LILT, la cultura della prevenzione

IL MELANOMA CUTANEO: autoesame e controlli periodici

per una corretta diagnosi precoce

LILT (Lega Italiana per la Lotta contri i Tumori)

è un Ente Pubblico di notevole rilievo su base associati

va vigilato dal Ministero della Salute ed impegnato nella

lotta contro i tumori. Dal 1922 opera senza fini di lucro

su tutto il territorio nazionale svolgendo un ruolo fonda

mentale nella promozione di attività di educazione alla

salute, di sensibilizzazione alla diagnosi precoce e all’as

sistenza psico-fisica del paziente oncologico. Obiettivo

della LILT è promuovere la cultura della prevenzione

come stile di vita perché prevenire è vivere!

Il melanoma è il più aggressivo dei tumori della pelle.

Origina dalla mutazione maligna dei melanociti, le cellule

della pelle che producono la melanina. Tumori anche di

piccole dimensioni possono avere un decorso molto

grave. L’incidenza, che è in progressivo aumento in tutto

il mondo negli ultimi decenni, viene imputata in parte

alla crescente ed eccessiva esposizione al sole. In Italia

l’incidenza è tendenzialmente in aumento sia nei maschi

(+4,4% anno) sia nelle donne (+3,1% anno) ed è più fre

quente al Nord. Nell’ Unione Europea l’incidenza di mela

noma è attualmente 9 casi ogni 100.000 abitanti/anno.

Il melanoma continua pertanto ad essere, per la bassa

sopravvivenza in stadi avanzati, un problema di sanità

pubblica a cui contribuisce anche l’invecchiamento pro

gressivo della popolazione.

Ne abbiamo parlato con la Dr.ssa Bianca Bernardini,

Specialista in Dermatologia presso la LILT di Padova.

È possibile prevenire il melanoma cutaneo?

Certo! Le strategie preventive a nostra disposizione si

basano su due approcci ben precisi e fra loro

complementari: la prevenzione primaria (corretti stili di

vita) che in questo ambito consiste nell’evitare,

soprattutto per i bambini, scottature ed esposizioni

prolungate e violente della cute ai raggi solari, in

particolare nelle ore centrali della giornata. E’

raccomandato inoltre l’utilizzo di creme solari ad alta

protezione che per essere efficaci devono essere appli appli

cate più volte nella giornata, essere resistenti all’acqua e

contenere filtri sia per raggi UVA sia UVB. La prevenzione

secondaria (diagnosi precoce) con controlli clinici

della cute indicativamente dai 18 anni in poi, che

andrebbero effettuati ogni 1-2 anni a seconda dei fattori

LILT (Italian League for the Fight against Tumors)

is a Public Body Association of considerable importance

supervised by the Ministry of Health and engaged in the

fight against cancer. Since 1922 it has operated in a nonprofit

capacity throughout Italy, playing a fundamental

role in promoting health education activities, raising

awareness of early diagnosis and psycho-physical assistance

for cancer patients. The aim of LILT is to promote

the culture of prevention as a lifestyle because prevention

means living!

Melanoma is the most aggressive of skin cancers. The

melanin-producing cells of the melanocyte, originate

from the malignant mutation. Even small tumors can

have a very serious effect. The incidence, which is

increasing all over the world in recent decades, is partly

attributed to the increasing and excessive exposure to

the sun. In Italy the incidence tends to increase both in

males (+ 4.4% year) and in women (+ 3.1% year) and

is more frequent in the North. Across the European

Union the incidence of melanoma is currently 9 cases

per 100,000 inhabitants / year. (Source AIOM 2018

Guidelines).

Therefore, due to low survival in advanced stages,

melanoma continues to be a public health problem

which also contributes to the progressive aging of the

population.

We talked about it with Dr. Bianca Bernardini,

Dermatology Specialist at the LILT of Padua.

Can skin melanoma be prevented?

