Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong><br />
Intended Use<br />
Enzyme immunoassay for the detection of antibodies to antigens of <strong>HIV</strong>1, <strong>HIV</strong>2 and <strong>HIV</strong>1 (subtype O) in<br />
serum or plasma. The enzyme immunoassay is processed on the BEP ® II, BEP ® III and BEP ® 2000 ELISA processors.<br />
The test was developed for investigation of individual samples, not pooled samples. The product is for in<br />
vitro diagnostic use only.<br />
Summary and Explanation<br />
Acquired immunodeficiency syndrome (AIDS) was first recognized in 1981 as a clinical picture in its own right.<br />
The disease is today considered to involve two different Human Immunodeficiency Viruses (<strong>HIV</strong>1 and <strong>HIV</strong>2) as<br />
causal agents. The <strong>HIV</strong>1 virus (synonym: LAV/HTLV-III) was isolated as early as in 1983 1,2 and a further such<br />
pathogenic human virus (<strong>HIV</strong>2) was isolated in 1986 3 . A high level of importance is attached to the serological<br />
determination of antibodies to <strong>HIV</strong>, especially in the field of transfusion medicine where measures are needed to<br />
prevent the further spread of the disease.<br />
Consequently, from 1985 onwards, donor blood was screened for <strong>HIV</strong>1 antibodies, both by blood banks and<br />
manufacturers of plasma products. Subsequent information on the spread of <strong>HIV</strong>2 infections revealed that blood<br />
donors need to be tested not only for anti-<strong>HIV</strong>1 antibodies but also for anti-<strong>HIV</strong>2 on a routine basis in order to<br />
exclude to the greatest possible extent the potential risk of transmission via blood transfusions or blood-derived<br />
products.<br />
In 1990 a new <strong>HIV</strong> subtype was reported. 4 In 1994 two independent teams succeeded, simultaneously, in sequencing<br />
this new subtype 5,6 . Parallel to this virological research, several teams were able to demonstrate the<br />
serological significance of the virus, by then known as <strong>HIV</strong>1 Subtype O 7 . This research showed that some anti-<br />
<strong>HIV</strong> 1+2 assays exhibit weaknesses in detecting Subtype O antibodies 8 . Consequently, a need arose for reliable<br />
concurrent detection of this new <strong>HIV</strong> variant along with the established <strong>HIV</strong>1 and <strong>HIV</strong>2 isolates.<br />
Although the detection of antibodies specific for <strong>HIV</strong>1 and <strong>HIV</strong>2 does not allow definite conclusions to be made<br />
on whether infectious <strong>HIV</strong>1 or <strong>HIV</strong>2 is present in the blood at the time of the test, in the same way that a negative<br />
result does not exclude the presence of <strong>HIV</strong>1 or <strong>HIV</strong>2 with certainty, currently the test provides the best means<br />
of detecting and eliminating blood donations from <strong>HIV</strong>-infected donors with a high level of probability.<br />
Principle of the Method<br />
<strong>HIV</strong>-specific immunoglobulins contained in the test sample bind to the recombinant <strong>HIV</strong>1, <strong>HIV</strong>2, and/or <strong>HIV</strong>1<br />
(Subtype O) proteins bound to the surface of the microtitration plate.<br />
The unbound sample components are washed out and then the conjugate is bound to the <strong>HIV</strong>-specific antibodies.<br />
The excess conjugate is removed and the enzyme activity of the bound conjugate is then determined (blue<br />
colour reaction). The enzymatic reaction with the chromogen is terminated by the addition of Stopping Solution<br />
POD (yellow colour reaction). The colour intensity is proportional to the concentration of antibody present in<br />
the sample.<br />

Composition<br />

Microtitration plate coated with a mixture of recombinant <strong>HIV</strong>(env) proteins (E. coli). The proteins represent<br />
immunoreactive regions of the following viral proteins:<br />
gp 41 (<strong>HIV</strong>1), gp 41 (<strong>HIV</strong>1 Subtype O), gp 36 (<strong>HIV</strong>2).<br />

Warnings and Precautions<br />
1. For in vitro diagnostic use.<br />
2. Each individual blood donation for use in manufacture of the control sera is tested for HBsAg, anti-HCV,<br />
anti-<strong>HIV</strong>1 and anti-<strong>HIV</strong>2 by FDA-required test. The Control Serum, negative, is produced using only sera<br />
with negative findings. The positive control serum has been heat-treated (no less than 1 hour at +56 °C).<br />
Nevertheless, since absence of infectious agents cannot be proven, all samples (e.g. patient sera) and<br />
products (e.g. control sera) obtained from human blood should always be handled with due care, observing<br />
the precautions recommended for biohazardous material9 .<br />
3. It is advisable to wear protective gloves throughout the entire test procedure.<br />
4. For disposal, it is recommended that solid infectious materials should be autoclaved for at least one hour<br />
at +121 °C. All aspirated solutions should be collected in two receptacles connected in series, which<br />
should both contain a disinfectant suitable for inactivating pathogenic human viruses. The concentrations<br />
and reaction times specified by the manufacturer must be observed.<br />
5. The pack contains a matched set of reagents. Reagents must not be interchanged with other kits unless<br />
the reagents are from an identical lot (i.e. they must display the required 6-digit lot numbers printed on the<br />
pack and given in the enclosed barcode table of values). Washing Solution POD, Stopping Solution POD<br />
and Working Chromogen Solution are exceptions to this requirement. Note that the Working Chromogen<br />
Solution must first be prepared from matched components (do not use Chromogen TMB and Buffer/Substrat<br />
TMB from kits with a different number).<br />
Preparation of the Reagents<br />
Bring all reagents and samples to +18 to +25 °C before the start of the test, without removing the test plates<br />
from the container.<br />
The pack of Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> 10 x 96 includes a barcoded label for an empty bottle for the <strong>HIV</strong>-Ag/<br />
POD Conjugate. When running large series of samples, to avoid frequent changing of the syringes on the<br />
BEP ® III it may be expedient to pour several vials of the <strong>HIV</strong>-Ag/POD Conjugate into a larger bottle (e.g. a<br />
surplus unused empty bottle for Working Chromogen Solution or a rinsed bottle of Stopping Solution POD), to<br />
label this bottle with the label "Empty bottle for conjugate" and to insert the labelled bottle into the reagent rotor.<br />

For each test plate, dilute 20 ml Washing Solution POD to 400 ml with distilled or demineralized water.<br />
Working Chromogen Solution: For each test plate, dilute 1 ml Chromogen TMB with 10 ml Buffer/Substrate<br />
TMB in the plastic bottle supplied with the kit (= working chromogen solution) and store closed and<br />
protected from light. Rinse the bottle thoroughly with distilled water after use.<br />
For technical reasons (overfill) it is not permissible to pour together the full contents of the Chromogen TMB<br />
vial and the full contents of the Buffer/Substrate TMB vial.<br />
Storage and Stability<br />
Stored unopened at the stated temperature all the components of the Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> kit may be<br />
used up to the dates given on the labels.<br />
For details on the stability and storage of the opened and diluted reagents, see Table 1 in the Appendix.<br />
Equipment required:<br />
BEP ® II: For automatic dispensing of reagent and washing<br />
BEP ® III: For automatic processing of the test after dispensing the samples as well as for evaluation<br />
BEP ® 2000: For fully automatic processing and evaluation of the test<br />
Pipettes: piston-type pipettes 25, 100 and 1000 μl<br />
Incubator: Covered water bath (+37 ± 1 °C) or comparable incubation methods<br />
All the equipment used in the test must have been validated.<br />
Specimens<br />
Suitable specimens are individual samples (human sera or EDTA/heparinized/citrated plasma) obtained by<br />
standard laboratory techniques. The samples should be stored for not more than 3 days at +2 to +8 °C. If the<br />
samples are to be stored for a longer period of time, they must be frozen.<br />
Procedure<br />
Test procedure using the BEP ® II<br />
1. Pipetting scheme:<br />
Ascertain the number of wells required (number of samples to be investigated plus 6 wells for controls). Remove<br />
from the holder any strips which are not required for the test and store for later use (See Table 1 for stability data).<br />
2. Introduce sample buffer:<br />
Fill each well with a receiving volume of 25 μl of sample buffer.<br />
3. Dispense samples:<br />
Pipette 100 μl/well of the negative control into 4 wells and 100 μl/well of the positive control into 2 wells.<br />
Dispense 100 μl/well of undiluted sample into the following wells. Cover the finished test plate with foil and<br />
place in the incubator as soon as possible, max. 15 min after completion of the sample dispensing step.<br />
As an alternative to the above pipetting scheme, it is also possible to pipette the positive control once at<br />
the start and once at the end of the series.<br />
Pipetting scheme: Pipette 100 μl/well of negative Control, into 4 wells, 100 μl of positive control into the<br />
next well and then fill the following wells with 100 μl/well of undiluted sample. At the end of the series /<br />
plate pipette 100 μl of positive control once more.<br />
4. Incubate:<br />
Incubate at +37 ± 1 °C for 30 ± 2 minutes, then proceed immediately to the washing step.<br />
5. Wash:<br />
Remove the foil, aspirate all the wells and wash 4 times with approx. 0.3 ml/well of washing solution.<br />
6. Dispense conjugate:<br />
Fill each well with 100 μl conjugate, cover with fresh foil and immediately place into the incubator.<br />
7. Incubate:<br />
Incubate at +37 ± 1 °C for 30 ± 2 minutes as described in "4. Incubate", then proceed immediately to the<br />
next wash step.<br />
8. Wash:<br />
Remove the foil and wash as described in "5. Wash".<br />
9. Dispense substrate:<br />
Pipette 100 μl of the Working Chromogen Solution into each well and cover the plate with fresh foil.<br />
10. Incubate:<br />
Incubate at +18 to +25 °C for 30 ± 2 minutes, protected from light.<br />
11. Stop reaction:<br />
Remove the foil and add 100 μl of Stopping Solution POD to each well, keeping to the same timing as<br />
in "9. Dispense substrate".<br />
12. Read:<br />
At 450 nm within one hour.<br />
The absorbances of the control and patient samples are to be measured at a wavelength of 450 nm. The<br />
wavelength recommended for the reference reading is 650 nm (if necessary between 615 nm and 690 nm).<br />

Test procedure using the BEP ® III<br />
Before using the BEP ® III, prepare the test plates and sample dispensing steps (Section 1 and 2 at "Test<br />
procedure with the BEP ® II"). Immediately afterwards place the uncovered test plates (i.e. not covered with<br />
adhesive foil) into the BEP ® III. Note that partially filled test plates need to be made up to at least half plates (6<br />
test strips) by adding "water-filled strips". The test is then processed fully automatically (see BEP ® III instruction<br />
manual).<br />
The settings for the incubation times in the BEP ® III software may differ from the times on the BEP ® II for<br />
technical reasons (system speed) but have been validated for Enzygnost * on the BEP ® III.<br />
Test procedure using the BEP ® 2000<br />
The sample dispensing steps and subsequent processing steps in the test are performed fully automatically in<br />
the instrument (see instruction manual for the BEP ® 2000).<br />
Test validation<br />
The individual values of the absorbances for the control sera must comply with the following specification:<br />
-0.010 ≤ A ≤ 0.150<br />

If one of the four absorbance values of the Control Serum, negative, is outside the specification, this value can<br />
be neglected.<br />
Both absorbance values of the positive control must comply with the specification.<br />
If these conditions are not fulfilled, the test is to be repeated.<br />
Evaluation<br />
On the BEP ® 2000 and BEP ® III the results are evaluated automatically. Please consult the instruction manuals<br />
for further details in this respect. The following sections apply if the results are evaluated without software<br />
assistance.<br />
Calculate the mean absorbance value of the valid negative controls and then calculate the cut-off value by<br />

Samples with absorbances within or above the retest range are to be retested in dual determination. If in the<br />
retest absorbances below the retest range are yielded, the sample is to be considered antibody negative as<br />
defined by the criteria of the test.<br />
If the mean absorbance of the sample is again within the retest range or above this range, the sample is to be<br />
considered repeatedly reactive. All samples which are equivocal or reactive in the retest must be checked in a<br />
confirmatory test (e.g. Western Blot analysis).<br />
Limitations of the Procedure<br />
1. <strong>Anti</strong>coagulants such as heparin, EDTA and citrate do not interfere with the test result.<br />
2. Lipaemic, icteric or haemolytic samples do not interfere with the test.<br />
3. Samples containing potential interference factors, rheumatoid factors, EBV/IgM antibodies, VZV/IgM antibodies,<br />
E.-Coli antibodies, ANA and AMA, as well as sera from pregnant women, were checked in the test.<br />
The samples used were found not to interfere with the results.<br />
4. No interference was observed with samples which had been repeatedly frozen and thawed, or with sera<br />
which had been heat-treated (30 min, +56 °C).<br />
5. Inadequately coagulated sera, or microbially contaminated samples, should not be used. Any particulate<br />
constituents (e.g. fibrin clots, erythrocytes) must be removed before the assay.<br />
6. For thawed samples ensure good homogenisation of the material.<br />
7. The test is suitable only for testing individual samples and not for pooled or diluted samples.<br />
8. Reagents must not be interchanged with other kits unless the reagents are from an identical lot (i.e. they<br />
must display the required 6-digit lot numbers printed on the pack and given in the enclosed barcode table).<br />
Exceptions are Washing Solution POD, Stopping Solution POD and the Working Chromogen Solution<br />
(which is prepared from Chromogen TMB and Buffer/Substrate TMB using the required lots).<br />

9. Buffer/Substrate TMB, Working Chromogen Solution and Stopping Solution POD must not be allowed to<br />
come into contact with heavy metal ions or oxidizing substances (do not use pipettes with metal parts<br />
which are in direct contact with the liquid). Do not perform the substrate reaction in the proximity of disinfectants<br />
containing hypochlorite. If the Working Chromogen Solution has developed a blue colour before<br />
transferral into the test plate, this indicates that the solution is contaminated; in such cases prepare a fresh<br />
solution in a clean container. Skin contact with the above-mentioned solutions is to be avoided.<br />
10. The test plate should be protected from vibration during the incubation phase (e.g. placed on a secured<br />
floatation aid, or in a non-circulating water bath); the wells of the plate must be in contact with the thermostated<br />
water. If stabilizers are used to prevent microbial contamination of the water, care must be taken that<br />
neither the surface of the test plate nor the wells come into contact with these solutions since such contamination<br />
can lead to unspecific reactions.<br />
11. Highly reactive samples may cause precipitation of the dye during the stopping reaction. This does not<br />
interfere with the photometric evaluation.<br />
12. The control sera have been prepared from native human sera. As a result, turbidity may occur but this has<br />
no effect on the test result.<br />
13. Dade Behring has validated use of these reagents on various analyzers to optimize product performance<br />
and meet product specifications. User defined modifications are not supported by Dade Behring as they<br />
may affect performance of the system and assay results. It is the responsibility of the user to validate<br />
modifications to these instructions or use of the reagents on analyzers other than those included in Dade<br />
Behring Application Sheets or these instructions for use.<br />
14. Results of this test should always be interpreted in conjunction with the patient’s medical history, clinical<br />
presentation and other findings.<br />
Specific Performance Characteristics<br />
Sensitivity and Specificity<br />
The results of the study on sensitivity and specificity are summarized in Tables 2 + 3 (see Appendix).<br />
To establish the sensitivity, a total of 566 anti-<strong>HIV</strong>1, 317 anti-<strong>HIV</strong>2 and 22 anti-<strong>HIV</strong>1 (Subtype O) positive<br />
samples were investigated. All the tested 905 anti-<strong>HIV</strong>-positive samples were found to be positive as defined<br />
by the criteria of the Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> test. The reactivity of the test with regard to seroconversion<br />
samples was studied using 31 seroconversion panels. The results of the study show that, in the<br />
detection of seroconversions, Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> exhibits a degree of sensitivity which matches<br />
that of comparable tests. Nevertheless the possibility cannot be ruled out that isolated samples may escape<br />
detection when the test is used on a large scale.<br />
To establish the specificity, a total of 9243 anti-<strong>HIV</strong>-negative samples were investigated and the specificity was<br />
found to be 99.3 to 100 % (initial testing) and 99.9 % (retest result). Deviations from these findings may occur<br />
due to variations in sample population, test procedure, etc.<br />
Current knowledge indicates that a positive result in the anti-<strong>HIV</strong> test does not yield definite evidence that the<br />
blood contains infectious <strong>HIV</strong>1 or <strong>HIV</strong>2, just as a negative test result also does not definitely exclude the<br />
presence of <strong>HIV</strong>1 or <strong>HIV</strong>2.<br />
Reproducibility<br />
The results on intra-assay and inter-assay reproducibility are summarized in Table 4 (in the Appendix). The<br />
data shown are typical results. Depending on various factors (procedure, etc), different values may well be<br />
obtained.<br />

