloodstream unless the muscle is damaged (Norris, 2019 p. 43). Other
diagnostic tests included an electrocardiogram with evident ST elevation.
Other medical diagnoses identified include anxiety, gastroesophageal reflux disease,
hypotension, type 2 diabetes, hyperlipidemia and hypertension. Generalized anxiety
disorder is characterized by excessive worry about events and certain activities (Norris,
2019, p. 533). This disorder typically causes stress and a decrease in one’s quality of life.
Gastroesophageal reflux disease is defined as the symptoms caused by the abnormal
reflux of gastric contents into to esophagus (Norris, 2019, p. 1080). Hypotension is
classified as a blood pressure lower than 90/60. Hypertension is classified as a blood
pressure above 130/80 and puts undue stress on the heart muscles (Norris, 2019, p.
742). Type 2 diabetes is a condition that causes high glucose levels in the bloodstream
due to insulin resistance or underproduction (Norris, 2019, p. 1219). Hyperlipidemia
is the presence of high amounts of lipids within the bloodstream and can lead to
occlusion of blood vessels (Norris, 2019, p. 722). In addition to the diagnoses of type 2
diabetes, hypertension, and hyperlipidemia, the patient is a smoker and is overweight.
All of these factors combine to form something called metabolic syndrome which is a
collection of factors that put somebody at higher risk for heart attacks and other heart
problems (Norris, 2019, p. 725).
Nursing Care Required in the Acute Care Setting
At first glance, the patient seems to be in general good health. Her appearance is
clean, skin color is pink, and hair is evenly distributed. The patient is fully conscious
and aware of her surroundings. Upon the first encounter the patient was asleep but
smiled upon awakening. Her behavior is appropriate for time, place, and situation.
Her mood seems to be happy as she was very talkative and polite. The patient
is independent of all activities of daily living. Her skin had no obvious lesions or
abnormal skin discoloration other than the cardiac catheterization incision site.
Upon physical assessment it is apparent that the patient is alert and oriented
to person, place, time, and situation. No abnormal head, ears, eyes, nose, throat,
skin, neurological, musculoskeletal, or abdominal/gastrointestinal findings evident
upon assessment. The patient has verbalized previous chest, left arm and jaw
pain that has disappeared with time. Her vital signs present a blood pressure of
90/53, a respiratory rate of 18, an oxygen saturation of 96, a heart rate of 97, and
a temperature of 98.7. The patient’s blood pressure is low evidenced by her blood
pressure reading. Her lungs are clear to auscultation bilaterally with no apparent
signs of dyspnea. Auscultation of her heart revealed present S1 and S2 sounds with
no S3 sounds or pedal edema. Her point of maximum impulse is located at the 5th
intercoastal space, mid clavicular line.
Why Do You Want to Be a Nurse?
Renee Oehler
BSN Student, Thomas More University
Recipient of 2020 Kentucky Nurses Foundation
Nursing Excellence Scholarship
During their educational journey, every nursing student
is inevitably asked the question, “Why do you want to be a
nurse?” Although it maintains a sentiment of caring curiosity,
this inquiry has the potential to conjure anxiety in even the
most confident of students as they ponder the seemingly
endless career possibilities in nursing. I regularly find myself
lost in thought at the various nursing career prospects in
my future, and I thoroughly survey each individual clinical
experience in an incessant effort to uncover the one type of
nursing perfectly suited to my skill set.
This scrutinizing, fervent attitude undoubtedly presented
Renee Oehler
an obstacle as I tried to focus on the present and learn the
lessons existent in each unique clinical experience. The problematic nature of this
mindset became strikingly evident when I experienced an unforgettable moment of
inspiration while following and assisting different nurses in an emergency department
setting. I observed as they treated patients from all walks of life. I was in awe of
how the nurses prioritized and treated all patients in an orderly manner to provide
individualized care effectively and efficiently.
While these aspects provided important education, the indelible inspiration
this experience lent arose from the genuine care and compassion the nurses
demonstrated for their patients. Every single person was treated with the same
dignity and respect regardless of the circumstances of their presentation. I saw nurses
look into the eyes of every patient with a gaze of assurance, comfort, and genuine
compassion. It is this unforgettable interaction that revolutionized my perception of
nursing and reframed consideration of my future career considering others’ needs
instead of my own desires. While certain amounts of anxiety about the future remain,
I no longer wonder, “Why do I want to be a nurse?” Instead, I ponder the question,
“How can I use my nursing career to give the fullest gift of myself to others?”