Sure! The preventive strategies at our disposal are

based on two very precise and complementary approaches:

primary prevention (correct lifestyles) which in

this area consists of avoiding sunburn and prolonged

and violent skin exposure to solar rays especially for

children, in particular in the central hours of the day. It

is also recommended to use high protection sunscreens

which, to be effective, must be applied several

times a day, be water resistant and contain filters for

both UVA and UVB rays. Secondary prevention (early

diagnosis) with clinical skin checks from 18 years

onwards, which should be performed every 1-2 years

depending on the personal risk factors identified by

the dermatologist. Early diagnosis is the winning wea-

di rischio personali individuati dal dermatologo. La dia

gnosi precoce rappresenta l’arma vincente nella lotta al

tumore, la sua scoperta in fase inziale permette cure

meno aggressive e maggiori possibilità di guarigione.

Quali sono gli esami che consentono una diagnosi

precoce?

L’esame principale è la visita dermatologica completa

della cute nella quale il dermatologo valuta la storia

familiare e i fattori di rischio del singolo paziente e l’e

ventuale presenza di segni di sospetto di melanoma

cutaneo o di altri tumori della pelle. Il dermatologo utiliz

za uno strumento chiamato dermatoscopio, una specia

le tecnica di ingrandimento e illuminazione della pelle

per poter meglio valutare le caratteristiche microscopi

che di ogni lesione cutanea. La diagnosi certa di melano

ma cutaneo richiede, nei casi di sospetto clinico, una

biopsia, in cui un campione di tessuto viene prelevato,

previa anestesia locale, e analizzato al microscopio. La

LILT di Padova è impegnata da oltre 30 anni nella pre

venzione e diagnosi precoce del melanoma cutaneo con

campagne di informazione e attraverso un servizio

ambulatoriale di diagnosi precoce altamente specializzato

e collegato alle principali strutture sanitarie pub

bliche del territorio.

AUTOESAME DELLA CUTE: Il Sistema A B C D E

Il melanoma, insorgendo sulla cute, è facilmente visibile

e pertanto può essere sospettato già dal paziente con un

“autoesame della pelle”, che deve essere effettuato

periodicamente. Statisticamente il melanoma è più fre

quente alle gambe nelle donne e al dorso nell’uomo. Per

un corretto autoesame è necessario spogliarsi comple

tamente ed osservare tutta la superficie cutanea, incluso

il cuoio capelluto, la regione ano-genitale, le pieghe

interdigitali, la pianta dei piedi e i palmi delle mani,

facendosi aiutare da un familiare o utilizzando due spec

chi per evidenziare le zone meno accessibili.

Il Sistema A B C D E è un semplice algoritmo che aiuta

ad individuare i segnali d’allarme. I criteri per individuare

le lesioni sospette sono i seguenti:

• A = asimmetria della lesione

• B = bordi irregolari, frastagliati

• C = colore policromo o nero

• D = dimensioni > 6 mm di diametro

• E = evoluzione: (parametro più importante) modifica

zioni in dimensione, forma e colore in un breve periodo

di tempo (6-8 mesi) nel paziente di età superiore ai

25-30 anni.

What are the tests that allow an early diagnosis?

The main examination is the complete dermatological

examination of the skin in which the dermatologist

evaluates the family history and risk factors of the

individual patient and the possible presence of signs

of suspicion of cutaneous melanoma or other skin

cancers. The dermatologist uses a tool called a dermatoscope,

a special technique for enlarging and illuminating

the skin to better evaluate the microscopic

characteristics of each skin lesion. The accurate diagnosis

of cutaneous melanoma requires, in cases of

clinical suspicion, a biopsy, in which a sample of tissue

is taken, under local anesthesia, and analyzed

under a microscope. The LILT of Padua has been

engaged for over 30 years in the prevention and early

diagnosis of cutaneous melanoma with information

campaigns and through a highly specialized outpatient

early diagnosis service connected to the main public

health facilities in the area.