Anwendungsbereich<br />
Enzymimmunoassay zum Nachweis von <strong>Anti</strong>körpern gegen <strong>HIV</strong>1-, <strong>HIV</strong>2- und <strong>HIV</strong>1 (Subtyp O)-<strong>Anti</strong>gene in Serum<br />
oder Plasma. Die Abarbeitung des Enzymimmunoassays erfolgt mit den ELISA Prozessoren BEP ® II, BEP ® III und<br />
BEP ® 2000. Der Test wurde entwickelt für die Untersuchung von Einzelproben, nicht von gepoolten Proben. Das<br />
Produkt darf nur für in vitro diagnostische Zwecke angewendet werden.<br />
Diagnostische Bedeutung<br />
Das erworbene Immundefektsystem (AIDS) wurde erstmalig 1981 als eigenständiges Krankheitsbild erkannt.<br />
Als ursächliche Erreger gelten heute zwei verschiedene Human Immundefizienz Viren (<strong>HIV</strong>1 und <strong>HIV</strong>2). Die<br />
Isolierung des <strong>HIV</strong>1 (Synonyom: LAV/HTLV III) gelang bereits 1983 1, 2 . Im Jahre 1986 wurde ein weiteres<br />
Virus (<strong>HIV</strong>2) isoliert, das ebenfalls als humanpathogen gilt 3 . Die Bedeutung der serologischen Bestimmung<br />
von <strong>Anti</strong>körpern gegen <strong>HIV</strong> hat besonders für die Transfusionsmedizin unter dem Gesichtspunkt der Prävention<br />
zur Verhütung der weiteren Ausbreitung der Erkrankung einen hohen Stellenwert.<br />
Folgerichtig wurde ab 1985 begonnen, Spenderblut auf <strong>Anti</strong>-<strong>HIV</strong>1-<strong>Anti</strong>körper sowohl in Blutbanken als auch<br />
bei Herstellern von Plasmaprodukten zu testen. Zwischenzeitliche Informationen über die Verbreitung von<br />
<strong>HIV</strong>2-Infektionen ergaben das Bedürfnis einer routinemäßigen Untersuchung von Blutspendern außer auf<br />
<strong>Anti</strong>-<strong>HIV</strong>1- auch auf <strong>Anti</strong>-<strong>HIV</strong>2-<strong>Anti</strong>körper, um eine potentielle Übertragung durch Transfusion oder aus Blut<br />
hergestellten Präparaten nach Möglichkeit auszuschließen.<br />
Im Jahre 1990 wurde ein neuer <strong>HIV</strong> Subtyp beschrieben 4 . 1994 gelang gleichzeitig die Sequenzierung dieses<br />
neuen Subtyps bei zwei unabhängigen Arbeitsgruppen 5 ,6 . Parallel zu den virologischen Arbeiten konnte von<br />
mehreren Arbeitsgruppen die serologische Bedeutung von dem nun <strong>HIV</strong>1-Subtyp O 7 genannten Virus gezeigt<br />
werden. Danach zeigen manche <strong>Anti</strong>-<strong>HIV</strong> 1+2-Assays Schwächen bei der Detektion von <strong>Anti</strong>-Subtyp O-<strong>Anti</strong>körpern<br />
8 . Folgerichtig entstand der Bedarf zum gleichzeitigen sicheren Nachweis dieser neuen <strong>HIV</strong>-Variante<br />
und den etablierten <strong>HIV</strong>1- und <strong>HIV</strong>2-Isolaten.<br />
Obwohl der Nachweis von <strong>HIV</strong>1- und <strong>HIV</strong>2-spezifischen <strong>Anti</strong>körpern keine sicheren Aussagen darüber erlaubt,<br />
ob zum Zeitpunkt des Testes infektiöses <strong>HIV</strong>1 bzw. <strong>HIV</strong>2 im Blut vorhanden ist, als auch bei negativem<br />
Ergebnis keineswegs die Anwesenheit von <strong>HIV</strong>1 oder <strong>HIV</strong>2 sicher ausgeschlossen werden kann, stellt der<br />
<strong>Anti</strong>körpertest dennoch die derzeit beste Möglichkeit dar, Blutspenden von <strong>HIV</strong>-Infizierten mit großer Wahrscheinlichkeit<br />
zu erkennen und zu eliminieren.<br />
Prinzip der Methode<br />
In der Untersuchungsprobe enthaltene <strong>HIV</strong>-spezifische Immunglobuline binden sich an die auf der Oberfläche<br />
der Mikrotitrationsplatte fixierten rekombinanten <strong>HIV</strong>1-, <strong>HIV</strong>2- und/oder <strong>HIV</strong>1 (Subtyp O)-Proteine.<br />
Nach Entfernung der ungebundenen Probenbestandteile wird das Konjugat an die <strong>HIV</strong>-spezifischen <strong>Anti</strong>körper<br />
gebunden.<br />
Nach Entfernung des überschüssigen Konjugates wird die gebundene Enzymaktivität des Konjugates bestimmt<br />
(blaue Farbreaktion). Die enzymatische Umsetzung des Chromogens wird durch Zusatz von Stopplösung<br />
POD unterbrochen (gelbe Farbreaktion). Die Farbintensität ist der in der Probe vorhandenen <strong>Anti</strong>körperkonzentration<br />
proportional.<br />

Mit einem Gemisch von rekombinanten <strong>HIV</strong>(env)-Proteinen (E.Coli) beschichtete Mikrotitrationsplatte. Die<br />
Proteine repräsentieren immunreaktive Bereiche folgender Virusproteine:<br />

Warnungen und Vorsichtsmaßnahmen<br />
1. Nur zur in vitro diagnostischen Anwendung.<br />
2. Jede individuelle Blutspende, die zur Herstellung von Kontroll-Sera vorgesehen ist, wurde auf HBsAg, <strong>Anti</strong>-HCV,<br />
<strong>Anti</strong>-<strong>HIV</strong>1 und <strong>Anti</strong>-<strong>HIV</strong>2 untersucht. Für die Herstellung des Kontroll-Serums, negativ, werden nur Sera mit negativem<br />
Befund verwendet. Das positive Kontroll-Serum wurde hitzebehandelt (mindestens 1 Stunde bei +56 °C).<br />
Unabhängig davon sollten alle aus menschlichem Blut gewonnenen Proben (z. B. Patientensera) und<br />
Produkte (z. B. Kontrollsera) wegen nie auszuschließender Gefährdung durch Krankheitserreger mit angemessener<br />
Sorgfalt unter Einhaltung der bei Biogefährdung empfohlenen Sicherheitsmaßnahmen gehandhabt<br />
werden9 .<br />
3. Das Tragen von Untersuchungshandschuhen während der gesamten Testdurchführung wird angeraten.<br />
4. Für die Entsorgung fester infektiöser Materialien empfiehlt sich eine Autoklavierung von mindestens 1<br />
Stunde bei +121 °C. Alle abgesaugten Lösungen sind in zwei hintereinandergeschalteten Vorlagen zu<br />
sammeln. Die Vorlagen sollten ein Desinfektionsmittel enthalten, das geeignet ist, human-pathogene Viren<br />
zu inaktivieren. Die vom Hersteller angegebenen Konzentrationen und Einwirkungszeiten müssen<br />
beachtet werden.<br />
5. Die Reagenzien (ausgenommen Waschlösung POD, Stopplösung POD und die aus den chargengebundenen<br />
Reagenzien Chromogen TMB und Puffer/Substrat TMB hergestellte Chromogen-Gebrauchslösung)<br />
sind nur chargengebunden zu verwenden, d.h. nur in der Kombination der einzelnen 6-ziffrigen<br />
Chargen-Bezeichnungen, die auf der Packung aufgedruckt bzw. der separat beigepackten Barcode-Wertetabelle<br />
zu entnehmen sind.<br />
Vorbereitung der Reagenzien<br />
Alle Reagenzien und Proben vor Testbeginn auf +18 bis +25 °C erwärmen. Dabei die Testplatte nicht dem<br />
Behältnis entnehmen.<br />
Der Packung Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> 10 x 96 liegt ein barcodiertes Etikett für eine Leerflasche für das <strong>HIV</strong>-<br />
Ag/POD-Konjugat bei. Um bei großen Serienlängen einen häufigen Spritzenwechsel am BEP ® III zu vermeiden,<br />
kann es sinnvoll sein, mehrere Abfüllungen des <strong>HIV</strong>-Ag/POD-Konjugats in eine größere Flasche (z.B. noch unbenutzte<br />
überzählige Leerflasche für Chromogen-Gebrauchslösung oder gespülte Flasche der Stopplösung POD)<br />
umzuschütten, mit dem Etikett "Leerflasche für Konjugat" zu etikettieren und ins Reagenzienrad einzustellen.<br />

Je Testplatte 20 ml Waschlösung POD mit destilliertem oder entionisiertem Wasser auf 400 ml verdünnen.<br />
Chromogen-Gebrauchslösung: Je Testplatte 1 ml Chromogen TMB mit 10 ml Puffer/Substrat TMB in der<br />
mitgelieferten Kunststoffflasche verdünnen (Chromogen-Gebrauchslösung) und verschlossen unter Lichtschutz<br />
aufbewahren. Flasche nach Gebrauch sorgfältig mit destilliertem Wasser spülen.<br />
Wegen technisch bedingter Überfüllung ist das Zusammengießen der Flascheninhalte von Chromogen TMB<br />
und Puffer/Substrat TMB nicht zulässig.<br />
Haltbarkeit und Lagerungsbedingungen<br />
Ungeöffnet sind alle Bestandteile der Kombinationspackung Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> bei der angegebenen<br />
Lagertemperatur bis zu den auf den Etiketten angegebenen Daten verwendbar.<br />
Die Haltbarkeit und Lagerbedingungen der geöffneten bzw. gebrauchsverdünnten Reagenzien sind der Tabelle<br />
1 im Anhang zu entnehmen.<br />
Erforderliche Geräte:<br />
BEP ® II: Zur automatischen Durchführung der Reagenzien-Dosierung und der Waschschritte<br />

Untersuchungsmaterial<br />
Zur Untersuchung können Einzelproben (Human-Seren oder -EDTA/Heparin/Citrat-Plasmen) verwendet werden,<br />
welche nach Standard-Labortechniken entnommen wurden. Die Proben sollten maximal 3 Tage bei +2 bis<br />
+8 °C gelagert werden. Zur längeren Lagerung sind die Proben einzufrieren.<br />
Testdurchführung<br />
Testdurchführung mit BEP ® II<br />
1. Ansatzschema:<br />
Benötigte Anzahl der Auftragsstellen feststellen (Anzahl der zu untersuchenden Proben plus 6 Vertiefungen<br />
für Kontrollen). Für die Testdurchführung nicht benötigte Riegel dem Halterahmen entnehmen und für<br />
die spätere Verwendung lagern (siehe Tabelle 1).<br />
2. Proben-Puffer-Vorlage:<br />
Je Vertiefung 25 μl Proben-Puffer vorlegen.<br />
3. Proben-Dosierung:<br />
In 4 Vertiefungen je 100 μl negative und in 2 Vertiefungen je 100 μl positive Kontrolle einfüllen; in die<br />
folgenden Vertiefungen je 100 μl unverdünnte Probe dosieren, mit Folie abkleben und baldmöglichst,<br />
maximal 15 min nach Beendigung der Proben-Dosierung, in den Inkubator stellen.<br />
Alternativ zu oben aufgeführtem Pipettierschema ist es auch erlaubt, die positive Kontrolle einmal zu<br />
Beginn und einmal am Ende der Testreihe aufzutragen.<br />
Pipettierschema: In 4 Vertiefungen je 100 μl negative, in eine Vertiefung 100 μl positive Kontrolle, in die<br />
folgenden Vertiefungen je 100 μl unverdünnte Probe und am Ende der Serie bzw. Testplatte noch einmal<br />
100 μl positive Kontrolle dosieren.<br />
4. Proben-Inkubation:<br />
30 ± 2 Minuten bei +37 ± 1 °C inkubieren, Waschvorgang unmittelbar anschließen.<br />
5. Waschen:<br />
Folie abziehen, alle Vertiefungen aussaugen und mit je ca. 0,3 ml Waschlösung 4mal waschen.<br />
6. Konjugat-Dosierung:<br />
In jede Vertiefung 100 μl Konjugat einfüllen, mit neuer Folie abkleben und sofort in den Inkubator stellen.<br />
7. Konjugat-Inkubation:<br />
30 ± 2 Minuten bei +37 ± 1 °C, wie unter 4. beschrieben, inkubieren, Waschvorgang unmittelbar anschließen.<br />
8. Waschen:<br />

9. Substrat-Dosierung:<br />
In jede Vertiefung 100 μl Chromogen-Gebrauchslösung einfüllen, Platte mit neuer Folie abkleben.<br />

30 ± 2 Minuten bei +18 bis +25 °C lichtgeschützt inkubieren.<br />
11. Stoppreaktion:<br />
Die Folie entfernen. Je Vertiefung 100 μl Stopplösung POD zugeben, dabei den gleichen Zeittakt wie bei<br />
Punkt 9. einhalten.<br />
12. Messung:<br />
Innerhalb einer Stunde bei 450 nm photometrieren.<br />
Die Extinktionsmessung der Kontroll- und Patientenproben erfolgt bei 450 nm, als Wellenlänge der Referenzmessung<br />
wird 650 nm (ggfs. zwischen 615 und 690 nm) empfohlen.<br />

Testdurchführung mit BEP ® III<br />
Bei der Abarbeitung mit BEP ® III müssen die Testplatten einschließlich der Probendosierung (Punkt 1 und 2<br />
der "Testdurchführung mit BEP ® II") vorbereitet werden. Unmittelbar im Anschluss daran werden die Testplatten<br />
offen, d.h. nicht mit Folie abgeklebt, in den BEP ® III eingegeben. Dabei ist zu beachten, dass teilbestückte<br />
Testplatten mit "Wasserriegeln" auf mindestens halbe Testplatten (6 Testriegel) zu ergänzen sind. Die anschließende<br />
Testabarbeitung erfolgt vollautomatisch (siehe BEP ® III-Bedienungsanleitung).<br />
Die in der BEP ® III Software eingestellten Inkubationszeiten können auf Grund der technischen Rahmenbedingungen<br />
(Gerätetaktung) von denen der BEP ® II Prozessierung abweichen, sind jedoch in der Kombination<br />
BEP ® III/Enzygnost * validiert worden.<br />
Testdurchführung mit BEP ® 2000<br />
Die Probendosierung und anschließende Testabarbeitung erfolgt vollautomatisch im Gerät (siehe BEP ® 2000-<br />
Bedienungsanleitung).<br />
Testvalidierung<br />
Die Einzelwerte der Extinktionen für die Kontroll-Sera müssen folgende Spezifikationen erfüllen:<br />

