Cape Town is usually idyllic at this time of the year and<br />

there should be plenty of opportunity to rekindle friendships<br />

and make new ones in a relaxed, congenial atmosphere<br />

and just enjoy our hospitality.<br />

On behalf of the South African Orthopaedic Association and<br />

its organising committee, we have great pleasure in welcoming<br />

you to Cape Town to participate in the 13th Combined<br />

Orthopaedic Meeting. At this meeting we will have active<br />

contributions from all our Sister Associations for which we<br />

thank you.<br />

The scientific committee has put together a very stimulating<br />

programme, with active input from the various subgroups,<br />

which is both interesting and varied. It will consist of symposia,<br />

debates, instructional courses, Plenary and Keynote lectures,<br />

free papers as well as posters. The challenge may well lie in<br />

planning which sessions to attend.<br />

We hope you will take part in all the lively social events and<br />

in the partner’s programme or take this opportunity to explore<br />

Cape Town and the Western Cape on your own.<br />

Sanford E. Emery MD, MBA<br />

Dr. Emery is Professor and Chair of<br />

the Department of Orthopaedics,<br />

Interim Chair of the Department of<br />

Surgery, and Director of Surgical<br />

Services at West Virginia University<br />

in Morgantown, West Virginia. He<br />

has been in the active practice of orthopaedic surgery for<br />

the last 27 years, sub-specializing in spine surgery.<br />

Dr. Emery did his undergraduate studies at Dartmouth<br />

College and medical school at Duke University School<br />

of Medicine. He completed his orthopaedic surgical<br />

residency at the University of Rochester/Strong Memorial<br />

Hospital in Rochester, New York and received his fellowship<br />

training at Case Western Reserve University Hospitals<br />

in Cleveland, Ohio. While on faculty at Case Western<br />

Reserve, he obtained his MBA from Weatherhead School<br />

of Management in 2003.<br />

He is the immediate past president of the ABOS and serves<br />

as a Senior Director of the ABOS. Dr. Emery is also a<br />

member of several professional medical societies including<br />

the American Academy of Orthopaedic Surgeons, North<br />

American Spine Society, Orthopaedic Research Society,<br />

West Virginia Orthopaedic Society, and is a past president<br />

of the Cervical Spine Research Society. He has over sixty<br />

publications, thirty-three book chapters, one textbook, and<br />

has been invited to present his work on over two hundred<br />

occasions at local, regional, national, and international<br />

meetings. He has received external funding for his research<br />

activities as a co-investigator or principal investigator<br />

from the NIH and foundation sources for translational<br />

and clinical outcome studies. Dr. Emery has chaired<br />

the Department of Orthopaedics at WVU since 2003.<br />

He has grown the department from five to twenty-five<br />

full-time faculty, established a resident research program,<br />

and fostered a significant increase in both funding and<br />

productivity of basic and clinical research in Orthopaedics.<br />

AMERICAN ACADEMY OF<br />

ORTHOPAEDIC SURGEONS<br />

David Teuscher<br />

David is the Immediate Past President<br />

after completing service as the 8<strong>3rd</strong><br />

President of the American Academy<br />

of Orthopaedic Surgeons in March<br />

<strong>2016</strong>. He has been in full-time private<br />

practice of orthopaedic surgery with<br />

specialty interest in sports medicine<br />

in Southeast Texas at Beaumont Bone<br />

and Joint Institute since completing thirteen years active<br />

duty U.S. Army service in 1993. A Team Physician for Lamar<br />

University NCAA intercollegiate athletics, he also works with<br />

several local high school athletic training departments.<br />

After completing a Bachelor’s degree at the University of<br />

Illinois at Urbana-Champaign, David joined the Army and<br />

was awarded his M.D. at the University of Texas Medical<br />

School at San Antonio. His Orthopaedic Chief residency<br />

