Managing Director,
Edge Services
Hello and welcome to our Summer 2024 newsletter.
I’d like to start with our recent success at The Royal Society for the Prevention of Accidents
(RoSPA) qualification awards. Everybody at EDGE Services take a bow, just WOW....to achieve two
nominations at the national awards held on board HMS Belfast in London is an amazing achievement.
Furthermore being awarded Highly Commended and Commended in our categories, especially
after being awarded the First Class Instructor-Led Provider of the Year last year demonstrates the
dedication, quality and hard work the team continuously put in both at the head office in York and
nationally at all our training venues.
We are all extremely proud of the EDGE team who continuously strive to give the best service to our
delegates and clients.
In February we were absolutely thrilled to have been award Silver Accreditation by Investors in People
following an assessment at the end of 2023. The assessment was a great opportunity to continue to
develop and we are now excited to confirm that EDGE Services Ltd has been identified as one of the
Top 20 Silver Investors in People accredited employers in the UK.
This is why EDGE Services continues to be one of the leading national training organisations within its
field and the reason we want to continue developing new and advanced training opportunities.
We have an exciting time ahead of us especially as we are now settled into our new head office in
York. The full team will shortly be taking part in our annual week-long summer meeting, during which
there will be opportunity for continuing professional development, team building and chance for us all
to catch up and enjoy some time together.
Thank you for reading, and for your continued support. We looking forward to delivering to all our
clients for many years to come.
CQC REPORT
An independent review of the CQC has identified
significant internal failings, revealing its struggle to
identify poor performance at care homes.
The interim report, led by Dr Penny Dash, chair of
the North West London Integrated Care Board, found
inspection levels were still well below where they were
pre-Covid, a lack of clinical expertise among inspectors,
a lack of consistency in assessments and problems with
the CQC’s IT system.
These failings entail that the CQC is unable to
consistently and effectively judge the quality of health
and care services, including those in need of urgent
improvement.
The Dr Dash’s report also found that social care
providers are waiting too long for their registration and
rating to be updated, with implications for local capacity.
The government is set to take immediate steps to restore
public confidence in the effectiveness of health and
social care regulation.
This includes increasing the level of oversight of the
CQC, ahead of a full report by Dr Dash which will be
published in the autumn.
This works forms part of the government’s wider efforts
to identify the challenges facing the NHS and take action
to address them head on as part of its mission to build a
health service fit for the future.
Health and Social Care Secretary, Wes Streeting,
said: “When I joined the department, it was already clear
that the NHS was broken and the social care system in
crisis.
“But I have been stunned by the extent of the failings
of the institution that is supposed to identify and act on
failings. It’s clear to me the CQC is not fit for purpose.
“We cannot wait to act on these findings, so I have
ordered the publication of this interim report so action
can begin immediately to improve regulation and ensure
transparency for patients.
“I know this will be a worrying development for patients
and families who rely on CQC assessments when
making choices about their care.
“I want to reassure them that I am determined to grip
this crisis and give people the confidence that the care
they’re receiving has been assessed. This government
will never turn a blind eye to failure.”
Streeting has also announced four steps the government
and CQC will take to restore public confidence in the
regulator and ensure patients can get an accurate picture
of the quality of care available.
These include:
• The appointment by the CQC of Professor Sir Mike
Richards to review CQC assessment frameworks. Sir
Mike was a hospital physician for more than 20 years
and became the CQC’s first Chief Inspector of Hospitals
in 2013, retiring from this role in 2017.
• Improving transparency in terms of how the CQC
determines its ratings for health and social care providers
• Increased government oversight of the CQC, with
the CQC regularly updating the department on progress,
to ensure that the recommendations in Dr Dash’s final
review are implemented
• Asking Dr Dash to review the effectiveness of all
patient safety organisations.
Dr Dash was asked to carry out a review of the CQC in
May 2024. Over the last two months, she has spoken to
around 200 senior managers, caregivers, and clinicians
working across the health and care sector, along with
over 50 senior managers and national professional
advisors at the CQC.
