It has not yet been decided<br />

which vaccines will be used.<br />

The Joint Committee on<br />

Vaccination and Immunisation<br />

(JCVI) advise that these boosters<br />

will assist in maintaining protection<br />

against Covid-19 and new variants<br />

for those most at risk before winter<br />

comes. Vaccines are thought to<br />

protect most people for at least<br />

six months but a lack of data on<br />

exactly how long immunity lasts is<br />

prompting a safety-first approach.<br />

Professor Jonathan Van-Tam,<br />

deputy chief medical officer for<br />

England, said: “We want to be on<br />

the front foot for Covid-19 booster<br />

vaccination to keep the probability<br />

of loss of vaccine protection, due<br />

to waning immunity or variants, as<br />

low as possible - especially over<br />

the coming autumn and winter.”<br />

He also advised that other<br />

respiratory viruses, particularly<br />

flu, were likely to be an additional<br />

problem this winter.<br />

Health and Social Care Secretary,<br />

Sajid Javid, said ministers were<br />

working with the NHS to rapidly<br />

deliver the programme from<br />

September across all nations and<br />

that the booster programme will<br />

protect the freedom that has been<br />

hard fought for.<br />

The JCVI’s final advice will be<br />

published before September,<br />

when better data will be available<br />

on how long protection from the<br />

first two doses of the vaccines<br />

lasts. The latest figures on<br />

hospitalisations, emerging<br />

variants and trials will also be<br />

taken into account at that point,<br />

and could change the advice<br />

accordingly.<br />

NHS Awarded St Georges Cross<br />

for 73 Years’ Service<br />

Her Majesty the Queen has awarded the NHS with the George Cross for 73 years of dedicated service and<br />

for their collective response to the coronavirus pandemic.<br />

In response to this unprecedented honour, bestowed collectively for only the third time, NHS chief executive Sir<br />

Simon Stevens, said that the “darkness” of the last 16 months had brought out the best in health and care staff<br />

and he paid tribute to everyone in the NHS and beyond who had played their part.<br />