year at The Brooke Army Medical Center was highlighted<br />

with deployment with the 144th Evacuation Hospital in<br />

Operations Desert Shield and Desert Storm in the liberation<br />

of the Iraqi invasion of Kuwait. He has held academic<br />

appointments with the University of Oklahoma and Baylor<br />

University, and clinical faculty with the University of Texas<br />

Medical Branch at Galveston.<br />

Over the past twenty years David has been deeply involved<br />

in a wide variety of public policy debates in Washington,<br />

Austin and numerous other state capitals. A four time<br />

Gubernatorial appointee; he has served the state of Texas<br />

on the Texas Tax Reform Commission, the Texas Inaugural<br />

Committee, Vice-Chairman of the Texas Youth Commission,<br />

and now serves as the Secretary of the Board of Directors of<br />

the Texas Higher Education Coordinating Board in charge<br />

of all public higher education in Texas including 38 colleges<br />

and universities, 50 community colleges and 12 academic<br />

health science centers. He was appointed by the Supreme<br />

Court of Texas in 2012 to serve on the Board of Directors of<br />

the State Bar of Texas.<br />

In addition to his many roles in the AAOS, he has served<br />

in volunteer leadership positions of the Texas Orthopaedic<br />

Association, Texas Medical Association, Western<br />

Orthopaedic Association, American Orthopaedic Society<br />

for Sports Medicine, American Orthopaedic Foot and Ankle<br />

Society, and the American Medical Association.<br />

He and his wife Connie have raised two fully-grown men as<br />

sons; Brandon and Derek. Connie and David now occupy<br />

primary residence on their ranch in northern Bastrop<br />

County in central Texas.<br />

AUSTRALIAN ORTHOPAEDIC<br />

ASSOCIATION<br />

Andreas Loefler<br />

Andreas Loefler is currently<br />

the President of the Australian<br />

Orthopaedic Association. He works<br />

at the Prince of Wales Hospital<br />

in Sydney, a university teaching<br />

hospital, where he has been the<br />

Head of Department for 15 years.<br />

His interests are Spinal Surgery<br />

and Arthroplasty of the Hip and Knee. He is involved in<br />

undergraduate and postgraduate teaching. He also has<br />

a keen interest in Orthopaedic Outreach, with regular<br />

humanitarian missions to Fiji.<br />

Andreas made Ethics and Professionalism the topic of his<br />

presidency to emphasise the importance of these core<br />

values in medical practice.<br />

He is a fellow of the Royal Australasian College of<br />

Surgeons, a member of the Spine Society of Australia, the<br />

American Academy of Orthopaedic Surgeons, and the<br />

European Federation of Orthopaedics and Traumatology.<br />

Robin Richards was born in Guelph, Ontario and attended<br />

McMaster University where he obtained BA (Geography<br />

- 1973) and MD (1976) degrees. He completed the<br />

University of Toronto Postgraduate Training Program in<br />

Orthopaedic Surgery and received his FRCSC in 1982.<br />

Dr. Richards was a MRC Fellow at Duke University in<br />

1983 and joined the staff of St. Michael’s Hospital in<br />

1984 where he was Head of the Division of Orthopaedic<br />

Surgery, Medical Director of the Mobility Program, Chair<br />

of the Medical Advisory Committee and served on the<br />

Board. Dr. Richards was Head of the Department of<br />

Surgery and Co-Director of the Operating Room and<br />

Related Services at Sunnybrook HSC from 2001 - 2012.<br />

Dr. Richards maintains an active surgical practice focusing<br />

on arthroplasty of the shoulder and elbow, upper extremity<br />

reconstruction following trauma, soft tissue procedures to<br />

control joint instabilities in the upper extremity and the<br />

surgical treatment of irreparable brachial plexus injury.<br />

Dr. Richards is Vice Chair – Clinical of the University of<br />

Toronto Department of Surgery, Chair of the Awards and<br />

Internal Evaluation Committees, Medical Director of the<br />

Sunnybrook Working Condition Program and Sunnybrook<br />

Centre for Independent Living, Past Secretary-Treasurer<br />

