INTENDED USE<br />
The <strong>ULTRASENSE</strong> <strong>EA</strong> is composed of liquid soy phosphatides with plasma activator for use in the determination of activated partial thromboplastin<br />
time (APTT) in citrated plasma and related coagulation procedures.<br />
SUMMARY<br />
The main use for the APTT is for the screening of coagulation defects and the presence of inhibitors. The test is prolonged by the deficiencies of<br />
factors VIII, IX, X, XI and XII and defects of the contact phase, e.g. prekallikrein, high molecular weight kininogen. It also may be prolonged by gross<br />
defects of factor II, V and fibrinogen. With reliable APTT systems, specific and non-specific inhibitors of intrinsic clotting factors are detected. The<br />
APTT is also the most widely used method for the laboratory monitoring of heparin administration (1,2) . The APTT is also used as in the ECAT study as<br />
an overall assessment of the intrinsic clotting system, Accelerated APTT have been reported after operations, oral contraceptive administration and<br />
withdrawal or oral anticoagulation (3) .<br />
R<strong>EA</strong>GENT<br />
The <strong>ULTRASENSE</strong> <strong>EA</strong> is a preparation of soy phospholipids and ellagic acid activator with buffer, stabilizers and preservatives. The reagent is<br />
provided ready to use. Mix reagent by gentle inversion (5 to 8 times) before use.<br />
STORAGE AND STABILITY<br />
The <strong>ULTRASENSE</strong> <strong>EA</strong> is stable according to the following specification. DO NOT FREEZE.<br />

SPECIMEN COLLECTION AND PREPARATION<br />
Test plasma should be prepared from citrated whole blood without heparin, EDTA or oxalate. To obtain the plasma, carefully mix 1 part sodium citrate<br />
solution (0.11 mol/L) with 9 parts venous blood, avoiding the formation of foam.<br />
Immediately centrifuge for 10 minutes at approximately 3000 rpm (1500 x g), remove the supernatant plasma and keep at + 15 to + 25°C until use in<br />
the test. In the US please refer to the NCCLS Document H21-A2, entitled “Collection, Transport and Processing of Blood Specimens for Coagulation<br />
Testing and Performance of Coagulation Assays” (4) .<br />

PROCEDURE<br />
This procedure pertains to manual or semi-automated coagulation systems. Refer to your instrument manual for more detailed instrument specific<br />
instructions.<br />
Pre-incubate the <strong>Hart</strong> <strong>Biologicals</strong> Calcium Chloride (0.025M) to 37 o C for at least 10 minutes (water-bath).<br />
Stepwise pipette scheme for manual systems<br />

120s Incubation at 37°C (in the measuring block).<br />

The clotting time in seconds will be recorded automatically by the analyzer. Refer to the instrument operator’s<br />

QUALITY CONTROL<br />
Reliability of test results should be monitored by use of Vericon Control Plasma 1-2. A control range should be established by the laboratory to<br />
determine the allowable variation in day to day performance for each level of control. Failure to meet quality control specifications should be<br />
investigated and resolved.<br />

CALCULATION OF RESULTS<br />
For best results, duplicate samples are recommended. APTT results should be reported as clotting time in seconds. Calculate the mean clotting time of<br />
duplicate samples and controls. Differences between duplicate results should be less than 5%. Repeat the test if necessary.<br />
LIMITATIONS<br />
Stress, exercise, pregnancy, the post-partum state and surgical operations result in acceleration of the test. Prothrombic changes associated with<br />
recent deep-vein thrombosis, thromboembolic disorders and oestrogen administration may result in accelerated clotting times (5) . Acquired pathological<br />
states such as liver disease, disseminated intravascular coagulation (DIC), and drug toxicity cause prolongation. Certain drugs, including oral<br />
anticoagulants, heparin and thrombolytic agents, prolong the APTT.<br />

