- Weber osteotomy
- DuToit capsulorrhaphy
- Gallie procedure (fascia lata graft)
Therapie
Labrum + GHL =probleem
→ herstel normaal anatomie
Therapie
Met huidige technieken < 5% recidief
Postop revalidatie
4-6 wk sling
Actieve flexie tot 120º na 2-3 wk
Exorotatie tot 20º, na 2-3 wk 30-40º
Return sport na 3 mnd
Therapie
Rotatorcuff = probleem
→ herstel cuff
→ reversed prothese
SLAP laesie
Superior Labrum Anterior Posterior
SLAP laesie
3-30% bij anterieure luxaties
Stabilisatie dmv labumfixatie
Mutidirectioneel
Capsulair probleem
Ehler Danlos
Met ernstige laxiteit in in zowel
anterieur als posterieur richting
Operatieve stabilisatie vaak
teleurstellend, hoog % recidief
Conservatief: Fysiotherapie!
Conclusie
1. Inschatten recidief anterieure luxatie
2. Complicaties? Bij APAS fractuur?
3. Ernst 1ste luxatie?
4. Bij acute luxatie, fysio individueel bepalen.
5. Persisterende anterieure instabiliteit?
6. Dan voorkeur operatief: oorzaak
instabiliteit?
7. Voorkeur scopische, anatomische
reconstructie.
Conclusie
8. Bij SLAP: classificatie / MRI
9. Voorkeur scopische stabilisatie indien los
bicepsanker.
10. Postop opvoeren PROM/ exo 30-40°met
return sport > 3 mnd
11. Val + frozen shoulder = post luxatie!