AUTOEXAMINATION OF THE SKIN: The System A B C D

Melanoma, arising on the skin, is easily visible and therefore

can be suspected by the patient with a “self-examination

of the skin”, which must be carried out periodically.

Statistically, melanoma is more common in the legs in

women and in the back in men. For a correct self-examination

it is necessary to completely undress and observe

the entire skin surface, including the scalp, the anogenital

region, the interdigital folds, the soles of the feet

and the palms of the hands, being helped by a family

member or using two mirrors to highlight the least

accessible areas.

The A B C D E System is a simple algorithm that helps

to identify alarm signals. The criteria for detecting

suspicious lesions are the following:

• A = asymmetry of the lesion

• B = irregular, jagged edges

• C = polychrome or black color

• D = dimensions> 6 mm in diameter

• E = evolution: (most important parameter) changes in

size, shape and color in a short period of time (6-8

months) in patients over 25-30 years old.

sibilità che la Cooperativa sociale La Mia Badante offre in aiuto alle

famiglie bisognose di assistenza per sé o per i propri famigliari.

I servizi che la cooperativa offre sono in linea con le normative e garan

tiscono una assistenza di qualità ad un costo equo ed accessibile su

tutto il territorio delle province di Padova e Vicenza, ma è prevista la

for themselves or their families.

The services offered by the cooperative are in line with regulations and

guarantee quality assistance at a fair and accessible cost throughout

the territory of the provinces of Padua and Vicenza, but a new opening

is also planned for the Lombardy region. So why not try this viable

Manrico Baldo (Padova, 1972) - alias Zero Mentale - Diplomato presso l’Istituto d’Arte P. Selvatico

di Padova, si è successivamente laureato in Architettura a Venezia.

Formatosi nella tecnica dell’areografia e nella ricerca di nuovi materiali è in continua evoluzione

emozionale tra esperienza e pittura. La passione per l’aerografia segna un importante apporto

all’identità artistica di Zero Mentale. Un interesse che in breve tempo lo porta a un affinamento

nella gestione e nella tecnica dei materiali.

Grazie al sapiente utilizzo di svariate vernici, le opere assumono in breve una superficie patinata e

traslucida e si distinguono per il forte impatto visivo, il valore stilistico e la precisione del tratto.

Dal 2005, presentandosi con il nome di “Zero Mentale”, Manrico Baldo trasforma le sue sensazioni

in istinti, in forme che si sintetizzano in un concetto di annullamento, da cui il nome Zero Mentale, con la volontà di lasciarsi alle spalle le

esperienze precedenti a favore di nuove sperimentazioni.

Il punto di partenza è una formulazione di osservazione che si proietta nelle sue composizioni attraverso la creazione di un “impulso

mentale” detto sinapsi che vede la genesi, partendo da una terminazione nervosa, di un complesso viaggio di un pensiero che si snoda

tra forme e colori. Sinapsi significa “toccare” e “connettere” due terminologie che consentono la comunicazione e la strutturazione di un

concetto. Il lavoro dell’artista si basa sulla composizione pittorica di configurazioni che inscenano la nascita di una forma.

Zero Mentale fissa non tanto la formulazione di un “impulso mentale” quanto quella di un “impulso creativo”: dà voce ai pensieri in

maniera surreale, si è davanti al predominio di un mondo creato solo da suggestioni, quello che arriva agli occhi dello spettatore sono le

basi della creazione mentale così come immaginate dal suo autore.

L’artista Zero Mentale, figlio di un’arte contemporanea sempre più urban life, negli ultimi anni si è avvicinato al mondo della Street art,

cimentandosi in opere murarie. Sui muri nascono cosi graffiti e messaggi per il cuore contemporaneo della vita metropolitana; l’uso di

spray, vernici, aerografo, stencil fa si che il messaggio passi sulla pelle della città.