“This unprecedented award rightly recognises the skill<br />

and compassion and the fortitude of staff right across the<br />

National Health Service – the nurses, the paramedics,<br />

the doctors, the cleaners, the therapists, the entire<br />

team– who under the most demanding of circumstances<br />

have responded to the worst pandemic in a century and<br />

the greatest challenge this country has faced since the<br />

Second World War.<br />

“Out of those dark times have come the best of what it<br />

means to be a carer and a health professional.<br />

“In the face of adversity we have seen extraordinary<br />

teamwork, not just across the NHS but involving<br />

hundreds of thousands of volunteers, millions of carers,<br />

key workers and the British public who have played<br />

an indispensable role in helping the health service to<br />

look after many hundreds of thousands of seriously ill<br />

patients with coronavirus.<br />

“And so, as we congratulate staff across the health<br />

service on this award, we recognise that completing the<br />

NHS COVID vaccination programme which is in the final<br />

stages is now the surest way out of this pandemic and<br />

provides a sense of hope.”<br />

Two Care Provider Companies Fined and a<br />

Manager Cautioned After Employee Stabbed<br />

A Liverpool care agency, that supports people with<br />

mental health issues, its manager and a care home have<br />

been fined after an employee was stabbed by one of its<br />

residents.<br />

Liverpool Crown Court heard that on the 2 November<br />

2014 an employee of the care company was undertaking<br />

a regular scheduled visit to assist the transition of<br />

services for a resident from one site to another. During<br />

the visit, the untrained employee was left alone in the<br />

kitchen with the individual despite the care plan stating<br />

that the resident, whose capacity for violence and<br />

aggression had been clearly identified, required the<br />

attendance of two care workers at all times. Whilst the<br />

employee was unaccompanied, the resident crossed the<br />

kitchen and stabbed the employee in the right side of her<br />

neck.<br />

While the employee made a physical recovery, she has<br />

suffered post-traumatic stress disorder (PTSD) and<br />

long-term psychological trauma and is still receiving<br />

counselling.<br />

An investigation by the Health and Safety Executive<br />

(HSE) found that despite care plans and risk<br />

assessments being in place from the City Council, the<br />

NHS Mental Health Trust and care company, all of which<br />

indicated the high risk the individual posed to themselves<br />

and others, neither care company took account of these<br />

documents prior to the visits.<br />

It was also found that dedicated training and a full risk<br />

assessment and care plan for that individual were not<br />

undertaken to identify the triggers for violence and<br />

aggression, and how the risk could be managed. The<br />

need for 2:1 supervision, triggers (things not to say<br />

or do) should have been identified and copies of the<br />

documents given to employees at the new site prior to<br />

their visits. Arrangements regarding communication and<br />

supervision by the two care agencies should also have<br />

been undertaken and adequate supervision during visits<br />

provided by both care companies.<br />

HSE inspector, Rose Leese-Weller, said after the<br />

hearing: “This was a tragic and wholly avoidable incident.<br />

Those in control of work have a responsibility to devise<br />

safe methods of working and to provide the necessary<br />

information, instruction, and training to their workers.<br />

“If a suitable safe system of work had been in place<br />

prior to the incident, the individual in care may not have<br />

reacted the way they did, and life changing injuries<br />

sustained, and trauma experienced by this employee<br />

could have been prevented.”<br />

Care Provider Fined £80,00 for Failing to<br />

Prevent Harm to Vulnerable Resident<br />

A care provider pleaded guilty to failing to provide safe<br />

care and treatment following a CQC prosecution and<br />

was fined £80,000 after an 87-year-old woman suffered<br />

avoidable harm at its nursing home.<br />

The court heard how the resident, who had suffered a<br />

stroke in her own home, was admitted to the care home<br />

in August 2016 having been fitted with a catheter during<br />

her stay in hospital.<br />

On 3 September 2016, her daughter visited the home<br />

where she found her mother semi-conscious in bed, in<br />

considerable distress and calling for help.<br />

She alerted staff who found that the catheter was<br />

blocked. The resident was admitted to hospital where<br />

she sadly died the same day. Urinary sepsis associated<br />

with her catheter was cited as her cause of death.<br />

A CQC investigation found serious failings in her<br />

care, including a lack of appropriate assessment<br />

before admission, gaps in staff training and poor<br />

communication. Carers were found to have not been<br />

trained in catheter care and the provider had not<br />

adequately monitored how much fluid the resident<br />

was taking through a feeding tube or whether this<br />

corresponded with the urine collected in her urine bag.<br />

The 87-year-old’s care plan also lacked detail and<br />

accuracy, with records referring to her by the wrong<br />

name and stating she should be encouraged to drink<br />

over a litre a day when she was nil by mouth.<br />

Mary Cridge, CQC deputy chief inspector for adult<br />

social care, said: “This is a distressing case and I<br />

welcome the guilty plea.<br />

“This vulnerable, elderly lady had every right to expect<br />

safe care, but the provider failed in its specific legal<br />

duty to protect her from avoidable harm. When a<br />

provider puts people in its care at risk, we take action to<br />

hold it to account and protect people.<br />

“I hope this prosecution reminds care providers they<br />

must always take all reasonable steps to manage risks<br />

to people’s safety. Today this home is rated by the<br />

CQC as good in all areas, and we are confident that it<br />

provides safe, compassionate care to everyone who<br />

lives there.”<br />

Videos<br />

The <strong>EDGE</strong> <strong>Services</strong> trainers spent time last year<br />