Von den vier Extinktionswerten des Kontroll-Serums, negativ, kann ein außerhalb der Spezifikation liegender<br />
Wert vernachlässigt werden.<br />
Die Extinktionswerte der positiven Kontrolle müssen beide die Spezifikation erfüllen.<br />
Werden diese Bedingungen nicht erfüllt, ist der Test zu wiederholen.<br />
Testauswertung<br />
Die Auswertungen erfolgen mit BEP ® 2000 und BEP ® III automatisch. Bitte dazu die Bedienungsanleitungen<br />
heranziehen. Die nachfolgenden Kapitel sind bei Auswertung ohne Softwareunterstützung zu beachten.<br />
Aus den validen Extinktionswerten der negativen Kontrolle wird der Mittelwert gebildet.<br />
Zur Grenzwertberechnung wird zum Extinktionsmittelwert der negativen Kontrolle ein Wert von 0,400 addiert:<br />

Proben, die Extinktionswerte in der Grauzone oder höhere Extinktionen ergeben, sind in Doppelbestimmung<br />
erneut zu testen. Ergeben sich bei der Wiederholungstestung Extinktionswerte unterhalb der Grauzone, ist die<br />
Probe nach den Kriterien des Tests als <strong>Anti</strong>körper-negativ anzusehen.<br />
Bei erneut grenzwertigen oder höheren Extinktionsmittelwerten gilt die Probe als wiederholt reaktiv. Alle wiederholt<br />
grenzwertig oder reaktiv reagierenden Proben müssen einer Bestätigung mit Hilfe sogenannter Konfirmationstests<br />
(z.B. Western-Blot-Analyse) zugeführt werden.<br />
Einschränkungen der Testdurchführung<br />
1. <strong>Anti</strong>koagulantien wie Heparin, EDTA und Citrat beeinflussen das Testergebnis nicht.<br />
2. Lipämische, ikterische oder hämolytische Proben führen zu keiner Testbeeinträchtigung.<br />
3. Proben mit den potentiell störenden Substanzen Rheumafaktoren, EBV/IgM-<strong>Anti</strong>körper, VZV/IgM-<strong>Anti</strong>körper,<br />
E.-Coli-<strong>Anti</strong>körper, ANA und AMA sowie Seren von Schwangeren wurden mit dem Test überprüft. Mit<br />
den eingesetzten Proben wurde keine Beeinflussung des Testergebnisses beobachtet.<br />
4. Bei mehrfach eingefrorenen und aufgetauten Proben sowie bei hitzebehandelten Seren (30 min. +56 °C)<br />
wurden keine Störungen beobachtet.<br />
5. Ungenügend geronnene Seren und mikrobiell kontaminierte Untersuchungsproben sollten nicht eingesetzt<br />
werden. Eventuell vorhandene partikuläre Komponenten (z.B. Fibrin-Gerinnsel, Erythrozyten) müssen<br />
vor der Testdurchführung entfernt werden.<br />
6. Bei aufgetauten Proben ist auf eine gute Homogenisierung des Materials zu achten.<br />
7. Der Test ist nur für die Untersuchung von Einzelproben, nicht von gepoolten oder verdünnten Proben geeignet.<br />
8. Die Reagenzien (ausgenommen Waschlösung POD, Stopplösung POD und die aus den chargengebundenen<br />
Reagenzien Chromogen TMB und Puffer/Substrat TMB hergestellte Chromogen-Gebrauchslösung)<br />
sind nur chargengebunden zu verwenden, d.h. nur in der Kombination der einzelnen 6-ziffrigen<br />
Chargen-Bezeichnung, die auf der Packung aufgedruckt bzw. der separat beigepackten Barcode-Tabelle<br />
zu entnehmen sind.<br />

9. Puffer/Substrat TMB, Chromogen-Gebrauchslösung und Stopplösung POD dürfen nicht in Kontakt mit<br />
Schwermetallionen oder oxidierenden Substanzen kommen (keine Pipetten mit flüssigkeitsführenden<br />
Metallteilen verwenden). Die Substratreaktionen nicht in der Nähe von Hypochlorit-haltigen Desinfektionsmitteln<br />
durchführen. Eine spontane Blaufärbung der Chromogen-Gebrauchslösung vor der Übertragung in<br />
die Testplatte deutet auf Kontamination hin; eine frische Lösung ist in einem sauberen Gefäß anzusetzen.<br />
Hautkontakt mit den vorgenannten Lösungen ist zu vermeiden.<br />
10. Die Testplatte soll während der Inkubation ruhig liegen (z.B. auf fixierter Schwimmhilfe oder im nicht zirkulierenden<br />
Wasserbad); die Kavitäten sind dabei in Kontakt mit dem temperierten Wasser. Werden Stabilisatoren<br />
zur Verhinderung der Verkeimung des Wassers verwendet, so ist sorgfältig darauf zu achten, dass<br />
weder die Testplattenoberfläche noch die Näpfchen mit diesen Lösungen in Kontakt kommen, da dadurch<br />
unspezifische Reaktionen hervorgerufen werden könnten.<br />
11. Bei stark reaktiven Proben kann bei der Stoppreaktion der Farbstoff ausfallen. Die photometrische Auswertung<br />
wird dadurch nicht beeinflusst.<br />
12. Die Kontrollsera sind unter Verwendung nativer Humansera hergestellt. Daher können Trübungen auftreten,<br />
die das Testergebnis jedoch nicht beeinflussen.<br />
13. Dade Behring hat den Einsatz dieser Reagenzien auf verschiedenen Analysengeräten auf optimale Produktleistung<br />
und Einhaltung der Produktspezifikationen überprüft. Vom Benutzer vorgenommene Änderungen<br />
werden von Dade Behring nicht unterstützt, da sie die Leistung des Systems und die Testergebnisse<br />
beeinflussen können. Es liegt in der Verantwortung des Benutzers, Änderungen an diesen Anleitungen<br />
oder die Verwendung dieser Reagenzien auf anderen als in den Applikationsvorschriften von Dade<br />
Behring oder diesen Gebrauchsanweisungen genannten Analysengeräten zu validieren.<br />
14. Resultate dieses Tests sollten stets in Verbindung mit der Vorgeschichte des Patienten, dem klinischen<br />
Bild und anderen Untersuchungsergebnissen interpretiert werden.<br />
Leistungsmerkmale des Tests<br />
Sensitivität und Spezifität<br />
Die Ergebnisse zur Prüfung der Sensitivität und Spezifität sind in den Tabellen 2 + 3 (im Anhang) zusammengefaßt.<br />
Bei der Ermittlung der Sensitivität wurden insgesamt 566 <strong>Anti</strong>-<strong>HIV</strong>1, 317 <strong>Anti</strong>-<strong>HIV</strong>2 und 22 <strong>Anti</strong>-<strong>HIV</strong>1 (Subtyp O) positive<br />
Proben untersucht. Alle getesteten 905 <strong>Anti</strong>-<strong>HIV</strong> positiven Proben wurden nach den Kriterien des Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2<br />
<strong>Plus</strong> positiv gefunden. Die Reaktivität des Tests bei Serokonversionsproben wurde anhand von 31 Serokonversionspanel<br />
untersucht. Dabei zeigte sich, dass Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> eine Sensivität bzgl. der Erkennung von Serokonversionen<br />
aufweist, die der Empfindlichkeit vergleichbarer Tests entspricht. Unabhängig davon kann nicht ausgeschlossen<br />
werden, daß sich bei breiter Anwendung des Tests einzelne Proben dem Nachweis entziehen können.<br />
Bei der Ermittlung der Spezifität wurden insgesamt 9243 <strong>Anti</strong>-<strong>HIV</strong>- negative Blutproben untersucht und eine<br />
Spezifität von 99,3 bis 100 % (initiale Testung) bzw. 99,9 % (Retestung) ermittelt. Bedingt durch das Untersuchungskollektiv,<br />
die Testdurchführung u.a. sind abweichende Werte möglich.<br />
Nach derzeitigem Kenntnisstand kann aus einem positiven Ergebnis der <strong>Anti</strong>-<strong>HIV</strong>-Testung nicht mit Sicherheit<br />
abgeleitet werden, dass infektiöses <strong>HIV</strong>1 oder <strong>HIV</strong>2 im Blut vorhanden ist, wie auch ein negatives Testergebnis<br />
keineswegs die Anwesenheit von <strong>HIV</strong>1 oder <strong>HIV</strong>2 sicher ausschließt.<br />
Reproduzierbarkeit<br />
Die Ergebnisse zur Intra/Inter-assay Reproduzierbarkeit sind in der Tabelle 4 (im Anhang) zusammengefasst.<br />
Es handelt sich hierbei um beispielhaft ermittelte Daten. Abhängig von der Testdurchführung u.a. sind durchaus<br />
abweichende Werte möglich.<br />

Domaines d'utilisation<br />
Test immunoenzymatique pour le dépistage des anticorps anti-antigènes VIH1, VIH2 et VIH1 (sous-type O)<br />
dans le sérum ou le plasma. La réalisation du test immunoenzymatique se fait à l’aide des ELISA Processeurs<br />
BEP ® II, BEP ® III et BEP ® 2000. Le test a été mis au point pour l'analyse d'échantillons individuels, et non de<br />
pools d'échantillons. Le produit ne doit être utilisé qu'à des fins de diagnostic in vitro.<br />
Intérêt Diagnostique<br />
Le Syndrome d’immunodéficience acquise (SIDA) a été reconnu pour la première fois en 1981 comme tableau<br />
clinique autonome. Deux virus différents d’immunodéficience humaine (VIH1 et VIH2) sont aujourd’hui<br />
considérés comme responsables de la maladie. L’isolement de l'VIH1 (synonymes : LAV/HTLV III) a réussi<br />
dès 1983 1,2 . En 1986, un deuxième virus (VIH2), également pathogène pour l’Homme, a été isolé 3 . Le dosage<br />
sérologique des anticorps anti-VIH présente un intérêt particulier pour la médecine transfusionnelle, puisqu’il<br />
permet de prévenir la propagation de la maladie.<br />
Depuis 1985, la recherche d’anticorps anti-VIH1 sur les dons de sang est obligatoire aussi bien pour les<br />
centres de transfusion sanguine que pour les fabricants de produits sanguins. Des informations complémentaires<br />
sur le développement d’infections à VIH2 ont depuis généralisé le dépistage en routine des anticorps<br />
anti-VIH1 et VIH2, dans le souci d’exclure le transfert possible de la maladie par la transfusion ou l’utilisation<br />
de préparations sanguines.<br />
En 1990, un nouveau sous-type d'VIH a été décrit 4 . Ce n'est qu'en 1994 que deux équipes indépendantes<br />
l'une de l'autre ont réussi simultanément à définir la séquence de ce nouveau sous-type 5, 6 . Parallèlement à<br />
ces travaux virologiques, plusieurs équipes de recherche ont pu montrer l'intérêt sérologique du virus,<br />
aujourd'hui appelé VIH1-sous-type O 7 . Certains test anti-VIH 1+2 ont alors montré leur faiblesse à révéler les<br />
anticorps anti-sous-type O 8 . Le besoin est donc apparu de disposer d'un test fiable pour la recherche simultanée<br />
de ce nouveau variant VIH et des isolats VIH1 et VIH2 déjà établis.<br />
Bien que la mise en évidence d’anticorps anti-VIH1 ou anti-VIH2-spécifiques ne permette pas de dire avec certitude si<br />
le sang contient de l’VIH1 ou de l’VIH2 infectieux au moment du test, de même qu’un résultat négatif ne permet pas<br />
d’exclure avec certitude la présence d’VIH1 ou d’VIH2, le test des anticorps constitue pourtant actuellement la meilleure<br />
solution pour dépister et éliminer avec une grande probabilité les donneurs infectés par l’VIH.<br />
Principe de la méthode<br />
Les immunoglobulines VIH-spécifiques contenues dans l'échantillon à tester se lient aux protéines VIH1, VIH2<br />
et/ou VIH1 (sous-type O) recombinantes fixées à la surface de la plaque de microtitration.<br />
Après élimination par lavage des éléments non liés de l'échantillon, le conjugué se fixe aux anticorps VIH-spécifiques.<br />
Après élimination du conjugué en excès, l'activité enzymatique du conjugué fixé est mesurée (réaction colorée<br />
bleue). La transformation enzymatique du chromogène est stoppée par l'addition de Solution d'arrêt POD<br />
(réaction colorée jaune). L'intensité de la coloration est proportionnelle à la concentration d'anticorps présents<br />
dans l'échantillon.<br />

Mises en garde et précautions d’emploi<br />
1. Ne doit être employé que pour un usage in vitro.<br />
2. Tout don de sang prévu pour la préparation des sérums de contrôle est soumis à une recherche<br />
d'AgHBs, d'anti-VHC, d'anti-VIH1 et d'anti-VIH2. Pour la préparation du sérum de contrôle négatif, seuls<br />
les sérums trouvés négatifs sont utilisés. Le sérum de contrôle positif est traité par la chaleur (au moins<br />
1 heure à +56 °C).<br />
Indépendamment de cela, tout échantillon (par ex. sérum de patient) ou produit (par ex. sérum de contrôle)<br />
obtenu à partir de sang humain doit être manipulé avec les précautions nécessaires en cas de risque<br />
biologique, dans la mesure où on ne peut exclure totalement tout risque d'infection9 .<br />
3. Il est recommandé de porter des gants de protection pendant toute la réalisation du test.<br />
4. Pour décontaminer le matériel de laboratoire infecté, l’autoclaver pendant au moins 1 heure à +121°C.<br />
Toutes les solutions aspirées doivent être récoltées dans deux récipients reliés l’un à l’autre et contenant<br />
chacun un désinfectant spécialement prévu pour inactiver les virus pathogènes pour l’homme. Respecter<br />
les concentrations et temps d’incubation indiqués par le fabricant.<br />
5. Les réactifs (à l’exception de la Solution de lavage POD, de la Solution d’arrêt POD et de la solution<br />
d’emploi du chromogène obtenue à partir des lots de réactifs indiqués pour le Chromogène TMB et le<br />
Tampon/substat TMB) ne peuvent être utilisés que selon la combinaison des numéros de lots à 6 chiffres<br />
indiqués sur le coffret et dans le tableau des valeurs codes-barres joint.<br />
Préparation des réactifs<br />
Porter tous les réactifs et échantillons à +18/+25 °C avant le début du test, sans sortir la plaque-test de son emballage.<br />
Le coffret Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> 10 x 96 contient une étiquette avec code à barres pour un flacon vide<br />
destiné au Conjugué Ag-<strong>HIV</strong>/POD. Pour éviter de changer trop souvent de seringue sur le BEP ® III si on<br />
effectue de grandes séries d’analyses, verser le contenu de plusieurs flacons de Conjugué Ag-<strong>HIV</strong>/POD dans<br />
un flacon plus grand (par ex. le flacon vide pour la solution d’emploi du chromogène si on ne l’utilise pas, ou un<br />
flacon rincé de Solution d’arrêt POD), apposer l’étiquette «Flacon vide pour Conjugué», et placer le flacon<br />
dans le carrousel réactifs.<br />

