light_16 The patient presented due to the mobile anterior teeth in the maxilla. The clinical examination revealed a tooth mobility of class 3 for the teeth 12, 11, 21, 22. The radiological findings showed severe root re- sorption and extended periodontal gaps, presumably due to the repeated apicoectomies. A preservation of the affected teeth was impossible and we decided to extract them followed by an immediate implant placement. According to our initial protocol, immediate non-functional loading at an adequte insertion torque of > 30 Ncm was planned additionally. After atraumatic tooth extraction and removal of the granulation tissue the alveolar socket was inspected very carefully in using a probe. To place implants immediately, an unscathed buccal lamella is mandatory. Subsequently, implant site preparation was performed with the drills in their sequence of use. However, the implant axes were angulated towards palatal to avoid trauma of the oral bone plate. After the preparation procedure was completed, 4 XiVE CELLplus implants (D 3.8) were placed. Additional cooling was avoided in order to preserve the primarily adapted blood proteins for bone regeneration (figs. 1, 2). Since contrary to the initial protocol, the insertion torque was less than 30 Ncm, the option of immediate function was abandoned. Standard gingiva formers were placed and the patient was temporarily restored with an adhesive Maryland bridge (figs. 3, 4). _1 _2 _3 _6 _6a _7 _8

After 8 weeks the standard gingiva formers were replaced by anatomical shaped abutments based on FRIADENT EstetiCaps. This naturally shaped gingiva former allows for an expansion of the previously narrowed and round-shaped emergence profile (figs. 5, 6). The healing period was completed after three months and the final resto- ration was produced. Based on the prefabricated anatomically shaped FRIADENT ® CERCON ® abutments completely veneered zirconium oxide caps (CERCON ® Smart Ceramics, DeguDent) were fabricated (figs. 7, 8, 9). The crowns were fixed in using composite-reinforced glassionomer cement (fig. 10). _4 Esthetics and function were successfully restored due to ideal material properties. The beauty of the crowns matches the soft tissue architecture ideally. Based on the positive biological characteristics of CERCON ® , long-term success can be guaranteed!