filming an extensive and thorough range of people<br />

handling techniques as a resource for our moving and<br />

handling ‘Key Trainer’ qualified delegates. These were<br />

made in direct response to requests received during<br />

the spring 2020 lockdown to make a video resource<br />

that would help make distance learning for the practical<br />

techniques a more viable option. While the videos<br />

are not intended to take the place of supervision and<br />

monitoring of staff, they have proven able to reduce<br />

the amount of hands-on contact that our key trainers<br />

have been required to have while face-to-face training<br />

opportunities have been limited. Mooreover, they<br />

provide our key trainers with a useful aide-memoire of<br />

safer handling techniques.<br />

We have received an overwhelmingly positive<br />

response to these new videos and are delighted to<br />

announce that we will be filming a Module 5 at our<br />

AGM in August <strong>2021</strong>.<br />

Modules 1 - 4 increase in complexity as staff move<br />

up them and they demonstrate and explain over 50<br />

individual practical techniques including:<br />

•Getting the client ready to stand.<br />

•Sit to stand techniques.<br />

•Chair to chair transfer techniques.<br />

•Managing the slouched client.<br />

•Managing the falling client.<br />

•Assisted walking etc.<br />

•Managing the fallen client with equipment.<br />

•The use of handling slings and handling belts.<br />

•The use of turntables/rotating boards.<br />

•The use of transfer boards.<br />

•The use of rotating turners and perched<br />

transferring aids.<br />

•The use of slide sheets and in-bed turning<br />

systems.<br />

•The use of oval gliding boards.<br />

•The use of a very wide range of hoists and<br />

slings including a gantry hoist system etc.<br />

Module 5 will include practical demonstrations and<br />

explanations of more complex practical techniques<br />

(with and without equipment) including assisting clients<br />

into/out of vehicles and up/downstairs.<br />

Legal Case<br />

NHS Trust Fined for Failing to Manage<br />

Environmental Risks<br />

An NHS Foundation Trust has been fined for failing to<br />

manage environmental risks within its mental health<br />

inpatient wards.<br />

Chelmsford Crown Court heard that, between 25 October<br />

2004 and 31 March 2015, the Trust failed to effectively<br />

manage recognised risks from potential fixed ligature<br />

points in its inpatient wards which resulted in mental<br />

health patients being exposed to unacceptable and<br />

avoidable risk at a time when they were most vulnerable.<br />

Eleven inpatients died during this timeframe; their tragic<br />

deaths involved access to fixed ligature points.<br />

An investigation by the Health and Safety Executive<br />

(HSE) found that the Trust failed to adequately identify,<br />

or address with sufficient urgency, the significance of the<br />

environmental risks within its inpatient wards.<br />

The Trust pleaded guilty to breaching Section 3(1) Health<br />

and Safety at Work Act 1974, they were fined £1,500,000<br />

and ordered to pay costs of £ 86222.23.<br />

Speaking after the hearing, HSE inspector Dominic Ellis<br />

said: “It has long been recognised that a key control<br />

in the prevention of inpatient death or self-harm is<br />

the identification and removal of potential fixed points<br />

of ligature from the ward environment. For a period<br />

of more than 10 years, this Trust repeatedly failed to<br />

manage these well documented risks, including learning<br />

from tragic experience, thereby needlessly exposing<br />

vulnerable patients in its care to unacceptable risk.<br />

“I hope this case acts as a reminder to all mental health<br />

trusts of the need to continue to review their current<br />

arrangements and ensure their service users receive<br />

the protection they need at, what is often, their most<br />

vulnerable time.”<br />

Report Claims Frontline Social Care Workers<br />

Community Integrated Care’s<br />

‘Unfair to Care’ report found that<br />

the average pay for many frontline<br />

social care workers remains at<br />

around 45p below the Real Living<br />

Wage, equating to an almost £7,000<br />

disparity compared to their peers<br />

working in publicly funded sectors.<br />

The report highlights how the role<br />

of social care workers has changed<br />

beyond recognition in the past<br />

