Anyone with serious heart and lung disease knows how overwhelming it can be. Panicked trips to 

the ER. The anxiety of mounting bills. The stress of just getting through the day. 

For advanced cardiac and pulmonary conditions, the most expensive care is often the least effective. A better 

solution – both to lower costs and to improve quality of life – is to care for patients at home. That’s why we 

created a special Hospice program for heart and lung patients. 

Optimum Care provides personalized, comprehensive care with an expert team of physicians, nurses, 

social workers, chaplains, aides and volunteers. Patients learn what to expect and how to deal with their 

condition. With team members visiting regularly, it’s easier to notice changes and make adjustments. 

With nurses available by phone around the clock, it’s easy to get answers and advice. Health care comes 

right to the home. 

With Optimum Care,patients are more comfortable. Families are relieved. Care is more customized. 

And costs are lower. It’s an excellent model of care.

A Success Story of Home Care for Heart Failure

Ipeeked in the exam room to find Shirley radiant and well dressed, flanked by 

her proud daughter. We congratulated each other on Shirley’s near-miraculous 

turnaround from what we all thought was “end-stage” heart disease. 

I recalled a much younger woman who first came to me in 1987 because of 

chest pains due to heart artery spasms. Since then, Shirley and I have navigated 

several challenging medical crises, culminating last year in a string of harrowing 

hospitalizations for severe heart failure. Out of frustration from her lack of improvement, 

we opted last fall to start Hospice Buffalo home care, hoping 

for less distress in her remaining days with her family nearby. 

I favored this approach because of my recent experience with a dozen of my 

sickest heart patients who were treated kindly and effectively by Hospice 

Buffalo in their homes. For two thirds, the comfort they sought came with 

the daily attention provided by Hospice professionals. But more remarkably, 

the remaining third gained so much strength that they ventured out 

to resume former outside activities.

Hope for better care 

before the end of life

Home Connections makes all the benefits of compassionate home-based care available long before 

Hospice is appropriate. We created the program to help people with advanced, progressive illnesses – 

heart failure, COPD, ALS, cancer and others – live safely at home. Working with a patient’s own physician, 

Home Connections assigns a team to coordinate care. Nurses visit frequently, and are on call 24 hours 

a day. Palliative care physicians also visit patients to get symptoms under control. Social workers connect 

families with community support services and help patients make decisions about their care. 

One patient, for example, has learned to manage her condition so well that she has actually “graduated” 

from our program. Before Home Connections, she had been rushed to the ER four times and hospitalized 

three times, all in just three months. 

Simple and cost-effective, Home Connections is succeeding remarkably in reducing ER visits and hospital 

stays, and increasing the number of patients with advance directives.

A Success Story… 

Over a 35-year medicine career, I’ve seen hospitals take over the care 

of chronically sick heart patients, presuming this was the 

best approach. But more recently, I’ve seen hospital costs rise alarmingly 

without any increase in the quality of outcomes. Patients are 

frustrated by stressful and expensive hospital stays. I now believe quality health 

care at reasonable cost requires at least some home-based care 

for patients with chronic diseases. 

Heart failure has become the most common cause for repeated 

hospitalizations, costing billions yearly. Chronic heart problems are 

particularly amenable to home care because symptoms such as shortness 

of breath and fluid retention are readily managed at home with simple 

treatments. Because heart failure is unpredictable, it’s better handled by 

a quick response team on call for home visits, rather than by hectic trips to 

hospital emergency departments. Of course, home care should come into play 

only after hospital treatments such as angioplasty, bypass surgery, 

and pacemakers have been fully utilized. Continued next page...

Often called comfort care, palliative care is really much more. It’s interdisciplinary care that considers 

the whole person – not only physical needs, but also emotional, spiritual, psychological and social 

well-being. This describes Hospice perfectly, but we believe the approach has value throughout life. 

Our hope of spreading the philosophy has inspired the Palliative Care Institute. Its mission is to establish 

excellence in education, research and clinical practice. 

The key to advancing palliative care is to educate both current and future health care professionals. We’re 

partnering with the University at Buffalo to make palliative care part of the curriculum for physicians, nurses, 

social workers, pharmacists and attorneys, and plan to collaborate with other colleges and universities as well. 

A deeper, more holistic approach will enable professionals to communicate with patients more clearly, treat 

their pain and symptoms more effectively, and help them make sound decisions about their care. 

As health care practice becomes more evidence-based, the need for research is growing ever more critical. 

We are already participating in projects to advance the body of knowledge and promote best practices in 

palliative and end-of-life care.

I’ve noted patients are reluctant to accept Hospice care, probably because of 

its connection with incurable cancer. But every one of 

my heart patients treated by Hospice Buffalo reported a favorable, 

even an uplifting experience. 

I’ve made a study of why Hospice care works so well. It starts with their 

convincing optimism and unconditional commitment to a patient’s well-being. 

I see an emphasis on what really counts: symptom relief, anxiety control, better 

nutrition, restful sleep, and family closeness. Hospice professionals have a 

knack for harnessing a patient’s own grit and determination, while amplifying the 

assistance of family and friends. 

I’ve also noticed a wide spectrum of expertise from Hospice doctors, nurse practitioners, 

nurses, therapists, nutritionists, and clergy. Their success comes from 

routine daily contact, punctuated by timely adjustments in 

treatments, keeping patients symptom-free and away from hospitals. I’ve noted 

how they minimize personnel changes to avoid care lapses, resulting in greater 

patient confidence. Team members appear motivated more by 

a calling to help sick patients than by expectations of personal gain.

Dr. Milch accepts national leadership award 

Robert A. Milch, MD, FACS, has long been acknowledged as a leader dedicated to improving end-of-life care. So it is 

very fitting that he was the top recipient of the prestigious new Hastings Center Cunniff-Dixon Physician Award for 

exceptional work in the field. Dr. Milch helped to shape The Center for Hospice & Palliative Care as medical director, 

and is now clinical director of our new Palliative Care Institute.


Iestimated that the total cost of Shirley’s hospitalizations last year 

exceeded $100,000, compared to $150 per day in Hospice Buffalo 

expenses over six months, which undercuts the hospital tab by 75%. 

There’s a clear lesson here for those seeking health care reform: Patients 

with ongoing diseases like chronic heart failure should 

be treated as much as possible at home by a team of experienced 

professionals if we ever hope to get a grip on rising health care costs. We 

must encourage simpler scientifically proven home protocols which amplify 

the inherent resources of family and friends. 

Shirley’s experience with Hospice Buffalo should make us rethink 

how we organize American health care in the next decade.

each year, Hospice staff 

members and volunteers 

arrange activities at the 

mitchell Campus to raise funds 

for holiday gifts and food for 

current Hospice patients and 

their families. We established 

the Family Care Fund in 2009 

to help families in need with 

such basics as food, clothing, 

shelter and utilities. last year’s 

bake sales, cook-outs, basket 

raffles and more brought in 

$30,000.
