Substitution of a generic product for a brand-name drug is mandatory when ageneric equivalent is available. If a member requests the brand-name drug in thissituation, a Prior Authorization may be required and the member must pay thedifference in cost between the generic and branded versions. Age Limitation (AG) -- a coverage limit based on minimum or maximum age of themember imposed as a result of safety, efficacy or dosage form considerations.2. Generics preferred/Generic equivalent available (GP -- if there is a genericavailable and the patient receives the brand name product, the patient pays the copayfor the generic drug plus the difference in cost between the brand and the generic.3. Medical <strong>Drug</strong>s (MED) -- Medications obtained through the medical benefit. Medicaldrugs are defined as medications administered in the office or facility that require ahealth care professional to administer. These medications include, but are not limited to,injectable, infused, oral or inhaled drugs. They may involve unique distribution and maybe provided by a specialty pharmacy vendor. Some Medical <strong>Drug</strong>s may require BenefitCertification before they can be obtained. Office administered applies to all outpatientsettings including, but are not limited to, physician's offices, emergency rooms, urgentcare facilities and outpatient surgery facilities.