Pour une plaque, diluer 20 ml de Solution de lavage POD avec de l'eau distillée ou désionisée en complétant à 400 ml.<br />
Solution d'emploi du chromogène : pour une plaque, diluer 1 ml de Chromogène TMB avec 10 ml de<br />
Tampon/substrat TMB dans le flacon vide joint au coffret (solution d'emploi du chromogène), et conserver la<br />
solution, le flacon fermé, à l’abri de la lumière. Après utilisation, bien rincer le flacon à l’eau distillée.<br />
Pour des raisons de remplissage du flacon, il est impossible de verser la totalité du contenu d'un flacon de<br />
Chromogène TMB dans un flacon de Tampon/substrat TMB.<br />
Stabilités et conditions de conservation<br />
Avant leur ouverture, tous les éléments du coffret Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> se conservent à la température<br />
indiquée jusqu'à la date indiquée sur l'étiquette.<br />
Pour les stabilités et conditions de conservation après ouverture des flacons et préparation des réactifs à leur<br />
dilution d'emploi, se reporter au tableau 1 en annexe.<br />
Matériel nécessaire:<br />
BEP ® II : pour la distribution automatique des réactifs et l'automatisation des étapes de lavage<br />
BEP ® III: pour l'automatisation du test après la distribution des échantillons et de l'exploitation<br />
des résultats<br />
BEP ® 2000 : pour l’entière automatisation du test et de l’ exploitation des résultats<br />
Pipettes: pipettes à embout de 25, 100 et 1000 μl<br />
Incubateur: bain-marie couvert (+37 ± 1 °C) ou méthode d'incubation équivalente<br />
Tout le matériel utilisé pour la réalisation du test doit être validé.<br />
Echantillons à tester<br />
Utiliser des échantillons (sérums humains ou plasmas citratés, héparinés ou prélevés sur EDTA) obtenus<br />
selon les techniques standard des laboratoires. Les échantillons peuvent être conservés 3 jours maximum à<br />
+2/+8 °C. Pour une conservation plus longue, les congeler.<br />
Réalisation du test<br />
Réalisation du test à l’aide du BEP ® II<br />
1. Plan de distribution :<br />
déterminer le nombre de cupules nécessaires (nombre d'échantillons à tester + 6 cupules pour les contrôles).<br />
Sortir du cadre les barrettes inutiles et les conserver pour un test futur (cf. Tableau 1).<br />
2. Prédistribution du Tampon échantillons :<br />
distribuer dans chaque cupule 25 μl de Tampon échantillons.<br />
3. Distribution des contrôles et échantillons :<br />
distribuer dans 4 cupules 100 μl de contrôle négatif, dans 2 cupules 100 μl de contrôle positif, puis 100 μl<br />
de chacun des échantillons non dilués dans les cupules suivantes. Couvrir d'une feuille adhésive et<br />
placer la plaque le plus rapidement possible dans l'incubateur, au plus tard 15 min après la fin de la<br />
distribution des échantillons.<br />
Comme alternative au schéma de pipetage indiqué ci-dessus, on peut également déposer le contrôle<br />
positif une fois en début et une fois en fin de série.<br />
Schéma de pipetage : distribuer 100 μl de contrôle négatif dans les 4 premières cupules, 100 μl de<br />
contrôle positif dans la cupule 5, 100 μl de chacun des échantillons non dilués dans les cupules suivantes,<br />
puis de nouveau 100 μl de contrôle positif à la fin de la série ou de la plaque.<br />
4. Incubation des échantillons :<br />
laisser incuber 30 ± 2 min à +37 ± 1 °C, et enchaîner immédiatement le processus de lavage.<br />
5. Lavage :<br />
retirer la feuille adhésive, aspirer le contenu de toutes les cupules, et laver 4 fois avec env. 0,3 ml de<br />
solution de lavage.<br />
6. Distribution du conjugué :<br />
ajouter dans chaque cupule 100 μl de conjugué, couvrir d'une feuille adhésive et placer la plaque immédiatement<br />
dans l'incubateur.<br />
7. Incubation du conjugué :<br />
laisser incuber 30 ± 2 min à +37 ± 1 °C comme décrit en 4., et enchaîner immédiatement le processus de<br />
lavage.<br />
8. Lavage :<br />
retirer la feuille adhésive et laver comme décrit en 5.<br />
9. Distribution du substrat :<br />
ajouter dans chaque cupule 100 μl de la solution d’emploi du chromogène et couvrir la plaque d’une<br />
nouvelle feuille adhésive.<br />
10. Incubation du substrat :<br />
laisser incuber 30 ± 2 minutes à +18/+25 °C à l’abri de la lumière.<br />

11. Arrêt de la réaction :<br />
retirer la feuille adhésive et ajouter dans chaque cupule 100 μl de Solution d'arrêt POD en respectant le<br />
même rythme qu’en 9.<br />
12. Mesure :<br />
faire une mesure photométrique dans l'heure qui suit à 450 nm.<br />
La mesure de la densité optique des contrôles et des échantillons de patients se fait à 450 nm. Une longueur<br />
d’onde de référence de 650 nm est recommandée (éventuellement comprise entre 615 et 690 nm).<br />
Réalisation du test sur le BEP ® III<br />
Pour une utilisation sur le BEP ® III, les plaques-tests doivent être préparées jusqu’y compris la distribution des<br />
échantillons (points 1 et 2 du paragraphe «Réalisation du test sur le BEP ® II»). Immédiatement après cette étape,<br />
placer la plaque ouverte, c’est-à-dire non recouverte d’une feuille adhésive, dans le BEP ® III. Si la plaque n’est<br />
pas totalement utilisée, la compléter au moins pour moitié (6 barrettes) avec des barrettes remplies d’eau. La<br />
suite du test est ensuite effectuée entièrement automatiquement (cf. Manuel d’utilisation du BEP ® III).<br />
Les temps d’incubation prévus dans le logiciel du BEP ® III peuvent, du fait de conditions techniques (cadence<br />
de l’appareil), diverger par rapport à ceux du BEP ® II. Ils ont toutefois été validés dans la combinaison BEP ® III/<br />
Enzygnost * .<br />
Réalisation du test sur le BEP ® 2000<br />
La distribution des échantillons ainsi que la réalisation du test sont effectuées entièrement automatiquement<br />
par l’appareil (cf. Manuel d’utilisation du BEP ® 2000).<br />
Validation du test<br />
Les différentes densités optiques obtenues pour les sérums de contrôle doivent répondre aux critères suivants :<br />
-0,010 ≤ E ≤ 0,150<br />

Pour les quatre valeurs du contrôle négatif, si une seule ne répond pas au critère indiqué, ne pas en tenir<br />
compte pour le calcul de la moyenne.<br />
Les deux valeurs du contrôle positif doivent répondre au critère indiqué.<br />
Si ces conditions ne sont pas remplies, le test doit être recommencé.<br />
Exploitation du test<br />
L’exploitation des résultats est effectuée automatiquement par le BEP ® 2000 et le BEP ® III. Respecter les<br />
instructions indiquées pour l’appareil utilisé. Ci-dessous le protocole d’exploitation du test en l’absence d’un<br />
logiciel.<br />
Calculer la valeur moyenne des densités optiques obtenues pour le contrôle négatif, et y ajouter une valeur de<br />
0,400 pour obtenir la valeur-seuil :<br />

Les échantillons trouvés dans la zone grise ou supérieurs à cette zone grise, doivent être retestés en double. Si<br />
un échantillon trouvé initialement douteux donne des valeurs inférieures à la zone grise dans le retest, le<br />
considérer comme négatif selon les critères du test.<br />
Si les deux valeurs du retest sont de nouveau trouvées dans la zone grise ou supérieures à la zone grise,<br />
l'échantillon doit être considéré comme de nouveau réactif. Tous les échantillons retrouvés douteux ou réactif<br />
doivent être confirmés à l'aide d'un test dit de confirmation (par ex. une analyse Western-Blot).<br />
Limites du test<br />
1. Les anticoagulants, comme l’héparine, l’EDTA et le citrate, n’ont pas d’influence sur le résultat du test.<br />
2. Les échantillons lipémiques, ictériques ou hémolytiques n’ont pas d’influence sur le test.<br />
3. Des tests ont été effectués sur échantillons contenant des substances éventuellement interférentes, tels<br />
les facteurs rhumatoides, les anticorps anti-EBV/IgM, anti-VZV/IgM, anti-E. Coli, les ANA et AMA, ainsi<br />
que sur sérums de femmes enceintes. Aucune interférence sur les résultats du test n'a été observée.<br />
4. L’utilisation d’échantillons plusieurs fois congelés ou de sérums traités à la chaleur (30 mn à +56 °C) n’a<br />
permis l’observation d’aucune perturbation.<br />

5. Ne pas utiliser de sérums incomplètement coagulés ni contaminés par voie microbienne. En cas de<br />
présence d’éléments particulaires (par ex. caillot de fibrine ou érythrocytes), les éliminer avant le test.<br />
6. Si on utilise des échantillons décongelés, veiller à bien les homogénéiser avant emploi.<br />
7. Le test ne permet l’analyse que d’échantillons individuels. Ne pas utiliser d’échantillons poolés ni dilués.<br />
8. Les réactifs du coffret (à l’exception de la Solution de lavage POD, de la Solution d’arrêt POD et de la<br />
solution d’emploi du chromogène obtenue à partir des lots indiqués de chromogène TMB et de Tampon/<br />
substrat TMB) ne peuvent être utilisés que selon la combinaison de lots indiquée sur le coffret et le tableau<br />
des codes à barres joint au coffret.<br />
9. Le Tampon/substrat TMB, la solution d’emploi de Chromogène et la Solution d’arrêt POD ne doivent pas<br />
entrer en contact avec des ions de métaux lourds ni des substances oxydantes (ne pas utiliser de pipettes<br />
à parties métalliques au niveau du passage des solutions). La réaction du substrat ne doit pas être effectuée<br />
à proximité de désinfectants contenant de l’eau de Javel. Une coloration bleue de la solution d’emploi<br />
du chromogène avant son transfert dans la plaque indique une contamination ; préparer une nouvelle<br />
solution dans un récipient propre. Eviter tout contact de la peau avec les solutions sus-mentionnées.<br />
10. La plaque doit rester immobile pendant l’incubation (la placer par ex. sur un support fixe ou dans un bainmarie<br />
sans circulation d’eau) ; le fond des cupules doit être en contact avec l’eau thermostatée. Si l’eau<br />
contient des stabilisateurs pour éviter la contamination, bien veiller à ce que ni la surface supérieure de la<br />
plaque ni l’intérieur des cupules n’entrent en contact avec cette eau, au risque d’obtenir des réactions nonspécifiques.<br />
11. En cas d’échantillons fortement réactifs, il peut y avoir absence de coloration au moment de l’arrêt de la<br />
réaction, sans que cela ait d’influence sur l’exploitation photométrique.<br />
12. Les sérums de contrôle sont obtenus à partir de sérums humains natifs. Ils peuvent donc devenir trouble,<br />
sans que cela ait d’influence sur le résultat du test.<br />
13. Dade Behring a validé l’utilisation de ces réactifs sur plusieurs analyseurs afin d’optimiser les performances<br />
du produit et répondre à ses spécifications. Les modifications apportées par l’utilisateur ne sont<br />
pas sous la responsabilité de Dade Behring dans la mesure où elles peuvent affecter les performances du<br />
système et les résultats des dosages. Il est de la responsabilité de l’utilisateur de valider toutes modifications<br />
apportées à ces instructions ou à l’utilisation des réactifs sur les analyseurs autres que ceux mentionnés<br />
dans les protocoles d’application Dade Behring ou dans la présente notice d’utilisation.<br />
14. Les résultats de ce test doivent toujours être interprétés en rapport avec les antécédents médicaux du<br />
patient, les signes cliniques et autres constatations.<br />
Charactéristiques du test<br />
Sensibilité et spécificité<br />
Les résultats des études de sensibilité et de spécificité sont résumés dans les tableaux 2 + 3 (en annexe).<br />
Pour déterminer la sensibilité du test, un total de 566 échantillons anti-VIH1-positifs, 317 anti-VIH2-positifs et<br />
22 anti-VIH1 (sous-type O)-positifs a été testé. La totalité des 905 échantillons anti-VIH-positifs ont été trouvés<br />
positifs d'après les critères du test Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong>. La réactivité du test vis-à-vis des échantillons<br />
en phase de séroconversion a été testée sur 31 panels de séroconversions. L'étude a montré que le test<br />
Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> présentait la même sensibilité, c'est-à-dire la même détection des séroconversions,<br />
que les tests comparables. Malgré ces résultats, on ne peut exclure, dans le cadre d'une large utilisation<br />
du test, que certains échantillons ne soient pas révélés.<br />
Pour déterminer la spécificité du test, un total de 9243 échantillons sanguins anti-VIH-négatifs a été testé, et<br />
une spécificité de 99,3 à 100 % a été trouvée en test initial, et de 99,9 % en retest. Des valeurs divergentes<br />
peuvent être trouvées, dues par ex. au collectif étudié ou à la réalisation du test.<br />
Selon l'état des connaissances actuelles, un résultat trouvé positif dans un test VIH ne permet pas de conclure<br />
avec certitude que le sang contient de l'VIH1 ou de l'VIH2 infectieux, de même qu'un résultat négatif ne permet<br />
en aucun cas d'exclure avec certitude la présence d'VIH1 ou d'VIH2.<br />
Reproductibilité<br />
Les résultats de l’étude de reproductibilité et de répétabilité sont résumés dans le tableau 4 (en annexe). Ils<br />
ont été obtenus à partir de données prises à titre d’exemple. Les valeurs obtenues peuvent donc varier en<br />
fonction en particulier des conditions de réalisation du test.<br />

