PRODUCT DESCRIPTION<br />
INTENDEND USE<br />
<strong>APC</strong> <strong>Resistance</strong> Kit is a plasma based functional assay for the<br />
determination of resistance to activated protein C caused by the factor V<br />
Leiden mutation (FV:Q 506 ). Activated protein C (<strong>APC</strong>) resistance is the most<br />
frequent hereditary defect associated with deep vein thrombosis. Over 95%<br />
of the <strong>APC</strong> resistance phenotype can be explained by the Factor V Leiden<br />
mutation [1,2,3,4,5,6]. This defect is caused by point mutation in the factor<br />
V gene resulting in a replacement of the amino acid Arg 506 by a Gln<br />
residue [2,3,7]. The heterozygous (het) defect is associated with a 5 to 10<br />
fold, the homozygous (hom) defect with a 50 to 100 fold increased<br />
thrombosis risk [5,8,9]. There are two possibilities of detecting factor V (FV)<br />
Leiden. Plasma based functional assays identifying the phenotype<br />
expression of the defect [1] or genotype determination which can be done<br />
by PCR technology [10].<br />
TEST PRINCIPLE<br />
<strong>APC</strong> <strong>Resistance</strong> Kit is a plasma based functional clotting assay and differs<br />
from other functional <strong>APC</strong> resistance tests by acting specifically on the<br />
prothrombinase complex level. It is based on a FV-dependent prothrombin<br />
activator isolated from snake venom. Robustness and specificity of the<br />
assay is enhanced by elimination of possible disturbing influences by<br />
factors upstream the coagulation cascade and independency from calcium.<br />
Interference from UFH, LMWH and Pentasaccharide in the blood sample is<br />
precluded by a heparin inhibitor added to reagents 1 and 2. Sample plasma<br />
is pre-diluted with reagent 4 (dilution plasma) and incubated at 37°C with<br />
FV activator from snake venom (RVV-V from Daboia russelli). Coagulation<br />
is triggered by the addition of a FV dependent prothrombin activator from<br />
snake venom from Notechis scutatus scutatus in the absence of calcium.<br />
The clotting times are recorded and the ratios (clotting time in the presence<br />
of <strong>APC</strong> / clotting time in the absence of <strong>APC</strong>) are calculated.<br />

potentially infectious and handled with appropriate care and in<br />
compliance with the biosafety regulations in force and must be disposed<br />
of in the same way as hospital waste.<br />
- The reagents R1, R4, C1 and C2 contain products derived from human<br />
blood and the samples have to be regarded as potentially infectious and<br />

waste. The reagents prepared from human blood and each single plasma<br />
used is HbSAg, HIV 1/2 Ab and HCV Ab negative.<br />

TEST PROCEDURE<br />
BLOOD COLLECTION AND PREPARATION OF PLASMA<br />
SAMPLES<br />
The patient should be at rest for 10 min prior sampling. Collect venous<br />
blood carefully in either 104 mM or 129 mM sodium citrate (volume ratio<br />
9+1). Mix gently blood and anticoagulant directly after sampling, avoid foam<br />
formation. Centrifuge immediately at no less than 2000x g for at least 20<br />
min at room temperature. Take care to avoid contaminations from the<br />
platelet layer into plasma when the plasma is separated from the cells. As a<br />
general rule hemolytic plasma samples should not be used. For storage<br />
freeze undiluted plasma rapidly at -70°C in aliquots. Freeze only once.<br />
Avoid repeated freezing and thawing cycles. Thawing should be done<br />
rapidly (within 5 min) in a water bath at 37°C. For more information see<br />
NCCLS document H21-A2 [11].<br />

PREPARATION OF REAGENT<br />
The lyophilized reagents are dissolved in the volume of distilled water<br />
indicated on the vial.<br />
Incubate reconstituted solutions R1-R4 and reconstituted C1 and C2<br />
controls in closed vials for 30 min at room temperature and swirl gently<br />
before use.<br />
PERFORMANCE OF THE TEST<br />
Prepare reagents as described above. Thaw frozen samples as described<br />
above ensuring negligible loss of activity of labile coagulation factors and<br />
absence of cryoprecipitate. Invert thawed sample for homogenization.<br />
Determine +<strong>APC</strong> clotting time (clotting time in the presence of Activated<br />
Protein C), -<strong>APC</strong> clotting time (clotting time in the absence of Activated<br />
Protein C) and calculate the ratio according to the following scheme:<br />

