Recomendaciones<br />
Si ALT aumenta 5 veces <strong>por</strong> encima <strong>de</strong>l valor<br />
basal, discontinuar <strong>el</strong> tratamiento y monitorear<br />
función hepática. Una vez resu<strong>el</strong>to <strong>el</strong> problema<br />
reiniciar TARV remp<strong>la</strong>zando <strong>el</strong> medicamento<br />
probablemente re<strong>la</strong>cionado con <strong>la</strong> condición<br />
(EFV <strong>por</strong> NVP).<br />
Discontinuar <strong>el</strong> tratamiento ARV y administrar<br />
tratamiento <strong>de</strong> apoyo. Después <strong>de</strong> haberse<br />
resu<strong>el</strong>to reiniciar <strong>el</strong> tratamiento, con TDF<br />
Reemp<strong>la</strong>zo temprano <strong>de</strong>l medicamento<br />
sospechoso (d4T <strong>por</strong> TDF o AZT).<br />
Generalmente autolimitados, sin necesidad <strong>de</strong><br />
discontinuar <strong>el</strong> tratamiento.<br />
Si resulta <strong>int</strong>olerable para <strong>el</strong> paciente,<br />
reemp<strong>la</strong>zar EFV <strong>por</strong> NVP o IP/r. Pue<strong>de</strong><br />
cambiarse solo este medicamento sin<br />
necesidad <strong>de</strong> suspen<strong>de</strong>r <strong>el</strong> tratamiento.<br />
Consi<strong>de</strong>rar sustituirlo <strong>por</strong> AZT.<br />
Consi<strong>de</strong>rar reemp<strong>la</strong>zo <strong>de</strong> d4T <strong>por</strong> AZT o TDF<br />
<strong>Tratamiento</strong> s<strong>int</strong>omático (amitriptilina, vitamina<br />