Settori d'impiego<br />
Metodo immunoenzimatico per I'identificazione degli anticorpi contro gli antigeni <strong>HIV</strong>1, <strong>HIV</strong>2 e <strong>HIV</strong>1 (sottotipo O)<br />
nel siero o nel plasma. Il test immunoenzimatico viene impiegato sugli analizzatori BEP ® II, BEP ® III e BEP ® 2000<br />
ELISA. Il test è stato realizzato per la determinazione di campioni singoli, e non per pool di campioni. Il prodotto<br />
deve essere utilizzato solo per scopi diagnostici in vitro.<br />
Significato diagnostico<br />
La sindrome da immunodeficienza acquisita (AIDS) è stata riconosciuta per la prima volta nel 1981 come<br />
forma clinica a sé stante. Come agenti patogeni della malattia sono stati fino ad oggi riconosciuti due diversi<br />
virus dell'immunodeficienza umana (<strong>HIV</strong>1 e <strong>HIV</strong>2). Il virus <strong>HIV</strong>1 (sinonimo LAV/HTLV III) è stato isolato nel<br />
1983. 1, 2 Nel 1986 è stato isolato un altro virus, l'<strong>HIV</strong>2, anch'esso risultato essere patogeno per l'uomo. 3 La<br />
determinazione sierologica degli anticorpi anti-<strong>HIV</strong> riveste particolare importanza nella medicina trasfusionale,<br />
per prevenire l'ulteriore diffusione della malattia.<br />
Di conseguenza a partire dal 1985 è iniziato il controllo degli anticorpi anti-<strong>HIV</strong>1 presso le banche del sangue<br />
e presso i produttori di emoderivati. Informazioni più recenti sulla diffusione delle infezioni da <strong>HIV</strong>2 hanno<br />
dimostrato la necessità di un controllo sistematico degli anticorpi anti-<strong>HIV</strong>1 e anti-<strong>HIV</strong>2 nei donatori di sangue,<br />
per escludere per quanto possibile, ogni potenziale trasmissione del virus mediante trasfusioni di sangue o da<br />
emoderivati.<br />
Nel 1990 è stato descritto un nuovo sottotipo di <strong>HIV</strong>. 4 Nel 1994 è stato possibile, da parte di due diversi gruppi di<br />
lavoro, determinare la sequenza nucleotidica di questo nuovo sottotipo. 5, 6 Parallelamente a queste ricerche<br />
virologiche è stato possibile dimostrare, da parte di diversi gruppi di lavoro, l’importanza sierologica del virus ora<br />
denominato <strong>HIV</strong>1- sottotipo O. 7 Inoltre alcuni test anti-<strong>HIV</strong> 1 + 2 dimostrano scarsa sensibilità per quanto riguarda<br />
la determinazione degli anticorpi anti-sottotipo O. 8 Di conseguenza è scaturita la necessità di un accertamento<br />
più sicuro di questa nuova variante dell’<strong>HIV</strong> accanto alle consuete indagini per i virus <strong>HIV</strong>1 e <strong>HIV</strong>2.<br />
Sebbene l’identificazione degli anticorpi specifici anti-<strong>HIV</strong>1 e anti-<strong>HIV</strong>2 non possa escludere con certezza la<br />
presenza dei virus <strong>HIV</strong>1 o <strong>HIV</strong>2 nel sangue al momento dell'esecuzione del test, così come non si può in<br />
nessun caso escludere con sicurezza la presenza di <strong>HIV</strong>1 o <strong>HIV</strong>2 in caso di risultato negativo, tuttavia il test<br />
anticorpale oggi rappresenta la migliore possibilità per riconoscere ed eliminare con alto grado di probabilità i<br />
campioni di sangue infetti da <strong>HIV</strong>.<br />
Principio del metodo<br />
Gli anticorpi specifici per l'<strong>HIV</strong> contenuti nel campione in esame, si legano con le proteine ricombinanti dell'<strong>HIV</strong>1,<br />
dell'<strong>HIV</strong>2 e/o dell’<strong>HIV</strong>1 (sottotipo O) fissate sulla superficie della piastra di microtitolazione.<br />
Dopo l'eliminazione delle componenti del campione non legate, il coniugato si lega agli anticorpi specifici dell'<strong>HIV</strong>.<br />
Dopo l'eliminazione del coniugato in eccesso viene valutata l'attività enzimatica del coniugato (reazione di<br />
colore blu). La trasformazione enzimatica del cromogeno viene interrotta mediante l'aggiunta di soluzione<br />
bloccante POD (reazione di colore giallo). L'intensità del colore sviluppato è proporzionale alla concentrazione<br />
degli anticorpi presenti nei campioni in esame.<br />

Avvertenze e precauzioni<br />
1. Solo per uso diagnostico in-vitro.<br />
2. Ogni donazione di sangue impiegata per la produzione dei sieri di controllo è stata esaminata per la ricerca<br />
dell’HBsAg, dell’anti HCV, dell'anti-<strong>HIV</strong>1 e dell'anti-<strong>HIV</strong>2. Per la produzione del siero di controllo negativo<br />
vengono impiegati solo sieri che risultano negativi. Il siero di controllo positivo viene inattivato per l'<strong>HIV</strong><br />
mediante un trattamento al calore (almeno 1 ora a +56 °C).<br />
Tuttavia tutti i campioni di siero umano (ad es. siero dei pazienti) ed i derivati (ad es. sieri di controllo)<br />
devono essere trattati con le necessarie precauzioni rispettando le norme di sicurezza sul rischio biologico<br />
in quanto non si può escludere con assoluta certezza il pericolo di agenti patogeni. 9<br />
3. Si consiglia l'uso di guanti di protezione durante l'intera esecuzione del test.<br />
4. Per la distruzione del materiale solido infettivo si consiglia il trattamento in autoclave per almeno 1 ora a +<br />
121 °C. Tutti i liquidi aspirati devono essere raccolti in due raccoglitori collegati in serie. I raccoglitori<br />
dovrebbero contenere un disinfettante adatto all'inattivazione dei virus patogeni umani. Osservare le concentrazioni<br />
ed i tempi di azione indicati dal produttore.<br />
5. E' necessario utilizzare solo reagenti appartenenti tutti ad uno stesso lotto (ad esclusione della soluzione<br />
di lavaggio POD, la Soluzione bloccante POD e la Soluzione d'uso del cromogeno preparata con i reagenti<br />
Cromogeno TMB e Tampone/Substrato TMB appartenenti tutti allo stesso lotto). La combinazione ammessa<br />
dei numeri di lotto a 6 cifre e quella stampata sulla confezione ed anche riportata nell'allegata tabella dei<br />
codici a barre.<br />
Preparazione dei Reagenti<br />
Portare tutti i reagenti ed i campioni in esame a + 18/+ 25 °C, prima dell’inizio del test, senza togliere le piastre<br />
dal contenitore.<br />
La confezione di Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> 10x96 contiene una etichetta con codice a barre per 1 flacone<br />
vuoto per il Coniugato <strong>HIV</strong>-Ag/POD. Analizzando una grande quantità di campioni, per evitare che la siringa<br />
venga cambiata spesso sul BEP ® III, si consiglia di versare il contenuto di diverse fiale del coniugato <strong>HIV</strong>-Ag/<br />
POD in un flacone più grande (es. un grande flacone vuoto non utilizzato per la Soluzione d’uso del Cromogeno<br />
oppure un flacone lavato della Soluzione Bloccante POD), etichettare questo flacone con la rispettiva<br />
etichetta «Flacone vuoto per coniugato» e posizionare il flacone etichettato nel rotore reagente.<br />

Per ogni piastra test, diluire 20 ml di Soluzione di Lavaggio POD con 400 ml di acqua distillata o deionizzata.<br />
Soluzione d’uso di Cromogeno: per ogni piastra test, diluire 1 ml di Cromogeno TMB con 10 ml di Tampone/Substrato<br />
TMB nel flacone vuoto appositamente fornito (soluzione d’uso di cromogeno) e conservare ben<br />
chiuso al riparo dalla luce. Dopo l‘uso, lavare accuratamente il flacone con acqua distillata.<br />
Per motivi tecnici, non è possibile versare il Cromogeno TMB direttamente nel flacone del Tampone/Substrato<br />
TMB.<br />
Conservazione e validità<br />
Prima dell'apertura, tutte le componenti della confezione Enzygnost * anti-<strong>HIV</strong> 1/2 <strong>Plus</strong> possono essere utilizzate<br />
fino alla data di scadenza indicata in etichetta, purché conservate alla temperatura indicata.<br />
La conservazione e la validità dei reagenti diluiti per l'uso, dopo l'apertura, sono riportate in appendice nella<br />
Tabella 1.<br />
Strumentazione necessaria:<br />
BEP ® II: Per la dispensazione automatica dei reagenti e della soluzione di lavaggio<br />
BEP ® III: Per l'esecuzione automatica e valutazione del test, dopo la distribuzione dei campioni<br />

Tutta la strumentazione usata nel test deve essere validata.<br />
Campioni in esame<br />
Si impiegano come campioni in esame quelli singoli (siero umano o plasma citratato/eparinizzato/EDTA) ottenuti<br />
da tecniche standard di laboratorio. I campioni devono essere conservati a + 2/+ 8 °C per non più di 3<br />
giorni. Se i campioni devono essere conservati per un lungo periodo di tempo, devono essere congelati.<br />
Esecuzione del test<br />
Esecuzione del test con il BEP ® II<br />
1. Schema di distribuzione:<br />
definire il numero dei pozzetti necessari (numero dei campioni in esame + 6 pozzetti per i controlli). Per<br />
l’esecuzione del test togliere le file di pozzetti non necessarie e conservarle per successive determinazioni<br />
(vedi. Tabella 1).<br />
2. Distribuzione del tampone per i campioni:<br />
distribuire 25 μL di tampone per i campioni in ogni pozzetto.<br />
3. Distribuzione dei campioni:<br />
distribuire 100 μL di siero di controllo negativo in ognuno dei primi 4 pozzetti e 100 μL di siero di controllo<br />
positivo nei 2 pozzetti successivi; distribuire in ognuno dei successivi pozzetti 100 μL di campione non<br />
diluito, coprire con un foglio adesivo e inserire la micropiastra nel sistema di incubazione il più presto<br />
possibile, al massimo entro 15 min dalla distribuzione dei campioni.<br />
In alternativa al suriportato schema di distribuzione, è possibile distribuire il controllo positivo all’inizio ed<br />
alla fine della serie.<br />
Schema di distribuzione: distribuire 100 μL/pozzetto di controllo negativo in 4 pozzetti, 100 μL di controllo<br />
positivo nel pozzetto successivo, quindi distribuire 100 μL/pozzetto del campione non diluito nei<br />
pozzetti successivi. Alla fine della serie o della piastra distribuire ancora una volta 100 μL di controllo<br />
positivo.<br />
4. Incubazione dei campioni:<br />
incubare per 30 ± 2 min a +37± 1 °C, iniziare quindi immediatamente la fase di lavaggio.<br />
5. Lavaggio:<br />
rimuovere il foglio adesivo, aspirare il contenuto di tutti i pozzetti e lavare utilizzando ogni volta ca. 0,3 mL<br />
di soluzione di lavaggio per 4 volte.<br />
6. Distribuzione del coniugato:<br />
distribuire in ogni pozzetto 100 μL di coniugato, ricoprire con un nuovo foglio adesivo e inserire subito la<br />
micropiastra nel sistema di incubazione.<br />
7. Incubazione del coniugato:<br />
ricoprire con un nuovo foglio adesivo e incubare per 30 ±2 min, a +37± 1 °C come descritto al punto 4. Al<br />
termine inserire immediatamente la piastra nel dispositivo di lavaggio.<br />
8. Lavaggio:<br />
rimuovere il foglio adesivo e lavare come descritto al punto 5.<br />
9. Distribuzione del substrato:<br />
distribuire in ogni pozzetto 100 μL di soluzione d'uso di cromogeno e ricoprire con un nuovo foglio adesivo.<br />
10. Incubazione del substrato:<br />
incubare per 30 ± 2 min. a + 18/+25 °C al riparo dalla luce.<br />
11. Arresto della reazione:<br />
rimuovere il foglio adesivo e aggiungere in ogni pozzetto 100 μL di soluzione bloccante POD, con lo stesso<br />
ordine seguito al punto 9.<br />

12. Lettura: leggere i risultati al fotometro a 450 nm entro 1 ora.<br />
Effettuare la misurazione dei valori di estinzione dei campioni di controllo e dei pazienti a 450 nm. La<br />
lunghezza d'onda della misura di riferimento dovrebbe essere 650 nm (evtl. fra 615 nm e 690 nm).<br />
Esecuzione del test usando il BEP ® III<br />
Prima di utilizzare il BEP ® III, preparare le piastre test ed eseguire le operazioni di dispensamento dei campioni<br />
(Paragrafo 1 e 2 in «Esecuzione del test con il BEP ® II»). Subito dopo porre le piastre non coperte da foglio<br />
adesivo nel BEP ® III. Attenzione: le piastre parzialmente riempite devono essere completate almeno a metà<br />
piastra (6 strip) aggiungendo «strip riempite con acqua». Il test viene quindi eseguito in modo completamente<br />
automatico (v. Manuale d’uso BEP ® III).<br />
Le impostazioni per i periodi di incubazione nel software del BEP ® III possono differire dai periodi del BEP ® II<br />
per motivi tecnici (velocità del sistema), ma sono state validate per i metodi Enzygnost * sul BEP ® III.<br />
Esecuzione del test usando BEP ® 2000<br />
Le fasi di dispensazione del campione e la successiva esecuzione del test sono eseguite in maniera completamente<br />
automatica dallo strumento (v. manuale d’uso del BEP ® 2000).<br />
Validità del test<br />
I singoli valori di estinzione per i sieri di controllo devono essere rispondenti ai seguenti protocolli:<br />

Se uno dei quattro valori di estinzione del siero di controllo negativo non è compreso nell'ambito specificato,<br />
può essere trascurato.<br />
I valori di estinzione dei controlli positivi devono entrambi essere compresi nell'ambito indicato. Se non sussistono<br />
queste condizioni, il test deve essere ripetuto.<br />
Valutazione del test<br />
Sul BEP ® 2000 e sul BEP ® III il calcolo dei risultati viene eseguito automaticamente. Per maggiori informazione<br />
consultare il manuale d'uso. I seguenti capitoli permettono la valutazione dei risultati senza l'ausilio del<br />
software.<br />
Dai valori di estinzione validi dei controlli negativi si calcola il valore medio.<br />
Per calcolare il valore soglia si somma al valore medio di estinzione dei controlli negativi il valore di 0,400:<br />

I campioni per i quali risultano valori di estinzione entro la zona grigia o superiori, devono essere risottoposti ad<br />
un nuovo test in doppio.<br />
Se, nella ripetizione del test, i valori di estinzione risultano inferiori alla zona grigia, tali campioni sono da<br />
considerarsi, secondo i criteri del test, negativi.<br />
Se il valore medio di estinzione risulta nuovamente entro l'ambito limite o più alto, il campione è da considerarsi<br />
reattivo. Tutti i campioni che reagiscono ripetutamente con valori entro l'ambito limite o in modo reattivo,<br />
dovrebbero in ogni caso essere sottoposti ai cosiddetti test di conferma (ad es. Western-Blot).<br />
Limitazioni della esecuzione del test<br />
1. <strong>Anti</strong>coagulanti come l’eparina, EDTA e citrato non interferiscono con il risultato del test.<br />
2. Campioni lipemici, itterici o emolitici non interferiscono con il test.<br />
3. Campioni contenenti sostanze potenzialmente interferenti come fattori reumatoidi, anticorpi EBV/IgM, anticorpi<br />
VZV/IgM, anticorpi E.-Coli, ANA e AMA, così come sieri di donne in gravidanza sono stati analizzati<br />
con il test. Sui campioni esaminati non si sono osservate interferenze sul risultato del test.<br />
4. Non sono state rilevate interferenze utilizzando campioni congelati e scongelati più volte, come pure i sieri<br />
inattivati al calore (30 min. a + 56 °C).<br />
5. Non devono essere utilizzati sieri coagulati inadeguatamente o campioni contaminati. Qualsiasi particella<br />
(es. coaguli di fibrina, eritrociti) devono essere eliminati prima del test.<br />
6. Per i campioni scongelati assicurare una buona omogeneizzazione del materiale.<br />