Differentiation of homozygous, heterozygous and negative samples is<br />
based on the typical ratio ranges measured with genotyped patient plasma<br />
samples<br />

geotyped patient samples.<br />
Attention: Factors mentioned under “Limitations and Interferences” can<br />
change the specific ratio ranges, so that differenciation of samples become<br />
impossible.<br />
QUALITY CONTROLS (C1 and C2)<br />
Negative control or wild-type (neg) shows normal response to <strong>APC</strong> whereas<br />
heterozygous control (het) shows response to the presence of the<br />
heterozygous type of FV:Q 506 mutation. A control run should be made with<br />
each test series. If values outside the certified range (ratio) are obtained, a<br />
complete check of reagents should be made and the analysis should be<br />
repeated. If the problem persists, a complete instrument check should be<br />
made and the analysis should be repeated.<br />

If values outside these ranges are obtained the test results are not valid and<br />
shall not be used for diagnostic purposes. When controls are used in<br />
combination with another suitable test or instrument expected ratio values<br />
may be different and have to be determined locally under appropriate<br />
conditions.<br />
SPECIFICITY AND SENSITIVITY<br />
With the samples tested so far <strong>APC</strong> <strong>Resistance</strong> Kit provided 100%<br />
sensitivity and 100% specificity for carriers of heterozygous and<br />
homozygous FV:Q 506 mutation as determined by BCS ® (n=340), KC-4/-10<br />
A micro (n=290), CA-1500/CA-7000 (n=293), ACL 9000 (n=270), STA ®<br />
C (n=244) and AMAX CS-190 (n=146). Due to the functional detection<br />
technique the assay is supposed to detect other FV mutations leading to<br />
<strong>APC</strong>-R phenotype as well. However their prevalence is very low compared<br />
to the FV Leiden mutation.<br />
LIMITATION AND INTERFERENCES<br />
No significant differences are observed when fresh or frozen plasma<br />
samples are used. It does neither matter whether buffered or un-buffered<br />
citrate plasma is used.<br />
There is no significant influence on ratio or test sensitivity in case of<br />
Fibrinogen, Prothrombin, FVIII, FX, ATIII, Protein C, or Protein S deficiency<br />
(up to 100%) or excess of Fibrinogen, FVIII, ATIII, or TFPI (up to 5 times<br />
normal value). Mechanical measurements are neither influenced by the<br />
hemolytic blood samples nor by samples containing platelet residues. In<br />
contrast, optical methods can be influenced by the use of hemolytic or<br />
lipemic plasmas. Lupus anticoagulant antibodies did not influence the test.<br />

PRODUKTBESCHREIBUNG<br />
ANWENDUNG<br />
<strong>APC</strong> <strong>Resistance</strong> Kit ist ein funktioneller Gerinnungstest zur Bestimmung<br />
einer auf der FV-Leiden Mutation (FV:Q 506 ) basierenden Resistenz gegen<br />
aktiviertes Protein C (<strong>APC</strong>-R). Häufigste Ursache einer erblich bedingten<br />
tiefen Beinvenenthrombose ist eine Resistenz gegen aktiviertes Protein C<br />
(<strong>APC</strong>). In über 95% der Fälle manifestiert sich diese phänotypisch durch die<br />
FV Leiden (FVL) Mutation [1,2,3,4,5,6]. FVL basiert auf einer Punktmutation<br />
im FV-Gen, einem Austausch der Aminosäure Arginin 506 durch Glutamin<br />
[2,3,7]. Ein heterozygoter Genotyp (het) ist mit einer 5- bis 10-fachen und<br />
ein homozygoter Genotyp (hom) mit einer 50- bis 100-fachen Erhöhung des<br />
Thromboserisikos verbunden [5,8,9]. Der Nachweis des FVL-Phänotyps<br />
erfolgt im Blutplasma mittels funktioneller Gerinnungstests [1]; die Genotypisierung<br />
mit Hilfe PCR-basierter Methoden [10].<br />
TESTPRINZIP<br />
Im Unterschied zu anderen am Markt erhältlichen funktionellen<br />
Gerinnungstesten erfolgt beim <strong>APC</strong> <strong>Resistance</strong> Kit die Aktivierung auf der<br />
Ebene des Prothrombinasekomplexes durch einen FVa abhängigen<br />
Prothrombinaktivator, isoliert aus dem Gift der australischen Tigerotter<br />
(Notechis scutatus). Dies eliminiert potenzielle Störeinflüsse durch<br />
vorgelagerte Faktoren der Gerinnungskaskade. Die geringe Störanfälligkeit<br />
und Spezifität des Tests wird des Weiteren unterstützt durch die<br />
Unabhängigkeit von Kalziumionen. Einflüsse von unfraktioniertem Heparin<br />
(UFH), niedermolekularem Heparin (LMWH) oder Pentasaccharid in der<br />
Blutprobe werden durch Polybrenzusatz in Reagenz 1 und 2 neutralisiert.<br />
Das Probenplasma wird mit Reagenz 4 (Dilution Plasma) verdünnt und in<br />
Gegenwart oder Abwesenheit von <strong>APC</strong> bei 37°C mit einem Faktor V<br />
Aktivator (RVV-V), isoliert aus dem Gift der Kettenviper (Daboia russelli),<br />
inkubiert. Die Gerinnung wird in Abwesenheit von Kalzium durch Zusatz des<br />
FVa abhängigen Prothrombinaktivators ausgelöst. Die Gerinnungszeiten<br />
werden bestimmt und die Ratio (Gerinnungszeit in Gegenwart von <strong>APC</strong> /<br />
Gerinnungszeit in Abwesenheit von <strong>APC</strong>) berechnet.<br />