of the American Shoulder and Elbow Surgeons and<br />

Past Deputy Editor for the Upper Extremity of the Journal<br />

of Bone and Joint Surgery (American). Dr. Richards is<br />

a member of the Ontario, Canadian and American<br />

Orthopaedic Associations and is Past-President of the<br />

Ontario Orthopaedic Association and Past-Secretary of<br />

the Canadian Orthopaedic Association.<br />

Dr. Richards enjoys stream fishing, cycling and<br />

wind-surfing. Dr. Richards has four children ages 26 –<br />

34 and one grandchild.<br />

He now has an entirely elective practice in lower limb<br />

arthroplasty and has specialised in Knee replacement and knee<br />

revision surgery for the last 26 years. He has a lifetime personal<br />

experience of over 2500 hip and 3500 knee replacements.<br />

He teaches widely on technical aspects of knee replacement<br />

and has been invited lecturer throughout the world and at<br />

the major European, North American and International Knee<br />

Surgery organisations.<br />

He is a founder member of the new European Knee Society. He<br />

is currently Secretary of Council of the Bone and Joint Journal.<br />

His research interests are mainly in Soft-tissue aspects of knee<br />

replacement and the outcome of primary and revision total<br />

knees.<br />

He is involved with managing the ODEP and Beyond<br />

Compliance systems for implant assessment and was involved<br />

in developing the NJR.<br />

He has previously served as President, Hon Secretary and Hon<br />

Treasurer of the British Association for Surgery of the Knee.<br />

He will be President of the British Orthopaedic Association for<br />

<strong>2016</strong>/17.<br />

Terrance D. Peabody<br />

Dr. Peabody is the Edwin Warner<br />

Ryerson Professor of Orthopaedic<br />

Surgery and Chair of the<br />

Department of Orthopaedic Surgery<br />

at Northwestern University Feinberg<br />

School of Medicine. He also<br />

provides care for patients at Lurie<br />

Children’s Hospital, and is a member of the Robert H. Lurie<br />

Cancer Centre.<br />

Dr. Peabody has served as President of the American<br />

Orthopaedic Association, President of the North American<br />

Musculoskeletal Tumor Society, Vice Chair of the Residency<br />

Review Committee for Orthopaedic Surgery and is currently<br />

Director for the American Board of Orthopaedic Surgery.<br />

Dr. Peabody previously served on the faculty of the<br />

University of California at Irvine from 1991 to 1993 and<br />

the University of Chicago, Pritzker School of Medicine<br />

from 1994 to 2011. He was the Simon and Kalt Families<br />

Professor of Orthopaedic Surgery and Chief of the Section<br />

of Orthopaedic Surgery and Rehabilitation Medicine<br />

at the University of Chicago. He also held secondary<br />

appointments as a professor of surgery in the University of<br />

Chicago Cancer Center and Instructor in the Department of<br />

Orthopaedic Surgery at Loyola University Chicago, Stritch<br />

School of Medicine. He earned his Doctor of Medicine<br />

degree and completed his residency and internshiptraining<br />

at the University of California-Irvine, and completed his<br />

fellowship at the University of Chicago Medical Centre.<br />

He received his Bachelor’s in Biology from the University<br />

of California-Irvine, where he also received his medical<br />

degree, completed an internship and his residency.<br />

Dr. Peabody’s research and clinical expertise focus on the<br />

treatment of benign and malignant bone and soft-tissue<br />

tumors including limb salvage surgery and functional<br />

restoration for adult and pediatric patients. A well-published<br />

investigator, Dr Peabody has authored multiple journal<br />

articles and book chapters and served as on the editorial<br />

board of several publications. Other than musculoskeletal<br />

oncology, his professional interests include challenges<br />

related to resident and fellow education.<br />

American Academy of<br />

Orthopaedic Surgeons<br />

Stuart L. Weinstein<br />

Dr. Weinstein is the Ignacio V. Ponseti<br />

Chair and Professor of Orthopaedic<br />