decade as the sector increasingly<br />

supports people with highly complex<br />

health and care needs and calls<br />

for the government to issue an<br />

immediate and fair pay rise to all<br />

frontline staff and to prioritise a<br />

social care workforce strategy to<br />

benchmark all roles to ensure parity<br />

of pay with other public funded<br />

sectors.<br />

Mark Adams, CEO of Community<br />

Integrated Care, said: “The moral<br />

case for investment in social care<br />

and its workforce has, sadly, been<br />

ignored for years. This research now<br />

provides cold hard facts, which surely<br />

cannot be ignored by the Government.<br />

Our research proves that in other<br />

related sectors, many frontline support<br />

workers would be getting paid at<br />

least £6,907 more per year, and<br />

almost £7,500 within the NHS. This<br />

is immoral, illogical and cannot be<br />

justified.”<br />

Oonagh Smyth, CEO of Skills for<br />

Care, said: “Community Integrated<br />

Care have brought together a wide<br />

range of data and thinking, including<br />

from our Adult Social Care Workforce<br />

Data Set, to make the clear case that<br />

our 1.5 million workforce are highly<br />

skilled professionals, who deserve to<br />

be recognised as such as we enter a<br />

period of promised reform.”<br />

Rachel Harrison, GMB National<br />

Officer, said: “Care workers need a<br />

proper living wage for the amazing<br />

work they do – and proper<br />

professionalisation of the service. We<br />

need nothing less if we want a care<br />

system worthy of the name.”<br />

A Department of Health and Social<br />

Care spokesperson said: “The social<br />

care sector has been an essential<br />

and valued part of the front line<br />

response to the pandemic. We have<br />

sought to protect the workforce<br />

and those receiving social care,<br />

providing over £2 billion for the<br />

sector, including infection prevention<br />

and control measures, free PPE and<br />

regular testing, and we prioritised<br />

staff for the vaccine.<br />

“We are committed to the sustainable<br />

improvement of the adult social<br />

care system and, as set out in the<br />

Queen’s Speech, we will bring<br />

forward proposals later this year to<br />

ensure every person receives the<br />

care they need, provided with the<br />

dignity they deserve.”<br />

At least 75,000 people in England Wait Six Months for<br />

Social Care Assessment<br />

Council care leaders have revealed<br />

that almost a quarter of a million<br />

vulnerable people in England are on<br />

adult social care “waiting lists” for<br />

up to six months, at risk of physical<br />

deterioration, abuse or neglect while<br />

they wait to be assessed after contacting<br />

their local authority for help.<br />

Stephen Chandler, president of The<br />

Association of Directors of Adult<br />

Social <strong>Services</strong> (Adass), said: “I don’t<br />

think a waiting list is acceptable in<br />

the NHS, and I certainly don’t think it<br />

is acceptable in adult social care. If<br />

we are going to have a system that<br />

we are proud of for our nearest and<br />

dearest, we should be creating one<br />

that does not rely on waiting lists as a<br />

way of managing the level of need in<br />

the system.”<br />

Chandler, who is the director of adult<br />

social care at Oxfordshire County<br />

Council, called on the government<br />

to urgently publish its promised but<br />

long-delayed plans for the reform of<br />

social care funding. Failure to do so<br />

would ensure “a grinding continuation<br />

of decline” in council services. He<br />

added: “We do not have time on our<br />

side.”<br />

Heads of care services said they<br />

believed the emergence of the waiting<br />

lists reflected a deluge of requests for<br />

support as the UK opened up after<br />

the pandemic and followed the 10<br />

years of financial cuts that had taken<br />

£8bn from care budgets and left care<br />

departments overstretched.<br />

Boris Johnson said he had a “clear<br />

plan” to reform social care when he<br />

became prime minister but has yet<br />

to deliver and, in not doing so, has<br />

added to 20 years of unmet promises<br />

from successive government’s<br />

to overhaul care funding. Proposals<br />

are not expected until the autumn<br />

spending review.<br />

This is an exciting opportunity to further develop skills by<br />