WARNHINWEISE UND VORSICHTSMASSNAHMEN<br />
- Nur zur Anwendung als in vitro Diagnostikum<br />
- Alle Blut- bzw. Plasmaprodukte und Proben müssen als potentiell<br />
infektiös angesehen werden. Sie sind mit der notwendigen Sorgfalt und<br />
entsprechend den Sicherheitsvorschriften zu behandeln und wie<br />
Krankenhausmüll zu entsorgen.<br />
- Die Reagenzien R1, R4, C1 und C2 sind Blut- bzw. Plasmaprodukte und<br />
müssen ebenso wie die Proben als potentiell infektiös angesehen<br />
werden. Sie sind mit der notwendigen Sorgfalt und entsprechend den<br />
Sicherheitsvorschriften zu behandeln und wie Krankenhausmüll zu<br />
entsorgen. Die Reagenzien R1, R4, C1 und C2, hergestellt aus<br />
humanem Blut, und jedes hierzu verwendete Einzelplasma sind HBSAg,<br />
HIV Ak und HCV Ak negativ.<br />

Der Testkit ist ungeöffnet bei 2-8°C bis zu dem auf dem Etikett<br />

TESTDURCHFÜHRUNG<br />
PLASMAGEWINNUNG UND VORBEREITUNG DER<br />
PLASMAPROBE<br />
Der Patient sollte vor der Probennahme mindestens 10 Minuten ruhen. Zur<br />
Plasmagewinnung 1 Teil Natriumcitrat (104 mM) mit 9 Teilen Venenblut<br />
sorgfältig unter Vermeidung von Schaumbildung mischen. Sofort 20<br />
Minuten bei Raumtemperatur mit mindestens 2000 g zentrifugieren. Beim<br />
Abheben des Plasmas Kontamination durch Thrombozyten vermeiden. Es<br />
sollten generell keine hämolytischen Plasmaproben verwendet werden. Für<br />
die Lagerung das aliquotierte, unverdünnte Plasma rasch bei -70°C<br />
einfrieren. Nur einmal einfrieren. Wiederholte Auftau- und Gefrierzyklen sind<br />
zu vermeiden. Das Auftauen sollte rasch (innerhalb 5 Minuten) in einem<br />
37°C Wasserbad erfolgen. Weitere Informationen entnehmen Sie dem<br />
NCCLS Dokument H21 A2 [11].<br />

VORBEREITUNG DER REAGENZIEN<br />
Die lyophillisierten Reagenzien werden mit destillierten Wasser im am<br />
Etikett angegebenen Volumen gelöst. Die aufgelösten Reagenzien R1-R4<br />
sowie die Kontrollen C1 und C2 werden 30 Minuten bei Raumtemperatur<br />
inkubiert und vor Gebrauch vorsichtig geschüttelt.<br />
TESTDURCHFÜHRUNG<br />
Bereiten Sie Reagenzien und Probenmaterial wie oben beschrieben vor.<br />
Tauen Sie zur Vermeidung von Aktivitätsverlusten labiler<br />
Gerinnungsfaktoren und Kryopräzipitaten gefrorene Proben rasch bei 37°C<br />
in standardisierter Art und Weise auf. Mischen Sie sie vorsichtig unter<br />
Vermeidung von Schaumbildung. Bestimmen Sie die Gerinnungszeiten in<br />
Anwesenheit (+<strong>APC</strong>) und Abwesenheit (-<strong>APC</strong>) von aktiviertem Protein C<br />
und berechnen Sie anschliessend deren Verhältnis (Ratio). Verfahren Sie<br />
dabei nach folgendem Schema:<br />

negativen Proben basiert auf typischen Bereichen für die Ratio, wie sie für<br />
Plasmaproben genotypisierter Patienten bestimmt werden (siehe untenstehende<br />

