TERMS: Payment, shipment and pricing terms are as set forth in your International Distributorship Agreement. STANDING ORDERS: Navilyst Medical accepts standing orders on all products. This convenience provides you with scheduled shipments using a single purchase order. Contact the Customer Service Contracts Department at 518.792.4112. TO RETURN GOODS: The circumstances in which you may return products, and the terms applicable to any such returns, are set forth in your International Distributorship Agreement. TO RETURN GOODS (as permitted by the terms of your International Distributorship Agreement): • Contact Customer Service for a return authorization number at 518.792.4112. • For product complaints, please email the complaint notification form to navilystcomplaintmailbox@navilyst.com. • Identify the customer account number, catalog number, size, quantity, lot number, invoice number and the reason for the return. • Clearly identify the RGA number on your shipping paperwork. • Ship product returns to: Navilyst Medical, Inc., 10 Glens Falls Technical Park, Glens Falls, NY 12801 SPECIFICATION CHANGES; NOTICE: As we are continually improving our products, the equipment supplied may not agree in all details with the description and specification shown in the catalog and/or price list.
