<strong>Service</strong> Area Competition (SAC) Technical Assistance<br />

<strong>Manual</strong><br />

Introduction<br />

This <strong>technical</strong> <strong>assistance</strong> manual addresses changes made to the new fiscal year 2014<br />

<strong>Service</strong> Area Competition grant, which is open to current grantees and new applicants.<br />

Eligible applicants include: public or nonprofit organizations, current grantees, health<br />

centers that are not currently funded, or current grantees that would like to serve an<br />

additional <strong>area</strong>. Further <strong>technical</strong> <strong>assistance</strong> is available to new applicants and current<br />

grantees through two National Cooperative Agreements at North American<br />

Management. Grantees who serve special populations such as public housing residents<br />

can receive direct support through the National Center for Health in Public Housing.<br />

This manual broadly covers changes implemented in the new SAC grants. Additional<br />

information can be found in the grant application.<br />

applicants should use UDS Mapper for maps and accompanying data tables.<br />

• Attachment 14: An implementation Plan is now required for new applicants and current<br />

grantees applying to serve a new service <strong>area</strong>. This plan is designed to outline action steps<br />

required for meeting the 120-day operational status requirement.<br />

II.<br />

Cancer Screening Changes<br />

Although the UDS table has not changed, HRSA has changed the method of measurement.<br />

Due to the U.S. Preventive <strong>Service</strong>s Task Force latest recommendation in 2012, the new<br />

measure accounts for 5-year intervals between screenings for women age 30-64 years.<br />

Form 2: Staffing Profile<br />

The Staffing Plan has been updated to include a column to report staff expenses to be<br />

charged to the SAC grant (i.e. requested federal dollars).<br />

Applicants must indicate any staff whose salary will be paid from the grant during the first<br />

year of the award. This information includes staff who may receive funds through indirect<br />

costs. All staff and salaries reported in Form 2 should be associated with the Health Center<br />

Program project. If staffing changes are to occur, applicants must provide additional<br />

information in Item 4 (Resources and Capabilities) of Form 2.<br />

Form 3: Income Analysis<br />

The Income Analysis has been revised to simplify the reporting of projected income.<br />

Applicants must report estimated income for first budget year. Income reported here should<br />

include 1) program income/patient service revenue, and 2) all other income. Do not include<br />

any funds from the grant in this section.<br />

Patient service revenue includes all forms of payment from the patient or a third-party such<br />

as reimbursement or payment from an insurance company.<br />

Form 6A: Current Board Member Characteristics<br />

Current Board Member Characteristics will be pre-populated and available for edit for<br />

current Health Center Program grantees.<br />

The new application system remembers previous data for current grantees, but grantees<br />

must make corrections to old information as necessary. New grantees must fill out Form 6A<br />

in its entirety. The only exclusions applicable for this required section include Tribal and<br />

Urban Indians and those who request a waiver. However, those requesting a waiver must<br />

list the current board members, even if the patient majority is not 51 percent. Exact<br />

specifications for all information requested may be found in the application.<br />

Form 6B: Request for Waiver of Governance Requirements<br />

Request for Waiver of Governance Requirements will be not applicable for current grantees<br />

receiving CHC (section 330(e)) funding<br />

The only grantees eligible to request a waiver for lack of patient majority board composition<br />

are:<br />

Homeless Health Centers<br />

Public Housing Primary Care<br />

Current grantees of the Community Health Center (CHC) program or those applying for<br />

CHC funding are not eligible for waivers. Waivers are available for current grantees, but a<br />

new waiver must be requested. Failure to meet the 51 percent patient majority must be<br />

explained.<br />

Form 9: Need for Assistance (NFA) Worksheet<br />

The Need for Assistance Worksheet has been revised to include more current indicators.<br />

Core Barriers and Health Indicators have been modified, added, or removed to include the<br />

most relevant and current indicators of need for which data are available.<br />

Form 9 includes 3 sections:<br />

1) Core Barriers include:<br />

a. Percent under 200% of the Federal Poverty Level<br />

b. Percent Uninsured<br />

c. Population to one FTE Primary Care Physician<br />

d. Distance or Travel time to Nearest Primary Care Provider accepting Medicaid<br />

and Uninsured Patients<br />

For sections 2 and 3, data required should be the most current and available sub-state data.<br />

Please limit response to one per barrier or indicator.<br />

Although an applicant may have multiple proposed sites, there should be one NFA<br />

worksheet per application.<br />

Please note a new change: The budget justification must detail the costs of each line item<br />

within each object class category from the Federal Object Class Categories form (federal<br />

section 330 funding request and non-federal funding). See Appendix C within the<br />

application for details.<br />

Project period length determining factors have been added, including a stipulation that a<br />

current Health Center Program grantee will not be funded if they would be awarded a third,<br />

consecutive one-year project period for FY 2014. The maximum project period length is<br />

three years for all applicants. See Table 7 below:<br />