Therapeutic ranges for monitoring anticoagulation therapy will vary from laboratory to laboratory, therefore it is essential that each laboratory establish<br />
relevant APTT ranges for its respective patient population.<br />
Abnormal results obtained with a plasma from a patient not on anticoagulant therapy may indicate a factor deficiency or the presence on an inhibitor.<br />
The result may also be due to the effects of certain drugs and medications. Additional procedures such as the PT test and mixing studies using factor<br />
deficient plasma are usually required.<br />
PERFORMANCE CHARACTERISTICS<br />
Heparin Sensitivity: Numerous variables can affect the behavior of heparinized plasma, when evaluated by the APTT test. As a general guideline,<br />
when increasing amounts of Heparin were added to normal plasma, the following results were obtained using the <strong>ULTRASENSE</strong> <strong>EA</strong> on a photooptical<br />

ANWENDUNGSBEREICH<br />

ZUSAMMENFASSUNG<br />
Die aktivierte partielle Thromboplastinzeit (APTT) ist der Globaltest der Gerinnung und der Gegenwart von Inhibitoren. Verlängerungen der APTT<br />
werden verursacht durch den Mangel der Faktoren VIII, IX, X, XI und XII und der Kontaktfaktoren, z. B. Prekallikrein und hochmolekulares Kininogen<br />
(HMWK). Eine Verlängerung kann zudem durch einen erheblichen Mangel der Faktoren II, V und Fibrinogen verursacht werden. Mit zuverlässigen<br />
APTT-Systemen werden spezifische und unspezifische Inhibitoren der intrinsischen Blutgerinnungsfaktoren erfasst. Die APTT ist zudem die am<br />
weitesten verwendete Labormethode zur Überwachung der Heparin-Gabe (1,2) . Ferner wird die APTT in der ECAT-Studie zur Gesamtbewertung des<br />
intrinsischen Gerinnungssystems verwendet. Eine Verlängerung der APTT wurde nach Operationen, nach der Gabe und dem Absetzen von oralen<br />
Verhütungsmitteln und der oralen Antikoagulation berichtet (3) .<br />

Die <strong>ULTRASENSE</strong> <strong>EA</strong> ist eine Zusammensetzung aus Sojaphospholipiden und Ellagsäure-Aktivator mit Puffer, Stabilisatoren und<br />
Konservierungsmitteln. Das Reagenz ist gebrauchsfertig. Reagenz vor Gebrauch durch vorsichtiges Überkopfdrehen (5 bis 8 Mal) mischen.<br />

PROBENABNAHME UND VORBEREITUNG<br />
Das Testplasma sollte aus Citratblutohne Heparin, EDTA oder Oxalat hergestellt werden. Um das Plasma zu erhalten, mischen Sie sorgfältig 1 Teil<br />
Natriumcitrat-Lösung (0.11 mol/L) mit 9 Teilen venösem Blut; vermeiden Sie dabei die Bildung von Schaum. Zentrifugieren Sie unmittelbar danach für<br />
10 Minuten bei ca. 3000 U/Min (1500 x g), entnehmen Sie das überstehende Plasma und verwahren es bis zu seinem Gebrauch bei +15 bis +25°C.<br />

QUALITÄTSKONTROLLE<br />
Die Zuverlässigkeit der Testergebnisse sollte in jedem Lauf mit <strong>Hart</strong> <strong>Biologicals</strong> Vericon 1 und 2 überwacht werden. Jedes Labor sollte hierfür einen<br />
Qualitätskontrollbereich mit den zulässigen Tag-zu-Tag-Abweichungen für jedes Kontrollplasma festlegen. Werden Vorgaben der Qualitätskontrolle<br />
nicht erfüllt, sind die Ursachen zu untersuchen und zu beheben.<br />

Stabilität der Proben/Kontrollen oder den Analyser.<br />
KALKULATION DER ERGEBNISSE<br />
Es wird die Messung von Proben in Doppelbestimmung empfohlen. APTT Ergebnisse sollten als Gerinnungszeit in Sekunden angegeben werden.<br />
Berechnen Sie die mittlere Gerinnungszeit aus der Doppelbestimmung der Proben und Kontrollen. Die Abweichung zwischen den Doppelwerten<br />
sollte dabei nicht größer als 5% sein. Wiederholen Sie die Messungen falls erforderlich.<br />
LIMITIERUNGEN<br />
Stress, körperliche Aktivität, Schwangerschaft, die Phase nach der Geburt und chirurgische Eingriffe führen zu einer Beschleunigung des Tests.<br />
Prothrombin-Änderungen in Verbindung mit kürzlicher tiefer Venenthrombose, thromboembolischen Störungen und der Gabe von Östrogen können<br />
zu einer Beschleunigung der Gerinnungszeiten führen (5) . Erworbene pathologische Zustände wie Lebererkrankung, disseminierte intravasale<br />
Gerinnung (DIC) und Arzneimitteltoxizität verursachen eine Verlängerung. Bestimmte Arzneimittel, einschließlich orale Antikoagulationsmittel,<br />
Heparin und Thrombolytica verlängern die APTT.<br />