Surgery and Professor of Pediatrics<br />

at The University of Iowa.<br />

Dr. Weinstein is an NIH funded<br />

researcher. He has published more<br />

than 200 scientific articles in peer review journals He has<br />

edited three major textbooks including The Pediatric Spine<br />

and Lovell and Winter’s Pediatric Orthopaedics .<br />

Dr. Weinstein was ABC Traveling Fellowship in 1985. His is<br />

the recipient of the Bristol-Myers Squibb/Zimmer Award for<br />

Distinguished Achievement in Orthopaedic Research; The<br />

Kappa Delta /OREF Clinical Research Award(twice, 1998,<br />

2015); The Russell Hibbs Award ( 1998, 2014), The Arthur<br />

H. Heune Memorial Award, and the Alfred R. Shands, Sr.,<br />

MD Award. In 2011, Dr. Weinstein received the William W.<br />

Tipton, Jr., MD, Award for Outstanding Leadership and in<br />

2012 he received the AOA’s Distinguished Contributions<br />

to Orthopaedics Award.<br />

He has been honored by honorary memberships in<br />

National Orthopaedic Associations around the world<br />

including Australia, New Zealand, Germany, Great Britain<br />

Thailand, China, Portugal, and Argentina. In 2007 he<br />

was made a Fellow of The Royal College of Surgeons of<br />

England and in 2014 he was made an Honorary Member<br />

of EFORT.<br />

Dr Weinstein is past president of the American Academy<br />

of Orthopaedic Surgeons, The American Orthopaedic<br />

Association, The American Board of Orthopaedic Surgery,<br />

The Paediatric Orthopaedic Society of North America, and<br />

Past Chairman of the Board of Trustees of the Journal of<br />

Bone and Joint Surgery (JBJS).<br />

He and his wife are the proud parents of four children.<br />

Dr Ian Incoll<br />

Dr Ian Incoll is an Australian<br />

Orthopaedic and hand surgeon. He<br />

is Chair of Education and Training<br />

and Vice President for the Australian<br />

Orthopaedic Association (AOA).<br />

He is currently involved in AOA 21,<br />

the exciting new initiative in Education<br />

and Training by AOA, incorporating global best practice to<br />

improve quality in patient care nationally.<br />

He is Conjoint Senior Lecturer at the University of<br />

Newcastle, Australia, a director of the Australian Society of<br />

Orthopaedic Surgeons and a past director of the Australian<br />

Medical Association.<br />

James Patterson Waddell MD,<br />

Dr. James Waddell graduated from the University of Alberta<br />

Medical School. He did his postgraduate training in<br />

orthopaedic surgery at the University of Toronto and assumed<br />

a staff position at St. Michael’s Hospital.<br />

He has occupied a number of positions at St. Michael’s<br />

Hospital including Surgeon-in-Chief and Medical Director of<br />

the Trauma Program; he completed 10 years as the Professor<br />

& Chairman, Division of Orthopaedic Surgery at University<br />

of Toronto in June of 2006. He has also occupied a number<br />

of positions in the Canadian Orthopaedic Association<br />

including President. He has completed a 10 year term as the<br />

Co-Editor of the Canadian Journal of Surgery and recently<br />

stepped down as the Coordinator for the Canadian National<br />

Action Network for the Bone and Joint Decade. He recently<br />

completed his four year term as the Board Chair of the<br />

Canadian Orthopaedic Foundation.<br />

He is currently the Chair of the Expert Panel for Orthopaedic<br />

Surgery for the Province of Ontario, the Co-Chair of the<br />

National Hip and Knee Knowledge Translation Network.<br />

In 2006 Dr. Wedge became an Officer of the Order of Canada<br />

for his outstanding contributions to Paediatric Orthopaedic<br />

Saskatchewan in 2007. In 2012 he was awarded the Queen<br />

Elizabeth II Diamond Jubilee Medal for “dedicated service to<br />

peers, community and Canada”.<br />

Professor Sue Stott<br />

Professor Susan Stott is based at<br />

the Department of Surgery, Faculty<br />

of Medical and Health Sciences,<br />

University of Auckland and is<br />

Children’s Hospital.<br />

Professor Stott gained her MB ChB in 1982 and, in 1988,<br />

was one of the first women to be awarded a Fellowship in<br />

