Rare Diseases
Rare diseases, including those of genetic origin, are
life-threatening or chronically debilitating diseases
which are of such low prevalence that special
combined efforts are needed to address them
Low prevalence is taken as prevalence of less than
5 per 10 000 persons in the European Union.
European Rare Diseases Task Force
Rare Diseases
It is estimated that between 5 000 and 8 000 distinct rare
diseases exist today, affecting between 6% and 8% of the
population in total - in other words, between 27 and 36
million people in the European Union
The United States definition is very similar to the European
one. In the US an orphan or rare disease is generally
considered to have a prevalence of fewer than 200 000
affected individuals.
Approccio al problema
• Sospettare una malattia ematologica
• Sospettare una malattia ematologica rara
• Sospettare una malattia ematologica rara
ereditaria
• Cosa fare
Sospettare una malattia
• Sintomatica
ematologica
- il soggetto si presenta per la comparsa
più o meno recente di sintomi o segni: astenia,
gengivorragie, metrorragie, petecchie, ecchimosi,
febbri ricorrenti….
• Non sintomatica
- riscontro occasionale di alterazioni dell’esame
emocromocitometrico o della bilirubina
Sospettare una malattia ematologica:
sintomatica
Come procedere dipenderà dalle condizioni
cliniche del soggetto:
- Anamnesi
- Approfondimento diagnostico: emocromo con formula,
reticolociti, stato del ferro, elettroforesi proteine, beta-2microglobulina,VES
- Diagnosi:
- possibilità di gestione ambulatoriale
- necessità di una visita specialistica
- ricovero
Emocromo suggestivo per:
• Anemia
• Leucocitosi/leucopenia
• La malattia di Gaucher è una patologia fenotipicamente
eterogenea
• Molti parametri sono estremamente variabili:
– Età d’insorgenza
– Grado di progressione di malattia
– Organi coinvolti
– Gravità di malattia nei differenti pazienti
– Gravità di coinvolgimento d’organo nello stesso
paziente
– Sintomi di presentazione
• Differente fenotipo in individui con lo stesso genotipo