Therapeutische Bereiche für die Überwachung der Antikoagulation können von Labor zu Labor unterschiedlich sein. Daher ist es wichtig, dass jedes<br />
Labor relevante APTT Bereiche ermittelt, die den jeweiligen Patientenpopulationen entsprechen.<br />
Abnormale Ergebnisse einer Probe eines Patienten, der sich nicht unter Antikoagulanztherapie befindet, weisen auf einen Faktormangel hin oder<br />
zeigen das Vorhandensein eines Inhibitors an. Das Ergebnis kann auch durch bestimmte Medikamente oder Drogen beeinflusst werden. Weitere<br />
Verfahren, z. B. die Messung einer PT oder die Anwendung von Faktor-Mangelplasmen sind dann in der Regel erforderlich.<br />
DURCHFÜHRUNGSMERKMALE<br />
Heparinempfindlichkeit: Das Verhalten von heparinisiertem Plasma kann von verschiedenen Faktoren beeinflusst werden, wenn es mittels APTT-<br />
Test bewertet wird. Im allgemeinen wurden bei Erhöhung der Heparindosis mit dem <strong>Hart</strong> <strong>Biologicals</strong> APTT folgende Ergebnisse auf einem<br />

USO PREVISTO<br />
<strong>ULTRASENSE</strong> <strong>EA</strong> è una preparazione liquida a base di fosfatidi di soia con un attivatore del plasma destinata alla determinazione del tempo di<br />
tromboplastina parziale attivata (APTT) nel sangue citratato e a procedure di coagulazione correlate.<br />
SOMMARIO<br />
L’APTT è utilizzato principalmente per lo screening dei difetti della coagulazione e per rilevare la presenza di inibitori. Il test è prolungato nelle<br />
carenze dei fattori VIII, IX, X, XI e XII e in caso di difetti nella fase di contatto, ad esempio carenza di precallicreina e chininogeno ad alto peso<br />
molecolare. Può essere prolungato anche da grossi difetti dei fattori II e V e del fibrinogeno. Con sistemi APTT affidabili vengono rilevati gli inibitori<br />
specifici e non specifici dei fattori della via intrinseca della coagulazione. L’APTT è anche il metodo più diffuso per il monitoraggio di laboratorio della<br />
somministrazione di eparina (1,2) . L’APTT è anche impiegato, ad esempio nello studio ECAT, per la valutazione globale del sistema di coagulazione<br />
per via intrinseca. Sono stati riportati APTT accelerati in seguito ad interventi chirurgici, a somministrazione e sospensione di contraccettivi orali o<br />
nella terapia con anticoagulanti orali (3) .<br />
R<strong>EA</strong>GENTE<br />
<strong>ULTRASENSE</strong> <strong>EA</strong> è una preparazione di fosfolipidi di soia ed attivatore acido ellagico con tampone, stabilizzatori e conservanti. Il reagente è pronto<br />
all’uso. Miscelare il reagente capovolgendo delicatamente (5-8 volte) prima dell’uso.<br />

PRELIEVO E PREPARAZIONE DEI CAMPIONI<br />
Preparare il plasma per il test da sangue intero citratato senza eparina, EDTA od ossalato. Per ottenere il plasma, miscelare con attenzione 1 parte<br />
di soluzione di sodio citrato (0.11 mol/L) con 9 parti di sangue venoso, evitando la formazione di schiuma.Centrifugare immediatamente per 10 minuti<br />
a circa 3000 rpm (1500 x g), rimuovere il plasma surnatante e conservare da +15 a + 25° C fino al momento dell’uso nel test. Fare riferimento al<br />
“NCCLS Document H21-A2”, intitolato “Collection, Transport and Processing of Blood Specimens for Coagulation Testing and Performance of<br />