Orthopaedic Surgery from the Royal Australasian College<br />

of Surgeons. She has served as Editorial Secretary for the<br />

New Zealand Orthopaedic Association and is a current<br />

Examiner for the Royal Australasian College of Surgeons.<br />

She is a past President of the Australasian Academy of<br />

Cerebral Palsy and Developmental Medicine and has<br />

served as a Board member on the Health Research Council<br />

of New Zealand.<br />

Her major research interests are in the area of physical<br />

disability in children and the impact of acquired<br />

neurological disorders such as cerebral palsy on musculoskeletal<br />

functioning in childhood. She has received<br />

funding from the Health Research Council, Lottery Health,<br />

Neurological Foundation and the Cerebral Palsy Society of<br />

New Zealand to support her research.<br />

He is a member of the National Osteoporosis Foundation<br />

of South Africa. His interest is paediatrics spine deformity,<br />

osteoporosis and medical education.<br />

He is married to Grace Motswana Lukhele and has 4 children.<br />

He enjoys road running and occasional cycling.<br />

Dr Mark Eltringham<br />

Dr Mark Eltringham is a Wits graduate<br />

who also did his postgrad training<br />

and Orthopaedic specialisation<br />

on the Wits circuit. His interest in<br />

Paediatric Orthopaedic Surgery and<br />

in Limb Reconstruction and Deformity<br />

Correction began in 1989, initially<br />

whilst working under Prof Erken at the<br />

then Johannesburg Hospital, continuing on into his full time<br />

Private Practice at Sunninghill Hospital where he has worked<br />

since 2002. He began using the Ponseti method in 2003, and<br />

has been involved since then in training and disseminating the<br />

method in Southern Africa.<br />

In his spare time he is an avid cyclist, enjoys DIY and spending<br />

time – preferably on an off-road camping adventure – with<br />

his family.<br />

Mkhululi “Mac “Lukhele is a general<br />

Orthopaedics surgeon with special<br />

interest in spine surgery. He is the<br />

clinical head of department of<br />

Orthopaedics at Charlotte Maxeke<br />

Johannesburg Academic Hospital.<br />

He is the head of the spine unit at<br />

CMJAH where he has been training<br />

spine surgeons.<br />

He is the adjunct Professor of Orthopaedics at the<br />

University of Witwatersrand where he is also the Head<br />

of the School of Clinical Medicine. He is one of the past<br />

presidents of the South African Medical Association and<br />

also a past president of the South African Spine Society &<br />

South African Orthopaedic Association.<br />

of the congress an administration fee of R1000.00 per<br />

person will be levied over above the cost of the congress<br />

an administration fee of R250.00 per day will be levied to the<br />

daily registration fee.<br />

Registrars/ Trainees & Profession Allied to Medicine when<br />

registering on site as a delegate for the full duration of the<br />

congress an administration fee of R500.00 per person will be<br />

levied over above the cost of the congress fee. The same will<br />

be applicable for daily registrations but an administration fee<br />

of R125.00 per day will be levied to the daily registration fee.<br />

NAME BADGES:<br />

Your name badge is the official pass and must be worn at<br />

all times to obtain entry to all Congress sessions, teas and<br />

coffees, and social events. Should you have pre-registered<br />

for the social functions or activities on Wednesday afternoon<br />

these will be reflective on your name badges.<br />

If you have registered more than one person for the above<br />

events you will receive coupons for the additional person’s<br />

There is a private <strong>COMOC</strong> Business Centre for all<br />

<strong>COMOC</strong> delegates. This is can be found on level 1, next<br />

to the Speaker Ready Room outside Auditorium 1.<br />

There will be laptops and printers available for your use.<br />

Should you require any assistance with uploading the<br />

<strong>COMOC</strong> App or connecting to the <strong>COMOC</strong> WiFi, there<br />

