higher risk of severe illness if they get COVID-19. Those over 70 should discuss their participation in<br />
recreational programs with their health care provider to understand their personal risk and act accordingly.<br />
In-Person Activities<br />
We encourage all Activity Groups to submit their return-to-play plans to the Program Committee as soon<br />
they are ready to start. The GWSA's waiver and vaccine requirement applies wherever in-person activities<br />
happen, such as meeting rooms and in the auditorium.<br />
An In-person Play ACCESS card will be provided to each member who completed the GWSA<br />
COVID-19 Liability Waiver and provided Proof of COVID-19 Vaccination.<br />
Note: The Evergreen Centre is open to the public. The City will maintain the facility consistent with<br />
current public health requirements, which may include COVID-19 screening, contact tracing, monitoring<br />
capacity limits, etc. The City does not collect vaccination information or require a waiver. Members may<br />
encounter unvaccinated individuals in the public space, including hallways and washrooms.<br />
Proof of COVID-19 Vaccination<br />
The GWSA requires Proof of COVID-19 Vaccination for in-person activities because:<br />
The GWSA serves a population that is inherently vulnerable to COVID-19.<br />
GWSA members have expressed strong support for a vaccination policy.<br />
Public Health advises individuals in our demographic to receive the vaccine.<br />
The decision to receive vaccinations is a personal one, and we shall continue to offer virtual activities to<br />
accommodate those who remain unvaccinated.<br />
Members wishing to participate in the in-person activities will be asked to show or provide Proof of<br />
COVID-19 Vaccination but the GWSA will not hold a copy of the document. A virtual copy may be stored<br />
securely with DocuSign for those submitting online.<br />

How to Get Your Official Ontario COVID-19 Vaccination Receipt<br />
To get the information, you first need to check the box to accept the 'terms of use' and have a green<br />
(photo) Ontario Health Card. If you have the older red and white card, you can call the Provincial Vaccine<br />
Booking Line at 1-833-943-3900.<br />

How to Add Your Out-of- Province Vaccinations<br />
Submit Out-of-Province Vaccination Information<br />
Use this to send your out-of-province vaccination documents to WDG Public Health, so they can update<br />
your Ontario vaccination record. It may take several days to be validated then included on your Ontario<br />
COVID-19 Vaccination Receipt.<br />
COVID-19 Liability Waiver<br />
The GWSA decided to require a COVID-19 Liability Waiver for in-person activities due to three<br />
considerations:<br />
Due diligence regarding the known or unknown outcomes to members and the organization from<br />
possible exposure to COVID-19.<br />
The limitations in coverage of the GWSA’s liability insurance.<br />
The GWSA population’s inherent vulnerability to COVID-19.<br />
The COVID-19 Liability Waiver includes:<br />

A promise to refrain from in-person activities if experiencing any COVID-19 symptoms.<br />
The COVID-19 Liability Waiver provides important legal protection to each participant and volunteer<br />
from unknown outcomes that may occur from COVID-19. This path allows us to return to in-person<br />
activities sooner, for those who understand and accept the unknown outcomes.<br />
The GWSA’s General Commercial Liability Insurance policy remains in effect to cover situations that are<br />
not associated with COVID-19.<br />
We revised and improved the COVID-19 Liability Waiver in August <strong>2021</strong> in response to member<br />