7. Il test è indicato solo per analizzare i campioni singoli e non per un pool di campioni o campioni diluiti.<br />
8. I reagenti non devono essere interscambiati con altri kit a meno che non siano dello stesso lotto (devono<br />
avere la combinazione ammessa dei numeri di lotto a 6 cifre stampati sulla confezione e riportata<br />
nell’allegata tabella dei codici a barre).<br />
Sono esclusi la Soluzione di lavaggio POD, la Soluzione bloccante POD e la Soluzione d’uso di cromogeno<br />
(preparata con il cromogeno TMB ed il Tampone/Substrato TMB appartenenti tutti allo stesso lotto).<br />
9. Il Tampone/Substrato TMB, la Soluzione d’uso di Cromogeno e la Soluzione Bloccante POD non devono<br />
venire in contatto con ioni di metalli pesanti o sostanze ossidanti (non usare pipette con parti metalliche a<br />
diretto contatto con il liquido). Non eseguire la reazione con il substrato in presenza di disinfettanti contenenti<br />
ipoclorito. Se la Soluzione d’uso di Cromogeno ha sviluppato un colore blu prima della dispensazione<br />
nella piastra test, questo indica che la soluzione è contaminata; in tal caso preparare una soluzione<br />
fresca in un contenitore pulito. Evitare il contatto delle suddette soluzioni con la cute.<br />
10. Durante il periodo di incubazione la piastra deve essere protetta da vibrazioni (ad es. utilizzare un supporto<br />
fisso ed evitare bagnomaria a circolazione d’acqua); i pozzetti devono venire a contatto con l’acqua<br />
termostatata. Se vengono utilizzati conservanti per evitare la contaminazione dell’acqua, occorre fare<br />
attenzione che la superficie della piastra test ed i pozzetti non vengano a contatto con queste soluzioni, in<br />
quanto simili contaminazioni possono produrre reazioni aspecifiche.<br />
11. Campioni altamente reattivi possono causare la formazione di un precipitato entro la soluzione colorata<br />
durante la reazione bloccante. Questo non interferisce con la valutazione fotometrica.<br />
12. Il siero di controllo è stato ottenuto da siero umano nativo. Possono manifestarsi intorbidamenti, che però<br />
non interferiscono sui risultati del test.<br />
13. Dade Behring ha validato l’uso di questi reagenti su vari analizzatori per ottimizzare le prestazioni e rispettare<br />
le specifiche dei prodotti. Le modifiche definite dall’utente non sono supportate da Dade Behring<br />
poiché possono influire sulle prestazioni del sistema e sui risultati del test. Pertanto è responsabilità<br />
dell’utente validare tutte le modifiche apportate a queste istruzioni, l’uso dei reagenti su analizzatori diversi<br />
da quelli inclusi nei fogli di istruzioni o nelle istruzioni d’uso fornite da Dade Behring.<br />
14. I risultati di questo test devono essere sempre interpretati alla luce della anamnesi del paziente, della<br />
presentazione clinica e valutando contestualmente l’esito di altri accertamenti.<br />
Caratteristiche del test<br />
Sensibilità e specificità<br />
I risultati per la valutazione della sensibilità e specificità sono riportati nelle tabelle 2 e 3 (in appendice).<br />
Per la valutazione della sensibilità sono stati esaminati in totale 566 campioni anti-<strong>HIV</strong>1 positivi, 317 anti-<strong>HIV</strong>2<br />
positivi e 22 anti-<strong>HIV</strong>1 (sottotipo O) positivi. Tutti i 905 campioni anti <strong>HIV</strong> positivi esaminati sono risultati<br />
positivi al test secondo i criteri dell'Enzygnost * anti-<strong>HIV</strong>1/2 <strong>Plus</strong>.<br />
La reattività al test per campioni in sieroconversione è stata verificata esaminando 31 pannelli di sieri di<br />
pazienti durante la fase di sieroconversione. L’Enzygnost * anti-<strong>HIV</strong> 1/ 2 <strong>Plus</strong> ha dimostrato una sensibilità<br />
corrispondente alla sensibilità di test analoghi in merito alla precocità di rilevamento della sieroconversione.<br />
Non si può tuttavia escludere la possibilità che, con l'impiego del test su vasta scala, qualche particolare<br />
campione possa risultare non identificato.<br />
Per la valutazione della specificità sono stati esaminati in totale 9243 campioni di sangue anti-<strong>HIV</strong> negativi ed<br />
è stata rilevata una specificità da 99,3 a 100% (test iniziale) e di 99,9% (dopo ripetizione del test). Possibili<br />
scostamenti da tali valori possono dipendere dal tipo di collettività esaminata, dalle modalità di esecuzione del<br />
test o da altre variabili.<br />
Secondo lo stato attuale delle conoscenze un risultato positivo al test anti-<strong>HIV</strong> non esclude con assoluta<br />
sicurezza la presenza del virus <strong>HIV</strong>1 o <strong>HIV</strong>2 nel sangue. Anche in caso di risultato negativo non si può escludere<br />
con assoluta certezza la presenza del virus <strong>HIV</strong>1 o <strong>HIV</strong>2.<br />
Riproducibilità<br />
I risultati sulla riproducibilità intra e inter-serie sono riassunti nella Tabella 4 (in Appendice). I dati indicati sono<br />
da considerarsi come risultati tipici. In base a vari fattori (procedure, ecc.) si possono ottenere valori differenti.<br />

Campos de aplicación<br />
Prueba inmunoenzimática para la detección de anticuerpos contra los antígenos <strong>HIV</strong>1, <strong>HIV</strong>2 y <strong>HIV</strong>1 (subtipo O) en<br />
sueros o plasmas. La realización del ensayo inmunológico se lleva a cabo con los ELISA Procesadores BEP ® II, BEP ®<br />
III y BEP ® 2000. El test fue desarollado para la investigación de muestras aisladas, no para muestras en pool. El<br />
producto debe ser usado únicamente en diagnósticos in vitro.<br />
Significado diagnóstico<br />
El síndrome de inmunodeficiencia adquirida (SIDA) fue reconocido por primera vez en 1981 como una enfermedad<br />
con un cuadro clínico propio. Como causantes de esta enfermedad se conocen hoy dos virus de la<br />
inmunodeficiencia humana diferentes (<strong>HIV</strong>1 y <strong>HIV</strong>2). En 1983 fue posible aislar el <strong>HIV</strong>1 (sinónimo: LAV/HTLV<br />
III) 1,2 . En el año 1986 se logró aislar un nuevo virus (<strong>HIV</strong>2), que también es válido como patógeno humano 3 . La<br />
determinación serológica de los anticuerpos contra <strong>HIV</strong> tiene un significado muy importante en especial, bajo<br />
el aspecto preventivo, para la medicina de transfusiones con el fin de evitar una mayor propagación de la<br />
enfermedad.<br />
En 1985 se empezó a investigar la existencia de anticuerpos contra <strong>Anti</strong>-HVI 1 en sangre donada, tanto en los bancos<br />
de sangre como en la produccion de productos del plasma. Informaciones aparecidas en este período sobre la<br />
propagación de infecciones por <strong>HIV</strong>2, crearon la necesidad de una investigación de rutina para las donaciones de<br />
sangre no sólo para anticuerpos contra <strong>Anti</strong>-<strong>HIV</strong>1 si no también contra <strong>Anti</strong>-<strong>HIV</strong>2, y así de esta forma poder evitar un<br />
potencial contagio por medio de transfuciones o por preparados producidos a partir de sangre.<br />
En 1990 se describió un nuevo subtipo de <strong>HIV</strong> 4 , pero sólo en 1994 pudieron dos grupos de trabajo, al mismo<br />
tiempo e independientemente el uno del otro, determinar la secuencia de este nuevo subtipo 5, 6 . Paralelamente<br />
con los trabajos virológicos pudieron otros grupos de trabajo demostrar el significado serológico del virus<br />
ahora denominado <strong>HIV</strong>1- subtipo O 7 . Después de esto se encontró que con algunos de los ensayos para <strong>Anti</strong>-<br />
<strong>HIV</strong>1+2 se tenía muchas dificultades para detectar el anticuerpo contra <strong>Anti</strong>-subtipo O 8 . Como consecuencia<br />
de esto se presentaba la necesidad de encontrar un método seguro para el reconocimiento de esta nueva<br />
variante de <strong>HIV</strong> y de los ya establecidos <strong>HIV</strong>1 y <strong>HIV</strong>2.<br />
Aunque el reconocimiento de anticuerpos específicos de <strong>HIV</strong>1 y <strong>HIV</strong>2 no permite una declaración segura<br />
sobre si existen, en el momento del test, <strong>HIV</strong>1 y <strong>HIV</strong>2 infecciosos en la sangre, ni tampoco un valor negativo<br />
excluye la presencia de <strong>HIV</strong>1 o <strong>HIV</strong>2, ofrece el test de anticuerpos la mayor posibilidad para reconocer y<br />
eliminar sangres donadas infectadas con <strong>HIV</strong>.<br />
Principio del método<br />
La inmunoglobulina específica para <strong>HIV</strong> existente en la muestra a investigar se une a la proteína recombinante<br />
<strong>HIV</strong>1, <strong>HIV</strong>2 y/o <strong>HIV</strong>1 (subtipo O) fijadas a la superficie de la placa de microtitulación.<br />
Después de retirar las partes no enlazadas de la muestra, se une el conjugado al anticuerpo específico para <strong>HIV</strong>.<br />
Después de retirar el exceso de conjugado se determina la actividad enzimática del conjugado (reacción cromática<br />
azul). La transformación enzimática del cromógeno se interrumpe por adición de la solución de parada POD (reacción<br />
cromática amarilla). La intensidad del color es proporcional a la concentración de anticuerpo en la muestra.<br />

Advertencias y medidas de precaución<br />

a pruebas para detectar el HBsAg, los anti-HCV, los anti-<strong>HIV</strong>1 y los anti-<strong>HIV</strong>2.<br />
Para la elaboración sólo se utilizan donaciones con resultados negativos. El suero de control positivo se<br />
somete a un tratamiento por calor (por lo menos 1 hora a + 56 °C)<br />

pacientes) y productos (p. ej. sueros de control) deben ser manipuladas con las precauciones necesarias,<br />
siguiendo las medidas de seguridad recomendadas en caso de riesgo biológico, puesto que nunca se<br />
puede excluir completamente el peligro de una contaminación bacteriana9 .<br />
3. Se aconseja utilizar guantes de seguridad durante el desarrollo del test.<br />
4. Para la eliminación del material infeccioso sólido se recomienda colocarlo en el autoclave por lo menos 1<br />
hora a + 121°C. Las soluciones aspiradas se deben recoger en dos recipientes conectados uno con el<br />
otro. Los recipientes deben contener un medio de desinfección apropiado para inactivar virus patógenos<br />
humanos. Deben observarse la concentración y el tiempo de acción dados por el fabricante.<br />
5. Los reactivos (con exepción de la solución de lavado POD, la solución de parada y la solución de uso del<br />
cromógeno, la cual ha sido preparada a partir de los reactivos dependientes del lote, cromógeno TMB y<br />
Tampón/sustrato TMB ) sólo se pueden utilizar juntos si provienen del mismo lote, esto es, con el mismo<br />
número de 6 cifras que está impreso en el envase o que se puede tomar de la tabla de valores de códigos<br />
de barras incluida en él.<br />
Preparación de reactivos<br />
Antes de iniciar el test calentar todos los reactivos y las muestras a investigar entre +18 y +25 °C. No sacar del<br />
recipiente la placa de prueba.<br />
En el envase Enzygnost * <strong>Anti</strong>-<strong>HIV</strong>1/2 <strong>Plus</strong> 10 x 96 viene una etiqueta para un frasco vacío para el conjugado<br />
<strong>HIV</strong>-Ag/POD. Para evitar el cambio frecuente de jeringa en el BEP ® III cuando se trabaja una serie grande de<br />
muestras, puede ser de gran utilidad mezclar varias soluciones de conjugado <strong>HIV</strong>-Ag/POD en un frasco<br />
grande (por ej. un frasco vacío sobrante de solución de uso de cromógeno o un frasco lavado de solución de<br />
parada), marcarlo con la etiqueta frasco vacío para conjugado y colocarlo en la rueda de reactivos.<br />

Para cada placa de prueba diluir 20 ml de la solución de lavado POD con agua destilada o desionizada<br />
hasta obtener 400 ml.<br />
Solución de uso del cromógeno: Por cada placa diluir 1 ml de cromógeno TMB con 10 ml de tampón/sustrato<br />
TMB dentro de la botella de plástico que viene en el envase para este fin (solución lista de cromógeno), mantenerla<br />
cerrada y protegida de la luz. Lavar el frasco esmeradamente con agua destilada después de su uso.<br />
Por razones técnicas (sobrellenado) no está permitido verter el contenido del cromógeno TMB en el frasco de<br />
tampón/sustrato TMB.<br />
Estabilidad y almacenaje<br />
Todos los componentes del envase combinado Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> aún cerrados, conservados a la<br />
temperatura indicada, son utilizables hasta las fechas de vencimiento dadas en las etiquetas.<br />
La estabilidad y las condiciones de almacenamiento de los envases ya abiertos o de los reactivos ya diluidos<br />
listos para el uso se pueden tomar de la Tabla 1 del apéndice.<br />
Equipo necesario:<br />
BEP ® II: Para el desarollo automático de la dosificación de reactivos y de las etapas de lavado<br />
BEP ® III: Para la realización completamente automática del test después de la distribución de<br />
las muestras, así como para la evaluación<br />
BEP ® 2000: Para la realización completamente automática del test<br />
Pipetas: Pipetas a émbolo 25, 100 y 1000 μl<br />
Incubadora: Baño de agua cubierto (+37 ± 1 °C) u otros métodos de incubación similares<br />
Todos los instrumentos utilizados para el desarrollo del test deben estar validados.<br />
Material a investigar<br />
Para la investigación se pueden utilizar muestras aisladas (sueros humanos o plasma con EDTA/heparina/citrato),<br />
las cuales hayan sido tomadas según las técnicas estándar de laboratorio. Las muestras se pueden almacenar<br />
máximo 3 días entre 2 y 8 °C. Para tiempos más largos de almacenamiento las muestras se deben congelar.<br />
Procedimiento<br />
Procedimiento con el BEP ® II<br />
1. Esquema de distribución:<br />
Determinar el número necesario de pocillos de la placa (número de muestras a investigar más 6 pocillos<br />
para los controles). Los elementos de la placa de prueba no utilizados se deben retirar del marco que los<br />
contiene y se deben guardar para ser utilizados posteriormente (ver Tabla 1).<br />
2. Distribución del tampón para muestras:<br />
En cada uno de los pocillos colocar 25 μl de tampón para muestras.<br />
3. Dosificación de las muestras:<br />
Llenar 4 pocillos c/u con 100 μl de control negativo y 2 pocillos c/u con 100 μl de control positivo; en los<br />
siguientes pocillos dosificar 100 μl de las muestras sin diluir por cada uno, cubrir con la lámina adhesiva,<br />
y lo más pronto posible, máximo 15 min después de terminar la dosificación de las muestras, colocar la<br />
placa en el incubador.<br />
Como una alternativa para el esquema de pipeteo descrito anteriormente se permite también colocar un<br />
control positivo al comienzo y otro al final de cada serie del test.<br />
Esquema de pipeteo: Dosificar en cuatro pocillos 100 μl del control negativo por c/u, en un pocillo 100 μl del<br />
control positivo y en cada uno de los siguientes pocillos 100 μl de la muestra sin diluir. Al final de la serie o de<br />
la placa de ensayo dosificar una vez más 100 μl del control positivo.<br />
4. Incubación de las muestras:<br />
Incubar durante 30 ± 2 min a + 37 ± 1 °C, lavar a continuación.<br />
5. Lavado:<br />
Retirar la lámina adhesiva, aspirar el contenido de todos los pocillos y lavar 4 veces con aprox. 0,3 ml de la<br />
solución de lavado cada vez.<br />
6. Distribución del conjugado:<br />
Colocar en cada pocillo 100 μl de conjugado, cubrir la placa con nueva lámina adhesiva e inmediatamente<br />
colocar en el incubador.<br />
7. Incubación del conjugado:<br />
Incubar durante 30 ± 2 min a +37 ± 1°C, como se describe en el punto 4 y lavar inmediatamente después.<br />
8. Lavado:<br />
Retirar la lámina adhesiva y lavar como está descrito en el punto 5.<br />
9. Distribución del sustrato:<br />
Agregar en cada pocillo 100 μl de solución de cromógeno lista para el uso y cubrir la placa con un nueva<br />
lámina adhesiva.<br />
10. Incubación del sustrato:<br />
30 ± 2 min entre +18 y +25 °C protegiendo de la luz.<br />