Es wird empfohlen mit genotypisierten Plasmen die Referenzbereiche und<br />
Cut-offs für jedes Labor und Koagulometer individuell zu ermitteln.<br />
Weiters ist zu beachten, dass verschiedene Störfaktoren (siehe<br />
„Einschränkungen der Testdurchführung“) bewirken, dass Ratios erhalten<br />
werden, die nicht sicher einem Genotyp zugeordnet werden können.<br />
QUALITÄTSKONTROLLEN (C1 und C2)<br />
Die Normalkontrolle (C1, FV-L Negative Kontrolle) repräsentiert den<br />
normalen Phänotyp, während die pathologische Kontrolle (C2, FV-L<br />
Heterozygote Kontrolle) den positiven Phänotyp für FV-Leiden<br />
repräsentiert. Die Kontrollen sollten in jeder Testserie mitgeführt werden.<br />
Falls die ermittelten Werte ausserhalb der für die Kontrollen zertifizierten<br />
Bereiche (Ratios) liegen, ist eine komplette Überprüfung der verwendeten<br />
Reagenzien durchzuführen und die Analyse sollte wiederholt werden.<br />
Besteht das Problem weiter, ist eine Überprüfung des verwendeten<br />
Gerinnungsgerätes notwendig. Die Analyse ist zu wiederholen.<br />

Sollten Ergebnisse erhalten werden, welche außerhalb der angegebenen<br />
Bereiche liegen, so sind die Probenergebnisse ungültig und dürfen nicht für<br />
diagnostische Befunde verwendet werden.<br />
Werden die Kontrollen in Kombination mit einem anderen <strong>APC</strong> <strong>Resistance</strong><br />
Testsystem oder anderen Koagulometern verwendet, so können andere<br />
Ratios erhalten werden. In diesem Fall müssen die Bereiche unter<br />
geeigneten Bedingungen im Labor ermittelt werden.<br />
SPEZIFITÄT UND SENSITIVITÄT<br />
Mit den bisher getesteten genotypisierten Patientenproben zeigt der <strong>APC</strong><br />
<strong>Resistance</strong> Kit eine 100%-ige Sensitivität und Spezifität für die Detektion<br />
heterozygoter und homozygoter Träger der FVL-Mutation. Die<br />
Bestimmungen wurden mit folgenden Geräten durchgeführt: BCS ® (n=340),<br />
KC-4/-10 A micro (n=290), CA-1500/CA-7000 (n=293), ACL 9000<br />
(n=270), STA ® C (n=244) und AMAX CS-190 (n=146). Aufgrund des<br />
funktionalen Nachweisprinzips sollte der Test neben der FV Leiden<br />
Mutation auch andere Störungen einer FV-basierten phänotypischen <strong>APC</strong>-<br />
R nachweisen können. Die Prävalenz ist verglichen mit der FVL-Mutation<br />
jedoch sehr gering.<br />
EINSCHRÄNKUNGEN DER TESTDURCHFÜHRUNG<br />
Für das Ergebnis macht es keinen signifikanten Unterschied, ob mit<br />
frischen oder eingefrorenen und wieder aufgetauten Proben gearbeitet wird.<br />
Auch die Verwendung von gepuffertem vs. ungepuffertem Zitratplasma hat<br />
keinen störenden Einfluss auf die Messergebnisse.<br />
Ein bis zu 100%-iger Mangel der Faktoren Fibrinogen, Prothrombin, FVIII,<br />
FX, ATIII, Protein C und Protein S, sowie ein Überschuss bis zum 5-fachen<br />
der Normalwerte von Fibrinogen, FVIII, ATIII und TFPI hat in<br />
experimentellen Untersuchungen keinen Einfluss auf die Ratio oder die<br />
Sensitivität des Testes gezeigt. Weder hämolytische Proben noch<br />
Kontamination durch Thrombozyten stören mechanische Messmethoden.<br />
Optische Messmethoden können hingegen durch hämolytische oder<br />
lipämische Plasmen beeinflusst werden. LA Antikörper haben keine Störung<br />