Some of Dr Dash’s emerging findings include:
• Of the locations the CQC has the power to inspect, it
is estimated that around one in five have never received
a rating
• Some organisations not being reinspected for several
years – with the oldest rating for an NHS hospital dating
from over 10 years ago and the oldest rating for a social
care provider dating from 2015
• A lack of experience among some inspectors – with
the review hearing of inspectors visiting hospitals and
saying they had never been in a hospital before and an
inspector of a care home who’d never met a person with
dementia.
Commenting on her findings so far, Dr Dash said: “The
contents of my interim report underscore the urgent need
for comprehensive reform within the CQC.
“By addressing these failings together, we can enhance
the regulator’s ability to inspect and rate the safety and
quality of health and social care services across England.
“Our ultimate goal is to build a robust, effective regulator
that can support a sustainable and high-performing
NHS and social care system which the general public
deserves.
To start to rebuild its credibility, Dr Dash has highlighted
urgent actions the CQC can take, including overhauling
the inspection and assessment system, rapidly improving
operational performance and fixing faltering IT systems.
In response, Kate Terroni, interim chief executive of
the CQC, said: “We accept in full the findings and
recommendations in this interim review, which identifies
clear areas where improvement is urgently needed.
“Many of these align with areas we have prioritised as part
of our work to restore trust with the public and providers by
listening better, working together more collaboratively and
being honest about what we’ve got wrong.
“We are working at pace and in
consultation with our stakeholders
to rebuild that trust and become
the strong, credible, and effective
regulator of health and care
services that the public and
providers need and deserve.
“Work is underway to improve how
we’re using our new regulatory
approach. We’ve committed
to increasing the number of
inspections we are doing so that
the public have an up-to-date
understanding of quality and
providers are able to demonstrate
improvement.
“We’re increasing the number of
people working in registration so we
can improve waiting times.
“We’re working to fix and improve
our provider portal, and this time
we’ll be listening to providers
and to our colleagues about the
improvements that are needed
and how we can design solutions
together.
“We’ll be working with people who
use services and providers to
develop a shared definition of what
good care looks like.
“And we’re also developing a
new approach to relationship
management that enables a closer
and more consistent contact point
for providers.
“Additionally, to strengthen our
senior level healthcare expertise,
we have appointed Professor
Sir Mike Richards to conduct a
targeted review of how the single
assessment framework is currently
working for NHS trusts and where
we can make improvements.
“Sir Mike’s career as a senior
clinician, and a distinguished leader
of high-profile national reviews,
as well as his direct experience
of driving improvement through
regulation, make him uniquely
placed to conduct this work.”
The King’s Speech
July 2024
The King’s speech was delivered in parliament on 17 July, setting out the
new UK government’s priorities for the coming parliamentary session. A total
of 40 bills were announced, with planned changes across health and care
services.
A crucial piece of legislation will be the Employment Rights Bill that
underpins the commitments made in Labour’s New Deal for Working
People. It will update trade union legislation, removing unnecessary
restrictions on trade union activity, including the previous government’s
approach to minimum service levels, which severely limited workers’
freedom to strike.
The Mental Health Bill aims to modernise the Mental Health Act to make it
fit for the 21st century. The government also specified mental health support
for young people.
The draft Conversion Practices Bill seeks to bring forward a full, transinclusive
ban on conversion practices relating to both sexual orientation and
gender identity.
While there has been positive forward focus on reforming and rebuilding
health care, efforts to stop current unsustainable practices such as corridor
care and the exploitation of social care staff failed to make an appearance.
In response, The Royal College of Nursing General Secretary and Chief
Executive Professor Nicola Ranger, said: “Nursing staff are expecting
a period of renewal for our health and care services and a reset with
government – today’s policy programme is a move towards delivering that.
We particularly welcome the new government’s commitment to repealing
anti-trade union legislation, which silences NHS and care workers from
speaking up for themselves and their patients.
“The New Deal for Working People and the potential for a historic fair pay
agreement in social care can help begin the journey to pay parity with
NHS colleagues. The government must honour its pledge to launch an
investigation into the exploitation of migrant care workers across the sector,
which we will support.
“Reforms to the Mental Health Act are long overdue and could improve care
for vulnerable people, raising the focus given to mental health and giving
patients more of a say in their treatment – but investment in mental health
nursing is needed to boost workforce numbers and transform outcomes.