Manrico Baldo (Padua, 1972) - aka Zero Mentale - Graduated from the P. Selvatico Institute of Art in Padua, he later graduated in Architecture

in Venice.

Trained in the technique of aerography and in the search for new materials, he is constantly evolving between experience and painting. The

passion for aerography marks an important contribution to the artistic identity of Zero Mentale. An interest that in a short time leads him to

a refinement in the management and technique of materials.

Thanks to the wise use of various paints, the works quickly take on a patinated and translucent surface and stand out for their strong visual

impact, stylistic value and the precision of the line.

Since 2005, presenting himself with the name “Zero Mental”, Manrico Baldo transforms his feelings into instincts, into forms that are summarized

in a concept of annulment, from which the name Zero Mentale, with the desire to leave behind previous experiences in favor of new

experiments.

The starting point is an observation formulation that is projected into his compositions through the creation of a “mental impulse” called

synapse which sees the genesis, starting from a nervous termination, of a complex journey of a thought that winds through forms and colors.

Sinapsi means “touching” and “connecting” two terminologies that allow the communication and structuring of a concept. The artist’s work

is based on the pictorial composition of configurations that stage the birth of a form.

Zero Mental fixes not so much the formulation of a “mental impulse” as that of a “creative impulse”:

it gives a voice to thoughts in a surreal way, one is in front of the dominance of a world created

only by suggestions, that which reaches the eyes of the spectator are the bases of mental creation

as imagined by its author.

The artist Zero Mentale, son of a contemporary art more and more urban life, in recent years has

approached the world of Street art, venturing into masonry works. On the walls graffiti and messages

are born for the contemporary heart of metropolitan life; the use of sprays, paints,

airbrushes and stencils causes the message to pass over the skin of the city.

Non c’è più la carne di una volta” è un’affermazione

che spesso si sente, con toni che sottolineano qualità

negative come la durezza delle carni che al tempo stes

so si vogliono magre e tenere.

Spesso si scorda che la qualità deriva dai metodi di

produzione che partendo dall’allevamento degli animali

arriva al banco della macelleria o del supermercato.

Il sistema di allevamento piu’ utilizzato, anche in Italia,

è quello “intensivo” che ha certamente permesso un

abbattimento del costo del prodotto finale a discapito

pero’ della qualità degli animali con conseguente pro

duzione di alimenti non sempre idonei alla salute dei

consumatori.

Nella bottega delle carni di Massimo Capelozza l’ob

biettivo è quello di invertire la attuale tendenza del

mercato preferendo la filiera corta, garanzia di qualità

ed eccellenza.

L’attenzione alla qualità di vita degli animali e l’effettiva

possibilità di controllo su di essi rende il rapporto qua

lità prezzo assolutamente equo se si pensa, per altro al

beneficio che ne consegue per la salute dei consuma

tori.

Dopotutto anche Fabrizio De Andrè diceva “Noi siamo

dei venditori. Bisogna vedere se siamo abbastanza

onesti da vendere carne fresca oppure carne marcia.”

PALIO DELLE ZATTERE. Nasce dalla ricostruzione

storica del passaggio delle zattere sul fiume Brenta,

dal porto fluviale di Valstagna fino ad arrivare a Venezia, dagli inizi del

1300 fino alla fine del 1800. Sei secoli d’intensa attività economica

legata all’economia del legname, portati a valle dagli “zattieri” lungo la

storica Calà del Sasso.

PALIO DELLE ZATTERE. It was born from the historical reconstruction

counts in her existence. Against the backdrop of the

stormy scenario of the Nordic winter, Thomas’s investigation

of the island and the capital tries to get to the bottom

of the life of a brave woman who, apparently, had

not been able to offer her neighbor the same dedication

to work. The more the inspector learns to know her, the

hanno imparato quant’è dura la vita. Mara non

dimentica l’ingiustizia subita, che le è costata il tra