DESCRIZIONE DEL PRODOTTO<br />
APPLICAZIONE<br />
<strong>APC</strong> <strong>Resistance</strong> Kit è un test per la determinazione nel plasma della<br />
resistenza alla Proteina C attivata, causata dalla mutazione del fattore<br />
V:Q506 La resistenza alla Proteina C attivata (<strong>APC</strong>) si riferisce alla più<br />
frequente causa ereditaria che predispone alla trombosi venosa profonda. Il<br />
fenotipo <strong>APC</strong> resistente è, in oltre 95% dei casi, dovuto ad una mutazione<br />
del gene del fattore V Leiden [1,2,3,4,5,6], che comporta la sostituzione<br />
dell’Arginina con la Glutamina nella posizione 506 della proteina fattore V<br />
[2,3,7]. I soggetti eterozigoti hanno un rischio di 5-10 volte superiore di<br />
sviluppare una trombosi venosa, mentre gli omozigoti hanno un rischio di<br />
50-100 volte [5,8,9].<br />
La rilevazione del fattore V Leiden viene effettuata con test funzionali nel<br />
plasma che indentificano il fenotipo della sindrome di Leiden [1] o con<br />
determinazioni del genotipo mediante tecniche di amplificazione genetica<br />
(PCR) [10].<br />
PRINCIPIO DEL TEST<br />
<strong>APC</strong> Resitance Kit è un test funzionale di coagulazione nel plasma e si<br />
differenzia da altri test funzionali perché agisce specificamente a livello del<br />
complesso protrombinase. Il test si basa su un attivatore della protrombina,<br />
dipendente dal fattore V, isolato da veleno di serpente (Notechis scutatus).<br />
La precisione e la specificità del test è molto alta a causa dell’eliminazione<br />
di possibili influenze dovute a fattori che provengono dalla parte superiore<br />
della cascata coagulativa ed in più il test è indipendente dal calcio. Nel<br />
campione di sangue vengono neutralizzate interferenze da parte<br />
dell'eparina non frazionata (UFH), delle eparine a basso peso molecolare<br />
(LMWH) e del pentasaccaride tramite l'aggiunta di polibrene nei reagenti 1<br />
e 2. I campioni di plasma vengono prediluiti con plasma di diluizione (R4)<br />
ed incubati a 37°C con l'attivatore del fattore V proveniente dal veleno di<br />
serpente (RVV-V, Daboia russelli) in presenza o assenza della <strong>APC</strong>. La<br />
coagulazione viene innescata attraverso l’aggiunta di un attivatore della<br />
protrombina estratto dal veleno del serpente Notechis scutatus in assenza<br />
di calcio. Si registra il tempo di formazione del coagulo e si calcola la ratio<br />
(tempo di coagulazione in presenza della <strong>APC</strong> / tempo di coagulazione in<br />
assenza della <strong>APC</strong>).<br />

PRELIEVO DEL SANGUE E PREPARAZIONE DEI CAMPIONI DI<br />
PLASMA<br />
Il paziente deve restare a riposo per 10 minuti prima del prelievo. Prelevare<br />
sangue venoso attentamente e mescolare accuratamente 1 parte di<br />
soluzione di citrato di sodio 104 mM con 9 parti di sangue venoso. Evitare<br />
la formazione di schiuma. Centrifugare immediatamente a una velocità non<br />
inferiore a 2000x g per almeno 20 minuti a temperatura ambiente. Avere<br />
cura di evitare la contaminazione piastrinica del plasma quando viene<br />
separato dalle cellule. Non utilizzare mai plasma emolitico. Per la<br />
conservazione congelare il plasma non diluito rapidamente a -70°C in<br />
aliquote. Congelare una sola volta. Evitare di ricongelare e disgelare<br />
ripetutamente. In modo di evitare il decremento dei fattori labili della<br />
coagulazione e garantire l'assenza di crioprecipitato, i campioni vanno<br />
scongelati rapidamente (entro 5 minuti) a bagno-maria a 37°C. Per ulteriori<br />

PREPARAZIONE DEI REAGENTI<br />
I reagenti liofilizzati vanno disciolti nel volume di acqua distillate indicate<br />