11. Reacción de parada:<br />
Retirar la lámina adhesiva y agregar en cada pocillo 100 μl de solución de parada POD; mantener el<br />
mismo ritmo que en el punto 9.<br />
12. Medición:<br />
Realizar la medición en el fotómetro a 450 nm en el termino de una hora.<br />
La medida de la extinción de los controles y de las muestras de pacientes se hace a 450 nm, como<br />
longitud de onda de la medida de referencia se aconseja 650 nm (eventualmente entre 615 y 690 nm).<br />
Procedimiento en el BEP ® III<br />
Para el desarrollo del test en el BEP ® III, las placas de prueba se deben preparar hasta la distribución de las<br />
muestras (punto 1 hasta punto 3 del "Procedimiento en el BEP ® II"). Directamente después de esto, colocar<br />
las placas de prueba abiertas, esto es, sin lámina adhesiva en el BEP ® III. Aquí es importante observar que las<br />
placas de prueba que no estén llenas se deben completar con "elementos con agua" hasta por lo menos la<br />
mitad de la placa de prueba (6 elementos). Todas las etapas subsiguientes van a ser realizadas de forma<br />
completamente automática por el aparato (ver manual de funcionamiento del BEP ® III).<br />
Los tiempos de incubación ajustados en el software del BEP ® III pueden desviarse de los del procedimiento<br />
en el BEP ® II debido a condiciones técnicas básicas (ritmo del aparato), están sin embargo validados en la<br />
combinación BEP ® III/Enzygnost * .<br />
Procedimiento con el BEP ® 2000<br />
La dosificación de las muestras y la subsiguiente realización del test se efectúan de forma completamente<br />
automática en el aparato (ver manual de funcionamiento del BEP ® 2000).<br />
Validez del test<br />
Cada valor de la extinción para los sueros de control debe cumplir las siguientes especificaciones:<br />
- 0,010 < E < 0,150<br />

De los cuatro valores de extinción del control negativo, puede uno estar fuera de las especificaciones y éste<br />
se puede descartar.<br />
Los dos valores de extinción del control positivo deben cumplir las especificaciones anteriores.<br />
Si estas condiciones no se cumplen se debe repetir el test.<br />
Valoración del test<br />
Las valoraciones se efectúan automáticamente con el BEP ® 2000 y el BEP ® III. Para esto consultar, por favor,<br />
los manuales de funcionamiento. Los siguientes capítulos son para tener en cuenta en caso de evaluaciones<br />
sin la ayuda del software.<br />
De los valores válidos de la extinción del control negativo se halla el valor promedio.<br />
Para calcular el valor límite se añade un valor de 0,400 al valor promedio de extinción del control negativo:<br />

Muestras que tengan valores de extinción en la zona gris o extinciones mayores se deben repetir en una doble<br />
determinación. Si después de la repetición del test se encuentran valores por debajo de éste, de acuerdo con<br />
los criterios del test, se deben tomar estas muestras como anticuerpo-negativo.<br />
Para nuevos valores promedios de extinción limítrofes o mayores se considera la prueba como reactiva.<br />
Todos los valores limítrofes repetitivos o todas las muestras 2 x reactivas deben ser necesariamente confirmadas<br />
con ayuda de los llamados test de confirmación (por ej. Análisis Western -Blot).<br />
Limitaciones del Procedimiento<br />
1. Los anticoagulantes como heparina, EDTA y citrato no influyen sobre los resultados del test.<br />
2. Las muestras lipémicas, ictéricas o hemolíticas no alteran el desarrollo del test.<br />
3. Con el test se examinaron sustancias potencialmente alterantes como factores reumatoides, anticuerpos<br />
EBV/IgM, anticuerpos VZV/IgM, anticuerpos E-coli, ANA y AMA, así como, sueros de embarazadas. En<br />
ninguna de las muestras se encontró influencia sobre los resultados del test.<br />
4. En muestras congeladas y descongelas varias veces, así como en sueros tratados por calor (30 min., +56<br />
°C) no se observó ninguna alteración.<br />

5. No deben ser utilizados sueros no coagulados totalmente y muestras contaminadas. Si eventualmente<br />
existentes partículas (por ej., coagulo de fibrina, eritocitos) estas deben ser retiradas antes de empezar<br />
con el desarrollo del test.<br />
6. En las muestras descongeladas se debe observar que haya una buena homogenización del material.<br />
7. El test es apropiado únicamente para el estudio de muestras individuales y no de muestras en pool o<br />
diluidas.<br />
8. Los reactivos (con excepción de la solución de lavado POD, la solución de parada POD y la solución de<br />
uso del cromógeno elaborada a partir de los reactivos dependientes del lote, cromógeno TMB y Tampón/<br />
sustrato TMB) sólo se pueden utilizar juntos si provienen del mismo lote, esto es, con el mismo número de<br />
6 cifras que está impreso en el envase o que se puede tomar de la Tabla de código de barras incluida.<br />
9. El tampón substrato TMB, la solución de uso del cromógeno y la solución de parada POD no deben entrar<br />
en contacto con iones de metales pesados ni con sustancias oxidantes (no utilizar pipetas con partes<br />
metálicas que entren en contacto con el líquido). No efectuar la reacción del sustrato en la proximidad de<br />
sustancias desinfectantes que contengan hipoclorito. Una coloración espontánea azul de la solución de<br />
uso del cromógeno antes de colocarla en la placa de prueba indica una contaminación; en este caso, se<br />
deberá preparar una solución fresca en un recipiente limpio. Evite el contacto cutáneo con las soluciones<br />
arriba mencionadas.<br />
10. Durante la incubación debe mantenerse la placa de ensayo quieta (p. ej. sobre un flotador fijo o en un<br />
baño de agua no circulante); los pocillos permanecen de esta manera en contacto con el agua. En el caso<br />
de usar medios de conservación en el agua, para evitar su contaminación, debe observarse cuidadosamente<br />
que ni la superficie de la placa de prueba ni los pocillos entren en contacto con estas soluciones, ya<br />
que una contaminación de este tipo puede ocasionar reacciones inespecíficas.<br />
11. Para muestras con reactividad alta puede, durante la reacción de parada, precipitarse el colorante. Esto<br />
no afecta el resultado de la valoración fotométrica.<br />
12. Los sueros de control son fabricados a partir de sueros humanos nativos. Por esta razón puede aparecer<br />
turbidez, la cual no influye sobre los resultados del test.<br />
13. Dade Behring ha validado el uso de los reactivos en varios analizadores para optimizar el rendimiento del<br />
producto y cumplir con las especificaciones del mismo. Las modificaciones definidas por el usuario no<br />
están garantizadas por Dade Behring dado que pueden afectar al rendimiento del sistema y a los resultados<br />
del ensayo. Es responsabilidad del usuario validar las modificaciones realizadas a estas instrucciones<br />
o el uso de los reactivos en analizadores distintos a los incluidos en las hojas de aplicaciones de Dade<br />
Behring o en estas instrucciones de uso.<br />
14. Los resultados de esta prueba deberán interpretarse siempre de acuerdo con la historia clínica del paciente,<br />
la sintomatologia clínica y otras observaciones.<br />
Caracteristicas del test<br />
Sensibilidad y especificidad<br />
Los resultados de la prueba de sensibilidad y especificidad se encuentran resumidos en las Tablas 2 + 3 (en el<br />
apéndice).<br />
Para averiguar la sensibilidad se investigaron en total 566 muestras positivas para <strong>Anti</strong>-<strong>HIV</strong>1, 317 para <strong>Anti</strong><br />
<strong>HIV</strong>-2 y 22 para <strong>Anti</strong>-<strong>HIV</strong>1 (subtipo O). Todas la 905 muestras <strong>Anti</strong>-<strong>HIV</strong> positivas se encontraron positivas,<br />
según los criterios del Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong>. La reactividad del ensayo con muestras con seroconversión<br />
se investigó usando 31 panels con seroconversión. En este caso, se pudo observar que el Enzygnost *<br />
<strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> presenta en relación al reconocimiento de seroconversiones una sensibilidad que corresponde<br />
a la sensibilidad de cualquier otro test similar a éste. Independientemente de esto, no se puede excluir<br />
que para un empleo más amplio del test algunas muestras aisladas no se puedan reconocer.<br />
Para averiguar la especificidad se investigaron en total 9243 muestras de sangre <strong>Anti</strong>-<strong>HIV</strong> negativas y se encontraron<br />
valores para ésta de 99,3 hasta 100% (ensayo inicial) o 99,9 % (repetición del ensayo)). Valores<br />
diferentes a estos son también posibles dependiendo entre otros, de la colectividad a investigar y del desarrollo<br />
del test.<br />
De acuerdo a los conocimientos actuales no se puede deducir de un resultado positivo del <strong>Anti</strong>-<strong>HIV</strong>-test, con<br />
seguridad, que existan en la sangre <strong>HIV</strong>1 o <strong>HIV</strong>2 infecciosos, como tampoco un resultado negativo de ninguna<br />
manera deja excluir con seguridad la presencia de <strong>HIV</strong>1 o de <strong>HIV</strong>2.<br />
Reproducibilidad<br />
Los resultados de los ensayos de reproducibilidad intra/inter-ensayo están resumidos en la Tabla 4 (en el<br />
apéndice). En este caso se trata de un datos obtenidos de un ejemplo. Debido a la diferencias, entre otras, en<br />
el desarrollo del test es posible que aparezcan valores discrepantes.<br />

Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong><br />
Campo de aplicação<br />
Teste imunoenzimático para detecção de anticorpos contra os antígenos <strong>HIV</strong>1, <strong>HIV</strong>2 e <strong>HIV</strong>1 (subtipo O) em<br />
soros ou plasmas. O processamento do teste imunoenzimático efectua-se com os processadores ELISA BEP ® II,<br />
BEP ® III e BEP ® 2000. O teste foi desenvolvido para o exame de amostras singulares, não de amostras em<br />
pool. O produto só pode ser utilizado para fins de diagnóstico in vitro.<br />
Significado diagnóstico<br />
A síndrome de imunodeficiência adquirida (SIDA) foi identificada pela primeira vez em 1981 como enfermidade<br />
com um quadro clínico próprio. Como seus agentes patogénicos são considerados hoje em dia dois vírus<br />
diferentes da imunodeficiência humana (<strong>HIV</strong>1 e <strong>HIV</strong>2). já em 1983 foi possível isolar o <strong>HIV</strong>1 (sinónimo: LAV/<br />
HTLV III) 1, 2 . Em 1986 foi isolado um novo vírus (<strong>HIV</strong>2), considerado tambémele patogénico em relação ao ser<br />
humano 3 . A determinação serológica dos anticorpos contra <strong>HIV</strong> é particularmente relevante, sobretudo sob o<br />
aspecto preventivo, para a medicina de transfusões com vista a evitar a propagação da enfermidade.<br />
Consequentamente deu-se iníco em 1985 à detecção sistemática de anticorpos anti-<strong>HIV</strong>1 dádivas de<br />
sangue, tanto em bancos de sangue como no fabrico de produtos do plasma. Informações divulgadas nesse<br />
período sobre a propagação de infecções por <strong>HIV</strong>2 tornaram necessário implementar também uma rotina de<br />
determinação de anticorpos anti <strong>HIV</strong>2, a fim de excluir na medida do possível qualger perigo de transmissão<br />
atraves de transfusões ou de preparados fabricados a partir do sangue.<br />
Em 1990 foi descrito um novo subtipo de <strong>HIV</strong> 4 , cuja sequência pôde ser definada, em 1994, simultaneamente,<br />
mas independentemente, por dois grupos de investigação 5, 6 . Paralelamente aos trabalhos virológicos<br />
lograram outros grupos demonstrar o significado serológico do vírus agora denominado <strong>HIV</strong>1-subtipo 0 7 .<br />
Estas investigações revelaram certas deficiências por parte de alguns testes anti-<strong>HIV</strong>1 e anti-<strong>HIV</strong>2 na detecção<br />
de anticorpos do subtipo 0 8 . Urgia pois desenvolver um método capaz de reconhecer com segurança<br />
não só os tipos já isolados <strong>HIV</strong>1 e <strong>HIV</strong>2, mas também esta nova variante.<br />
Ainda que a identificação de anticorpos específicos <strong>Anti</strong>-<strong>HIV</strong>1 e <strong>Anti</strong>-<strong>HIV</strong>2 não permita concluir com segurança<br />
que, no momento de realização do teste, existiam efectivamente vírus infecciosos <strong>HIV</strong>1 ou <strong>HIV</strong>2 no sangue e<br />
um resultado negativo tão-pouco permita excluir com segurança a sua presença, o teste de anticorpos constitui,<br />
ainda assim, o melhor meio ao nosso alcance de detectar e eliminar, com alto grau de probabilidade,<br />
dádivas de sangue infectadas pelo <strong>HIV</strong>.<br />
Princípio metodológico<br />
As imunoglobulinas <strong>HIV</strong>-específicas contidas na amostra ligam-se às proteínas recombinantes dos <strong>HIV</strong> 1,<br />
<strong>HIV</strong> 2 e/ou <strong>HIV</strong> 1 (subtipo O) fixadas à superfície da placa de microtitulação.<br />
Após eliminação dos componentes não ligados, o complexo conjugado é ligado aos anticorpos<br />
<strong>HIV</strong>-específicos.<br />
Após eliminação do conjugado em excesso, é determinada a actividade enzimática (reacção cromática azul).<br />
A transformação enzimática do cromógeno é interrompida por adição da solução para bloqueio POD (reacção<br />
cromática amarela). A intensidade da cor é proporcional à concentração de anticorpos na amostra.<br />

