Case Reports TNI ® 20 oxy / TNI ® 20s oxy 56 Sex: male Age: 72 years Therapy Diagnosis: 1. Bronchial – CA re OL, T2 N2 Mx, inoperative due to 2. severe COPD with respiratory global insufficiency Problem: severe, especially night-time dyspnea, frequent nocturnal alarm of the medical emergency service. Rest – BGA: O 2 : 52 mm Hg CO 2 : 48 mm Hg Furthermore phase night-time desaturation to minimum 65 % Diagnostics Rest – BGA below 2,5l O 2 : O 2 : 59 mm Hg CO 2 : 56 mm Hg, thus due to CO 2 retention O 2 – supply not possible via nasal applicator. Previous therapy form/values: BIPAP therapy with O 2 supply for subjective intolerance and significantly overinflated lungs failed (FEV1: o,7 l; ITGV 180 % plan, TLC 160 % plan) TNI ® 20 oxy therapy/values: 2l/min O 2 and 16l/min room air: rest – BGA: O 2 : 65 mm Hg CO 2 : 43 mm Hg Comment: Subjectively free of complaints, nocturnal continued sleep possible, night-time minimum saturation: 85 % (versus 65 % before therapy) Sex: male Age: 72 years Diagnosis: 1. Central sleep-related respiratory disorders ( AHI 40 per hour ) for 2. Morbus parkinson 3. condition after apoplectic insult Previous therapy form/values: CPAP / BIPAP: therapy not possible due to mask intolerance, O 2 : Insufflation per nasal applicator (2l and 4l) without effect on AHI TNI ® 20 oxy therapy/values: 2l/min O 2 and 18l/min room air: AHI 11 per hour and significant improvement of the diurnal symptoms.
Case Reports Children TNI ® 20 Case report 1-year child: Successful breathing support of a previously early birth with nasal insufflation Therapy Diagnostics Summary of the case example: The patient was a one year old boy, born prematurely in the 27th week, with multiple disorders: acute respiratory insufficiency, epilepsy, infantile cerebral paresis, tetra spastic, retinopathy, BDP, supply of a gastrostroma and bronchopneumonia. He was admitted as an inpatient to the ward with a feverish pulmonary infection with a significantly obstructive component. With copious tracheobronchial secretions, he exhibited an acute respiratory insufficiency with hypoxemia and hypercapnia. An increasing deterioration of the pulmonary situation with an oxygen desaturation between 9% and 25% (determined by a monitor) could not be prevented despite intensive suctioning off of secretion and supply of oxygen. Thereafter the nasal insufflation was applied with sedation with an initial flow of 10-12 l/min of medical air and additionally 3-4 l/min of oxygen over 24 hours. After completed stabilization, the flow could be reduced to 8 l/min air and 1-2 l/min oxygen. The therapy was applied for several days until significant improvement of the pulmonary situation.