Incubare le soluzioni ricostituite R1-R4 e i controlli ricostituiti C1 e C2 in<br />
fiale chiuse per 30 min a temperatura ambiente e agitare delicatamente<br />

Attenzione: gli aspetti indicate nella sezione “Limitazioni ed interferenze”<br />

dei campioni diviene impossibile.<br />
CONTROLLI DI QUALITÀ (C1 e C2)<br />
Il controllo negative o wild-type (neg) mostra normale risposta alla <strong>APC</strong><br />
laddove il controllo eterozigote (het) mostra risposta alla presenza della<br />
mutazione eterozigote di FV:Q 506 . si raccomanda di eseguire il controllo ad<br />
ogni seduta di lavoro. In caso si ottengano risultati fuori dall’intervallo<br />
certificato si raccomanda di controllare tutti I reattivi e di Ripetere l’analisi.<br />
Se il problema persiste controllare anche lo strumento e Ripetere<br />
nuovamente l’analisi.<br />

In caso si ottengano nuovamente risultati fuori dagli intervalli considerare la<br />
seduta non valida allo scopo diagnostico.<br />
In caso I controlli siano usati in combinazione con un altro test idoneo o su<br />
un altro strumento I valori di ratio potrebbero essere diversi dall’atteso e<br />
vanno quindi ricalcolati in base alle nuove condizioni di lavoro appropriate.<br />
SPECIFICITÀ E SENSIBILITÀ<br />
La determinazione dei campioni di plasma genotipizzati finora effettuata con<br />
il kit <strong>APC</strong> <strong>Resistance</strong> ha indicato valori di sensibilità del 100% e di<br />
specificità del 100% per il fattore V Leiden (FV:Q 506 ) etero- ed omozigote. I<br />
risultati sono stati rilevati con i seguenti strumenti: BCS ® (n=340), KC-4/-10<br />
A micro (n=290), CA-1500/CA-7000 (n=293), ACL 9000 (n=270), STA®<br />
C (n=244) e AMAX CS-190 (n=146).<br />
In base alla tecnica di rilevazione funzionale si suppone che il test<br />
determini anche altre mutazioni del Fattore V che conducono al fenotipo<br />
<strong>APC</strong>-R. Tuttavia la loro prevalenza è molto bassa paragonata alla<br />
mutazione del fattore V Leiden.<br />
LIMITAZIONI ED INTERFERENZE DEL TEST<br />
Per i risultati non sono state osservate differenze significative tra campioni<br />
di plasma fresco, di plasma congelato o plasma scongelato più volte.<br />
L'utilizzo di plasma citrato tamponato vs plasma citrato non tamponato non<br />
interferisce sulla misurazione dei risultati. Non esistono influenze rilevanti<br />
sulla ratio o perdite di sensibilità nel caso di deficienza fino al 100% di<br />
fibrinogeno, della protrombina, del FVIII, del FX, della ATIII, della Proteina<br />
C o della Proteina S o nel caso di eccesso fino a 5 volte al di sopra dei<br />
valori normali di fibrinogeno, del FVIII, della ATIII o del TFPI. Nel caso di<br />
strumenti con metodo di rilevazione meccanica il test non è condizionato da<br />
campioni di sangue emolitici o da campioni di sangue contenenti piastrine.<br />
Nel caso di strumenti con metodo di rilevazione ottica il test può essere<br />
condizionato da campioni di sangue emolitici o da campioni di sangue<br />
lipemici. Inoltre gli anticorpi lupus anticoagulanti non interagiscono con il<br />