providing adult and children manual handling training to<br />

delegates from a wide variety of work place settings. The<br />

successful candidate will provide both 4 day and 2 day<br />

training events UK wide, therefore several nights will be<br />

spent away from home in accommodation provided and<br />

In addition to a salary of £30,500 - £32,500 (pro rata)<br />

New Health and Care Bill<br />

for England<br />

A new Health and Care Bill, designed in response to<br />

changes set out by the government in February <strong>2021</strong><br />

to improve oversight and accountability in the delivery<br />

of social care services, was laid out in Parliament on 6<br />

July <strong>2021</strong>.<br />

Implementing changes set out by the government<br />

in February <strong>2021</strong> its introduction follows extensive<br />

discussions with NHS England, the Local Government<br />

Association and the health and care sector and will<br />

ensure that each part of England has an Integrated Care<br />

Board and an Integrated Care Partnership responsible<br />

for bringing together local NHS and local government and<br />

delivering joined up care.<br />

Health and Social Care Secretary Sajid Javid said: “The<br />

astonishing response of our health and care services<br />

to the Covid-19 pandemic has hit fast-forward on<br />

some of the bold changes the NHS set out to deliver<br />

in its Long-Term Plan and shone the spotlight on other<br />

areas that require change to achieve better care for our<br />

communities.<br />

“To help meet demand, build a better health service, and<br />

bust the backlog, we need to back the NHS and embed<br />

lessons learned from the pandemic. This will support<br />

our health and care services to be more integrated and<br />

innovative so the NHS can deliver for people in the<br />

decades to come.”<br />

The COVID-19 pandemic has reinforced the need for<br />

services to collaborate more closely so as to be able to<br />

deliver improved care outcomes. In recent years, the<br />

existing legal framework has made this kind of joined<br />

up working difficult to undertake. It is hoped, however,<br />

that this bill will both enable and empower clinicians,<br />

carers, and experts to work collaboratively in a more<br />

effective way.<br />

The Bill builds on the proposals for legislative change<br />

set out by NHS England in its long-term plan, while also<br />

incorporating valuable lessons learnt from the pandemic<br />

that will benefit both staff and patients. Its intention is<br />

to dispose of unnecessary bureaucracy so that health<br />

and care staff can focus on patients, as opposed to<br />

paperwork, and ensure the system is able to flex to<br />

changing needs in the years to come.<br />

Sir Simon Stevens, NHS chief executive: “This Bill<br />

contains widely supported proposals for integrated care,<br />

which have been developed and consulted on over recent<br />

years by the NHS itself. They go with the grain of what<br />

our staff and patients can see is needed, by removing<br />

outdated and bureaucratic legal barriers to joined<br />

up working between GPs, hospitals, and community<br />

services.<br />

“In doing so, these pragmatic reforms build on the<br />

sensible and practical changes already well underway<br />

right across the NHS. And by enabling mutual support<br />

between different parts of the local health and care<br />

services they will undoubtedly both help tackle health<br />

inequalities and speed the recovery of care disrupted by<br />

the covid pandemic.”<br />

How to Measure Manual Handling Training<br />

Effectiveness<br />

As you deliver manual handling<br />

training to your team of staff within<br />

your organisation then you probably<br />

know how important it is to measure<br />

its effectiveness. This enables you<br />

to ensure that you are not spending<br />

money or time on training that does<br />

not provide a good return.<br />

This is where the ‘Kirkpatrick Four-<br />

Level Training Evaluation Model’<br />

can assist you to objectively analyse<br />

the effectiveness and impact of your<br />

training sessions. Donald Kirkpatrick,<br />

Professor Emeritus at the University<br />

of Wisconsin and past president of<br />

American Society for Training and<br />

Development (ASTD), first published<br />

his Four-Level Training Evaluation<br />

Model in 1959, in the US Training<br />

and Development Journal. The model<br />