Nursing Care and Teaching continued from page 18
A large factor that played into her most recent ST elevated myocardial infarction is
lack of education regarding the risks of lifestyle habits and smoking. Despite having
a very similar episode in 2005, the patient admitted to smoking, not exercising, and
eating a diet high in fats, sugar, and sodium. No changes in her lifestyle habits had
been made between her ST elevated myocardial infarctions, which indicates that
patient education is needed as a primary intervention in maintaining her health.
Several nursing diagnoses fit her case very well, but the most important one’s
deal with prevention of future episodes. After her most recent myocardial infarction,
the patient seems stable and is not at immediate risk of life-threatening harm, so
preventing another similar episode is essential as the episode itself is life threatening
(Dreyer, et al., 2020, p. 1). One diagnosis that fits this patient’s current state is
decreased cardiac tissue perfusion related to insufficient knowledge of modifiable
factors. This is evidenced by her unchanged lifestyle habits and seemingly indifferent
demeanor regarding a need to change her diet and exercise habits. For this reason,
the patient unfortunately suffered another myocardial infarction and resulting
decreased cardiac tissue perfusion that could have been prevented. In order to
reduce her risk of heart attack and promote health, one of the essential nursing
interventions is education regarding an appropriate cardiac diet that includes foods
she enjoys eating. An appropriate cardiac diet that is low in fats, sugar, and salt
is essential in maintaining a healthy heart (Wood & Gordon, 2019, p. 15). Upon
interview, the patient expressed that she enjoys southern cooking and sweets.
For many, eating is strongly based on culture and evidence shows that many are
reluctant to give up the foods that they grew up with (Sullivan, Hays, & Alexander,
2019, p.81). For this reason, her favorite recipes should be altered with the help
of the nurse so that they fit into her new cardiac diet. Information regarding how
to get in touch with a dietician to help her implement a cardiac diet should also
be offered as an educational and supportive resource. Research has shown that
collaboration with other individuals and healthcare providers has been shown
to improve results and the desire to change health practices (Sullivan, Hays, &
Alexander, 2019, p.80 -81). Another area of her lifestyle that needs to be addressed
is her exercise habits. The patient should be properly educated on the importance
of exercise and its benefits regarding the cardiovascular system. Studies have shown
that an increase in exercise results in a decrease in factors related to metabolic
syndrome, and thus better heart health (Wright et al., 2018, p. E5-E7). Like changing
one’s diet, changing one’s exercise habits can be very difficult. For this reason, her
exercise goals need to work with her schedule and be kept within her limits. Upon
interview, the patient expressed that she likes to watch TV in her free time. An
appropriate goal would be to watch one of her favorite episodes on TV while riding
a stationary bike for approximately 30 minutes 4 times a week.
In addition to insufficient knowledge regarding diet and exercise, the client also
does not seem to recognize or acknowledge the effects of her smoking. Thus,
another nursing diagnosis is ineffective health maintenance related to denial of
the effects of smoking. An appropriate nursing intervention should be to educate
the client about smoking’s effect on the heart. Smoking in and of itself is very
dangerous to overall health and has detrimental effects on the cardiovascular
system (Wood & Gordon, 2019, p. 13-14). Smoking cessation is necessary in order
to prevent her heart health from getting worse. Therefore, before discharge the
nurse should collaborate with the patient to develop a patient centered plan for
smoking cessation. This plan should include a gradual decrease in the number of
cigarettes consumed per week and at a rate that the patient is comfortable with.
Research has shown that people are more likely to follow a smoking cessation plan
with the help of others going through smoking cessation. Thus, a support group is
a good recommendation, and the information regarding how to get involved in one
should be provided (Arslanian-Engoren et al., 2015, p. 481-482).
Patient teaching regarding healthy eating, diet, and lifestyle practices is essential
to meet both the discharge and long-term goals of this patient. Outcomes for each
nursing diagnosis identified can be evaluated through vital signs and evaluating
her understanding of the plans to be implemented after discharge. Her vital signs
should be within normal limits as an appropriate goal prior to her discharge.
The patient should be able to verbalize a plan for diet and exercise and express
familiarity with the negative effects of smoking on her heart. These goals can be
further evaluated through follow up appointments with her healthcare provider as
well as a therapist and/or dietician.
Removal of CAPA-CS Requirement for APRNs Who Meet Criteria – To
ensure nurse practitioners can practice to the full extent of their education and
training to increase access to underserved populations throughout Kentucky.