Advertências e medidas de precaução<br />
1. Só para uso diagnóstico in-vitro.<br />
2. Cada dádiva individual de sangue destinada à preparação de soros de controlo foi previamente examinada<br />
com vista à detecção de HBsAg, <strong>Anti</strong>-HCV, <strong>Anti</strong>-<strong>HIV</strong>1 e <strong>Anti</strong>-Hiv2. Na preparação de soro de controlo<br />
negativo, só se utilizam soros com resultado negativo. O soro de controlo positivo foi submetido a um<br />
tratamento por calor (pelo menos 1 hora a + 56 °C)<br />
Independentemente disto, todas as amostras obtidas a partir de sangue humano (p. ex. soros de pacientes)<br />
e produtos (p. ex. soros de controlo) devem ser manipuladas com as precauções necessárias, seguindo<br />
as medidas de segurança recomendadas em caso de risco biológico, uma vez que nunca se pode<br />
excluir inteiramente o risco de contaminação por agentes patogénicos9 .<br />
3. Recomenda-se utilizar luvas para análise durante toda a realização do teste.<br />
4. Para desinfecção de materiais sólidos, convém colocá-los na autoclave durante pelo menos 1 hora a uma<br />
temperatura de, pelo menos, + 121 °C. Todas as soluções aspiradas devem ser recolhidas em dois receptáculos<br />
ligados em série, que devem conter um desinfectante apropriado para inactivação de vírus patogénicos<br />
humanos. Devem observar-se a concentração e o tempo de acção indicados pelo fabricante.<br />
5. Os reagentes (com excepção da solução de lavagem POD, da solução de interrupção POD e da solução<br />
de uso do cromogénio, preparada esta última a partir dos reagentes específicos de cada lote Cromogénio<br />
TMB e Tampão/substrato TMB) só podem ser utilizados como constituintes de um mesmo lote, isto é,<br />
somente na combinação de 6 cifras que vem impressa na embalagem e indicada na tabela de valores do<br />
código de barras adjunta.<br />
Preparação dos reagentes<br />
Antes de iniciar o teste, levar todos os reagentes e amostras à temperatura de +18 a +25 °C, tirar a placa de<br />
ensaio do seu recipiente.<br />
A embalagem de Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> 10 x 96 contém em anexo um rótulo com códico de barras para um<br />
frasco vazio destinado ao conjugado <strong>HIV</strong>-Ag/POD. Para evitar ter de mudar muitas vezes a seringa no BEP ® III<br />
quando se procede a uma longa série de análises, pode ser útil verter várias soluções de conjugado <strong>HIV</strong>-Ag/POD<br />
para um frasco maior (p. ex. um frasco vazio, supranumerário e ainda não utilizado, destinado à solução cromógena<br />
usual ou um frasco lavado da solução para bloqueio POD), etiquetá-lo com o rótulo «Frasco vazio para conjugado»<br />
e colocá-lo no suporte de reagentes.<br />

Para cada placa de ensaio, diluir 20 ml da solução de lavagem POD com água destilada ou desionizada até<br />
obter 400 ml.<br />
Solução cromógena usual: Diluir, por placa, 1 ml de cromógeno TMB com 10 ml de tampão/substrato<br />
TMB no frasco de plástico anexo à embalagem (solução cromógena usual) e conservar fechado e protegido<br />
da luz. Depois do uso, lavar bem o frasco com água destilada.<br />
Por razões técnicas (tamanho dos frascos) não é permitido verter conjuntamente os conteúdos dos frascos<br />
de cromógeno TMB e tampão/substrato TMB.<br />
Estabilidade e condições de conservação<br />
Antes de abertos, e enquanto conservados à temperatura de armazenagem indicada, todos os componentes<br />
da embalagem combinada Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong> mantêm-se utilizáveis até ao termo do prazo de<br />
validade indicado nos rótulos.<br />
A estabilidade e condições de conservação dos reagentes abertos ou diluídos para uso podem depreender-se<br />
da tabela 1 em anexo.<br />
Aparelhos necessários:<br />
BEP ® II: para a execução automática da dosagem dos reagentes e das fases de lavagem<br />
BEP ® III: processamento automático do ensaio e interpretação após dosagem das amostras<br />
BEP ® 2000: para o processamento automático e avaliação do teste<br />
Pipetas: pipetas com êmbolo 25, 100 e 1000 μl<br />
Incubador: banho-maria tapado (+37 ± 1 °C) ou outros métodos de incubação comparáveis<br />
Todos os aparelhos utilizados no ensaio têm de estar validados.<br />
Amostras<br />
Para a análise podem ser utilizadas amostras individuais (soros humanos e plasmas EDTA/Heparina/Citrato),<br />
colhidos de acordo com as técnicas do laboratório. As amostras poderão ser conservadas durante de 3 dias,<br />
no máximo, entre 2 - 8 °C. Se se pretender conservá-las durante um período de tempo superior, deverão ser<br />
congeladas.<br />
Procedimento<br />
Realização do teste com o BEP ® II<br />
1. Esquema de distribuição<br />
Determinar o número de poços de ensaio necessários (número das amostras a investigar mais 6 poços<br />
para os controlos). Retirar do caixilho as fileiras de poços não necessárias e guardá-las para serem<br />
utilizadas mais tarde (v. tabela 1).<br />
2. Distribuição do tampão para amostras<br />
Colocar em cada cavidade 25 μl de tampão para amostras<br />
3. Distribuição de controlos e amostras<br />
Deitar 100 μl de controlo negativo em cada uma de 4 cavidades e 100 μl de controlo positivo em cada uma<br />
de 2 cavidades, depois 100 μl de amostra não-diluída em cada uma das cavidades seguintes. Cobrir com<br />
película aderente e, o mais brevemente possível, no máximo 15 min após a distribuição das amostras,<br />
colocar na incubadora.<br />
Alternativamente ao esquema de pipetagem acima indicado é permitido também distribuir o controlo positivo<br />
uma vez no início e outra vez no fim da série de teste.<br />
Esquema de pipetagem: distribuir em 4 poços, 100 μl em cada um, de soro de controlo negativo, em 1<br />
poço 100 μl de soro de controlo positivo, nos poços seguintes 100 μl, em cada um, de amostra não diluída<br />
e no fim da série de teste ou da placa de ensaio novamente 100 μl de soro de controlo positivo.<br />
4. Incubação das amostras<br />
Deixar incubar durante 30 ± 2 min aprox. a + 37 ± 1 °C, imediatamente a seguir dar início à lavagem.<br />
5. Lavagem<br />
Retirar a película aderente, aspirar o conteúdo de todas as cavidades e lavar 4 vezes com aprox. 0,3 ml,<br />
para cada um, de solução de lavagem.<br />
6. Distribuição do conjugado<br />
Deitar 100 μl de conjugado em cada cavidade, cobrir com uma nova película aderente e colocar imediatamente<br />
na incubadora.<br />
7. Incubação do conjugado<br />
Deixar incubar durante 30 ± 2 min aprox. a + 37 ± 1 °C, como descrito na alínea 4, e imediatamente a<br />
seguir dar iníco à lavagem.<br />
8. Lavagem<br />
Retirar a película aderente e lavar como descrito em 5.<br />
9. Distribuição do substrato<br />
Deitar 100 μl de solução cromógena em cada cavidade, cobrir a placa com nova película aderente.<br />
10. Incubação do substrato<br />
Deixar incubar durante 30 ± 2 min protegido da luz entre + 18 e + 25 °C.<br />

11. Reacção de paragem<br />
Retirar a película aderente. Acrescentar a cada cavidade 100 μl de solução de paragem, mantendo o<br />
mesmo ritmo que em 9.<br />
12. Leitura<br />
Fazer uma leitura fotométrica no espaço de 1 hora.<br />
Efectuar a medição das extinções das amostras de controlo e das dos pacientes a 450 nm; como comprimento<br />
de onda de referência aconselha-se 650 nm (eventualmente entre 615 e 690 nm).<br />
Realização do teste com BEP ® III<br />
No caso do processamento ser feito com BEP ® III é necessário preparar as placas de ensaio incluindo a<br />
dosagem das amostras (ponto 1 e 2 da «execução do teste com BEP ® II»). Imediatamente a seguir, as placas<br />
de ensaio são colocadas no BEP ® III abertas, ou seja, sem película colada. Nas placas de teste ”incompletas”,<br />
estas necessitam de ter no mínimo 6 strips ainda que sejam preenchidas com água. O processamento subsequente<br />
do teste realiza-se automaticamente (vide instruções de serviço do BEP ® III).<br />
Devido às condições técnicas básicas (ciclo dos aparelhos), os tempos de incubação ajustados no software<br />
do BEP ® III podem divergir dos tempos do processamento com o BEP ® II, mas foram validados na combinação<br />
BEP ® III/Enzygnost * .<br />
Execução do teste com o BEP ® 2000<br />
A dosagem das amostras e subsequente processamento do teste efectua-se de forma completamente automática<br />
no aparelho (vide instruções de serviço do BEP ® 2000).<br />
Validação do teste<br />
Cada valor de extinção para os soros de controlo deve obedecer às seguintes especificações:<br />
- 0,010 ≤ E ≤0,150<br />

E ≥0,700<br />
pos.<br />
Dos quatro valores de extinção do soro de controlo, negativo, pode estar um fora da especificação e ser<br />
desprezado.<br />
Os dois valores do controlo positivo têm de corresponder ambos à especificação.<br />
Não estando preenchidas estas condições, há que repetir o teste.<br />
Avaliação do teste<br />
As avaliações efectuam-se automaticamente com o BEP ® 2000 e BEP ® III. Para o efeito, consultar as instruções<br />
de serviço. Deverão observar-se os capítulos seguintes no caso da avaliação ser efectuada sem a assistência<br />
de Software.<br />
De todos os valores de extinção (positivos e negativos) válidos, tira-se o valor médio para cada conjunto de<br />
valores positivos e negativos.<br />
Para calcular o valor-limite (cut off) junta-se um valor de 0,400 ao valor médio de extinção do controle negativo:<br />

De acordo com os critérios do teste, as amostras são classificadas do seguinte modo:<br />

Amostras que mostrem extinções na zona de incerteza ou superiores devem ser sujeitas a novo teste, numa<br />
dupla determinação.<br />
No caso de reanálise da amostra, esta deve ser considerada anticorpo-negativa caso as absorvâncias se<br />
encontrem fora do intervalo estipulado, definido no teste.<br />
Se os valores médios do novo teste se acharem novamente na zona de incerteza ou acima dele, a amostra será<br />
considera novamente reactiva. Todas as amostras que acusem repetidamente valores duvidosos ou reactivos<br />
têm de ser sujeitas a um teste dito de confirmação (p. ex. uma análise Western-Blot).<br />
Limitações do procedimento<br />
1. Os anticoagulantes como a heparina, EDTA e citrato não influenciam o resultado do teste.<br />
2. As amostras lipémicas, ictéricas ou hemolíticas não afectam o teste.<br />
3. As amostras com substâncias potencialmente perturbadoras de factores reumatóides, anticorpos de EBV/<br />
IgM, anticorpos de VZV/IgM, anticorpos de E.-Coli, ANA e AMA, bem como os soros de mulheres grávidas<br />
foram controladas com o teste. Não foi observada qualquer influência no resultado do teste com as amostras<br />
usadas.<br />

4. Nas amostras congeladas e descongeladas várias vezes, assim como nos soros tratados com calor (30<br />
min., +56 °C) não se registaram quaisquer perturbações.<br />
5. Não deverão utilizar-se soros insuficientemente coagulados nem amostras de teste contaminadas por<br />
micróbios. Antes da realização do teste, os componentes de partículas existentes (p. ex., coágulo de<br />
fibrina, eritrócitos) têm de ser eliminados.<br />
6. Tratando-se de amostras descongeladas, é necessário prestar atenção a que o material fique bem homogeneizado.<br />
7. O teste só é adequado para a análise de amostras individuais, não para amostras de pool ou diluídas.<br />
8. Os reagentes (excluindo a solução de lavagem POD, a solução de paragem e a solução cromógena<br />
pronta para uso fabricada a partir de reagentes associados a lotes de cromógeno TMB e tampão/substrato<br />
TMB) só deverão ser utilizados associados a lotes, ou seja, unicamente na combinação de 6 números<br />
da designação do lote impressa na embalagem ou retirados, respectivamente, da tabela do código de<br />
barras embalada separadamente.<br />
9. O tampão/substrato TMB, a solução cromógena pronta para uso e a solução de paragem POD não podem<br />
entrar em contacto com iões de metal pesado ou substâncias oxidantes (não utilizar pipetas que contenham<br />
partes metálicas que contactem com soluções). Não executar as reacções do substrato na proximidade de<br />
desinfectantes contendo hipoclorito. A coloração espontânea da solução cromógena pronta a usar antes de<br />
ser transferida para a placa de ensaio indicia que há contaminação; é necessário preparar uma solução<br />
fresca num recipiente limpo. Evitar o contacto da pele com as soluções referidas anteriormente.<br />
10. Durante a incubação, a placa de ensaio deverá repousar (p. ex., sobre um flutuador fixo ou em banhomaria<br />
não circulante); neste caso, as cavidades ficam em contacto com a água temperada. Se forem<br />
utilizados estabilizadores para impedir a propagação dos germes na água, é necessário ter todo o cuidado<br />
para que nem a superfície das placas de ensaio, nem os poços entrem em contacto com estas soluções,<br />
uma vez que essa circunstância pode provocar reacções não específicas.<br />
11. Nas amostras fortemente reactivas, na reacção de paragem o corante pode perder-se. A interpretação<br />
fotométrica não é influenciada por esse facto.<br />
12. Os soros de controlo são preparados utilizando soros humanos congénitos. Por essa razão, podem ocorrer<br />
turvações que, todavia, não influenciam o resultado do teste.<br />
13. A Dade Behring validou a utilização destes reagentes em vários analisadores, com o intuito de optimizar<br />
o desempenho do produto e satisfazer as especificações do produto. As modificações definidas pelo<br />
utilizador não são suportadas pela Dade Behring, na medida em que podem afectar o desempenho do<br />
sistema e os resultados do ensaio. Constitui responsabilidade do utilizador validar as modificações efectuadas<br />
nestas instruções ou em relação ao uso dos reagentes noutros analisadores que não os incluídos<br />
nas folhas de instruções de aplicação da Dade Behring ou nestas instruções de utilização.<br />
14. Os resultados deste teste devem sempre ser interpretados em conjunto com o histórico médico do doente,<br />
estado clínico e outros dados de interesse.<br />
Características do teste<br />
Sensibilidade e especificidade<br />
Os resultados das provas de sensibilidade e especificidade encontram-se resumidos nas tabelas 2 + 3 (em anexo).<br />
Para averiguar a sensibilidade foram investigadas um total de 566 amostras positivas para <strong>Anti</strong>-<strong>HIV</strong>1, 317<br />
para <strong>Anti</strong>-<strong>HIV</strong>2 e 22 para <strong>Anti</strong>-<strong>HIV</strong>1 (subtipo 0). Todas as 905 amostras <strong>Anti</strong>-<strong>HIV</strong> positivas foram declaradas<br />
positivas segundo os critérios do Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong>.<br />
A reactividade ao ensaio relativamente a amostras em seroconversão foi testada utilizando 31 painéis de<br />
soros de pacientes durante a fase de seroconversão. Pôde observar-se que o Enzygnost * <strong>Anti</strong>-<strong>HIV</strong> 1/2 <strong>Plus</strong><br />
apresenta, no que diz respeito à sensibilidade a ou reconhecimento de seroconversões, um grau de sensibilidade<br />
correspondente à de métodos similares. Não pode todavia excluir-se que, num emprego do teste a<br />
mais alta escala, algumas amostras escapem a um reconhecimento.<br />
Para determinação da especificidade foi testado um total de 9243 amostras sanguíneas <strong>Anti</strong>-<strong>HIV</strong> negativas e<br />
achados valores de 99,3% a 100% (ensaio inicial) ou de 99,9% (repetição do ensaio). Valores diferentes<br />
destes são também possíveis, dependendo isso, entre outros factores, da amostragem testada ou da modalidade<br />
de execução do teste.<br />
No estado actual de conhecimentos, não se pode inferir com certeza de um resultado positivo do teste <strong>Anti</strong>-<br />
<strong>HIV</strong> que existam efectivamente no sangue <strong>HIV</strong>1 ou <strong>HIV</strong>2 infecciosos, nem tão-pouco um resultado negativo<br />
permite excluir com certeza a sua presença.<br />
Reproduetibilidade<br />
Os resultados relacionados com a reprodutibilidade Intra/Inter-ensaio encontram-se resumidos na tabela 4<br />
(apêndice). Tratam-se de dados determinados a título de exemplo. Independentemente da realização do<br />
teste, é perfeitamente possível obterem-se valores divergentes.<br />
