• at jeg
- a) er folkeregistreret i Københavns Kommune eller
- b) er bosat i udlandet, er skattepligtig i Danmark, arbejder eller har virksomhed i
Københavns Kommune, og er statsborger i et EU/EØS-land eller Schweiz.
• at jeg ikke er tilmeldt danskundervisning hos et andet sprogcenter eller udbyder,
• at jeg var over 18 år ved indrejse til Danmark,
• at jeg ikke tidligere har modtaget tilbud om danskuddannelse (Modul-dansk)
• at jeg kender og nu eller tidligere har fået udleveret informationspjece ”Lær Dansk, Lær om
Danmark”,
• at jeg er bekendt med at tilbuddet omfatter 5 forløb á 50 timer, i alt 250 timers undervisning,
og at de 5 forløb skal gennemføres indenfor 1 år og 6 måneder fra første startdato.
• at jeg kan få 50 timer ekstra og perioden forlænget, hvis jeg dokumenterer at jeg er
forhindret på grund af graviditet, barsel eller adoption og som følge deraf har lov til fravær
efter Lov om ret til orlov og dagpenge ved barsel. Dokumentations skal indleveres til
Jobcenter Copenhagen International.
• at jeg er bekendt med, at deltagelse i danskundervisning kræver en henvisning fra
kommunen, og at henvisningen skal gives for hvert forløb.
• at jeg er bekendt med, at jeg ikke kan skifte til et andet sprogcenter eller en anden udbyder,
før jeg har gennemført et forløb
• at jeg er bekendt med at tilbudte timer ikke erstattes, hvis jeg udebliver fra timerne eller en
del af dem på grund af sygdom eller af andre grunde.
• At jeg er bekendt med, at jeg først efter gennemførslen af alle 5 forløb arbejdsmarkedsrettet
danskundervisning og i umiddelbar forlængelse heraf har ret til at fortsætte i
danskuddannelse i 3 år.
• at jeg giver samtykke til, at mine personoplysninger må bruges af sprogcentret og af
Københavns Kommune ved behandling af ansøgninger om henvisning til danskuddannelse
for min person.

I wish to receive Danish language tuition at IA Sprog.
I, the undersigned, do hereby declare:
• That I have legal residence in Denmark (residence permit, EU certificate of registration, EU resident
card, or other documentation of legal residence in Denmark),
• That a) I am a registered resident of Copenhagen municipality or b) reside abroad, have tax liability
in Denmark, work or maintain a business in Copenhagen municipality, and am a citizen of an
EU/EEA country or Switzerland,
• That I am not registered for a Danish education course at another language centre or institution.
• That I was over 18 years old when I arrived in Denmark.
• That I have not previously been offered Danish education (module Danish).
• That I am familiar with and have either now or previously been given the information brochure “Lær
Dansk, Lær om Danmark” (Learn Danish, Learn about Denmark).
• That I am aware that this course is divided into five 50-hour subsections, totalling 250 lessons of
tuition, and that the five subsections must be completed within 1 year and 6 months from the start
date of the first subsection.
• That I am aware that participation in Danish instruction requires a referral from the local authorities,
and that a referral is required for each 50-hour subsection of the course.
• That I am aware that I may not change to another language centre or another institution before I have
completed a 50-hour subsection,
• That I am aware that the lessons offered (or any part of them) will not be substituted if I am absent
due to illness or other reasons.
• That I may get 50 extra lessons if my attendance is prevented due to pregnancy, childbirth, or
adoption and I, as a result thereof, am allowed to be absent in accordance with the law governing
“right to leave of absence and unemployment benefits upon childbirth”.
• That I am aware that I, upon completion of the 5th workplace-oriented Danish subsection, and as a
direct continuation thereof, can continue to ordinary Danish education (module Danish).
• That I give consent to the use of my personal data by the language centre and Copenhagen
municipality when processing Danish education referral applications on my behalf.
