Kanserleşme sürecinde değişikliğe<br />
uğrayan proteinler, uyarıcı ve baskılayıcı<br />
etkileri olanlar şeklinde başlıca<br />
iki grupta toplanabilirler. Uyarıcı<br />
proteinleri kodlayan onkogenler ve<br />
baskılayıcı proteinleri kodlayan tümör<br />
baskılayıcı genlerdeki bozukluklar kanser<br />
etyopatogenezinde önemli bir yere sahiptir.<br />
Kanser, somatik hücrelerdeki mutasyonlar sonucunda<br />
oluşmaktadır ve tek bir mutasyon hastalığın ortaya<br />
çıkması için yeterli değildir. Hücrede çok sayıda<br />
mutasyon oluşmasında, zaman ve mutajenlere maruz kalma<br />
önemli faktörlerdir. Ancak bundan daha önemlisi genom<br />
dengesizliği olarak adlandırılan durumdur. DNA onarımı<br />
enzimlerinde bozukluk olduğunda ortaya akan genom<br />
dengesizliği, mutasyonların yığılma sürecini hızlandırır. Genom<br />
dengesizliği olan bir hücrede bir de apoptoz mekanizması<br />
ile ilgili bir bozukluk oluşursa, hücre döngüsü hatalı DNA<br />
sentezine rağmen ilerlemeye ve hücre her döngüde eklenen<br />
yeni mutasyonlarla çoğalmaya devam eder. Kanserleşme<br />
sürecindeki en kritik eşik budur.<br />
Onkogenler: Normal koşullarda hücre büyüme ve çoğalmasını<br />
uyaran proteinleri kodlayan genlere proto-onkogenler adı<br />
verilir. Proto-onkogenlerde bir bozukluk olduğunda ise<br />
onkogenler oluşmakta ve sonuçta kodlanan protein kanserli<br />
hücreye dönüşümde rol oynamaktadır. Onkogenler; büyüme<br />
faktörlerini, büyüme faktörlerinin reseptörlerini, Ras gibi<br />
reseptör sonrası efektör molekülleri, kinazları, transkripsiyon<br />
faktörlerini, hücre döngüsünü kontrol eden siklinler ve siklin<br />
bağımlı kinazları ve Bcl-2 gibi antiapoptotik proteinleri kodlar.<br />
Onkogenlerdeki mutasyonlar işlev kazandırıcı mutasyonlardır;<br />
yani proteinin daha fazla miktarda yapılmasına (ekspresyon<br />
artışı) veya denetimsiz şekilde sürekli aktif olmasına sebep<br />
olurlar.<br />

process of cancer can be grouped<br />
into two main groups as stimulants<br />
and repressive effects. Oncogenes<br />
that encode stimulatory proteins and<br />
disorders in tumor suppressor genes<br />
that encode repressor proteins have an<br />
important role in the etiopathogenesis<br />
of cancer.<br />
C<br />
ancer is the result of mutations in somatic cells,<br />
and a single mutation is not enough to cause the<br />
disease to appear. When multiple mutations occur<br />
in the cell, time and exposure to mutagen are important<br />
factors. But more importantly thing is genomic imbalance.<br />
The genomic imbalance that flows when there is a defect<br />
in DNA repair enzymes accelerates the accumulation<br />
process of mutations. If a disorder of the apoptosis<br />
mechanism occurs in a cell with genomic imbalance,<br />
the cell cycle does not progress despite the faulty DNA<br />
synthesis and the cell continues to multiply with new<br />
mutations added in each cycle. This is the most critical<br />
threshold in the cancer process.<br />
Oncogenes: The genes that encode proteins that<br />
stimulate cell growth and proliferation under normal<br />
conditions are termed proto-oncogenes. Oncogenes are<br />
formed when a disorder occurs in the proto-oncogenes, so<br />
protein is involved in the transformation of the cancerous<br />
cell. Oncogenes code growth factors, receptors for growth<br />
factors, post-receptor effector molecules such as Ras,<br />
kinases, transcription factors, cyclins controlling cycling<br />
and cyclin dependent kinases and antiapoptotic proteins<br />
such as Bcl-2. Mutations in oncogenes are functional<br />
mutations; they cause the protein to be made in greater<br />
amounts (increased expression) or continuously active in<br />
uncontrolled fashion.<br />
