Incident Reporting<br />

• FIRST AID CASES: -<br />

• All first aid case must be referred to HSE Dept. first before proceeding to our<br />

clinic or hospital. Supervisor-in-charge of the worker must accompany<br />

throughout the whole process i.e. to accompany the injured until medical<br />

attention if necessary. Supervisor will submit report within 48 hours<br />

otherwise the case is considered CLOSE.<br />

• MAJOR CASES: -<br />

• In such case if need to refer to hospital immediately, the Supervisor will bring<br />

the injured directly to the hospital, admit the injured first then informed HSE<br />

Dept. 6866 0054 or leave message with the operator 6866 0000 where is the<br />

injured person.<br />

• Report must be made within 24 hours and cordon off the area for<br />

investigation.<br />

Emergency Response<br />

• In case of an emergency, STOP all work, SWITCH OFF all<br />

equipment & proceed to the assembly area.<br />

• Report to your respective fire wardens/supervisors/HSE<br />

personnel of your presence & wait for instructions.

• To empower all of Rotary’s employees<br />

during the intervention of any hazardous/<br />

dangerous/unsafe situations without the<br />

fear of reprisals.<br />

• To empower all of Rotary’s employees to<br />

stop or decline any hazardous/dangerous/<br />

unsafe tasks without the fear of reprisals.


EEP – Employees’ Responsibilities<br />

• Intervene or stop the work immediately if the employee<br />

believes a situation exists which places himself/herself,<br />

other employees or the environment in danger or at risk.<br />

• Stop or decline any assigned tasks if the employee believes<br />

a situation exists which places himself/herself, other<br />

employees or the environment in danger or at risk.<br />

• Report any activity or situation that he/she believes is<br />

unsafe.<br />

• Discuss & resolve work & safety matters without the fear<br />

or reprisals.<br />

•3.2.1 Resolve any issues that have resulted in an individual<br />

stopping a specific task(s) or activity.<br />

•3.2.2 Provide feedback to individual/s and the affected work group<br />

who have exercised their Stop Work responsibility on the<br />

resolution of their concern prior to resuming work.<br />

•3.2.3 Ensure no actions are taken as reprisal or retribution against<br />

individuals who raise safety concerns or stop an activity they<br />

believe is unsafe.<br />

•3.2.4 Cultivate a culture where employee intervention is used when<br />

necessary and valued as a proactive process.<br />

•3.2.5 Log and track all employee interventions to ensure timely<br />

implementation of corrective / preventive actions.

• 1 Amber -> Situations/Actions that serves as a warning<br />

sign which may lead to an accident.<br />

• 3 Ambers -> Combination of the 3 different warning signs<br />

that can immediately lead to an accident.<br />

• 3 Ambers = 1 Red -> STOP<br />

• Also known as the ‘Rule of 3’.