CONTROLLO DI QUALITÀ<br />
L’affidabilità dei risultati del test va monitorata in ogni processo utilizzando plasma di controllo normale o anomalo. Spetta al laboratorio il compito di<br />
definire un intervallo di controllo allo scopo di determinare le variazioni accettabili nelle operazioni quotidiane per ciascun livello di controllo. I test che<br />

LIMITAZIONI<br />
Stress, sforzo fisico, gravidanza, puerperio e interventi chirurgiche possono causare l’accelerazione del test. Variazioni dell’attività protrombinica<br />
associate a trombosi venosa profonda recente, a disturbi tromboembolici e alla somministrazione di estrogeni possono accelerare i tempi di<br />
coagulazione (5) . Causano un prolungamento dell’APTT stati patologici acquisiti come malattie epatiche, coagulazione intravascolare disseminata<br />

RESUMEN<br />
El tiempo de tromboplastina parcial activado (APTT) se usa principalmente para la detección de anormalidades de coagulación y la presencia de<br />
inhibidores. La prueba es sensible a deficiencias de los factores VIII, IX, X, XI y XII y a anormalidades de la fase de contacto, p. ej. la precalicreína, del<br />
quininógeno de alto peso molecular. También puede ser sensible a causa de anormalidades graves del factor II, V y fibrinógeno. Con sistemas de<br />
APTT confiables se detectan inhibidores específicos y no específicos de factores de coagulación intrínseca. El APTT también es el método que más<br />
se usa para el control de la administración de heparina en el laboratorio (1, 2) . El APTT también se usa como en el estudio ECAT como una valoración<br />
general del sistema de coagulación intrínseca. Un APTT acelerado ha sido informado después de operaciones, administración de contraceptivo oral y<br />
retirada o anticoagulación oral ( 3) .<br />

El <strong>ULTRASENSE</strong> <strong>EA</strong> es un preparado de fosfolípidos de soja y ácido elágico como activador con tampón, estabilizadores y conservantes. El reactivo<br />

RECOLECCIÓN DE MUESTRAS Y PREPARACIÓN<br />
El plasma para la prueba debe prepararse a partir de sangre total citratada sin heparina, EDTA (ácido etilendiaminotetraacético, por sus siglas en<br />
inglés) ni oxalato. Para obtener el plasma, mezcle cuidadosamente 1 parte de solución de citrato de sodio (0.11 mol/L) con 9 partes de sangre<br />
venosa, evitando la formación de espuma.<br />
Centrifugue la mezcla inmediatamente durante 10 minutos a aproximadamente 3000 rpm (1500 x g), retire el plasma sobrenadante y manténgala<br />
entre + 15°C y + 25°C hasta que se use en la prueba. En los Estados Unidos consulte el documento del NCCLS H21-A2, titulado "Recolección,<br />
transporte y procesamiento de muestras de sangre para pruebas de coagulación y conducción de ensayos de coagulación" (4) .<br />

CONTROL DE CALIDAD<br />

CÁLCULO DE LOS RESULTADOS<br />
Para obtener los mejores resultados, se recomienda duplicar las muestras. Los resultados del APTT se deben presentar como tiempo de coagulación<br />
en segundos. Calcule la media del tiempo de coagulación de las muestras y controles duplicados. Las diferencias entre los resultados de los<br />
duplicados deben ser inferiores al 5%. Repita la prueba si es necesario.<br />
LIMITACIONES<br />
El estrés, ejercicio físico, embarazo, estado posterior al parto y operaciones quirúrgicas pueden acelerar la prueba. Los cambios protrombóticos<br />
relacionados con reciente trombosis venosa, trastornos tromboembólicos y administración de estrógeno pueden dar lugar a tiempos de coagulación<br />
acelerados (5) . Los estados patológicos adquiridos tales como enfermedad hepática, coagulación intravascular diseminada (CID) y toxicidad de<br />
medicamentos causan prolongación. Ciertos medicamentos, incluidos los anticoagulantes orales, heparina y agentes trombolíticos, prolongan el<br />