DESCRIPCIÓN DEL PRODUCTO<br />
USO PREVISTO<br />
<strong>APC</strong> <strong>Resistance</strong> Kit es una prueba de coagulación funcional para la<br />
determinación de la resistencia a la proteína C activada causada por una<br />
mutación en el factor V Leiden (FV:Q 506 ). La causa más frecuente de la<br />
afección hereditaria trombosis venosa profunda de las piernas es una<br />
resistencia a la proteína C activada (<strong>APC</strong>). En más del 95% de los casos,<br />
ésta se manifiesta fenotípicamente debido a una mutación del FV Leiden<br />
(FVL) [1,2,3,4,5,6]. El FVL se debe a una mutación puntual en el gen del<br />
FV, en el que se ha producido un cambio del aminoácido 506 arginina por<br />
glutamina [2,3,7]. La condición heterocigota (het) está asociada a un<br />
aumento del riesgo de trombosis de 5 a 10 veces, mientras que la<br />
condición homocigota (hom) está asociada a un aumento de este riesgo de<br />
50 a 100 veces [5,8,9]. La determinación del fenotipo FVL se realiza en el<br />
plasma mediante una prueba de coagulación funcional [1] y la del genotipo<br />
se realiza mediante métodos basados en la PCR [10].<br />
PRINCIPIO DEL TEST<br />
A diferencia de otras pruebas de coagulación funcional comerciales, en el<br />
caso del <strong>APC</strong> <strong>Resistance</strong> Kit, la activación del complejo de protrombina se<br />
produce por un activador de la protrombina dependiente del factor FVa,<br />
aislado del veneno de la víbora tigre australiana (Notechis scutatus), lo cual<br />
elimina potenciales interferencias debidas a factores de la cascada de<br />
coagulación acumulados. Esta menor interferencia y especificidad de la<br />
prueba se ve favorecida, además, por el hecho de ser independiente del<br />
calcio endógeno. Las interferencias producidas en la muestra de sangre<br />
por heparina no fraccionada (UFH), heparina de bajo peso molecular<br />
(LMWH) o por Pentasacárido, también se neutralizan por la adición de<br />
Polibreno (un inhibidor de la heparina) a los reactivos 1 y 2. El plasma<br />
problema se mezcla con el reactivo 4 (Dilution Plasma) y se incuba a 37°C,<br />
en presencia o ausencia de <strong>APC</strong>, con un activador del factor V (RVV-V),<br />
aislado del veneno de víbora de Russel (Daboia russelli). La coagulación se<br />
desencadena en ausencia de calcio por la adición del activador de la<br />
protrombina dependiente del FVa. Se determinan los tiempos de<br />
coagulación y se calcula el cociente (tiempo de coagulación en presencia<br />
de <strong>APC</strong> / tiempo de coagulación en ausencia de <strong>APC</strong>).<br />

ADVERTENCIAS Y MEDIDAS DE PRECAUCIÓN<br />
- Sólo para su uso en diagnóstico in vitro.<br />
- Todas las muestras de sangre y plasma y productos deben considerarse<br />
como potencialmente infecciosos y manipularse con el correspondiente<br />
cuidado y de conformidad con las normas de seguridad aplicables,<br />
siendo por tanto desechados como residuos del hospital.<br />
- Los reactivos R1, R4, C1 y C2 que contienen productos derivados de<br />
sangre humana y las muestras deben ser tratadas como potencialmente<br />
infecciosas, esto es, manipuladas con el apropiado cuidado y de<br />
acuerdo con las normas de seguridad vigentes y desechadas como<br />
residuos hospitalarios. Los reactivos preparados a partir de sangre<br />
humana y cada uno de los plasmas usados, son negativos para las<br />
siguientes pruebas: Antígeno de superficie de la Hepatitis B (HBsAg),<br />
VIH 1/2 Ab y HCV Ab.<br />

cuidadosamente antes de su uso. Congelar sólo una vez.<br />
REALIZACIÓN DEL TEST<br />
OBTENCIÓN DE SANGRE Y PREPARACIÓN DE LAS MUESTRAS<br />
DE PLASMA<br />
El paciente deberá estar en reposo durante 10 minutos como mínimo antes<br />
de la extracción de la muestra. Para la obtención del plasma se mezclará<br />
cuidadosamente 1 parte de citrato sódico (104 Mm o 129 Mm) con 9 partes<br />
de sangre venosa, evitando en todo momento la formación de espuma.<br />
Centrifugar inmediatamente después, durante al menos 20 minutos a<br />
temperatura ambiente y a 2000xg como mínimo. Al extraer el plasma<br />
deberá evitarse la contaminación con trombocitos. Por regla general, no se<br />
debería usar ninguna muestra de plasma hemolítica. El plasma sin diluir y<br />
en alícuotas deberá congelarse inmediatamente a -70°C. Congelar sólo<br />
una vez. Evitar ciclos repetidos de congelación-descongelación. Para evitar<br />
pérdidas de actividad de los factores de coagulación más lábiles y la<br />
formación de crioprecipitados; la descongelación se deberá realizar de<br />
forma rápida (en el plazo de 5 minutos) en un baño de agua a 37°C. Puede<br />
obtener más información en el documento NCCLS H21 A2 [11].<br />

