Justification : The department of VAQUEZ receives a lot of patients suffering from cardiovascular<br />
diseases. None study approached specifically the main causes of dilated<br />
cardiomyopathies.<br />
Objective : To report the main causes of dilated cardiomyopathies.<br />
Methodology : This is a retrospective study including 124 patients hospitalised in this<br />
department from January 1996 to mai 2001. The inclusion criterium is based on the existence<br />
of a dilatation of the cardiac cavities at the cardiac ultrasonography in patients hospitalised for<br />
heart failure. We have evaluated the social, clinical and paraclinical parameters and the<br />
associated risk factors.<br />
Results : This cohort was composed of 74 men and 40 women whose the average age was 48<br />
=1 years. Male people were more concerned. Primitive dilated cardiomyopathy constitut the<br />
main cause of dilated cardiomyopathies because et represents 41,12 % of them. Secondary<br />
dilated cardiomyopathies represent 58,88% of their causes with sever main etiopathgenical<br />
factors incriminated: alcohol, Meadow’s syndrome, high blood pressure, valvulopathies, heart<br />
decease secondary, to chronic pulmonary, coronaropthy, malnutrition. Half of the patients<br />
belong to a poor socio-economical class. The discovery of the decease is always very late, and<br />
60% of the patients have already global heart failure at the admission. Mortality rate is<br />
19,35% in the department.<br />
Conclusion : Dilated cardiomyopathies are a decease whose prognosis remains always bad.<br />
They require appropriated management and follow-up to limit and slow down the evolution to<br />
congestive heart failure.<br />
Prevention is possible to avoid all the causal factors that may give secondary dilated<br />
cardiomyopathies. Education, information and communication of these patients will be<br />
necessary.<br />

Justification : Les Services de VAQUEZ reçoivent beaucoup de patients ayant une affection<br />
cardio-vasculaire dans le CHU de Befelatanana. Or, aucune étude n’aborde spécifiquement les<br />
principales étiologies des CMD.<br />
Objectif : Rapporter les principales étiologies des cardiomyopathies dilatées ;<br />
Patients et méthodes : Il s’agit d’une étude rétrospective portant sur 124 patients admis dans le<br />
service du mois de janvier 1996 au mois de mai 2001. Le critère d’inclusion est basé sur<br />
l’existence d’une ou des cavités cardiaques ( CMD ) à l’échocardiographie chez les patients<br />
admis pour insuffisance cardiaque. Nous avons évalué les paramètres : démographique,<br />
clinique, paraclinique et les facteurs de risques associés.<br />
Résultats : Notre échantillon était constitué de 74 hommes et 50 femmes dont l’âge moyenne<br />
était de 48=1 ans. Le sexe masculin était le plus exposé. La Myocardiopathie dilatée<br />
primitive constitue la principale cause des CMD, elle représente le 41,12%. Les CMD<br />
secondaires représentent le 58,88% de CMD avec sept principaux facteurs étiopathogéniques<br />
incriminés : alcool, syndrome de Meadow, HTA, valvulopathie, CPC, ischémie myocardique<br />
silencieuse, carence alimentaire. La moitié ( 52,41% ) des patients était de niveau socioéconomique<br />
bas. La découverte de la maladie était tardive, plus de 60% étaient en étant<br />
d’insufisance cardiaque globale à l’admission. La mortalité était de 19,35% à l’hôpital.<br />
Conclusion : Les CMD est une affection dont le pronostic reste toujours mauvais. Elle mérite<br />
une prise en charge et suivis adéquats pour limiter et ralentir l’évolution vers l’insuffisance<br />
cardiaque congestive.<br />
La prévention est possible en évitant tous les facteurs étiopathogéniques susceptibles<br />
d’entraîner une CMD secondaire. L’IEC sera donc indispensable.<br />