“The new government is right to focus on our ailing NHS, but measures to
increase recruitment into nursing, that are not legislative and not present
today, should follow soon. The NHS Long Term Workforce Plan is off target,
and it needs fresh political energy and investment. Applications to study
nursing have collapsed, and ministers should introduce government-funded
degrees in the interests of patient safety.
“A corridor care national emergency is claiming lives and requires
immediate government intervention. The first step must be to introduce
mandatory reporting to reveal the extent of hospital overcrowding. Ministers
must also work with clinical leaders at the RCN to look at the evidence that
supports the introduction of safety-critical nurse-patient ratios.”
The evolution of
‘Controversial
Techniques’
When I started my nursing training back in 1977, patient
handling techniques were vastly different from those
taught today, all the ‘lifts’ that I was taught and practised
well into the 1980’s are now considered ‘controversial
‘or ‘poor handling’, only to be used in life threatening,
emergency or exceptional circumstances, when safer
techniques are not possible. Some of these, such as the
drag lift, were proved to be unsafe and condemned as far
back as 1981.
Terminology has changed over
the years, in 2001 The Derbyshire
Interagency Group referred to the
drag lift as ‘high risk’. In 2005
we saw a further change when it
was referred to as ‘controversial’
in HOP5. Now in HOP7, I see
this, and other high risk handling
methods are documented as
controversial or poor handling
techniques.
In our training courses and course
material we give examples of
several controversial techniques,
these being drag lift, front assisted
lift, orthodox lift, through arm lift,
arm and leg lug and bear hug/pivot
transfer. We used to include the
The Drag Lift
Front Assisted Lift
Through-Arm Lift
The Bear Hug/Pivot Transfer
(Arms Around Care Worker’s Neck)
Australian and poles and canvas lifts but slimmed our list
down because we felt these were no longer commonly
seen.
There are others to be aware of, such as linking arms
in supported walking, which puts the care worker at
Chapter 7 Controversial Techniques
considerable risk should the client stumble. As such this
should be discouraged for clients who require physical
support.
CONTROVERSIAL TECHNIQUES
103
Grabbing hold of waistbands to lift the client up, and
incorrect use of handling belts when care workers
use them to lift, are also considered as poor handling
techniques. Handling belts need to be correctly fitted,
The Drag Lift
Orthodox Lift
Arm and Leg Lug/Hammock Transfer
The Bear Hug/Pivot Transfer
(Arms Around Care Worker’s Waist)
An example of the training materials used during
EDGE Services’ training.
Chapter 7 Controversial Techniques
usually securely around the
client’s waist, and the care
worker needs to ensure they do
not lift otherwise it can rise and
put pressure on the shoulder
joint, like a drag lift. Also, their
hand hold needs to be one that
enables easy release from the
grip, in the event the client falls,
as handling belts should not
be used to support the client’s
weight. This would result in
considerable musculoskeletal
forces for the care worker.
Risks to both client and
care worker are well known
and evidenced in various
professional publications. If we
look at the drag lift for instance,
we can see this increases
strain on the back, as it has been identified as resulting
in maximum loading to the back. Looking at risks to the
client, their shoulder joint is particularly vulnerable with
risk of dislocation, fractures and nerve damage as well as
force related tissue damage.
Any of the controversial techniques mentioned
will impact hugely on the client’s ability to move
independently as normal patterns of movement
are impeded.
We have known for a long time the dangers,
to both client and care worker, that the use of
controversial techniques create. Through training
of safer methods, where staff understand the
underlying principles and are aware of the risks
from using these controversial techniques,
ongoing supervision, and with advances in
equipment choices over the years, there really
should be no reason for care workers to resort
to these controversial technique unless it is in
an emergency, or when it has, through robust
risk assessment involving a multi-disciplinary
approach shown that no safer method would
work. This would be an exceptional circumstance
and rationale for it’s use would have to be clearly
documented, and evidence shown as to how safer
methods of handling have failed.
Employers have legislative responsibilities
to ensure staff are suitably trained and that
appropriate equipment is provided as far as is
reasonably practicable, and that suitable and
sufficient risk assessment are carried out by a
competent person. One can hope that at some
point controversial techniques will be a thing of the
past in normal day to day people handling.