PREPARACIÓN DE LOS REACTIVOS<br />
Los reactivos liofilizados son disueltos con el volumen de agua destilada<br />
indicado en el vial.<br />
Incubar las soluciones reconstituidas de R1-R4 y de los controles C1 y C2<br />
en viales cerrados durante 30 min. a temperatura ambiente y agitar con<br />
delicadeza antes de su uso.<br />
REALIZACIÓN DEL TEST<br />
Prepare los reactivos como se describe en el apartado anterior.<br />
Descongele las muestras como se describe más arriba dando por hecho<br />
una insignificante pérdida de actividad de los factores lábiles de<br />
coagulación y ausencia de crioprecipitados. Mezcle las muestras<br />
descongeladas cuidadosamente, evitando la formación de espuma.<br />
Determine los tiempos de coagulación en presencia (+<strong>APC</strong>) y en ausencia<br />
(-<strong>APC</strong>) de la proteína C activada y calcule el cociente (ratio) de acuerdo<br />

La distinción entre homocigotos, heterocigotos y muestras negativas<br />
respecto al FVL se basa en los intervalos típicos del cociente determinados<br />
en las muestras de plasma de pacientes genotipados (véase las tablas<br />
siguientes).<br />

Se recomienda determinar intervalos de cociente específicos de laboratorio<br />
con muestras de pacientes genotipadas.<br />
Atención: Los factores que se mencionan en el apartado “Limitaciones e<br />
Interferencias” pueden cambiar los intervalos de cociente específicos, de tal<br />
modo que la diferenciación de las muestras puede resultar imposible.<br />
CONTROLES DE CALIDAD (C1 y C2)<br />
El control negativo o el tipo salvaje “wild-type” (neg) muestra una respuesta<br />
normal frente a la <strong>APC</strong> mientras que el control heterocigoto (het) muestra<br />
respuesta en presencia de la forma heterocigota de la mutación FV:Q 506 .<br />
Se debería utilizar un control cada vez que se realiza el test. Si se obtienen<br />
valores fuera del rango (ratio), debería de hacerse un completo chequeo de<br />
los reactivos y repetir el análisis. Si el problema persiste, se debería hacer<br />
una comprobación completa de los instrumentos y los análisis deberían ser<br />
repetidos.<br />

Si se obtienen valores fuera de estos rangos, los resultados del test no son<br />
válidos y no deberían usarse para diagnóstico. Cuando se usan controles<br />
en combinación con otro test o instrumento aplicable, los valores del<br />
cociente esperados pueden ser diferentes y han de ser determinados<br />
localmente bajo unas condiciones apropiadas.<br />
ESPECIFICIDAD Y SENSIBILIDAD<br />
Con las muestras testadas hasta el momento, <strong>APC</strong> <strong>Resistance</strong> Kit presenta<br />
una sensibilidad y especificidad del 100% para la detección de los<br />
portadores heterocigotos y homocigotos de la mutación FV:Q506. Las<br />
determinaciones se han realizado mediante los siguientes aparatos: BCS®<br />
(n=340), KC-4/-10 A micro (n=290), CA-1500/CA-7000 (n=293), ACL<br />
9000 (n=270), STA® C (n=244) y AMAX CS-190 (n=146).<br />
Debido a la técnica de detección funcional, el test tiene que ser capaz de<br />
detectar otras mutaciones de FV que dan lugar también a un fenotipo <strong>APC</strong>-<br />
R. Sin embargo, su prevalencia comparada con la de la mutación FV<br />
Leiden es muy pequeña.<br />
LIMITACIÓN E INTERFERENCIAS<br />
No se observan diferencias significativas cuando se utilizan muestras de<br />
plasma frescas o congeladas. Asimismo, la utilización de plasma citratado<br />
tamponado o no tamponado tampoco tiene ninguna influencia sobre los<br />
resultados de la determinación.<br />
En los estudios experimentales se ha demostrado que una deficiencia de<br />
hasta el 100% de los factores fibrinógeno, protrombina, FVIII, FX, ATIII,<br />
proteína C y proteína S, así como un exceso de hasta 5 veces el valor<br />
normal de fibrinógeno, FVIII, ATIII y TFPI no afectan en absoluto al<br />
cociente o a la sensibilidad de la prueba. Ni las muestras de sangre<br />
hemolítica ni las muestras que contienen residuos de plaquetas alteran los<br />
métodos de determinación mecánicos. Por el contrario, los métodos de<br />
determinación ópticos pueden verse alterados por el uso de plasmas<br />
hemolíticos o lipémicos. Los anticuerpos anticoagulantes lupus no<br />