Los rangos terapéuticos para el control de la terapia de anticoagulación varían dependiendo del laboratorio, por lo tanto es esencial que cada<br />
laboratorio establezca rangos relevantes del APTT para su respectiva población de pacientes.<br />
Los resultados anormales obtenidos con una muestra de plasma de un paciente que no está bajo tratamiento con anticoagulantes pueden indicar una<br />
deficiencia en los factores de coagulación o la presencia de un inhibidor. El resultado también puede ser debido a los efectos de ciertas drogas o<br />
medicamentos. Generalmente se requiere la realización de procedimientos adicionales, como la prueba TP (tiempo de protrombina) y estudios de<br />
mezclado mediante el uso de plasma de factor deficiente.<br />

RÉSUMÉ<br />
La principale utilisation de l’APTT est le dépistage de défauts de coagulation et de la présence d’inhibiteurs. Le test est allongé par les déficiences<br />
des facteurs VIII, IX, X, XI et XII et les défauts de la phase de contact, par ex. prékallicréine, kininogène de haut poids moléculaire. Il peut également<br />
être allongé en raison de défauts importants des facteurs II, V et du fibrinogène. Les systèmes APTT fiables permettent de détecter les inhibiteurs<br />
spécifiques et non spécifiques des facteurs de coagulation intrinsèque. L’APTT est également la méthode la plus largement utilisée pour la<br />
surveillance de l’administration d’héparine (1,2) par analyse en laboratoire. L’APTT est également utilisé, comme dans l’étude ECAT, en tant<br />
qu’évaluation générale du système de coagulation intrinsèque. On a reporté un APTT accéléré à la suite d’opérations, d’administration et d’arrêt de<br />

PRÉLÈVEMENT D'ÉCHANTILLON ET PRÉPARATION<br />
Le plasma testé doit être préparé à partir de sang total citraté sans héparine, EDTA ni oxalate. Pour obtenir le plasma, mélanger<br />
précautionneusement 1 volume de solution de citrate de sodium (0,11 mol/L) avec 9 volumes de sang veineux, en évitant la formation de mousse.<br />
Centrifuger immédiatement pendant 10 minutes à environ 3000 tr/min (1500 x g), enlever le plasma surnageant et conserver à + 15 to + 25 °C<br />

CONTRÔLE DE QUALITÉ<br />
La fiabilité des résultats de test doit être surveillée par l’utilisation du plasma de contrôle Vericon 1-2. Une plage de contrôle doit être établie par le<br />
laboratoire afin de déterminer la variation admissible sur une base quotidienne pour chaque niveau de contrôle. Si les spécifications de contrôle de<br />
qualité ne sont pas atteintes, rechercher et résoudre le problème.<br />

CALCUL DES RÉSULTATS<br />
Pour de meilleurs résultats, il est recommandé de prendre des doubles échantillons. Les résultats d’APTT doivent être enregistrés sous forme de<br />
temps de coagulation en secondes. Calculer le temps de coagulation moyen des échantillons doubles et témoins. La différence entre les résultats<br />
doubles doit être inférieure à 5%. Répéter le test si nécessaire.<br />
RESTRICTIONS<br />
Le test est accéléré par les facteurs suivants : stress, activité physique, grossesse, état post-partum et opérations chirurgicales. Les changements<br />
prothrombiques associés avec une thrombose veineuse profonde récente, des anomalies thromboemboliques et l’administration d’œstrogène<br />
peuvent entraîner une accélération du temps de coagulation (5) . Les états pathologiques acquis tels qu'une maladie hépatique, une coagulation intravasculaire<br />
disséminée (CIVD) et la toxicité médicamenteuse entraînent un allongement. Certains médicaments, y compris les anticoagulants oraux,<br />
l’héparine et les agents thrombolytiques, allongent l’APTT.<br />

Les plages thérapeutiques pour la surveillance de traitement anticoagulant varient d’un laboratoire à l’autre. Il est donc essentiel que chaque<br />
laboratoire établisse les plages d’APTT pertinentes pour ses patients respectifs.<br />
Des résultats anormaux obtenus avec du plasma appartenant à un patient non traité par des anticoagulants sont susceptibles d’indiquer un<br />
dysfonctionnement de facteur ou la présence d’un inhibiteur. Le résultat peut également être dû aux effets de certains médicaments et traitements.<br />
En règle générale, des procédures supplémentaires telles que le test PT (temps de prothrombine) et une étude de l’effet de mélange sur du plasma<br />
présentant une anomalie de facteur sont requises.<br />