was then updated in 1975 and again<br />

1994 when he published his bestknown<br />

work, ‘Evaluating Training<br />

Programs’.<br />

The four levels are:<br />

Level 1 - Reaction<br />

This level measures how your<br />

trainees (the people being trained)<br />

reacted to the training. Was this<br />

a valuable training experience for<br />

them? Did they feel good about<br />

the topic? Did they feel good about<br />

the presenter and/or the style of<br />

presenting? The course materials<br />

and/ or the training venue? Usually,<br />

this is captured in a reasonably<br />

detailed course evaluation document<br />

as the training concludes. These<br />

are important measures to assess<br />

what worked or did not work for the<br />

trainees. Course evaluations should<br />

be read thoroughly by the trainer<br />

and, where possible, acted upon to<br />

ensure future training events have a<br />

positive reaction from trainees.<br />

Level 2 - Learning<br />

This is where you measure how<br />

much of your trainees’ knowledge<br />

was increased as a result of the<br />

training. When you planned the<br />

training program you hopefully<br />

started with a list of specific learning<br />

objectives. It may be that you wanted<br />

to change levels of knowledge, skills<br />

or attitude as a result of your training.<br />

it is important to measure these to<br />

ensure you know what your trainees<br />

are learning and what they are not<br />

learning. This links to the next level.<br />

Level 3 - Behaviour<br />

At this level you can evaluate how<br />

far your trainees have changed their<br />

workplace behaviours based on the<br />

training that they have received.<br />

Specifically, this considers how<br />

trainees apply the information. From<br />

a manual handling point of view are<br />

trainees using handling equipment<br />

as you advised them to? Are risk<br />

assessments being adhered to? Are<br />

trainees using handling techniques<br />

as you advised? Obviously,<br />

conditions must be conducive to<br />

enable a behaviour change. Staff<br />

will need the support of their line<br />

manager/s. Policies and procedures<br />

must also support changed<br />

behaviour.<br />

Level 4 – Results<br />

At this level you analyse the final<br />

results of your training sessions.<br />

Can training actually be translated<br />

into tangible benefits for the<br />

organisation? Have handling<br />

techniques and procedures across<br />

the organisation improved? Are there<br />

fewer handling accidents/incidents<br />

and has this lowered insurance<br />

premiums? Do we have improved<br />

staff moral and reduced stress? Are<br />

risk assessments thorough, regularly<br />

reviewed and acted upon across all<br />

levels and have these reduced legal<br />

costs and/or compensation cases?<br />

Does the organisation have an<br />

enhanced reputation and are there<br />

fewer relative complaints? Are staff<br />

more highly skilled and is there less<br />

staff turnover?<br />

These are the metrics that really<br />

have an impact on the ‘bottom<br />

line’ as training must make a real<br />

difference to quality performance and<br />

staff competence. It must be proven<br />

to deliver results perhaps in line with<br />

corporate key performance indicators<br />

or KPIs and be cost effective at<br />

the same time. In conclusion, if<br />

we now accept that the majority of<br />

organisations carry out training to<br />

change behaviour of the workforce<br />

and improve staff performance and<br />

staff safety, we must accept that this<br />

cannot be achieved without a clear<br />

plan and strategy.<br />

This plan can then be revisited at<br />

the conclusion of training events to<br />

assess the effectiveness using the<br />

aforementioned KPIs. An example<br />

might be a KPI to reduce staff<br />

sickness and absence because of<br />

manual handling activities by 10%<br />

over one year. Say this equated to a<br />

monetary value to the organisation<br />

of £12,000. If say the cost of the<br />

training package (including staff time<br />

out of the workplace etc) cost £8,000<br />

that is a direct saving of £4,000.<br />

With a system in place that was<br />

able to measure and evaluate this<br />

process there is a tangible correlation<br />

between the investment (time and<br />

money) into training programs and<br />

the benefit to the organisation.<br />
