Standing Together: Heart-to-Heart<br />

Baltimore City Health Department’s neighborhood<br />

health profiles show areas of Southwest Baltimore with<br />

some of the highest rates of premature death due to<br />

cardiovascular disease. These areas have death rates<br />

nearly six-times higher than other areas in the City.<br />

Saint Agnes has long been a leader in the treatment of<br />

cardiovascular disease, and this grant will significantly<br />

accelerate the pace at which we can connect directly<br />

with women in our community to provide the education<br />

and tools they need to make a difference in their<br />

heart health.<br />

Areas of Southwest Baltimore have<br />

some of the highest rates of premature<br />

death due to cardiovascular disease. 1<br />

To combat this, the Heart-to-Heart<br />

program at the Saint Agnes Hospital<br />

<strong>Foundation</strong>, Inc will identify and<br />

assess underserved, low-income<br />

African American women at high risk<br />

for cardiovascular disease. A new<br />

companion program to Red Dress<br />

Sunday SM , Heart-to-Heart will provide<br />

community-based, heart-risk screenings<br />

based on the Women’s Heart Center<br />

at Saint Agnes, followed by intervention<br />

programs with nutrition, physical<br />

activity, and healthy lifestyle education<br />

to reduce the risks for heart disease.<br />

The program will be implemented at<br />

nine churches in Baltimore City, where<br />

each church will serve as a site for<br />

heart-risk screenings and intervention<br />

programs. 2012 is the first year the<br />

CCH program awarded funding to<br />

Heart-to-Heart, totaling $244,455 to date.<br />