Nurse practitioners fill a large gap in access to care. This will not only improve
access, but health outcomes for Kentuckians. Result: SB78 with amendment
considered; bill not passed.
School Nurse in Every School, All Day, Every Day – This initiative ensures
access to health providers for public school children throughout KY. Studies
suggest that schools with a school nurse have higher graduation rates and better
overall outcomes. Result: Not a GA bill in the 2021 session; KNA School Nurse
Task Force members are seeking Coronavirus Aid, Relief and Economic Security
(CARES) and Elementary and Secondary School Emergency Relief (ESSER) funds
to place nurses in schools for the next budgeted year.
The Kentucky Nurses Association welcomes the University
of Kentucky College of Social Work (CoSW) as a Friend of
KNA affiliate organization. Read more about CoSW:
For over 80 years, the College of Social Work (CoSW) has
Increase Access to Sexual Assault Nurse Examiners (SANE) – Sexual
Assault Nurse Examiners are specifically trained to care for victims of sexual
assault. Care by a SANE nurses results in better patient outcomes and higher
conviction rates. Sexual assault nurse examiners have become the gold standard
in care for victims of sexual assault, but Kentucky has a huge deficit which results
in a delay in care. We must ensure that victims have access to this specialized care
to improve outcomes, decrease costs, and improve the safety of our communities.
Result: Introduced, not assigned as a bill.
Evacuation of Surgical Smoke – Surgical smoke contains hundreds of
carcinogens that puts patients and medical providers at risk. The answer, that
many states have already mandated, is very simple. We must mandate the use
of smoke evacuation systems in our operating rooms. We must ensure we are
safeguarding those who save lives. Result: Gov. Beshear signed Kentucky SB
38 into law, requiring hospitals and outpatient centers to use a surgical smoke
evacuation system during any procedure that generates smoke.
Kentucky Nurses
Foundation
Highlights and
Accomplishments
Since its inception in 2017, our “Honor a
Nurse – Remember a Nurse” campaign has
brought in more than $20,073. This campaign
affords us all a great way to highlight those
special nurses in our lives. Our COVID-19 Nurses
Support Fund brought in more than $20,300 to
help nurses and nursing students. We need to
raise more funds to give more funds.
We are happy to report that we just kicked
off our 2021 Kentucky Nurses Foundation
Virtual Raffle. From chances on exclusive
packages to a “Split-the-Pot” drawing, there’s
something for everyone. Proceeds benefit
nursing education, scholarships and research
across the state. To learn more visit page 22.
Thanks to your charitable contributions, we
are providing the type of support Kentucky
nurses and students have come to depend
upon. Most recently, we have participated
in research surrounding diversity, equity and
inclusion, nurse suicide prevention and infection
control.
Last fiscal year (July 1, 2019 – June 30,
2020), we honored 36 Kentucky nurses with
Nightingale Tributes. We have temporarily
discontinued in-person Nightingale Tributes;
however, we continue to reach out to
the families of legacy nurses to offer our
condolences and let them know that we will
conduct in-person Tributes when it is safe to
do so. Later this summer, look for scholarship
applications and help us spread the word about
our scholarship program among students,
nurses and faculty in your community.
On behalf of all Kentucky nurses, I encourage
you to make a tax-deductible donation to the
Complete Kentucky Nursing Survey today –
COVID-19, other nurse stressors and intimate
partner violence; deadline July 15
Complete the Kentucky Nursing Survey today.
Conducted by Kathryn Showalter, PhD, assistant
professor, College of Social Work at the University
of Kentucky, the survey aims to understand the
stressful experiences of working while being
employed as a nurse. Two components of the
survey include a questionnaire related to COVID-19
and work-stressors that takes about 10 minutes and
the main survey with questions about relationships
and intimate partner violence for qualifying
participants that takes about 25 minutes. In total,
the survey should take about 35-45 minutes. If
you qualify for both sections, they must be taken
at once. Findings will help Kentucky healthcare
employers better support nurses during this time
and in the future.
The study will not ask for identifying information
and cannot be tracked to you. If you qualify for
participation in the first portion you will receive
one $5.00 Walmart electronic gift card upon
completion and if you qualify for both portions, you
will receive a $15.00 Walmart electronic gift card
upon completion for your time.
Please complete the survey by Thursday, July 15.
For questions and information, contact Kathryn.