will be technical personnel to assist you.<br />

You are also able to print off hard copies of the daily<br />

programme or general information you might need from<br />

the <strong>COMOC</strong> website.<br />

There will also be a CHARGING STATION at the INVESTEC<br />

Booth(100) in the EXHIBITION HALL where you can<br />

charge your phones and devices or alternatively charge<br />

your phones in the Business centre (please ensure that you<br />

bring your own charger & international adaptor along,if<br />

you are not a South African <strong>Delegate</strong>). You can leave<br />

devices to charge but <strong>COMOC</strong>, CTICC and Congress<br />

DISCOVERY CPD SCANNING:<br />

SOUTH AFRICAN DELEGATES ONLY:<br />

Discovery Health is facilitating the CPD process at the<br />

congress. The Discovery MYCPD staff will be scanning the<br />

QR Code on your congress name badge twice a day, once in<br />

the morning and again in the afternoon. CPD points will be<br />

allocated for every scan so please ensure that you have your<br />

QR coded name badge scanned twice a day for maximum<br />

points.<br />

The data is logged on the MYCPD website www.mycpd.co.za<br />

where you can view and download your CPD Certificate.<br />

You will receive notification via email with your unique<br />

username and password 2-3 weeks after the congress.<br />

any assistance with your CPD Certificate.<br />

CPD points will be allocated according to the rules and<br />

regulations of the Health Professions Council of South<br />

Africa (HPCSA). South African/local delegates will receive<br />

29 Clinical and 17 Ethics points<br />

International delegates can accrue 37 South African CPD<br />

points.<br />

CME points should also be accrued for international<br />

delegates via their local accreditation bodies for equivalent<br />

points.<br />

All South African delegates will need to scan in twice a<br />

day to claim their CPD points – once between 08h00 and<br />

12h30 to claim their morning session and once between<br />

13h30 and 17h00 to claim their afternoon session CPD<br />

points. Ethics points will be scanned in the respective<br />

venues. Accreditation of ethics points is only valid for South<br />

African <strong>Delegate</strong>s which are relevant to South Africa law.<br />

Ethics CPD allocation: Ethics sessions will be scanned<br />

outside the venue from 15 minutes prior to the presentation<br />

times listed below.<br />

The <strong>COMOC</strong> medic will be stationed in the registration area, please come to the registration area if there is an emergency.<br />

SAFETY TIPS<br />

1. Be street smart like the locals; don’t walk around flaunting paraphernalia like cameras, smartphones or jewellery.<br />

2. Always ask for local advice, especially when going out at night. Your hotel is a good place to start.<br />

3. Book a tour guide, they have local knowledge and will really look after you. African Eagle Tours are on site in the<br />

registration area.<br />

4. For a mountain or beach walks or anywhere off the beaten track, there is always a guide for your needs.<br />

5. Don’t catch lifts in the minibus taxis - rather use a metered taxi, Mycity busses or UBER are your best & safest options<br />

6. Anyone visiting the townships should make use of guided tours and should do it during daylight hours. African Eagle Tours<br />

are on site in the registration area and can book this tour option for you.<br />

7. Don’t walk alone to your hotel at night - rather take a metered taxi or UBER.<br />

8. Keep your windows up and your doors locked when driving at night and if lost, drive to the nearest petrol station - most<br />

petrol attendants in RSA are very good at giving directions.<br />

There is always a small criminal element in every large city and Cape Town is no exception.<br />

CURRENCY<br />

The currency of South Africa is the South African Rand (ZAR)<br />

divided into 100 cents. Notes come in denominations of 200,<br />

100, 50, 20 and 10 Rands, and coins in 5, 2, 1 Rands and 50,<br />

20, 10 and 5 cents. Foreign currency and travellers cheques<br />

can be exchanged at banks, exchange bureaux, hotels and at<br />

the airport.<br />

LANGUAGE<br />

The official language of the Congress will be English and there<br />

will be no translations.<br />

South Africa has 11 official languages,including Afrikaans,<br />

DRINKING WATER<br />

Water from the taps is perfectly safe to drinks in all the cities,<br />

towns and game reserves. At The <strong>COMOC</strong> Congress, you will<br />

be supplied with water bottles that can be refilled at numerous<br />

ELECTRICITY<br />

The electricity supply in South Africa is 220/230 volts AC<br />

50Hz. Appliances of lesser voltage require a transformer<br />

(especially those with a heating element like a hairdryer or flat<br />

iron). Adapters are obtainable locally. Most hotel rooms have<br />

110-volt outlets for electric shavers and small appliances.<br />

TIME<br />

South Africa is UT+2 (2 hours ahead of GMT – Greenwich<br />

Mean Time).<br />

A valid passport is required. Passport holders of more than 80<br />

countries, including the USA, Canada, Japan and the EU, can<br />
